정신분열병에 대한 리스페리돈의 효과 및 안정성

연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

행정서비스품질의 모델 중심적 분석에 관한 연구

본 연구는 점차 그 중요성이 증대되고 있는 행정분야에서의 고객만족을 위한 우수한 행정서비스 품질을 제공하기 위해 서비스품질의 측정방법인 SERVQUAL 모형과 SERVPERF 모형간의 적합성 여부를 3가지 비교방법에 의해 분석하고 행정서비스품질의 5가지 구성차원간의 중요도가 전반적 서비스품질과 고객만족에 어떤 차이가 있는지, 그리고 전반적 서비스품질과 고객만족과의 선행관계를 실증 분석하였다. 실증분석 결과 차원별 서비스품질이 전반적 서비스품질에 미치는 영향에 있어서는 SERVQUAL는 영향이 미치지 않았고, SERVPERF은 3요인이 영향을 미치고, 고객만족의 차원별 영향은 SERVQUAL은 1요인만이 영향을 미치고 SERVPERF는 2요인이 영향을 미치는 것으로 나타나 두 모형간의 차원별 중요도는 차이가 있는 것으로 나타났다. 또한 두 요인간의 선행변수는 전반적 서비스품질이 고객만족에 영향을 미치는 것으로 나타났고 행정서비스품질의 측정방법의 적합성은 SERVPERF 모형이 더 적합하였음을 밝혀 보았다. 이상의 본 연구는 행정서비스품질의 5가지 요인 중 전반적서비스품질에는 공감성이 고객만족에는 확신성이 가장 영향력이 높은 변수임을 밝혀내어 행정 분야에 대한 연구를 부여하였으며, 지금까지 논쟁이 되고 있는 SERVQUAL과 SERVPERF 모형의 우월성을 행정 분야에 접목시켜 성과 측정이 더 적합하였음을 시사하고 있다.

This study was carry out to examine the treatment methods of waste water contaminated with mercury by mercury resistant bacteria Pseudomonas spp Isolated from the soils. The results obtained from this study were as follows 1. The mercury contents was respectivelly 2.4ppm, 0.8ppm and 0.3ppm at A, B, and C site from the soils polluted with mercury. 2. 12 strains of bacteria which showed a remarkable tolerance against Thimerosal was isolated from soils contaminted with Mercury. 3. Isolated bacteria from 40ppm of Thimerosal concentration was identified as Pseudomonas spp. by morphological and biochemical test.

This study was carry out to examin removal efficiency of mercury in wast water polluted with mercury by mercury resistant Pseudomonas spp. The results obtained were as follows. 1. The optimal PH, temperature and aeration rate for the Mercury resistant bacteria growth were 7.0, 25℃ and 1ℓ/min. respectively. 2. The relations treatment(culture) time and residal mercury were in invers proportion, maximum cell growth shows at 4days, the other side, minimum residal mercury content was revealed at 4days. 3. Removal efficiency of mercury was about 75-81% in the treatment of 10ppm and 30ppm artificial thimerosal waste water using this bacteria Pseudomonas spp. 4. On the MIC test of isolates to 2 Kinds of antibiotics; streptomycine and lincomycin. MIC of the streptomycin and the lincomycin showed 3.12 to 50mcg / ml, 25 to 50mcg / ml. respectively.

산사태 취약성 평가를 위한 GIS와 AHP법의 적용

The goal of this study is to generate a landslide potential map using GIS(Geographic Information System) based method. A simple and efficient algorithm is proposed to generate a landslide potentialities map from DEM(Digital Elevation Model) and existing maps. The categories of controlling factors for landslides, aspect of slope, soil, vegetation are defined. The weight value for landslide potentialities is calculated from AHP(Analytic Hierarchy Process) method. Slope and Slope-direction is extracted from DEM, and soil information is extracted from digital soil map. Also, vegetation information is extracted from digital vegetation map. Finally, as overlaying, landslide potentialities map is made out, and it is compared with landslide place.

레이저 스페클간섭법에 있어서 스페클크기와 측정 한계에 관한 연구

The high coherence of laser beam has made it possible to observe interference effects even in the light scattered from rough surfaces. That's why, when object with a scattering surface is illuminated with laser light, we do see a speckled appearance due to random interference. This sort of unique property of laser speckle has brought into existence the new noncontacting techniques such as speckle metrology method of measuring deformation, displacement, and vibration etc of objects with high optical sensitivity. The measurable range of speckle metrology especially used to measure in-plane information, however, is limited by some factors, the so-called strain, rotation, tilt of surface and out of displacement perpendicular to the plane of analysis. This restrictions severly limits the measurable range of speckle metrology by causing the decorrelation of speckle patterns. It is the purpose of this paper to give a survey on the measurable limitation of speckle photography method that is one of speckle metrology. Namely we will discuss the mutual relationships and problems of each limitations adding the restriction of the largest and smallest displacement measurable with speckle methods.

Nahhodla and the surrounding Partizansk district were created Russia's first Free Exonomic Zone in 1990. The legal and regulatory foundations of the FEZ's activities were determined by the law On Foreign Investments, a Decree of the Supreme Soviet of the Russian Federation of 24 October 1990, a Decree of the Council of Minsters of the Russian Gederation of 23 November 1990 and by the Presidential Decree of 4 June 1992. These documents regulate the FEZ's activities and sprcify the develementation of experimental tax and customs regimes to stimulate the development of Nakhodka and to allow the FEZ's management body (the Administrative Committee) to receive State loans for improving infrastructure.A policy of local privileges and targeted tax exemption loans by the Administrative Committee has played a positive role in creating a favourable investment climate and in attracting foreign investment. The FEZ now has a competitive environment, increased export/import activities and generally favourable business and economic conditions.The legal, business and infrastructural conditions created in the territory have facilitated the implementation of infrastructural projects, encouraging some US$300 million of foreign investment into manufacturing, transportation and business infrastructure. In addition, the FEZ has started investment projects aimed at upgrading old and constructing new objects of regional and federal significance(reconstruction of Golden Valley airfield, construction of electric power plants, construction of timber and oil ports, construction of grain and mineral fertilizers re-loading complexes, etc.). Nakhodka has also entered a number of international projects, participation in which stimulates investment in concrete programmes (e.g. the Russo-Korean and Russo-American industral parks, participation in Tuman River Delta project and in the Gore- Chernomyrdin committee).

Testicular microlithiasis is a rare disease characterized by calcifications within the seminiferous tubules. Testicular microlithiasis has been found with a wide variety of testicular pathology, but to our knowledge its association with torsion of appendix testis has been reported only one case. Therefore, we report on an 13-year-old boy with torsion of testis appendix and bilaterally diffusely echogenic testes on sonography.

남중 육상경기 선수의 Foot Ratio와 체력과의 상관

Foot ratio and physical fitness were measured and multiple-compared in the level of P<.05 on the subject of 28 prize winners from Men's part of middle school in the field and track events of the "P" youth athletic tournament in 1998 and 7 general students adopted by random sampling. These are conclusions from the result of relative analysis of foot length measured by X-ray, Medial longitudinal arch's height, foot ratio and physical fitness in the level of P<.05. 1. No significance was discovered in FR but throwing group is better than medium and long distance group. 2. Flexibility and endurance have meaning in physical fitness. The short distance group has better flexibility than the medium and long, throwing and general one. And medium and long distance/ throwing group have better endurance than general one. 3. FR and physical fitness show a negative correlation in the muscular strength of throwing group.
