      • 파라캇에 의한 대뇌 피질 손상 : 증례 보고

        Paraquat is widely used as a universal herbicide. Paraquat induces brain damage in case of fatal poisoning. It is well documented that paraquat exposure is associated with Parkinson's disease. However, many reports suggest paraquat is not selective for the dopaminergic nigro-strial system. Preferably, there are reports that cortical neurons were found to be more sensitive to paraquat. A39-year-old woman committed suicide by drinking 20 ml of a 20% solution of Paraquat. After 5 days, she had a convulsive seizure and lost consciousness. Brain MRI showed diffuse cerebral cortical edema of bilateral frontal and temporal lobes. She died 2 days after progressive pulmonary fibrosis. Here, we report a patient with paraquat induced cortical lesion. 파라캇은 세계적으로 널리 쓰이는 제초제이다. 파라캇은 중독되었을 경우 뇌손상을 일으키기도 한다. 파라캇과 파킨슨병과의 관련성은 비교적 잘 알려져 있다. 하지만, 파라캇이 도파민 줄무늬흑체 계통(dopaminergic nifro-striatal system)에 선택적이지 않다는 많은 보고들이 있다. 오히려, 대뇌피질이 파라캇 중독에 더욱 민감하다는 일부 보고들도 있다. 39세 여자환자가 자살할 목적으로 20% 농도의 파라캇 20 ml를 마시고 본원에 내원하였다. 환자는 입원 5일째 갑자기 전신 발작 증상과 함께 의식 저하 소견을 보였다. 다음날 시행한 뇌자기공명영상 촬영상 양쪽 측두엽과 전두엽에 대칭성 세포 도겅부종 소견을 보이고 있었다. 그녀는 2일 후 폐섬유화증에 의한 호흡부전으로 사망하였다. 저자들은 파라캇 중독 환자에서 대뇌 피질 손상을 확인한 1예를 경험하였기에 보고하는 바이다.

      • 암환자에서 다른 신경학적 증상이 동반되지 않는 두통 : 예비 보고

        연구목적 : 암환자에서 신경학적 결손이나 발작 등이 동반되지 않는 두통의 원인과 이런 경우 심각한 두개내 질환에 의한 이차두통을 의심할 수 있는 소견의 진단적 유용성을 알아보고자 하였다. 대상 및 방법 : 2005년 10월부터 2009년 1월까지 신경학적 결손이나 발작을 동반하지 않는 두통이 발생한 71명의 암환자를 대상으로 하였다. 자세한 병력을 청취하고 신경학적 검사를 하였으며, 환자의 상황에 따라 적절한 조합의 뇌MRI, 뇌CT, 및 뇌척수액 검사를 시행하였고, 최소한 3개월 이상 추적 관찰하였다. 이 소견을 종합하여 두통의 국제분류 기준(The International Classification of Headache Disorders, second edition) 에 따라 두통을 진단하였다. 또한 심각한 두개내 질환에 의한 이차두통을 의심할 수 있는 소견인 있는 환자군과 없는 환자군 사이에 중추신경계로의 전이빈도가 차이가 나는지 알아보았다. 결과 : 71명 중에 8.5%인 6명에서 중추신경계로의 전이가 관찰되었는데 심각한 두개내 질환에 의한 이차두통을 의삼할 수 있는 소견이 있었던 18명 중에 4명(22.2%), 이런 소견이 없었던 53명 중에 2명(3.8%)이었다. (상대위험도 5.89, 95% 신뢰구간 1.18-29.49). 두통의 다른 원인으로는 지주막하 출혈이 1명, 중추신경계 이외의 감염에 의한 두통이 6명, 항암제에 의한 두통이 1명, 고혈압성 두통이 1명, 방사선 치료와 관련된 두통이 1명, 원발성 찌름두통이 3명, 긴장성 두통이 51명이었다. 결론 : 심각한 두개내 질환에 의한 이차두통을 의심할 수 있는 소견의 유무가 암환자에서 발생하는 두통의 감별진단에 유용하며, 이러한 소견이 동반되지 않는 경우의 두통은 대부분 양성이다. 하지만 이러한 결론을 바탕으로 향후 보다 많은 대상 환자군을 통한 연구가 필요할 것이다. Purpose: Headache in patients with systemic malignancy is a great concern to neurologists. We performed this study to investigate the frequency of metastasis as the cause of headache in the patients with systemic malignancy and the usefulness of symptoms suggesting underlying serious pathologies especially when headaches do not accompany any other neurological symptoms. Materials and Methods: From October 2005 to January 2009, we prospectively collected 71 patients with systemic malignancy complaining headache but not having any other accompanying neurological symptoms. After thorough history taking and neurological examination, we performed a neuroimaging (MRI or CT) and CSF study according to the clinical situations of individual patients and followed them at least 3 months and diagnosed headaches according to The International Classification of Headache Disorders, second edition (ICHD-II). We compared the incidence of intracranial metastasis between headaches with symptoms suggesting underlying serious illness and those without. Results: Six (8.5%) out of 71 patients had intracranial metastasis: 4(22.2%) out of 18 patients with symptoms suggesting underlying serious illness, 2(3.8%) out of 53 patients without them (relative risk 5.89, 95% confidence interval 1.18-29.49). Other causes included subarachnoid hemorrhage (1 patient), headache attributed to infection (6), headache attributed to drug (2), hypertensive headache (1), headache attributed to radiotherapy (1), primary stabbing headache (3), and tension headache (51). Conclusion: This study suggests that headache symptoms suggesting underlying serious disorder are valuable in differentiating the causes of headache without any other accompanying neurological symptoms in patients with systemic malignancy and headaches without those symptoms might be usually benign. However, more broad study should be made for these value and relationship.

