        褥瘡에 대한 治驗 1 例 報告 : an External Therapy with ASTRAGALI RADIX Powder

        Pressure sore is the ulceration and necrosis of the skin and underlying tissue usually occurred on the bony prominence of the body after prolonged or repeated pressure. Recently, the incidence of the pressure sore has been increased due to the development of the industry, increased number of the traffic accident and the cerebrovascular accident. Authors reviewed 4 sites of a patient who had received external application with ASTRAGALI RADIX powder for the pressure sore. Pressure sore of the patient was improved in about 4 months. ASTRAGALI RADIX has two effects: promoting tissue regeneration(生?) and evacuating the pus by administering tonics(????). So ASTRAGALI RADIX is proper for the external application of the pressure sore.

        자발성 뇌지주막하출혈 환자에서 Cardiac Troponin Ⅰ를 이용한 심근손상의 발생율

        Background : More than 90% of acute stroke patients have measurable cardiovascular sequelae, but we have been often overlooked in formal discussions of treatment. If we estimate the incidence of myocardial injury in patients with spontaneous SAH, we may figure the possibility of cardiac dysfunction in such patients. This study was designed to investigate the incidence of myocardial injury in patients with spontaneous SAH using cardiac troponin I(cTnI). Methods : A prospective single emergency center study was performed to determined preoperative incidence of unrecognized cardiac injury in patients suffering spontaneous SAH. We include the spontaneous SAH patients who underwent serum measurements of the cardic troponin I immediately upon admission last six month period. ECG, CK, CK-MB and myoglobin were also performed at admission. We excluded the spontaneous SAH patients who had past history of myocardial ischemia and ECG abnormality. Results : Fifty-two patients(34 females, 18 males) with spontaneous SAH were studied prospectively. 18 patients(34.6% of the total study population) had cTnI level above 0.5ng/ml. ECG was performed in 52 patients and was abnormal in 15 of the 52 patients(28.8%). Conclusion : The measurement of cTnI has provided physicians with a myocardial marker that has a cardiac sensitivity for cardiac injury equal to that of CK-MB yet with greater specificity. So, cardiac troponin I is useful to estimate the incidence of myocardial injury in patients with spontaneous SAH. And we may estimate the possibility of cardiac dysfunction in such patients. This knowledge will hopefully aid in the care and improve the outcome.

        농업인 평생학습체계 구축을 위한 농업인교육 발전방안 연구

        이 연구의 목적은 우리나라 농업인의 평생학습체계 구축을 위한 농촌진흥기관의 역할을 정립하고, 한국농업대학의 농업인대학 운영 본부화 방안을 수립하는데 있다. 이를 위해 관련 문헌고찰, 농업인 및 농촌진흥기관의 농업인 교육 담당자들의 인식 및 요구분석, 전문가 자문 및 협의회 등의 방법을 활용하였으며, 이를 토대로 도출된 연구의 결과를 제시하면 다음과 같다. 첫째, 농촌진흥기관의 농업인교육관련 연계를 위해서는 한국농업대학이 농업인평생교육기관으로 평가인정을 받고, 전국의 농업인대학을 총체적으로 지원하는 역할을 수행하는 것이 바람직하다. 이때 지역의 농업기술센터는 한국농업대학과 협력적 관계를 구축하여 학점은행제 운영에 소요되는 교육 및 이력관리를 지원하는 역할을 수행하는 것이 필요하다. 둘째, 한국농업대학의 농업인대학 운영본부화를 위하여 평생학습원의 조직이 추가되는 것이 바람직하다. 이에 따라 평생학습원의 비전, 미션 및 전략이 제시되었으며, 조직의 기능 및 세부과제가 제시되었다. 셋째, 농업인교육의 학점은행제를 도입을 위한 체계를 설정한 후, 농업관련 기존 표준교육과정 분석, 농업인 요구분석, 전국 농업인대학 교육과정 분석, 기존 농업관련 전공 교육과정 분석, 전문가 검토 등의 과정을 거쳐서 '영농 전문학사'와 '농업 경영 학사'의 표준교육과정을 개발하였다. 넷째, 고졸미만 농업인을 위한 사전학습제도화를 위하여 농업인교육의 평가인정을 통한 방안과 One-Stop 평생학습모델을 통한 방안을 제시하였다. The purposes of this research were to assign the role of rural development organizations, and to establish the strategy for the making headquarter of farmer's university. The study was performed as follows: (1) analyzing the related literatures, documents, records review, (2) need analysis of farmers and members ig rural development organizations in terms of farmer's lifelong learning, (3) consulting experts. The following four findings are the results of this study: (1) Korean national agricultural college should be headquarter of farmer's universities and be approve for lifelong learning credit. Also, agricultural technology & extension centers should run educational program for the lifelong learning credit. (2) For the doing role of headquarter of farmer's university, division of lifelong learning should be add to department of technology training in Korea agricultural college. In relation with this, vision, mission and strategy of division of lifelong learning were suggested. (3) Steps for introduction of credit bank system of farmer education were suggested. Also standard curriculums of 'farming associate degree' and 'agricultural management bachelor's degree' were developed through analyzing existing standard curriculum related with agriculture, analyzing need of farmer, analyzing curriculum of farmer's university, analyzing curriculum of agriculture major. (4) For farmer who do not have high school graduate, two solutions were suggested which are approving farmer's education and one-stop lifelong learning model.

