A glomus tumor is an uncommon soft tissue tumor that is most commonly found in the subungual area and a glomus originating in the trachea is extremely rare. Histologically and ultrastructurally, these tumors have been divided into three subtypes: classic glomus tumors, glomangiomas, and glomangiomyomas. Glomangiomyomas account for less than 10% of all glomus tumors and are the least common type. We report a case of a 54-year-old man with glomangiomyoma of the trachea who presented with stridor. We treated the tumor by segmental resection and primary repair via a transcervical approach.

This program was applied to 50 mentally disabled persons (final objects of study are 40) for 12 months through medical rehabilitation (basic health examination, parental counseling on health management), sports activities (physical strength training, swimming, bowling, and billiards), and leisure activities (weekend leisure program, excursion, sports viewing, and winter camp) to investigate physical construct, basic body strength, body fat, behavioral adapt and participation in programs to lay an emphasis on the importance of sports leisure programs for mentally disabled persons and to develop sports areas effective to fatness prevention and basic body strength. The program, on the purpose of offering basic materials, leaded to the results as followings. 1. physical construct, body fat rate, body bone quantity, and physical development index had a meaningful difference at the level of .001, and weight showed a meaningful change at the level of .01, which showed a meaningful change in all of the body construct areas. In addition, 91.3% of the total 40 persons showed an affirmative change. 2. Basic body strength showed a meaningful change at the level of .001 in all of the areas of muscle power, muscle endurance, explosive muscular strength, and flexibility. 96.4% of 40 persons showed an affirmative change. 3. Blood lipid showed a meaningful change at the level of .001 in all of the areas such as T-C, HDL-C, LDL-C. In addition, 95.8% of 40 persons showed an affirmative change. 4. Adaptation activities increased, such as swimming 13.6 point, bowling 14.1 point, billiards 9.3 point respectively, which showed that bowling was the event easiest for disabled persons to adapt in behaviors, and swimming and billiards in order. 5. As for an investigation of satisfaction, in the areas of medical rehabilitation one person responded, “No”, on the question about basic health examination and parental counseling. In the area of sports five persons responded, “No”, on the question about the frequency of physical training and sports-for-all. They wanted physical training and sports-for-all to be more frequent. In the area of leisure activities, respective one person responded, “No”, in the area of leisure activities, stress erase, and week-end leisure programs. They asked for an expansion of kind in leisure activities.

<P><B>Abstract</B></P><P>We fabricate single-ended resistive W-band millimeter-wave monolithic IC (MIMIC) mixers based on 0.1μm InGaAs/InAlAs/GaAs metamorphic HEMT technology. The mixers show good characteristics in linearity and LO-RF isolation, and the mixers with IF amplifiers exhibit a conversion loss of ∼0.4dB, which is an improvement of ∼7.8dB compared to that of the mixers without IF amplifiers. We obtain P-1dB of 10 and 9dBm from the mixers with and without the IF amplifiers, respectively.</P>

골연골종은 흔한 질환이나 조갑하에 생기는 경우는 드물고 특히 수지의 조갑하에 다발성으로 발생한 경우는 문헌상의 보고가 전무한 실정이다. 저자들은 수지의 원위 지골에 발생하여 조갑 변형과 통증을 일으킨 다발성 조갑하 골연골종을 경험하여 이를 보고하고자 한다. The osteochondroma is a common benign bone tumor, which is only rarely observed in a subungal location, especially in the fingers. We present a case of multiple subungal osteochondromas, which occurred in the distal phalanges causing nail deformities and pain.

Effects of Porphyromonas endodontalis lipopolysaccharide on IL-1β, TNF-α and IL-1ra production by human polymorphonuclear leukocytes