        오존처리에 의한 하수슬러지의 감량화

        The effects of ozone treatment on the dewaterability of activated sludge wasted from a sewage treatment plant were investigated in this study. Following ozone treatment, sludge was dewatered using a pressure fitter. The sludge filterability evaluated by SRF deteriorated with ozone dosage at relatively low ozone doses due to sludge solubilization and particle size decrease. However, at ozone doses higher than 0.6gO_3/gSS, the SRF of the ozone treated sludge became comparable with the value for the sludge without ozone treatment. It is suggested that the oxidation of the viscous, high molecule organics attached at cell surface reduces the surface charge of sludge particle and promotes flocculation at relatively high ozone doses. Ozone treatment also cause the release of bound water trapped inside cells or flocs, resulting in significant reduction of cake water content after dewatering. In terms of cake volume, more than 70% reduction was obtained at 0.6gO_3/gSS. These observations suggest that ozone treatment has potential to significantly enhance sludge dewaterability and to produce a more compact cake without severe deterioration of its filterability.

        방사선조사에 따른 구개점막상피의 형태학적 변화에 관한 실험적 연구

        The purpose of this study was to investigate the irradiation effects on the palatal mucosa. For this study, Sprague-Dawley strain rats were irradiated to their head and neck region with the dose of 5Gy and 10Gy by 6MV X-radiation and sacrificed on the experimental periods after irradiation. The authors observed the histological changes of the hard and soft palatal mucosae. The results were as follows: In the light microscopic examination, hydropic change on the basal cells, increased cell size of the epithelium, and decreased epithelial cell layers were observed on the 3hours, 6hours, and 12hours groups after irradiation. But, basal cell hyperplasia, increased epithelial cell layers, and elongated rete pegs were observed on the 3days group after irradiation. After then, these changes were recovered in the mucosa of the hard palate on the 7days and 14days groups, and in the mucosa of the soft palate on the 14days and 28days groups after irradiation. And such changes were greater in the mucosa of the soft palate than in that of the hard palate, and more prominent in 10Gy irradiated groups than in 5Gy irradiated groups.

