병원의무기록과에서 생성되는 각종 통계와 그전산화율에 대한 조사

정확한 국가보건통계 데이터베이스는 보건정책의 기초가 되지만, 우리나라에서는 아직 신뢰할만한 통계를 생산해내지 못하고 있는 것이 사실이다. 의료보험자료를 기초로 해서 만드는 보건통계가 있지만 여러 면에서 신뢰도가 떨어진다는 의견들이 많다. 이에 저자들은 대안으로서 각 병원의 의무기록실에서 생성하고 있는 통계들이 국가보건통계의 기초가 될 수 있는 지를 알아보기 위해 78개 병원을 대상으로 통계 생성여부와 전산화 정도를 조사하였다. 설문은 병원표준화 심사와 서비스평가, OECD에서 요구하는 통계들을 참고로 하여 작성하였으며, 해당 통계를 생성하고 있는지, 통계생성 작업이 어느정도 전산화되어 있는지를 물었다. 응답한 병원의 50% 이상이 통계를 생성하고 있다고 하였으며, 통계를 생성하고 있는 병원에서 전산망을 이용하지 않고 수작업으로 하고있는 비율은 1.3%에서 7.7% 사이였다. 설문서에서 질문한 통계를 생성하지 않고 있다고 응답한 병원의 대부분은 전산화 되어있지 않았다. 이상의 결과로부터 각 병원의 의무기록실에서는 많은 부분 국가보건 통계에 필요한 각종 통계량들을 생성해 내고 있으며, 통계 생성 방법은 전산화 되어있고, 따라서 이를 수집, 가공하면 국가보건통계 데이터베이스를 구축할 수도 있을 것이라는 결론을 얻었다. 이를 위해서는 통계량을 정의하는 표준화 사업이 선행되어야 할 것이다 We surveyed the generation rate of health statistics by medical records offices of the 78 hospitals and its automation rate using computerized hospital information system. Structured questionnaire was given to one medical record officer of each hospital. Items in the questionnaire was selected from statistics required for hospital service evaluation or OECD health statistics. More than 50% of the medical record office generated questioned health statistics, and most of them was automated. Because many of the medical record offices of the hospitals are producing essential health statistics and automated, the is a possibility that we can collect and use these datas to build up national health database if adequate standardization procedure can be implemented.

The purpose of the study was to analyze the primary environmental education of the 7th Practical Arts Curriculum and suggest the direction of practical environmental education in the 'Living Technology' Part. The conclusions of this study were followed: 1. As a result of analysis of environmental education goal of the Practical Arts Curriculum. 'Information & Knowledge, Awareness', 'Value & Attitude' and 'Action & Participation' were evenly emphasized. And in environmental education contents, all contents were treated except 'ESSD'. Especially 'the Environmental Preservation and measures' was most frequently treated among them. 2. The part of 'Living Technology' in the Practical Arts was divided into the five categories ('Food & nutrition', 'clothing', 'electricity, electron & handicraft', 'cultivation & breeding', and 'computer manipulation') and the practical methods of environmental education were suggested. The section of cultivation & breeding was achieved and educated by experiencing education, the case method, experimentation and practice. The section of Food & nutrition was educated by composing of research paper, discussion and experience-oriented class. The section of clothing was educated by the case method, role playing, experimentation and practice. And the section of information technology was educated by discussion of investigation and surfing on sites related to environment.

The binding of interleukin-6 (IL-6) cytokine family ligands to the gp130 receptor complex activates the Janus kinase (JAK)/ signal transducer and activator of transcription 3 (STAT3) signal transduction pathway, where STA T3 plays an important role in cell survival and tumorigenesis. Constitutive activation of STAT3 has been frequently observed in many cancer tissues, and thus, blocking of the gp130 signaling pathway, at the JAK level, might be a useful therapeutic approach for the suppression of STAT3 activity, as anticancer therapy. AG490 is a tyrphostin tyrosine kinase inhibitor that has been extensively used for inhibiting JAK2 in vitro and in vivo. In this study, we demonstrate a novel mechanism associated with AG490 that inhibits the JAK/STAT3 pathway. AG490 induced downregulation of gp130, a common receptor for the IL-6 cytokine family compounds, but not JAK2 or STAT3, within three hours of exposure. The downregulation of gp130 was not caused by enhanced degradation of gp130 or by inhibition of mRNA transcription. It most likely occurred by translation inhibition of gp130 in association with phosphorylation of the eukaryotic initiation factor-2 a. The inhibition of protein synthesis of gp130 by AG490 led to immediate loss of mature gp130 in cell membranes, due to its short half-life, thereby resulting in reduction in the STAT3 response to IL-6. Taken together, these results suggest that AG490 blocks the STAT3 activation pathway via a novel pathway.
Exploring the Diffuse X-ray Emission of Supernova Remnant Kesteven 69 with XMM-Newton

We have investigated the X-ray emission from the shock-heated plasma of the Galactic supernova remnant Kesteven 69 withXMM-Newton. Assuming the plasma is at collisional ionization equilibrium, a plasma temperature and a column absorptionare found to be kT ~ 0.62 keV and NH ~ 2.85 x1022 cm-2 respectively by imaging spectroscopy. Together with the deducedemission measure, we place constraints on its Sedov parameters.

Reappraisal of the Immunogenicity and Safety of Three Hepatitis A Vaccines in Adolescents

<P>Although the overall incidence of hepatitis A in Korea has been decreasing, adolescents remain highly vulnerable to its outbreaks. This study was conducted to compare the immunogenicity and safety of three hepatitis A vaccines in Korean adolescents. Healthy anti-hepatitis A virus seronegative subjects aged 13 to 19 yr were randomized in three equal groups to receive two doses of Avaxim™, Epaxal®, or Havrix®, 6 to 12 months apart. Seroconversion rates one month after the first dose were 98%, 95%, and 93% for Avaxim™, Epaxal®, and Havrix®, respectively. Seroconversion rates reached 100% for all vaccine groups one month after the second dose. Anti-HAV geometric mean concentrations (GMCs) were 7,207.7 mIU/mL (95% CI, 6023.1-8684.7), 1,750.5 mIU/mL (95% CI, 1362.9-2248.3), and 1,953.5 mIU/mL (95% CI, 1459.4-2614.7) after two doses of Avaxim™, Epaxal®, and Havrix® respectively. Avaxim™ was significantly more immunogenic than Epaxal® and Havrix®, whereas there were no significant differences in antibody responses between Epaxal® and Havrix®. Local and systemic solicited adverse events (AEs) were mostly of mild-to-moderate intensity and resolved within 5 days. No serious AEs were reported. In conclusion, all three vaccines are highly immunogenic and well-tolerated in Korean adolescents. (Clinical Trial Registry NCT00483470)</P>
