      • Tumour Markers in Peritoneal Washing Fluid - Contribution to Cytology

        Background: Peritoneal washing cytology (PWC) that shows the microscopic intra-peritoneal spread of gynaecologic cancers is not used in staging but is known as prognostic factor and effective in planning the intensity of the therapy. False negative or false positive results clearly affect the ability to make the best decision for therapy. In this study we assessed levels of tumour markers, carcinoembryonic antigen (CEA), cancer antigen 125 (CA-125) and carbohydrate antigen (CA19-9), in peritoneal washing fluid to establish any possible contribution to the peritoneal washing cytology in patients operated for gynaecologic cancer. Materials and Methods: Preoperative tumour markers were studied in serum of blood samples obtained from the patients for preoperative evaluation of a gynaecologic operation. In the same group peritoneal tumour markers were studied in the washing fluid obtained for intraoperative cytological evaluation. Results: This study included a total of 94 patients, 62 with malignant and 32 with benign histopathology. The sensitivity of the cytological examination was found to be 21% with a specificity of 100%. When evaluated with CEA the sensitivity of the cytological examination has increased to 37%. Conclusions: In addition to examination of PWC, the level of CEA, a tumour marker, in peritoneal washing fluid can make a diagnostic contribution. Determining the level of CEA in peritoneal washing fluid will be useful in the management of gynaecologic cancers.

        Intravesical Sodium Chondroitin Sulphate to Treat Overactive Bladder: Preliminary Result

        Purpose: This study aimed to verify the efficacy and safety of intravesical treatment with sodium chondroitin sulfate (CS) in patients with overactive bladder (OAB) who are refractory to previous antimuscarinic treatment. Methods: This study was performed between June 2012 and January 2015 and included 31 consecutive women (mean age, 42.10±7.34 years) with OAB who had been previously treated with two types of antimuscarinic drugs. The results of gynecologic and cystoscopic examinations were normal, and OAB comorbidity was absent. Treatment with intravesical instillations containing 40 mL CS (0.2%; 2 mg/mL) was administered for 6 weeks; after weekly treatments, monthly treatments were administered. The OAB-validated 8 (OAB-V8) symptom scores, nocturia, frequency, urgency, urge incontinence, and urinary volumes measured by uroflowmetry were evaluated for all the patients. The values obtained before the treatment were statistically compared with those obtained six months after the treatment. Results: The duration of the symptoms was 18.36±6.19 months. A statistically significant improvement of the patients’ conditions was observed in terms of the OAB-V8 symptom scores, nocturia, frequency, urgency, urge incontinence, and urinary volumes measured by uroflowmetry after the treatment. Conclusions: Despite the limitations of this study, the outcomes confirmed that CS therapy is safe and effective for the treatment of OAB.

        Using FEM and artificial networks to predict on elastic buckling load of perforated rectangular plates under linearly varying in-plane normal load

        Elastic buckling load of perforated steel plates is typically predicted using the finite element or conjugate load/displacement methods. In this paper an artificial neural network (ANN)-based formula is presented for the prediction of the elastic buckling load of rectangular plates having a circular cutout. By using this formula, the elastic buckling load of perforated plates can be calculated easily without setting up an ANN platform. In this study, the center of a circular cutout was chosen at different locations along the longitudinal x-axis of plates subjected to linearly varying loading. The results of the finite element method (FEM) produced by the commercial software package ANSYS are used to train and test the network. The accuracy of the proposed formula based on the trained ANN model is evaluated by comparing with the results of different researchers. The results show that the presented ANN-based formula is practical in predicting the elastic buckling load of perforated plates without the need of an ANN platform.

      • Effect of aggregate mineralogical properties on high strength concrete modulus of elasticity

        Aggregates mineralogical, and petrographic properties directly affect the mechanical properties of the produced high strength. This study is focused on the effects of magmatic, sedimentary, and metamorphic aggregates on the performance of high strength concrete. In this study, the effect of the mineralogical properties of aggregates on the compressive strength and modulus of elasticity of high-strength concrete was estimated by Artifical Neural Network (ANN). To estimate the compressive strength and elasticity modules, 96 test specimens were produced. After 28 days under suitable conditions, tests were carried out to determine the compressive strength and modulus of elasticity of the test specimens. This study also focused on the application of artificial neural networks (ANN) to predict the 28-day compressive strength and the modulus of elasticity of high-strength concrete. An ANN model is developed, trained, and tested by using the available test data obtained from the experimental studies. The ANN model is found to predict the modulus of elasticity, and 28 days compressive strength of high strength concrete well, within the ranges of the input parameters. These comparisons show that ANNs have a strong potential to predict the compressive strength and modulus of elasticity of high-strength concrete over the range of input parameters considered.

