      • Studies on Anopheles farauti in the Republic of Vanuatu

        1986년부터 1988년에 걸쳐 바누아투공하국 내 수개 섬에서 말라리아 매개종인 A. farauti의 서식습성, 흡혈습성 및 방제효과 등을 조사하여 다음과 같은 결과를 얻었다. 1) 유충의 서식장소는 인공용기나 기수등을 제외한 거의 모든 장소에서 채집되었는데, 개울, 여울, creek taro 경작지, 늪, 물웅덩이 등이 주요 발생원이었다. Tanna와 Nguna섬의 경우는 서식장소가 극히 제안되어 있어서 우기중에 일시적 방생장소가 급증하여 말라리아 환자가 증가하는 것으로 나타났다. 2) 대부분의 Taro 경작지, 늪, 못 등 항시 물이 고여있는 발생장소에는 생물학적 방제 목적으로 방사한 Gambusia 물고기가 높은 밀도로 번식하고 있었고 모기 유충은 전혀 발견할 수 없었다. 3) 인체흡혈모기채집 조사 결과 A. farauti의 흡혈활동은 밤 8∼10시에 가장 활발하였고, 옥내 : 옥외흡혈비는 3월(우기)에는 1:1.5였고 8월(건기)에는 1:0.6이였다. 이와같은 변화는 기후의 특히 온도 차이에 기인한 것 같았다. 4) 저녁에는 미경산(nulliparous) 모기가 높은 비율로 출현하였고(저녁 7∼8시에 85.7%의 미경산 모기 채집), 깊은 밤과 이른 새벽에는 경산(parous)모기가 다수 채집되었다(밤 10시에서 새벽 5시 사이에는 72.3%의 경산모기가 채집). 5) DDT 옥내 살포한 지역에서는 A. farauti의 개체군 밀도와 경산률(parous rate)이 모두 낮아 방제효과가 현저하였다. Studies on Anopheles farauti, the sole vector species of malaria in the Republic of Vanuatu were carried out in 1986-1988, and the results are summarized as follows: 1) Larvae of A. farauti were found almost in all the breeding sources available, except artificial containers and brackish water. The main breeding places were streams, creeks and taro fields in Santo Island, and a few swamps/marshes and wells in Emae Island and Ambae Island. Very limited breeding places were found in Nguna Islet and Tanna Island. During the wet season, temporary breeding places, such as puddles, ground pools, waster accumulations in road ruts. etc. were widely provided, causing high density of the vector population, so high malaria transmission. 2) In most of the taro fields, ponds, swamps and marshes, the larvivorous fish, Gambusia affinis were well established with high desnity, and no mosquito larvae were found without exception. 3) Human biting collections at Santo showed that the peak of the feeding time of A. farauti was 20:00-22:00 hours. The biting ratio of indoors to outdoors was 1 to 1.5 in March and 1 to 0.6 in August. 4) Nulliparous females predominated in the early evening, showing a 14.3% parous rate at 10:00-20:00 hours, whereas a 72.3% parous rate was shown during the period of 22:00-05:00 hours. 5) The impact of DDT residual spraying on A. farauti control was evident in aspects of both population density and longevity (parous rate) of the vector mosquitos.

        Effects of Age and High Frequency Hearing Loss on Binaural Speech Understanding Using HINT Study

        It has long been known that high-frequency sensorineural hearing loss (HFHL) can contribute to difficulty in speech understanding by elderly listeners. This study evaluated the relative contribution of HFHL and age to speech understanding. Subjects included adult middle-aged and old groups with normal hearing or with limited HFHL. The Hearing-in-Noise-Test (HINT) was used to measure speech perception performance in quiet and in noise. The middle-aged groups showed significant effects of HFHL for speech intelligibility in quiet and in noise, but the old groups showed the difference in quiet only due to high frequency hearing. The results suggest that HFHL may affect speech intelligibility differently with age and therefore hearing aid selection needs to take into account the influence of age.

        A case report of a carbon dioxide embolism caused by endoscopic vein harvesting during cardiac surgery -A case report-

        A carbon dioxide (CO2) embolism during endoscopic vein harvesting is a rare but potentially fatal complication. Early and accurate diagnosis is crucial for limiting the extent of the embolism and stabilizing the resulting cardiovascular compromise. We report a case of CO2 embolization during endoscopic vein harvesting. Transesophageal echocardiography was instrumental in the diagnosis and management of this patient by further improving the decision making process, which resulted in the best outcome. Mid-esophageal bicaval view is the best view to determine whether a CO2 embolism is coming from the upper or lower extremities.
