Isolated Facial Muscle Metastasis From Renal Cell Carcinoma on F-18 FDG PET/CT

Facial muscle is a very rare site for distant metastasis of renal cell carcinoma (RCC). We present a 71-year-old man with isolated facial muscle metastasis 12 years after right nephrectomy for RCC. Magnetic resonance imaging showed a soft-tissue mass in the masticator space. F-18 fluorodeoxyglucose positron emission tomography/computed tomography showed a large hypermetabolic lesion in the right masticator space without other hypermetabolic lesion in the whole body. Subsequent biopsy and histologic study confirmed metastatic cancer from RCC.

Perception of Long-Term Oxygen Therapy for Chronic Lung Diseases May Affect Poor Adherence in Korea

Background: Long-term oxygen therapy (LTOT) improves the survival of patients withhypoxemia due to chronic respiratory diseases. The clinical outcomes of LTOT arestrongly associated with patient adherence. To improve the adherence of patients,physicians have focused on the efficacy of LTOT. However, poor adherence may stemfrom patients’ perceptions of LTOT. Herein we evaluated patients’ perceptions of LTOTaffecting adherence. Methods: We conducted a cross-sectional survey study using descriptive, open, andclosed-ended questionnaire. Patients using oxygen therapy (OT) or requiring it butavoiding OT responded to the questionnaires at three university hospitals. Results: Seventy-nine patients responded to the questionnaires. The number of patientsusing home and portable OT was 69 (93%) and 37 (46.3%), respectively. Patientswith good adherence were 22 (30.1%). Among patients with good adherence, 90.9%used oxygen according to physicians’ prescriptions whereas only 37.3% of those withpoor adherence followed physicians’ prescriptions (p<0.01). The reasons for avoidingusing home OT were fear of permanent use (50%), unwanted attention (40%), and lackof symptoms (40%). They avoided portable OT because of unwanted attention (39%),heaviness (31.7%), and lack of symptoms (21.6%). Conclusion: Patients on LTOT had the perception of the misunderstanding the effectsof OT and of psychosocial barriers to initiate or use LTOT. Considering these findings,health professionals need to provide effective education on the purpose of LTOT toimprove patient adherence to OT and provide sufficient support for the management ofpsychosocial barriers in patients using LTOT.

<P>The directed assembly of block copolymer nanostructures with large periods exceeding 100 nm remains challenging because the translational ordering of long-chained block copolymer is hindered by its very low chain mobility. Using a solvent-vapor annealing process with a neutral solvent, which was sequentially combined with a thermal annealing process, we demonstrate the rapid evolution of a perpendicularly oriented lamellar morphology in high molecular weight block copolymer films on neutral substrate. The synergy with the topographically patterned substrate facilitated unidirectionally structural development of ultrahigh molecular weight block copolymer thin filmseven for the structures with a large period of 200 nmleading to perfectly guided, parallel, and highly ordered line-arrays of perpendicularly oriented lamellae in the trenched confinement. Fluorescence enhancement of a tetrazole-based pyridine coordination polymer hydrogel

<P>Hydrogelation of a pyridine derivative (1) possessing tetrazole moieties as end groups, without long alkyl chain groups, results in the formation of a Mg(NO<SUB>3</SUB>)<SUB>2</SUB> coordination polymer gel. The polymer exhibits a strong fluorescence enhancement upon gel formation. 1 can also be gelated with a variety of magnesium anions such as SO<SUB>4</SUB><SUP>2−</SUP>, NO<SUB>3</SUB><SUP>−</SUP>, Cl<SUP>−</SUP>, Br<SUP>−</SUP> and I<SUP>−</SUP>, indicating that the coordination polymer gel formation of 1 does not strongly depend on anions. The SEM and AFM images of Mg<SUP>2+</SUP> coordination polymer gel 1 display a fibrillar network several micrometres long, with widths in the range of 60–70 nm and thicknesses of about 3 nm. In addition, photophysical studies show that the hydrogel exhibits a typical π–π* transition and gives rise to high fluorescence behavior. PWM-PFM hybrid modulation for DAB converter based on RL algorithm

This paper proposes a hybrid modulation method to control a dual active bridge(DAB) converter. To improve the efficiency of the system, the proposed hybrid modulation is consisted with pulse width modulation(PWM) and pulse frequency modulation(PFM) in addition to single phase shift(SPS) control which is widely used in DAB converter. By adopting the proposed method, the RMS current of the switching device can be reduced while keeping ZVS operation capability. Therefore, the system can accomplish the high efficiency. Also, this method can be used to a wide output voltage range and light load which is traditionally referred to as a drawback of the DAB converter. To realize optimized control variables for hybrid modulation, reinforcement learning(RL) method is adopted. To verify the effectiveness of the proposed method, a 2 kW DAB converter is built and tested. The efficiency results shows that the efficiency is increased in 1.05% at full load.
