New PCBN for Heavy Interrupted Cutting of Hardened Steel

PCBN tools are used worldwide for machining of hardened steel parts in automotive industries. But in heavy interrupted cutting of hardened steel, the tool life is not so stable by sudden breakage of the cutting edge, and total cost of cutting by PCBN is not so economical compared to the grinding. To solve this problem, new PCBN has been developed. New PCBN has very fine and homogeneous microstructure to increase the toughness of sintered body that it provides a reliable tool life for heavy interrupted cutting.

Objective: The guidelines on indications for prophylactic use of Bacille Calmette-Gu$\acute{e}$rin (BCG) against non-muscle-invasive bladder cancer (NMIBC) have changed over the years. In order to assess the impact on outcome, the present retrospective comparison of BCG efficacy by time period with Japanese patients was conducted. Patients and Methods: A total of 146 cases of NMIBC treated with BCG since February 1985 were retrospectively evaluated. All patients received 80 mg of BCG (Tokyo 172 strain) six to eight times a week for prophylactic use. Comparison was made among three historical groups (Group A: 1980's, 39 cases; Group B: 1990's, 61 cases; Group C: 2000's, 46 cases). Results: In total, recurrence was seen in 55 of the 146 cases (37.7%), and progression in 14 (9.6%), 1 patient dying of cancer. These overall results were similar to those outlined in previous reports. However, the outcomes of this time-period-based analysis indicated a tendency for a shorter time to recurrence in patients after 2000, although a log-rank test showed no significance (P=0.229). Seven of the cases featuring progression (i.e., half of all such cases) were among the 46 Group C patients (15.2%). Excluding these progressive cases, there was no significant difference among the remaining 132 patients in the three groups. Conclusion: This study results revealed a tendency for a lower non-recurrence rate after 2000 in our series. This could stem from a number of factors, including changes in BCG indication criteria and the evolution of histopathological diagnostic criteria.

An outline is given of our development of mosaic CCD cameras. Hardware and data reduction software of two operational cameras are described. Scientific objectives of wide-field imaging with the cameras are briefly described.
Accuracy of Endoscopic Diagnosis for Mild Atrophic Gastritis Infected with Helicobacter pylori

Background/Aims: This study examined the accuracy of endoscopic evaluation for determining the Helicobacter pylori infection statusin patients with mild atrophy who might not exhibit characteristic endoscopic findings. Methods: Forty endoscopists determined the H. pylori infection status of 50 randomly presented H. pylori-positive and H. pylorinegativecases on the basis of a list of established findings. Results: The median clinical endoscopy experience was 7 years (range, 1–35 years), including 22 board-certified endoscopists (55%) of theJapan Gastroenterological Endoscopy Society. The mean accuracy rate of endoscopic diagnosis was 67% and was unrelated to experiencestatus (experienced vs. trainee: 69% vs. 65%, p=0.089) and total years of experience (R2=0.022). The most frequently selected endoscopicfindings were regular arrangement of collecting venules (59%), atrophy (45%), and red streak (22%), which had fair accuracy rates of67%, 65%, and 73%, respectively. By contrast, the accuracy rates of nodularity (89%) and mucosal swelling (77%) were highest. The 20endoscopists who more frequently identified these findings diagnosed H. pylori infection significantly more accurately than did the otherendoscopists (71% vs. 64%, p=0.008). Conclusions: Careful attention to nodularity and mucosal swelling in patients with mild atrophy may enhance diagnosis, enable prompttreatment, and avoid possible long-term carcinogenesis.
Effect of Protein Nutrition Timing on Muscle Protein Synthesis

N/A Maintaining the appropriate amount of skeletal muscle is important for high-level physical performance, and for both the treatment and prevention of some health issues. These health issues include lifestyle-related diseases as well as sarcopenia and osteopenis. or aging-related loss of skeletal muscle mass and bone mass, respectively. Since protein is one of the major components of skeletal muscle, it is generally acknowledged that the dietary requirement of protein is greater in individuals who engage in regular exercise than in sedentary individuals. Protein intake at 1.2-1.4 g/kg BW/day for endurance exorcist and 1.7-1.8 g/kg BW/day for strength exercise is recommended in Japan, and these values are 20-80% higher than the current recommended dietary allowances in this country. Carbohydrates have been shown to influence muscle protein metabolism; consuming carbohydrates together with protein would facilitate skeletal muscle protein synthesis due to insulin stimulated by ingested carbohydrates. In addition, recent studies suggest the ingestion timing of these nutrients could influence muscle protein metabolism. Some studies conducted with both animals and humans show the possibility that ingesting protein and carbohydrate shortly after completing exercise is more effective in promoting skeletal muscle protein synthesis than ingesting the nutrients several hours after exercise. Increased insulin sensitivity in skeletal muscle after exercise is assumed to be involved in this mechanism. Thus. it appears that ingestion timing as well as what to eat and how much to eat should be considered to establish a nutrition exercise regimen in order to obtain a appropriate skeletal muscle mass.
