        외상성 횡격막 손상

        Fifty two cases of traumatic diaphragmatic injuries that we have experienced from Jan. 1973 to Oct. 1994 were evaluated. The following results were obtained. The age of the patient was ranged from 1 to 74 years. Male was 38 and female was 14 in number with a ratio of 2.7 : 1. The traumatic diaphragmatic injuries were due to blunt trauma in 35 cases(motor vehicle accident 25, fall down 8, press 1, kick by fight 1) and penetrating trauma in 17 cases(stab wound 15, shot wound 1, explosion 1). In the blunt trauma, the preoperative diagnosis of the diaphragmatic injury was possible in 25 out of 35 cases(71%) and in the penetrating trauma, 15 out of 17 cases(88%). In the blunt trauma, the repture site was located in the left in 22 cases(63%) and in the right in 13 cases(31%). In the penetrating trauma, the rupture site was located in the left in 9 cases(53%) and in the right in 8 cases(47%). In the blunt trauma, 20 cases(63%) were treated within 24 hours and in the penetrating, 15 cases(88%) within 24 hours. In the blunt trauma, the herniated organs into the thorax were stomach(7), omentum(6), spleen(6), liver(5), colon(4), small bowel(2) and in the penetrating, stomach(7), colon(6), omentum(3), liver(2), and spleen(1) were herniated. Injury severity score(ISS) of 35-blunt trauma ranged from 11 to 66 with mean value of 30.6. Mean ISS of survivors and nonsurvivors was 27.6 and 52.7 respectively. The diaphragmatic repair of 49 cases was performed with thoracic approach in 23 cases, thoracoabdominal approach in 7 cases and abdominal approach in 19 cases, and 3 cases were not operated. The postoperative complication and mortality were developed in 16 out of 49 cases(33%) and in 5 cases(9.6%) respectively, and the causes of death were hypovolemic shock(1), combined head injury(2), asphyxia(1), and pulomnary edema and renal failure(1). In conclusion, the injuries of the diaphragm should be suspected in all patients with severe blunt trauma or penetrating injuries at thorax and upper abdominal area near the diaphragm. All of the cases had associated injury and most of deaths were related to the severity of associated injuries.

        Road or Railway construction over soft ground is needed to be considered on secondary consolidation which will be caused differential settlement, lack of transport serviceability, higher maintenance cost. Especially for the railway construction in the second phase of Gyung-Bu or Ho-Nam high speed railway, concrete slab track has been adapted as a safe and cost effective geotechnical solution. In this case controlling the total settlement under the tolerance is essential. And pile supported geogrid reinforced construction method is suggested as a solution for the problem of the traditional method on soft soil treatments. Pile supported geogrid reinforced construction method consists of piles that are designed to transfer the load of the embankment through the compressible soil layer to a firm foundation. The load from the embankment must be effectively transferred to the piles to prevent punching of the piles through the embankment fill creating differential settlement at the surface of the embankment. The arrangement of the piles can create soil arching to carry the load of embankment to the piles. In order to minimize the number of piles geogrid reinforced pile supported construction method is being used on a regular basis. This method consists of one or more layers of geogrid reinforcement placed between the top of the piles and the bottom of the embankment. This paper presents several methods of pile supported geogrid reinforced construction and calculation results from the several methods and comparison of them.

        3차원 지형모델링에 기반한 도시하천의 계획 및 설계

        치수적 안전은 하천의 계획에 있어 가장 먼저 고려해야 할 사항이며 최근 폭넓게 진행 중인 하천의 복원에 있어서도 치수적 안전은 기본적인 전제조건이 된다. 안전한 하천의 계획은 정확한 측량자료로부터 시작된다. 본 연구에서는 스마트 지형공간정보를 이용하여 하천의 3차원 지형모델을 제작하고 제작된 3차원 지형모델을 이용하여 하천의 수리해석 및 하천 복원에 적용하였다. 이를 통해 상세한 하천 현황 데이터를 추출함으로써 보다 정확한 수리해석이 가능하였다. 또한, LiDAR 데이터를 도시하천의 수리해석에 이용할 경우 적용할 수 있도록 효율적인 데이터 처리와 수리해석의 정확성을 고려한 최적 횡단면 간격을 결정하였다. 하천 복원 설계를 위한 3차원 설계방안과 하천의 다양한 공간계획에 3차원 지형모델을 이용할 수 있는 활용 방안을 제시하였다. When planning for streams, adequate and definite flood control should be in the primary consideration of the planner; likewise, flood control is the basic prerequisite for the recent river restoration taking place nationwide. Planning ‘safe’ streams and rivers that are predictable and controllable begins from accurate survey data. In this research, we will create streams in 3D terrain models and apply them through hydraulic analysis and restoration using smart geospatial information. This process allows the extraction of more accurate data regarding streams and rivers, which makes possible precise hydraulic analysis that is superior in details to the conventional methods. The study also proposes optimal vertical section interval for efficient data processing on hydraulic analysis, applicable when LiDAR data is utilized on hydraulic analysis of urban streams. The study proposes 3D design plan and various applications for spatially planning and restoring rivers and streams.

