Ischiorectal Block with Bupivacaine for Post Hemorrhoidectomy Pain

Background: Hemorrhoid is one of the most common surgical diseases occurring in the anorectal region. In this study, we evaluated the effect of ischiorectal fossa block on alleviating post hemorrhoidectomy pain. Methods: In this study, 90 patients suffering from hemorrhoids were evaluated. They were randomly divided into 3 groups. The first group had no block, the second group an ischiorectal block with placebo (normal saline), and the third group a preemptive ischiorectal block with bupivacaine. Postoperative variables such as pain intensity, pethidine consumption, nausea, and vomiting were compared between the groups. Results: The postoperative pain score in group 1 was $8.5{\pm}1.3$ and $8.1{\pm}0.9$ (P=NS) in group 2. The post operative analgesic demand was $3.1{\pm}1.5$ and $3.3{\pm}1.8$ hours in groups 1 and 2, respectively (P=NS). The post operative pain score and analgesic demand were $4.2{\pm}2.1$ and $9.3{\pm}2.7$ hours, respectively, in group 3 (P < 0.0001). Conclusions: Preemptive ischiorectal block reduces the posthemorrhoidectomy pain and opioid demand.
A Cost/Worth Approach to Evaluate UPFC Impact on ATC

Available transfer capability (ATC) is a measure of the transfer capability remaining in a transmission system. Application of unified power flow controllers (UPFCs) could have positive impacts on the ATC of some paths while it might have a negative impact on the ATC of other paths. This paper presents an approach to evaluate the impacts of UPFCs on the ATC from a cost/worth point of view. The UPFC application worth is considered as the maximum cost saving in enhancing the ATC of the paths due to the UPFC implementation. The cost saving is considered as the cost of optimal application of other system reinforcement alternatives (except for UPFC) to reach the same ATC level obtained by UPFC application. UPFC application costs include the maximum cost of alleviating the probable negative impact on the ATC of some paths caused by implementing UPFCs. Optimal system reinforcement is used for systems with UPFCs to determine the aforementioned cost. The proposed method is applied to the IEEERTS and the results are evaluated through a sensitivity analysis. The cost/worth of UPFC application is also used to develop an index for optimal UPFC location and the results are compared with those of other indices. A comparison is finally made with the results obtained using an existing ATC allocation profit-based approach to determine UPFC application worth.

In this study, the volumetric heat source in electron beam welding (EBW) is modeled through finite element method taking advantage of ABAQUS software package. Since this welding method is being applied in plates with different thicknesses and also considering that residual stresses reduce the strength of these weldments, the effect of thickness in the distribution and magnitude of residual stresses after welding is studied. Regarding the vast application of Inconel 706 super-alloy in aerospace industries, this material was selected in the current research. In order to validate the finite element model, the obtained results were compared to those of other researchers in this area, and good agreement was observed. The simulation results revealed that increase in the plate thickness leads to increase in the residual stresses. In addition heat treatment in the base metal (before welding) increases the residual stresses significantly.

MWCNTs functionalized with the carboxylate group are used as efficient adsorbent for the rapid removaland fast adsorption of malachite green from the aqueous solutions. The optimized contact time and pHwere 10 min and 9, respectively. The effect of temperature revealed that the adsorption capacity ofmalachite green (MG) dye increased with increasing contact time, temperature and pH of the workingsolution. The adsorption equilibrium and kinetic data was well fitted and found to be in good agreementwith the Langmuir isotherm model and pseudo second order kinetic model respectively with highcorrelation coefficient.
Perceived Stress, Alexithymia, and Psychological Health as Predictors of Sedative Abuse

Background: The harmful effects of sedative medications and substances in conjunction with limited research regarding predictive psychological constructs of drug abuse necessitate further investigation of associated factors. Therefore, the present study aimed to elucidate the roles of perceived stress, alexithymia, and psychological health as predictors of sedative abuse in medical students. Methods: In this cross-sectional study, 548 students at Kermanshah University of Medical Sciences, Iran, were selected using stratified random sampling. The data were obtained using the Perceived Stress Scale, an alexithymia scale (Farsi version of the Toronto Alexithymia Scale-20), and a General Health Questionnaire to assess psychological health. Data were analyzed using discriminant analyses. Results: The results demonstrated that the user and non-user of sedative substances groups had significantly different predictive variables (except for social function disorder) (P>0.05). Physical complaints, alexithymia, and perceived stress, which had standard coefficients of 0.80, 0.60, and -0.27, respectively, predicted sedative drug use. Conclusion: The results of the present study indicate that perceived stress, alexithymia, physical complaints, anxiety, and depression are associated with sedative drug abuse.

In this research, bending analysis of a micro sandwich skew plate with isotropic core and piezoelectric composite face sheets reinforced by carbon nanotube on the elastic foundations are studied. The classical plate theory (CPT) are used to model micro sandwich skew plate and to apply size dependent effects based on modified strain gradient theory. Eshelby-Mori-Tanaka approach is considered for the effective mechanical properties of the nanocomposite face sheets. The governing equations of equilibrium are derived using minimum principle of total potential energy and then solved by extended Kantorovich method (EKM). The effects of width to thickness ratio and length to width of the sandwich plate, core-to-face sheet thickness ratio, the material length scale parameters, volume fraction of CNT, the angle of skew plate, different boundary conditions and types of cores on the deflection of micro sandwich skew plate are investigated. One of the most important results is the reduction of the deflection by increasing the angle of the micro sandwich skew plate and decreasing the deflection by decreasing the thickness of the structural core. The results of this research can be used in modern construction in the form of reinforced slabs or stiffened plates and also used in construction of bridges, the wing of airplane.
