        Comparative Evaluation of Depth of Cure of Three High Viscosity Bulk Fill Composites versus Conventional Composite: An In Vitro Study

        Objective: The aim of this study was to evaluate the microhardness and depth of cure of three high viscosity bulk fill resin composites and to compare it with a conventional composite. Methods: To evaluate depth of cure, the composite specimen was prepared using metallic mold with a hole of 4 mm depth and 4 mm internal diameter which was bulk filled with each of the three bulk fill composites (SonicFill, Filtek Bulk Fill Posterior & Beautifil Bulk Restorative) and light cured for 20 seconds. The conventional composite (Tetric N Ceram) was filled in the increments of 2-mm. The top and bottom surface hardness was measured using a Vickers microhardness indenter. The depth of cure was then calculated by dividing the bottom & top mean hardness values. Results: Sonicfill showed maximum surface microhardness followed by Beautifil Bulk restorative, Tetric N Ceram and Filtek Bulk Fill. Maximum depth of cure was seen in SonicFill followed by Filtek Bulk Fill, Tetric N Ceram and Beautifil Bulk restorative and the difference was statistically significant. SonicFill and Filtek Bulk Fill were able to achieve the minimum depth of cure value ≥0.80 at 4-mm depth. Beautiful bulk restorative composite was not able to achieve the minimum bottom to top ratio of 0.80. Conclusion: Microhardness and depth of cure is a material specific property and factors such as composition; content and size of fillers; viscosity and translucency of the material play an important role in determining the properties of the bulk fill composite.

        In recent years, the image processing mechanisms are used widely in several medical areas for improving earlier detection and treatment stages, in which the time factor is very important to discover the disease in the patient as possible as fast, especially in various cancer tumors such as the liver cancer. Liver cancer has been attracting the attention of medical and sciatic communities in the latest years because of its high prevalence allied with the difficult treatment. Statistics indicate that liver cancer, throughout world, is the one that attacks the greatest number of people. Over the time, study of MR images related to cancer detection in the liver or abdominal area has been difficult. Early detection of liver cancer is very important for successful treatment. There are few methods available to detect cancerous cells. In this paper, an automatic approach that integrates the intensity-based segmentation and k-means clustering approach for detection of cancer region in MRI scan images of liver.

        An In Vitro Study to Evaluate and Compare the Effects of Various Commercially Available Remineralizing Agents on Surface Microhardness of Artificially Produced Enamel Lesions

        Objective: To evaluate the remineralization potential of various commercially available remineralizing agents on artificially produced enamel lesions. Methods: Twenty five human molar teeth extracted for periodontal reasons were collected for this study. Specimens were divided into various groups according to the remineralizing agent used, G1- Colgate Prevident, G2- GC Tooth Mousse, G3- GC Tooth Mousse Plus, G4- Clinpro and G5- SHY-NM Novamin. The baseline surface microhardness (SMH) was measured for all the specimens using the vickersmicrohardness testing machine. Artificial enamel lesions were created by inserting the specimens in demineralizing solution for 96 h. SMH of demineralized specimens was evaluated. Following which 10 days of pH-cycling regimen was carried out and then SMH of remineralized specimens was evaluated. Statistical analysiswas done using Friedman test with Wilcoxon signed rank test for intra group comparison and Kruskal Wallis test with Mann Whitney U test for inter group comparison. p<0.0001 was considered to be significant. Results: Regain in SMH was higher in Clinpro group as compared to Prevident group. Regain in SMH in Clinprogroup & Previdentgroupwas significantly higherthan Novamin group, which was significantly higher than GC Tooth Mousse plus group, which was further significantly higher than GC Tooth Mousse group. Conclusion: All remineralizing pastes used in this study could effectively remineralize the artificially produced early enamel lesions; however, none of them could regain the surface microhardness to the level found at baseline. Clinpro showed best remineralizing efficacy among all test groups.

        External Cervical Resorption - A Diagnostic Challenge

        External cervical resorption (ECR) is a relatively uncommon form of external resorption. Diagnosis & treatment of ECR is highly challenging to the clinician. This article discusses the etiology, histology, clinical features, differential diagnosis and management of ECR. It also discusses a case report of ECR which was earlier misdiagnosed as cervical caries with pulp and periapical pathology.

        Dentin Hypersensitivity - A Vexing Clinical Problem

        Dentinal hypersensitivity (DH) is a common clinical condition usually associated with exposed dentinal surfaces. It can affect patients of any age group and most commonly affects the canines and premolars of both the arches. This article concisely re- views the patho-physiology, mechanism and clinical management of the DH. Treatment of DH should start with an accurate diagnosis. Differential diagnosis should be made and all other probable causes should be excluded. An often neglected phase of clinical management of DH is the identification and treatment of the causative factors of DH. By removing the etio- logical factors, the condition can be even prevented from occurring or recurring. There are various treatment modalities available which can be used at home or may be professionally applied. The “at home” desensitizing agents include tooth- pastes, mouthwashes or chewing gums and they act by either occluding the dentinal tubules or blocking the neural transmission. This article also discusses the recent treatment options like bioglass, Portland cement, lasers and casein phosphopeptide.