      • Maturation effect on strength of high-strength concretes which produced with different origin aggregates

        This paper presents an application of the maturation effect on the strength of high-strength concrete which is produced with different origin aggregates. While investigating the maturation effect on HSC 384 specimens were prepared with 22 different origin aggregates. These prepared specimens were subjected to the standard compressive tests which were applied after curing for 2, 7, 28, and 56 days under appropriate conditions. The test results revealed that bright surface-low adherence behavior is valid in normal strength concretes, but is not as effective as expected in high-strength concretes. The application of artificial neural networks (ANNs) to predict 2, 7, 28, and 56 day compressive strength of HSC is also investigated in this paper. An ANN model is built, trained, and tested using the available test data gathered from experimental studies. The ANN model is found to predict 2, 7, 28, and 56 days of compressive strength of high-strength concrete well within the ranges of the input parameters considered. These comparisons show that ANNs have strong potential as a feasible tool for predicting the compressive strength of high-strength concrete within the range of the input parameters considered.

        Follicular fluid cerebellin and betatrophin regulate the metabolic functions of growing follicles in polycystic ovary syndrome

        Objective: The aim of this study was to assess the changes of follicular fluid (FF) and serum levels of cerebellin precursor protein 1 (cbln1) and betatrophin in patients with polycystic ovary syndrome (PCOS) undergoing in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) with a gonadotropin-releasing hormone (GnRH) antagonist protocol. Methods: Twenty infertile women with PCOS and 20 control women diagnosed as poor responders undergoing ovarian stimulation with a GnRH antagonist were included. Blood samples were obtained during ovum pick-up. Follicular fluid from a dominant follicle was collected from the subjects. Using enzyme-linked immunosorbent assays, FF and serum levels of cbln1 and betatrophin were measured in both groups of participants. Metabolic and hormonal parameters were also determined and correlated with each other. Results: Both groups of women had similar serum and FF betatrophin levels ($55.0{\pm}8.9ng/mL$ vs. $53.1{\pm}10.3ng/mL$, p=0.11). The serum and FF betatrophin levels of poor responders were found to be similar ($49.9{\pm}5.9ng/mL$ vs. $48.9{\pm}10.7ng/mL$, p=0.22). Conversely, the FF cbln1 levels of PCOS women were found to be significantly higher than the serum cbln1 levels ($589.1{\pm}147.6ng/L$ vs. $531.7{\pm}74.3ng/L$, p<0.02). The FF cbln1 levels of control participants without PCOS were significantly higher than their serum cbln1 levels ($599.3{\pm}211.5ng/L$ vs. $525.3{\pm}87.0ng/L$, p=0.01). Positive correlations were detected among body mass index, insulin resistance, serum insulin, total testosterone, and betatrophin levels in the PCOS group. Conclusion: Follicular fluid betatrophin and cbln1 concentrations may play a pivotal role on follicular growth in PCOS subjects undergoing IVF/ICSI with an antagonist protocol.

        Pleomorphic Carcinoma of the Lung with High Serum Betahuman Chorionic Gonadotropin Level and Gynecomastia

        Although gynecomastia is a well-defined paraneoplastic syndrome in patients with nonsmall cell lung cancer, the association with pleomorphic carcinoma has not been reported. A 50-yr-old man presented with bilateral gynecomastia and elevated serum beta-human chorionic gonadotropin (βhCG) level. Chest tomography showed a mass in the right middle lobe. Right middle lobectomy and mediastinal lymph node dissection were performed. βhCG levels decreased rapidly after surgery. Histological examination revealed pleomorphic carcinoma with positive immunostaining for βhCG. Serum βhCG levels began to increase gradually on postoperatively 4th month. Computed tomography detected recurrence and chemotherapy was started. After second cycle of chemotherapy, βhCG levels decreased dramatically again and tomography showed regression in mass. Patient died 6 months later due to brain metastasis. βhCG expression may be associated with aggressive clinical course and increased risk of recurrence, also βhCG levels may be used to evaluate therapy response in patients with pleomorphic carcinoma.
