Bilobar Liver Metastasis with Portal Vein Tumor Thrombus of Colorectal Cancer

Aims: Treatment of Liver metastasis of colorectal cancer (CLM) is now evolving rapidly with regards to oncological and technical resectability. Little is known about multiple CLM with tumor thrombus in the portal vein (PVTT). On the literatures, Authors suggested at least that macroscopic PVTT is not a contraindication to liver surgery. Herein, we report the clinical outcomes of two patients with progressive PVTT and multiple CLM underwent hepatectomy. Results: Case 1. A 73-year-old man presented with a metastatic liver tumor accompanied by a macroscopic tumor thrombus in the right portal branch and multiple CLM on both lobes. Extended right hemi-hepatectomy with and removal of the tumor thrombus were performed, and the liver metastasis and tumor thrombus were successfully resected. The patient had the multiple CLM on remnant lobe with left main PVTT at 8 months after the liver surgery. Case 2. A 64-year-old woman who had previously undergone palliative chemotherapy for ascending colon cancer presented with CLM accompanied by a macroscopic tumor thrombus in the left portal branch. Tumor and PVTT were progressed regardless of various chemoagents. Left hemihepatectomy with radiofrequency ablation on right lobe and right hemicolectomy were performed. But, CLM occurred just at 3 months after the liver surgery. Conclusions: The prognosis of patients with liver metastasis accompanied by a PVTT remains unknown. Considering previous our cases, a poor prognosis may be expected even though the tumor is successfully removed by liver resection. We introspected whether surgery is the treatment option for such pattern of liver metastasis in this era.

본 논문에서는 가상의 V상차 작업을 위한 이벤트 기반의 휠로더 운전자 모델을 개발하였다. 운전자 모델 개발의 목적은 휠로더의 일반적인 작업인 V상차 작업 시 동역학적 해석과 작업성능을 가상의 시뮬레이션 모델과 운전자 입력을 이용해 예측 및 평가하는 것이다. V상차 작업은 4단계로 이루어져 있으며, 총 8 개의 이벤트로 인해 순차적으로 작업이 진행된다. 개발된 3D 휠로더 시뮬레이션 모델은 Matlab/Simulink 환경에서 구성 되었으며, 시뮬레이션 결과는 V상차 작업의 실차 데이터와 비교 되었다. 본 연구에서 개발된 운전자 모델로 향후 가상의 V상차 작업에 대한 작업성능 및 동역학적 해석이 가능할 것으로 본다. This paper presents the development of an event-based operator model of a wheel loader for virtual V-pattern working. The objective of this study is to analyze the performance and dynamic behavior of the wheel loader for a typical V-pattern. The proposed typical V-pattern working is divided into four stages. The developed operator model is based on eight events, and the operator"s inputs are occurred sequentially by event. A 3D dynamic simulation model of the wheel loader is developed and used to analyze the dynamic behavior during working, and the simulation results are compared with the experimental data of V-pattern working. The proposed 3D dynamic simulation model and operator model are constructed using MATLAB/Simulink. The proposed operator model for V-pattern working is expected to enable evaluation of the working performance and dynamic behavior of the wheel loader.
An investigation of energy efficiency of a wheel loader with automated manual transmission

This paper describes an investigation of energy efficiency by applying an advanced powertrain system in a conventional wheel loader. A conventional powertrain of a wheel loader consists of an engine, torque converter and transmission. A torque converter in a conventional system generally causes a significant amount of energy loss, as determined by analyzing energy flow based on V-pattern working. To prevent energy loss in a torque converter, Automated manual transmission (AMT) was proposed and modeled in this paper as an advanced powertrain. A wheel loader based on AMT does not need to use a torque converter since the single clutch system is used between the engine and transmission with subsystems of engine controller, clutch actuator and controller. A simplified single clutch system and controller were constructed for V-pattern working of a wheel loader. Additionally, a PI-controller was used as a control algorithm for engine speed control to prevent energy loss while the clutch is not engaged. All simulation models have been constructed in the Matlab/Simulink environment, and simulation studies were conducted by using a simulation model of a wheel loader with a driver model based on V-pattern working. Simulation results of the AMT-based wheel loader were analyzed by comparison with the results of the torque converter-based wheel loader, and the results show that the AMT-based wheel loader is more energy efficient than the conventional wheel loader.

This paper presents the development of event based wheel loader operator model for virtual V-pattern working. The objective of this study is to analyze the performance and dynamical behavior of wheel loader for typical V-pattern. Proposed typical V-pattern working is divided into 4 stages. Operator model is based on 8 events and operator’s inputs are occurred sequentially by event. 3D wheel loader dynamic simulation model is used to analyze dynamical behavior in working and simulation results are compared to the experimental V-pattern working data. The presented 3D dynamic simulation model and operator model in this paper are constructed by Matlab/Simulink. It is expected that proposed operator model V-pattern working enable to evaluate working performance and dynamical behavior.