        부산·울산·경남지역 직업병 감시체계

        Objective: Occupational medicine specialists in the Busan, Ulsan and Kyung-Nam areas established an area-based occupational disease surveillance system and used this system to collected case information for the purpose of preventing occupational diseases Methods: l l hospital participated in this system. The authors selected five main diseases, which were hand-arm vibration syndrome (HAVS), work-related musculoskeletal disorder (WRMSD), occupational asthma, occupational skin disease and occupational and occupational lung cancer and established their case definitions. All cases were reported on the web, and real time analysis was conducted. Results: Between April 2001 and April 2003, 192 cases of HAVS, 118 cases of WRMSD, 33 cases of occupational asthma, 17 cases of occupational skin disease and 17 cases of occupational lung cancer (for a total of 377 cases of these five main diseases) were reported. most of the HAVS cases came from shipbuilding, and 172 of them (92.7%) were associated with grinding. Of the four main types of WRMSD, carpal tunnel syndrome (CTS) was the most prevalent with 46 cases and shipbuilding was also the main industry involved (83.9%). The ergonomic risk factors involved mainly associated with the hand. In 19 (57.6%) and 4 (12.1%) cases of occupational asthma, the agents involved. The causative agents of occupational lung cancer included Cr, welding fume, PAH, Ni, etc. 10(58.8%) of the cases were diagnosed as squamous cell carcinoma and 5(29.4%) as adenoma. Conclusion: This result showed that an area-based occupational disease surveillance system might provide an effective method of evaluating the prevalence of such diseases, however the Busan, Ulsan and Kyung-Nam provinces are too large to be treated as individual areas. Therefore, the authors suggest that each province should establish its own surveillance system.

        조기위암의 근치적 치료로서의 내시경적 점막절제술

        The endoscopic mucosal resection(EMR) is proposed by some Japanese investigators as a curative therapy of early gastric cancer(EGC) because of its minimal invasiveness and excellent results. To evaluate the possible role of EMR as a curative treatment modality of EGC, we retrospectively analyzed l9 casea with EGC initially treated by EMR in Seoul National University Hospital from December 1993 and January 1996. 1) The histologic diagnosis prior to EMR was adenocarcinoma in 12 cases(63%) and adenomatous polyp in 7 cases(38%), which were confirmed as adenocarcinoma after EMR. 2) The histologic curative resection was done in 7 cases(37%). Two cases of them showed recurrence of the gastric cancer and were treated by radical surgery. The other five cases have been closely observed by regular endoscopic examination without recurrence for the maximal period of 1~3 months. 3) Histoiogically inadequate resection(positive cancer cell in resection margin, submucosal cancer infiltration, or no cancer tissue in resected specimen) was done in 12 cases(73%). But two surgically resected cimens of them have no residual cancer cells, and there was no cancer cells in the follow-up biopsy of the other two patients. There 4 cases were seemed to he examples of buring effect of EMR. 4) After excluding five depressed lesions greater than 1 cm is long diameter, nine lesions were curatively treated by EMR. So successful EMR rate for strictly indicated lesions wsa 64%. 5) There was no signifieant complication related to the procedure. We thnnk that endoscopic mucosal resection has a potential role as a curative treatment modality in a highly selected patient with darly gastric cancer.

      • Atomically thin p–n junctions with van der Waals heterointerfaces

        Semiconductor p–n junctions are essential building blocks for electronic and optoelectronic devices. In conventional p–n junctions, regions depleted of free charge carriers form on either side of the junction, generating built-in potentials associated with uncompensated dopant atoms. Carrier transport across the junction occurs by diffusion and drift processes influenced by the spatial extent of this depletion region. With the advent of atomically thin van der Waals materials and their heterostructures, it is now possible to realize a p–n junction at the ultimate thickness limit. Van der Waals junctions composed of p- and n-type semiconductors—each just one unit cell thick—are predicted to exhibit completely different charge transport characteristics than bulk heterojunctions. Here, we report the characterization of the electronic and optoelectronic properties of atomically thin p–n heterojunctions fabricated using van der Waals assembly of transition-metal dichalcogenides. We observe gate-tunable diode-like current rectification and a photovoltaic response across the p–n interface. We find that the tunnelling-assisted interlayer recombination of the majority carriers is responsible for the tunability of the electronic and optoelectronic processes. Sandwiching an atomic p–n junction between graphene layers enhances the collection of the photoexcited carriers. The atomically scaled van der Waals p–n heterostructures presented here constitute the ultimate functional unit for nanoscale electronic and optoelectronic devices.
