        Anti-VEGF Monotherapy versus Combined Anti-VEGF and Endpoint Management Laser for Diabetic Macular Edema (END-DME Study)

        Purpose: To compare the efficacy of a combination of anti-vascular endothelial growth factor (VEGF) and endpoint management (EpM) grid laser versus monotherapy with anti-VEGF in the management of diabetic macular edema (DME). Methods: This was a prospective, multicenter, open-label, nonrandomized interventional study (NCT02462304). Anti-VEGF therapy consisted of one initial injection administered pro re nata in both groups. EpM laser was performed within 1 month after the first anti-VEGF injection. The main outcome evaluated was the number of anti-VEGF injections performed in 6 months. Best-corrected visual acuity (BCVA) and central foveal thickness (CFT) were also evaluated monthly. The anti-VEGF agent used in Japan and Hong Kong was ranibizumab, while bevacizumab was used in Korea. Results: This study enrolled a total of 42 subjects, with 19 (45.2%) allocated to the monotherapy arm and 23 (54.8%) allocated to the combination arm (EpM group). Overall, there was no significant difference in the number of injections between patients in the two groups (+0.12, p = 0.81). Mixed regression analysis with all the covariates plus an interaction term for the ranibizumab/bevacizumab and the monotherapy/combination arm showed that the EpM group needed 0.94 less injections (95% confidence interval [CI], -1.73 to -0.15; p = 0.02). The logarithm of the minimum angle of resolution BCVA at 24 weeks was significantly improved in the monotherapy arm (p = 0.03), but not in the EpM group (p = 0.09). CFT at 24 weeks was significantly improved in both groups (p < 0.01). Conclusions: These data suggest that combination therapy with the EpM laser may be effective for reducing the number of anti-VEGF injections required for DME treatment.

        Oral Testofen, L-Citrulline, Resveratrol, and Caffeine Supplement Drink Improves Sexual Function in Men with Phosphodiesterase 5 Inhibitors: Randomized, Double-Blind, Placebo-Controlled Crossover Pilot Study

        Purpose: Sexuality is very important for men’s health. Currently, phosphodiesterase type 5 inhibitors (PDE5i) remain the firstline therapy for erectile dysfunction, and no other oral drugs or drinks are approved and used clinically to treat male libido or orgasm dysfunction. In reference to a pharmaceutical dosage document, growing health consciousness is said to have boosted yearly sales of supplement drinks to more than 170 billion yen (1.58 billion US dollars) in Japan. We have created a supplement drink comprised of testofen, L-citrulline, resveratrol, and caffeine. We hypothesized that our supplement drink would be effective for men with sexual dysfunction, and especially libido dysfunction. Materials and Methods: In this study, men with sexual dysfunction received a placebo drink for 14 days or our supplement drink (testofen, 600 mg/d; L-citrulline, 800 mg/d; resveratrol, 300 mg/d; and caffeine, 40 mg/d) for another 14 days separated by a 7-day washout period. Patients continued on-demand use of PDE5i. The International Index of Erectile Function (IIEF) score and adverse events were assessed. Results: Twenty patients aged 30–47 years old were enrolled in and concluded the study with no adverse events experienced. The IIEF domains of desire frequency, erection frequency, erection firmness, erection confidence, intercourse satisfaction, ejaculation frequency, orgasm frequency, and overall satisfaction were significantly improved with our supplement drink. Conclusions: This is the first study to show that our supplement drink may not only be effective for erection but also for libido, orgasm, and ejaculation.

      • Characterization of Lignin Structure in Chemithermomechanical Pulp Predicting Photo-Yellowing Level by Pyrolysis-Gas Chromatography with Tetrabuthylammonium Hydroxide

        Pyrolysis-gas chromatography (Py-GC) in the presence of organic alkali of tetrabutylammonium hydroxide (TBAH) was applied to characterize the polyphenol fragments with a carbonyl group causing different magnitude of photoyellowing in chemithermomechanical pulp (CTMP) papers. Two different origin of CTMP papers prepared from different individuals of Eucalyptus globulus trees showing high and low yellowing after photo-irradiation was compared before photo-irradiation. As a result, 7 peaks assigned to a series of phenol compounds with a carbonyl group, derived mainly from lignin, gave significant amount of phenol compounds with a carbonyl group for the paper sample of latent high yellowing, i.e., butoxy- and syringaldehyde, butoxy- and syringylacetone, butoxy-acetoguaiacone, butoxy-acetosyringone, butoxy-acetoethylsyringone, 3-methoxy-4-butoxy butyl ester, and 3,5-dimethoxy-4-butoxy butyl ester, using Py-GC/mass spectrometry (MS). The Py-GC method combined with TBAH successfully characterized polyphenol fragments with a carbonyl group causing differ high photo-yellowing in CTMP papers using a microgram order of samples.