        Effects of Isometric Exercise on Muscle Activity and Body Balance Ability in Asymmetrical Walkers

        Purpose: This study was to investigate the effects of isometric exercise on muscle activity and body balance ability in asymmetrical walkers. Research design, data, and methodology: Twenty gait asymmetry people were divided to unilateral exercise group (UG, n=10) and bilateral exercise group (BG, n=10). UG were performed unilateral exercise for 60 minutes, three times a week, and 16 weeks, and BG were performed one side and then the other side alternately for 60 minutes, three times a week, and 16 weeks. Muscle activity and body balance ability were measured before, after 4 and 16 weeks isometric exercise. Moreover, SI (symmetry index; SI) was calculated from the measured value of SL (step length). Statistical analyses were conducted using one-way ANOVA and two-way ANOVA with repeated measures, a paired t-test, and multiple comparisons according to Scheffe. Results: In the muscle activity, ST decreased significantly in short step length(S-SL) and BG of LS compared to before isometric exercise(p<.05), and GCM decreased significantly in BG of S-SL(p<.05). As for body balance ability, the mSEBT-A difference between L-SL and S-SL was decreased significantly in UG(p<.05). And the respective total scores of L-SL and S-SL, mSEBT-PM and mSEBT-PL were increased significantly in BG(p<.05). Conclusions: As a result, in this study above, it was confirmed that isometric exercise improved muscle activity and body balance ability in asymmetrical walkers.

      • N-terminal arginylation generates a bimodal degron that modulates autophagic proteolysis

        <P>The conjugation of amino acids to the protein N termini is universally observed in eukaryotes and prokaryotes, yet its functions remain poorly understood. In eukaryotes, the amino acid L-arginine (L-Arg) is conjugated to N-terminal Asp (Nt-Asp), Glu, Gln, Asn, and Cys, directly or associated with posttranslational modifications. Following Ntarginylation, the Nt-Arg is recognized by UBR boxes of N-recognins such as UBR1, UBR2, UBR4/p600, and UBR5/EDD, leading to substrate ubiquitination and proteasomal degradation via the N-end rule pathway. It has been a mystery, however, why studies for the past five decades identified only a handful of Nt-arginylated substrates in mammals, although five of 20 principal amino acids are eligible for arginylation. Here, we show that the Nt-Arg functions as a bimodal degron that directs substrates to either the ubiquitin (Ub)proteasome system (UPS) or macroautophagy depending on physiological states. In normal conditions, the arginylated forms of proteolytic cleavage products, D101-CDC6 and D1156-BRCA1, are targeted to UBR box-containing N-recognins and degraded by the proteasome. However, when proteostasis by the UPS is perturbed, their Nt-Arg redirects these otherwise cellularwastes tomacroautophagy through its binding to the ZZ domain of the autophagic adaptor p62/STQSM/Sequestosome-1. Upon binding to the Nt-Arg, p62 acts as an autophagic N-recognin that undergoes self-polymerization, facilitating cargo collection and lysosomal degradation of p62-cargo complexes. A chemical mimic of Nt-Arg redirects Ub-conjugated substrates from the UPS to macroautophagy and promotes their lysosomal degradation. Our results suggest that the Nt-Arg proteome of arginylated proteins contributes to reprogramming global proteolytic flux under stresses.</P>

        새로운 소프트 스위칭 승압형 정류기의 토폴로지

        This paper has proposed a new boost rectifier. Adequate input current was obtained by keeping the duty ratio constant without complicated conventional control methods and the improvement of the waveform was increased. With a decrease of distortion up to 12.9[%], the scope of restriction on harmonics was set to 13.0[%](3<SUP>td</SUP> harmonics), 1.1[%](5<SUP>td</SUP> harmonics) and 0.6[%](7<SUP>td</SUP> harmonics), respectively. Because complicated methods of control are avoided, the circuit configuration is simple and practical. In particular, the said effect turned out to be highly efficient in the low boosting ratio range(boosting ratio(α)=1.25). The feasibility of these facts has been proven both theoretically and experimentally.

