Terra firma-forme dermatosis (TFFD) is a bizarre, acquired and idiopathic dermatosis that etiology has still not been fully defined. It is characterized by the presence of asymptomatic, brownish dirt-like lesion maybe due to disorder of keratinization. These lesions cannot be removed with ordinary cleansing. Therefore, TFFD can be differentiated from dermatosis neglecta. Patient was a 17-year-old man with brownish lesions on his face for 1 month. The patient had a history of regular washing habit with soap and water. The lesions were asymptomatic however due to cosmetic reasons, the patient wanted to treat his skin lesions. Punch biopsy revealed hyperkeratosis and fungal spore are in stratum corneum. Salicylic acid peeling with alcohol base was performed on the patient`s face. The skin lesions disappeared completely on gentle swabbing with peeling. In this point, the diagnosis of TFFD could be considered. Since, this disease can be confused with dermatosis neglecta, we report this case with literature review. (Ann Dermatol 29(1) 83∼85, 2017)

Background: Scabies is an ectoparasite caused by the mite Sarcoptes scabiei var. hominis. This very contagious dermatologic disease is not rare even today. In Goyang city, there are a lot of nursing home and the elderly. So scabies will not be rare disease and its prevalence could be increasing. Objectives: To evaluate the clinical characteristics of scabies patients and find out etiologic factors in transmission. Methods: We reviewed clinically 377 scabies patients diagnosed by microscopic examination in retrospective manner at the Myongji Hospital (2009-2013). Results: The mean age of the patients was 58.7 (male: 54.7/female: 62.8), and 75% of patients were over 50 years. Scabies was found to be most common in November and December. The suspected routes of transmission is nursing home (38.5%), caregivers (15.4%) and house(13.3%). 67.4% of scabies patients had underlying disease such as hypertension (33.7%), dementia (11.9%) and diabetes mellitus (11.9%). Conclusion: We report this clinical study to evaluate the clinical characteristics of scabies patients and find out etiologic factor in transmission.

SAPHO (synovitis, acne, pustulosis, hyperostosis, osteitis) syndrome is a systemic inflammatory disorder characterized by involving the skin, bone and joints. This osteoarticular and dermatological inflammatory disease was first reported 1987. Inflammatory osteitis can occur in sternoclavicular and sacroiliac joints with pain and tenderness; these symptoms can be a clue for diagnosis. In dermatologic field, acne on face and pustulosis on palm or sole can be seen. Patient was a 53-year-old woman with neck, chest and back tenderness who was consulted from rheumatologic department for evaluation of pustulosis on the palm and sole. Patient had acne scar on the face. Radiologic evaluation revealed that sternoclavicular osteitis without any infectious sign. Skin biopsy had been obtained from the palm and revealed that intraepidermal cavity filled with polymorphonuclear leukocytes associated with spongiform changes within the surrounding epidermis. The diagnosis of SAPHO syndrome was considered because of these clinical findings. We report a case of SAPHO syndrome, because it is rare and can be helpful for dermatologic doctors to diagnosis this disease.

Rheumatoid nodules (RN) are the most characteristic extra-articular manifestation of rheumatoid arthritis (RA),occurring in approximately 25% of patients with seropositive rheumatoid arthritis and rarely in seronegative patients. ‘Pseudorheumatoid nodules’ is a term that has been applied to nodules in the subcutis that mimic rheumatoid nodules histologically, without the development in the absence of joint pain, rheumatoid arthritis, or systemic lupus erythematosus. Because some of these nodules occur in patients with other lesions that are typical of intradermal granuloma annulare, the nodules are now generally considered to represent a subcutaneous variant of granuloma annulare. Patient was a 53-year-old man with multiple nodules on his joints of both hand for 1 year. He is a patient without RA, and seronegative RF and ANA. Histopathological examination of the skin biopsy specimen showed granulomatous lesion in dermis and subcutis. Eventually, this report is a case of a 53-year-old man with multiple pseudorheumatoid nodules on his hand who is patient without RA.

Eccrine spiradenoma is a benign uncommon neoplasm of skin adnexa which probably originates in the coil of the eccrine sweat gland. It presents usually as a solitary, slowly growing, sometimes painful, reddish-brown, and intradermal or deeply subcutaneous nodule. Lesions can arise on the head and trunk but also in a variety of locations. Histopathologically, eccrine spiradenoma may consist of one large, sharply demarcated lobule or several lobules that are often appear deeply basophilic because of the dense packing of nuclei on low magnification. The patient was 32-year-old woman with small subcutaneous soft nodule on the arm for 2 years. Due to its tenderness, the patient wanted it removed, and 4mm punch biopsy had been obtained. Histopathological examination had revealed the characteristic features of an eccrine spiradenoma.

Actinic granuloma (O’Brien) is an annular granulomatous lesion that develops mainly in sun exposed area. This rare skin disorder was first described in 1975 by O’Brien. This disease presents in middle aged individuals with asymptomatic annular plaque with mild central atrophy. Patient was a 76-year-old woman with yellowish, annular and crust like lesion on the face and anterior neck for 6 months. The lesions gradually increased with mild pruritus. Microscopic examination of skin scraping was negative for fungal elements. Punch biopsy showed granulmatous reaction and giant cells with degenerative elastic fibers. The histopathologic findings and photodistribution were consistent with actinic granuloma. We report a case of actinic granuloma that showed typical clinical and histopathological findings.

Background: Numerous dermatological problems develop in hospitalized patients and dermatological consultations are getting more important by the day. Objectives: We analyzed 3 years of dermatological consultation data from inpatients in our hospital and evaluated its clinical characteristics. Methods: We reviewed inpatients consulted from other departments for three years in retrospective manner at the Myongji Hospital (2012-2014). Results: Consulted patients numbered 2390 in three years. The total number of male and female patients were 128(53.8%) and 1104 (46.2%), respectively (M:F=1.16:1). The most frequent age group was the 6th decade (20.8%) and the second most frequent age group was the 8th decade (18.2%). Most of the consultations (34.2%) were made by internal medicine, followed by neurosurgery (12.1%), and orthopedic surgery (11.3%). Fungal infections (22.5%), pruritus (21.8%), eczematous dermatitis (18.5%), xerosis (16.4%), drug eruption (12.3%), contact dermatitis (9.5%), seborrheic dermatitis (9.0%), herpes zoster (6.2%) and urticaria (5.9%) were 9 dermatological disorders that were most commonly referred to dermatologists. Conclusion: We report this clinical study to evaluate the characteristics of consulted inpatients. Key words: Consulted patients, Dermatology