        Optimum Hybrid SVPWM Technique for Three-level Inverter on the Basis of Minimum RMS Flux Ripple

        This paper presents an optimum hybrid SVPWM technique for three-level voltage source inverters (VSIs). The proposed hybrid SVPWM technique aims to minimize total harmonic distortion (THD). A new parameter is introduced to incorporate the heterogeneous nature of switching sequences of SVPWM technique. The proposed hybrid SVPWM technique is implemented on a low-cost PIC microcontroller (PIC18F452) and verified experimentally with a 2 KVA three-phase three-level insulated gate bipolar transistor-based VSI. Optimum switching sequence results in the three-level inverter configuration are demonstrated. The proposed hybrid SVPWM technique improves the THD performance by 17.3% compared with the best available three-level SVPWM technique.

        Performance Evaluation of Various Bus Clamped Space Vector Pulse Width Modulation Techniques

        The space vector pulse width modulation (SVPWM) technique is a popular PWM method for medium voltage drive applications. Conventional SVPWM (CSVPWM) and bus clamped SVPWM (BCSVPWM) are the most common SVPWM techniques. This paper evaluates the performance of various advanced BCSVPWM strategies in terms of their harmonic distortion and switching loss based on a uniform frame work. A uniform frame work, pulse number captures the performance parameter variations of different SVPWM strategies for various number of samples with heterogeneous pulse numbers. This work compares different advanced BCSVPWM techniques based on the modulation index and location of the clamping position (zero vector changing angle ) of a phase in a line cycle. The frame work provides a fixed fundamental frequency of 50Hz. The different BCSVPWM switching strategies are implemented and compared experimentally on a 415V, 2.2kW, 50Hz, 3-phase induction motor drive which is fed from an IGBT based 2 KVA voltage source inverter (VSI) with a DC bus voltage of 400 V. A low cost PIC microcontroller (PIC18F452) is used as the controller platform.

        Automated segmentation and delineation of morphological properties of retinal vascular network had now become the most important research area in the treatment of ophthalmologic disorders. With the advancement of computational efficiency, image processing methodologies are widely used in ophthalmology. This paper gives the review of various segmentation techniques implemented by various authors in conjunction with performance metrics like sensitivity, specificity, accuracy and area under the curve. Results of various algorithms has been compared and analyzed. For the extraction of retinal vasculature, 2D retinal image from various databases has been considered.

        C-Shaped Canals: A Review

        C-shaped canal configuration is a variation that has a racial predilection and is commonly seen in mandibular second molars. The intricacies present in this variation of canal morphology can pose a challenge to the clinician during negotiation, debridement and obturation knowledge of the C-shaped canal configuration is essential to achieve success in endodontic therapy. Radiographic and clinical diagnosis can aid in identification and negotiation of the fan-shaped areas and intricacies of the C-shaped anatomy. Effective management of this anomalous canal configuration can be achieved with rotary and hand instrumentation assisted with sonics and ultrasonics. Modifications in the obturation techniques will ensure a 3-dimensional fill of the canal system and chamber retained restorations like amalgam or composites, serve as satisfactory post endodontic restorations.

        Acupressure as an Adjunct to Pharmacological Treatment for Depression, Anxiety, and Stress in Patients with Knee Osteoarthritis

        Background: Osteoarthritis is the most common type of arthritis. Depression, anxiety, and stress are associated with knee osteoarthritis. Objectives: The aim of the study was to check the effectiveness of acupressure therapy with pharmacological treatment on pain, depression, anxiety, and stress in patients with knee osteoarthritis and to assess the effect of pain improvement on psychological health. Methods: Eligible 212 patients with knee osteoarthritis were divided into two groups (intervention and control group). The intervention group (n = 106) received acupressure therapy in combination with pharmacological treatment, whereas the control group (n = 106) continued pharmacological treatment only. Pain and psychological symptoms were measured using the visual analog scale and Depression Anxiety Stress Scale-21. Pearson's correlation was used to check the effect of pain improvement on psychological health. Results: Patients of both groups reported severe pain initially. On analyzing the results after completion of the study, it was found that patients in the intervention group scored better on the pain scale (p < 0.001) and DASS-21 (p ≤ 0.0001). However, it may be noted that reduction in the DASS-21 score was not found to be significant for the control group (p = 0.08). Pearson's correlation coefficients value ranged from 0.231 to 0.412 for DASS-21 (p < 0.05). Conclusions: On analysis, it can be concluded that acupressure can be used as add-on therapy in combination with conventional treatment (pharmacological treatment), which may assist in pain reduction. The reduction in pain directly contributes to improvement in the physiological wellness among patients with knee osteoarthritis.
