Curative transvenous embolization for congenital multi-hole pial arteriovenous fistula

Objective: Congenital intracranial pial arteriovenous fistula (PAVF) is a rare cerebral vascular pathology characterized by a direct shunt between one or more pial feeding arteries and a cortical draining vein. Transarterial endovascular embolization (TAE) is widely considered first line therapy. Curative TAE may not be achievable in the multihole variant due to the potential to harbor innumerable small feeding arteries. Transvenous embolization (TVE) may be considered to target the final common outlet of the lesion. Here, we present a series of four patients with complex multi-hole congenital PAVF treated with staged TAE followed by TVE.Methods: A retrospective review was conducted on patients who underwent treatment for congenital, multi-hole PAVFs treated by a combined TAE/TVE approach at our institution since 2013.Results: We identified four patients with multi-hole PAVF treated by a combined TAE/TVE. Median age was 5.2 (0-14.7) years. Median follow-up of 8 (1-15) months by catheter angiography and 38 (23-53) months by MRI/MRA was obtained. TVE achieved complete occlusion in three patients that proved durable on radiographic follow-up and demonstrated excellent clinical outcomes with a modified Rankin Score (mRS) of 0 or 1. Complete occlusion of the draining vein was not achieved by TVE in one case. This patient is graded as pediatric mRS=5 three years post-procedure.Conclusions: With thorough technical considerations, our series indicates that TVE of multi-hole PAVF that are refractory to TAE is feasible and effective in arresting the consequences of chronic, high-flow AV shunting produced by this pathology.
Trade Specialization, Endogenous Innovation and Growth

In this paper, we develop a two-sector model of economic growth, in which both sectors can enjoy productivity growth by employing an ever-increasing variety of differentiated intermediate inputs. In contrast to many two-sector models of growth and trade, specialization in the "backward" sector is fully consistent with an output growth rate equal to the global output growth rate, even though such a country does no R&D and produces no intermediate goods. International trade improves the growth rate of all countries, but is especially beneficial for "backward" countries. These results support a more optimistic interpretation of the relationship between growth, technology and trade than other recent models also making a distinction between progressive and nonprogressive sectors. Breast radiologists are increasingly seeing patients with axillary adenopathy related to COVID-19 vaccination. Vaccination can cause levels I–III axillary as well as cervical lymphadenopathy. Appropriate management of vaccine-related adenopathy may vary depending on clinical context. In patients with current or past history of malignancy, vaccine-related adenopathy can be indistinguishable from nodal metastasis. This article presents imaging findings of oncology patients with adenopathy seen in the axilla or neck on cross-sectional imaging (breast MRI, CT, or PET-CT) after COVID-19 vaccination. Management approach and rationale is discussed, along with consideration on strategies to minimize false positives in vaccinated cancer patients. Time interval between vaccination and adenopathy seen on breast MRI, CT, or PET-CT is also reported.

It has been reported for several decades that microbes, which naturally contaminate cotton fibres during crop growth and subsequent storage can have an adverse effect on the structural quality of cotton lint. Although several studies have analysed the relationship between numbers of Gram-negative bacteria or bacterial endotoxin and particular physical properties, these studies have been limited to cotton from the United States, and the possible effects of fungal contamination have not been examined in detail. This study quantified the Gram-negative bacteria and fungal cells, as well as measuring concentrations of bacterial endotoxin and fungal glucan, on cotton lint samples from international sources. Spearman's rank correlation coefficients calculated between these results and quality data analysed by an automated testing instrument revealed several significant correlations. Findings included inverse correlations between the biological contamination parameters and fibre elongation, micronaire and reflectance. The possible causes and implications of these findings were also discussed.

CR asthma is associated with disease chronicity, a positive family history of asthma and in vitro and in vivo defects in mononuclear cell function. The HPA axis in CR asthmatics is suppressed normally by dexamethasone and the pharmacokinetic profile of an oral dose of prednisolone is similar to that found in CS subjects. In addition, competitive binding studies have shown that the ligand binding and nuclear translocation functions of the GR are similar in the two groups. Studies using gel retardation assay have indicated a defect in DNA binding in CR subjects. Chemical mutational analysis of the GR has shown that is not due to a defect in its structure at the cDNA level. Scatchard analysis of the GR/DNA and GR/ligand interactions suggests that there may be transcriptional interference of the GR with other transcriptionally active molecules leading to defective gene transcription.
Interrogating Love: Romance in The Empire Strikes Back and The Force Awakens

The Star Wars sequel trilogy has improved in its female representation with numbers surpassing the scarce female cast of the original trilogy. The character Leia Organa from the original trilogy is popular among fans for her feminism, but since critics generally disagree, an analysis of Rey, the protagonist of the sequel trilogy, is necessary to determine if the series has improved the quality of its representation. A commonality between the two characters is their sexuality; both Rey and Leia undergo interrogations with implied sexual content, combining sexual assault from the romance genre with interrogations in the science fiction genre. Using theory on interrogatory rape from the romance fiction genre, connections between the two genres are made evident, as well as the feminist potential arising from such a combination. This paper examines the interrogatory scenes between the female characters Rey and Leia and their love interests Kylo Ren and Han Solo using theories of rape in the romance fiction genre, investigating how the character’s reactions can be analyzed for subversive gender performances. Deviating from claims of feminist science fiction as empowered through the hybridity of the human-technological and male-female binaries, subversive gendered performances are instead made possible for Rey through the use of the mystical Force power. Despite her Force sensitivity, the same is not true for Leia, and her submissive reactions in response to interrogatory scenes with the dangerous hero Han Solo align with traditional gender performances from the romance genre. In contrast, subversive gender performances by the character Rey using defiance through both physicality and language indicate achievement of subjectivity in response to her interrogation, demonstrating an improvement in feminist representation within the sequel trilogy.