      • Complementary and Alternative Medicine Use among Cancer Patients at the End of Life: Korean National Study

        Objectives: To investigate in depth the use of complementary and alternative medicines (CAMs) by cancer patients at the end-of-life (EOL) and how they communicate with physicians about them. Design and location: In 17 hospitals in Korea between January and December 2004 we identified 4,042 families of cancer patients. Results: The prevalence of CAM use among cancer patients at the EOL was 37.0%, and 93.1% had used pharmacologic types of agents. The most frequent motive for CAM use was the recommendation of friends or a close relative (53.4%) or a physician (1.6%). Only 42.5% discussed CAM use with their physicians. Satisfaction with CAMS was recalled for 37.1%. The most common reason given for that satisfaction was improvement of emotional or physical well-being, while ineffectiveness was the most common reason given for dissatisfaction. The average cost of CAM during the last month of life was $US 900. CAM use was associated with longer disease periods, primary cancers other than liver, biliary, and pancreatic, and need of support from physicians or religion. Conclusions: CAM use among cancer patients at the EOL was common, not discussed with physicians, and associated with expectation of cure. Expectations were generally unmet while the treatments were a financial burden. Further studies evaluating the effects of CAM at the EOL and factors that enhance communication with the physician are needed.

        Daidzein Activates Choline Acetyltransferase from MC-IXC Cells and Improves Drug-Induced Amnesia

        <P>The choline acetyltransferase (ChAT) activator, which enhances cholinergic transmission <I>via</I> an augmentation of the enzymatic production of acetylcholine (ACh), is an important factor in the treatment of Alzheimer’s disease (AD). Methanolic extracts from <I>Pueraria thunbergiana</I> exhibited an activation effect (46%) on ChAT <I>in vitro</I>. <I>Via</I> the sequential isolation of <I>Pueraria thunbergiana</I>, the active component was ultimately identified as daidzein (4′,7-dihydroxy-isoflavone). In order to investigate the effects of daidzein from <I>Pueraria thunbergiana</I> on scopolamine-induced impairments of learning and memory, we conducted a series of <I>in vivo</I> tests. Administration of daidzein (4.5 mg/kg body weight) to mice was shown significantly to reverse scopolamine-induced amnesia, according to the results of a Y-maze test. Injections of scopolamine into mice resulted in impaired performance on Y-maze tests (a 37% decreases in alternation behavior). By way of contrast, mice treated with daidzein prior to the scopolamine injections were noticeably protected from this performance impairment (an approximately 12%–21% decrease in alternation behavior). These results indicate that daidzein might play a role in acetylcholine biosynthesis as a ChAT activator, and that it also ameliorates scopolamine-induced amnesia.</P>

        Intravenous Patient-controlled Analgesia Has a Positive Effect on the Prognosis of Delirium in Patients Undergoing Orthopedic Surgery

        Background: Postoperative delirium is relatively common. However, the relationship between intravenous patient-controlled analgesia (IV-PCA) and delirium has not been thoroughly investigated. The aim of this study was to evaluate the effects of IV-PCA on the prognosis of postoperative delirium in patients undergoing orthopedic surgery. Methods: Medical records of 129 patients with postoperative delirium were reviewed. Patients were divided into two groups according to whether they used IV-PCA with fentanyl and ketorolac. The IV-PCA group consisted of 73 patients who were managed with IV-PCA; the NO-PCA group consisted of 56 patients who were managed without PCA. Results: Incidences of multiple psychiatric consultations and prolonged delirium were significantly lower in patients using IV-PCA with fentanyl and ketorolac than in those without PCA. Conclusions: We recommend the use of IV-PCA for pain control and management of delirium in patients with postoperative delirium.

        Skin benefits of postbiotics derived from Micrococcus luteus derived from human skin: an untapped potential for dermatological health

        Background The skin microbiome, a diverse community of microorganisms, plays a crucial role in maintaining skin health. Among these microorganisms, the gram-positive bacterium Micrococcus luteus exhibits potential for promoting skin health. This study focuses on postbiotics derived from M. luteus YM-4, a strain isolated from human skin. Objective Our objective is to explore the beneficial effects of YM-4 culture filtrate on dermatological health, including enhancing barrier function, modulating immune response, and aiding recovery from environmental damage. Methods The effects of the YM-4 culture filtrate were tested on human keratinocytes and fibroblasts under various conditions using real-time PCR for gene expression analysis and fibroblast migration assays. A dehydration-simulated model was employed to prepare RNA-Seq samples from HaCaT cells treated with the YM-4 culture filtrate. Differentially expressed genes were identified and functionally classified through k-means clustering, gene ontology terms enrichment analyses, and protein-protein interactions mapping. Results The YM-4 culture filtrate enhanced the expression of genes involved in skin hydration, hyaluronic acid synthesis, barrier function, and cell proliferation. It also reduced inflammation markers in keratinocytes and fibroblasts under stress conditions. It mitigated UVB-induced collagen degradation while promoted collagen synthesis, suggesting anti-aging properties, and accelerated wound healing processes by promoting cell proliferation and migration. RNA sequencing analysis revealed that the YM-4 culture filtrate could reverse dehydration-induced transcriptional changes towards a state similar to untreated cells. Conclusion M. luteus YM-4 culture filtrate exhibits significant therapeutic potential for dermatological applications.