      • Affinity Labeling of E. coli GTP Cyclohydrolase I by a Dialdehyde Derivative of Guanosine Triphosphate

        Time-dependent inactivation of E. coli GTP cyclohydrolase I with a 2',3'-dialdehyde derivative of GTP (oGTP) was directed to the active site of the enzyme, and was dependent on the concentration of oGTP. The kinetics of inactivation were biphasic with a rapid reaction occurring immediately upon exposure of the enzyme to oGTP followed by a slow rate of inactivation. The $K_i$ value of oGTP for the enzyme was 0.25 mM. Inactivation was prevented by preincubation of the enzyme with GTP, the substrate of the enzyme. At 100% inactivation, 2.3 mol of [8.5'-$^3H$</EX>]oGTP were bound per each enzyme subunit, which consists of two identical polypeptides. The active site residue which reacted with the affinity label was lysine. oGTP interacted selectively with the ${\varepsilon}$-amino group of lysine in the GTP-binding site to form a morpholine-like structure which was stable without sodium borohydride treatment. However, triphosphate group was eliminated during the hydrolysis step. To identify the active site of the enzyme, [8.5'-$^3H$]oGTP-labeled enzyme was cleaved by endoproteinase Lys-C, and the $^3H$-labeled peptide was purified by HPLC. The amino acid sequence of the active site peptide was Pro-Ser-Leu-Ser-Lys, which corresponds to the aminoterminal sequence of the enzyme.

        Intensity-modulated radiotherapy for stage I glottic cancer: a short-term outcomes compared with three-dimensional conformal radiotherapy

        Purpose: To investigate the differences in treatment outcomes between two radiation techniques, intensity-modulated radiotherapy (IMRT) and three-dimensional conformal radiotherapy (3DCRT). Materials and Methods: We retrospectively analyzed 160 (IMRT = 23, 3DCRT = 137) patients with stage I glottic cancer treated from January 2005 through December 2016. The IMRT was performed with TomoTherapy (16 patients), volumetric-modulated arc therapy (6 patients), and step-and-shoot technique (1 patient), respectively. The 3DCRT was performed with bilateral parallel opposing fields. The median follow-up duration was 30 months (range, 31 to 42 months) in the IMRT group and 65 months (range, 20 to 143 months) in the 3DCRT group. Results: The 5-year overall survival and 3-year local control rates of the 160 patients were 95.7% and 91.4%, respectively. There was no significant difference in 3-year local control rates between the IMRT and 3DCRT groups (94.4% vs. 91.0%; p = 0.587). Thirteen of 137 patients in the 3DCRT group had recurrences. In the IMRT group, one patient had a recurrence at the true vocal cord. Patients treated with IMRT had less grade 2 skin reaction than the 3DCRT group, but this had no statistical significance (4.3% vs. 21.2%; p = 0.080). Conclusion: IMRT had comparable outcomes with 3DCRT, and a trend of less acute skin reaction in stage I glottic cancer patients

        PIVKA-II as a surrogate marker for prognosis in patients with localized hepatocellular carcinoma receiving stereotactic body radiotherapy

        Purpose: This study aimed to determine the correlation between protein induced by vitamin K absence or antagonist-II (PIVKA-II) and stereotactic body radiotherapy (SBRT) in patients with hepatocellular carcinoma (HCC). Materials and Methods: Sixty-one patients received SBRT between 2015 and 2020 with a median dose of 48 Gy (range, 39 to 60 Gy) with a median of 4 fractions. Changes in tumor markers before and after SBRT were analyzed. Results: The median follow-up period was 31 months (range, 12 to 64 months). The estimated 2-year in-field failure-free survival, progression-free survival (PFS), and overall survival rates were 82.0%, 39.3%, and 96.7%, respectively. Patients with decreased PIVKA-II levels through SBRT had significantly few in-field failures (p = 0.005). Patients with PIVKA-II levels of ≤25 mAU/mL after SBRT had significantly long PFS (p = 0.004). Conclusion: PIVKA-II could be a useful surrogate marker for response or survival outcomes in patients with localized HCC receiving SBRT.