목 적 Inflammatory cytokine으로 알려진 interleukin 1β, tumor necrosis factor α는 치수 및 치근단질환에서 주요한 역할을 하며, 골흡수를 자극하고 골형성을 방해하는 것으로 알려져 왔다. 이들 cytokine은 주로 단핵세포/대식세포가 형성하는 것으로 알려져 왔으나 최근 연구에 의하면, PMN도 또한 이런 cytokine들을 형성할 수 있다는 것이 보고되었다. 오랫동안 염증반응이나 면역반응에서 PMN의 역할이 주로 포식작용 을 통해 병원균을 제거하는 것이라고만 생각되어져 왔던 것을 생각하면, 새로운 발견이라 할 수 있다. 또, PMN은 IL-1ra도 생성하는 것으로 보고되었는데, IL-1ra란 IL-1의 생물학적 작용을 방해하는 인자이므로, IL-1과 밀접한 관련을 가지는 질환의 발전에 있어서 IL-1과 IL-1ra의 balance가 매우 중요한 역할을 할 것으로 생각된다. 즉, IL-1ra는 IL-1β의 proinflammatory effect를 제한할 수 있는 negative feedback mechanism이라고 할 수 있다. 이 연구의 목적은 치수 및 치근단 조직의 감염에 있어서 주요 원인균인 Porphyromonas endodontalis 의 LPS가 PMN의 IL-1β, TNF-α, IL-1ra생성에 미치는 영향을 단백질과 mRNA수준에서 관찰하는 것이다. 잘 알려진 non-oral bacterium인 E.coli의 LPS를 positive control로 사용하였으며, IL-1ra가 IL-1β의 생물학적 작용을 방해하는 작용을 관찰하기 위해, IL-1의 biological assay도 시행하였다. 방 법 P. endodontalis ATCC 35406을 혐기성 조건에서 배양하고, hot phenol-water extraction의 방법으로 LPS를 추출(crude LPS)한 후, 제조회사로부터 구입한 E.coli의 crude LPS와 함께 정제하였다. 건강한 자원 자들을 대상으로 말초혈액을 채취한 후 dextran sedimentaion을 거쳐, Lymphoprep을 이용하여 PMN층을 분리하였다. 얻어진 세포들은 RPMI 1640 (supplemented with fetal bovine serum, antibiotics)에 5×106cells/ml이 되도록 resuspend시킨 후 각기 다른 농도 (0, 0.01, 0.1, 1 and 10㎍/m1)의 LPS를 처리하여, 각기 다른 시간 (Northern blot : 1, 2, 4시간, ELISA : 2. 6, 12, 18시간)동안 37℃ in 5% CO2의 조건으로 배양하였다. 상층액은 -70℃에 보관하였다가 추후에 ELISA를 이용한 단백질 농도 측정과 IL-1 biological assay에 사용되어졌으며 , 배양된 세포로부터 RNA를 추출하여 Northern hybridization을 통해 mRNA expression을 관찰하였다. 결 과 1. 세 가지의 cytokine모두 각각의 LPS로 처리된 PMN에서 LPS로 처리되지 않은 PMN에서보다 통계적으로 유의성 있게 높은 level의 protein농도와 mRNA발현을 나타냈다(p<0.05). 이는 LPS로 자극된 PMN이 염증성 cytokine을 분비할 수 있다는 것을 나타낸다. 2. E.coli LPS는 P. endodontalis LPS에 비해 통계적 유의성 있게 많은 양의 protein synthesis와 mRNA 발현을 나타냈다. 3. 세 가지 cytotine모두에서 mRNA는 두 가지 LPS모두 1시간 만에 peak를 나타냈고, IL-1β만이 4시간 만에 두 번째 peak를 나타냈다. 4. PMN으로부터 cytotine을 분비하게 하는 데는 0.01㎍/ml의 LPS 정도면 충분하였으며, 세 가지 cytotine 모두에서 분비된 cytokine 양은 dose-dependent manner를 나타냈다. 5. 모든 sample의 IL-1ra: IL-1β ratio가 anti-inflammatory range에 속하지 못했다. 결과적으로, LPS로 자극된 PMN supernatant내에 상당한 수준의 biological IL-1 activity가 발견되었다. 6. IL-1ra:IL-1β ratio는 배양시간이 증가함에 따라 감소하였다. 이는 시간이 지남에 따라 IL-1β의 level은 현저히 증가하지만, IL-1ra의 level은 큰 변화가 없는 것에 따른 결과이며, 결과적으로 시간이 증가함에 따라 IL-1ra의 IL-1β에 대한 inhibitory effect가 점점 감소한다는 것을 의미한다. 주요어 : Porphyromonas endodontalis, Lipopolysaccharide, interleukin 1β, tumor necrosis factor α, interleukin 1 receptor antagonist

안검하수증 치료에 관한 임상적 고찰

The blepharoptosis is a disease which is the drooping of the eyelid and is caused by the weakness of the levator or the Muller's muscle. The author had treated 60 patients(71 eyes) from January of 1985 to January of 1991 and the follow-up study was carried out in 44 of them(59 eyes) The choice of operative procedures was determined by the levator function : When the amout of the levator function was 2∼9mm(13 patients, 14 eyes), the patient was treated by the levator resection, and when the amount of the levator function was below 2mm, the patient was treated by the frontalis suspension(23 patients, 32 eyes)or frontalis transfer(8 patients, 13 eyes) The followings were the results. 1. There were 43 congenital and one acquired blepharoptosis. 2. The amout of the levator function was below 2mm in 45 cases, and 2∼9mm in 14 cases. 3. In the postoperative complications, the lagophthalmos in sleeping and the ptosis in upward gazing were observed in all cases treated by frontalis suspension or levator resection, and in 1 case treated by frontalis transfer. The other complications were overcorrection, undercorrection, lid lag in downward gazing, eyelid deformity, notching and sensory disturbance of forehead. 4.The above results have shown that in the case of a high degree of levator weakness, the frontalis transfer was the more effective method of treatment than the frontalis suspension in the correction of the blepharoptosis.
