      • Fournier`s gangrene in Sweet syndrome

        Fournier``s Gangrene is a rapidly progressive necrotizing fasciitis of the groin, perianal and perineal region that is often polymicrobial in nature, often averaging 3 species of bacteria per patient. Sweet syndrome is a disorder of unknown etiology. It has been associated with autoimmune processes, malignancies, infections, drug reactions, and gastrointestinal disorders such as inflammatory bowel disease. A 31-year-old woman who presented with severe pain in her left elbow, along with fever, myalgia of 48 hour`s duration. Cultures and laboratory test yielded a diagnosis of Sweet syndrome and she was received intravenous injections of glucocorticoid for several days during hospitalization. After three days of discharge, the patient``s left buttock was noted to be mottled with a paucity of granulation tissue, therefore, the patient was taken for buttock debridement. After additional test, a diagnosis of Fournier`s gangerene was made. And antibiotics for gram negative, gram positive was injected for 7days. Gradually, wound healed copmpletely without severe sequelae and we performed primary-closure. Fournier``s gangrene is a rarely occurring, life-threatening emergency situation. In order to keep the patient alive, it is of critical importance to make a diagnosis as soon as possible, start broad-spectrum antibiotics, perform emergency surgical debridement, support the nutrition of the patient, and carry out suitable wound care.

        <P>We present the hybrid-integrated silicon photonic receiver and transmitter based on silicon photonic devices and 65 nm bulk CMOS interface circuits operating over 30 Gb/s with a 10(-12) bit error rate (BER) for λ ~1550nm. The silicon photonic receiver, operating up to 36 Gb/s, is based on a vertical-illumination type Ge-on-Si photodetector (Ge PD) hybrid-integrated with a CMOS receiver front-end circuit (CMOS Rx IC), and exhibits high sensitivities of -11 dBm, -8 dBm, and -2 dBm for data rates of 25 Gb/s, 30 Gb/s and 36 Gb/s, respectively, at a BER of 10(-12). The measured energy efficiency of the Si-photonic receiver is 2.6 pJ/bit at 25 Gb/s with an optical input power of -11 dBm, and 2.1 pJ/bit at 36 Gb/s with an optical power of -2 dBm. The hybrid-integrated silicon photonic transmitter, comprised of a depletion-type Mach-Zehnder modulator (MZM) and a CMOS driver circuit (CMOS Tx IC), shows better than 5.7 dB extinction ratio (ER) for 25 Gb/s, and 3 dB ER for 36 Gb/s. The silicon photonic transmitter achieves the data transmission with less than 10(-15) BER at 25 Gb/s, 10(-14) BER at 28 Gb/s, and 6 x 10(-13) BER with the energy efficiency of ~6 pJ/bit at 30 Gb/s.</P>

      • Investigation of the growth and in situ heating transmission electron microscopy analysis of Ag<sub>2</sub>S-catalyzed ZnS nanowires

        <P><B>Abstract</B></P> <P>We investigated the semiconductor-catalyzed formation of semiconductor nanowires (NWs) – silver sulfide (Ag<SUB>2</SUB>S)-catalyzed zinc sulfide (ZnS) NWs – based on a vapor-liquid-solid (VLS) growth mechanism through metal-organic chemical vapor deposition (MOCVD) with a Ag thin film. The Ag<SUB>2</SUB>S-catalyzed ZnS NWs were confirmed to have a wurtzite structure with a width and length in the range of ∼30 nm to ∼80 nm and ∼1 μm, respectively. Using extensive transmission electron microscopy (TEM) and energy-dispersive X-ray spectroscopy (EDS) analyses from plane and cross-sectional viewpoints, the ZnS NWs were determined to have a c-axis, [0001] growth direction. In addition, the catalyst at the top of the ZnS NWs was determined to consist of a Ag<SUB>2</SUB>S phase. To support the Ag<SUB>2</SUB>S-catalyzed growth of the ZnS NWs by a VLS reaction, an in situ heating TEM experiment was conducted from room temperature to 840 °C. During the experiment, the melting of the Ag<SUB>2</SUB>S catalyst in the direction of the ZnS NWs was first observed at approximately 480 °C along with the formation of a carbon (C) shell. Subsequently, the Ag<SUB>2</SUB>S catalyst melted completely into the ZnS NWs at approximately 825 °C. As the temperature further increased, the Ag<SUB>2</SUB>S and ZnS NWs continuously melted and vaporized up to 840 °C, leaving only the C shell behind.       • Ulocladium Infection in a Healthy Individual

        Ulocladium, which is phylogenetically related to Alternaria, contains species that are food spoilers and plant pathogens, but also species that have potential as enzyme producers and bio-control agents. Ulocladium spp. are often found on dead vegetation, in soil, air and dust, but also on food and feedstuffs and on water-damaged building materials. A 67-year-old man presented at our clinic with 3-month history of an ulcer covered with crust on his both fore arms. The primary fungal medium had been incubated at 28°C. A colony appeared at the inoculation site after 4 days. By day 8, the colony had a diameter of 2 cm and was greyish brown and powdery. It was sporulating by this time. Many of the conidia were dark brown and had transverse and longitudinal septa, suggestive of Alternaria species. However, some conidia were coarsely verrucose. The isolate was sent, with clinical details only, to a fungal reference laboratory. It was identified as a Ulocladium species on morphological grounds. Based on the morphological features and molecular identification, the patient was diagnosed as Ulocladium atrum keratitis. He was successfully cured by oral sporanox and topical use of travogen cream. Diagnosis was done by culture isolation, histopathological examination. The main interest of this case is the rareness of Ulocladium atrum keratitis in an healthy individual and we also want to emphasize the good response to therapy.

      • Pitch Autopilot Design for Agile Missiles with Uncertain Aerodynamic Coefficients

        <P>The work presented here proposes two pitch autopilot designs for agile missiles which experience high pitch angles or angles of attack (AoA) during their manoeuvres. One great challenge pertaining to high AoA is that aerodynamic coefficients cannot be precisely predicted in a conventional wind tunnel environment. To directly address this challenge, the proposed autopilot designs (based on the integrator backstepping and H<SUB>∞</SUB>-norm minimization) do not require precise knowledge of such aerodynamic coefficients and so has a strong practical merit. Simulation results clearly testify this practical merit.</P>

        <P>This brief presents two area-efficient drivers for a 50-Omega terminated optical modulator. Driver 1 adopts a double cascode with dynamic biasing that enables sufficient high-speed operation owing to the high transition frequency of the thin-oxide transistor. Therefore, it does not require area-consuming additional peaking inductors. A custom-designed shared inductor is used in Driver 2 for bandwidth enhancement with a small area penalty; the required total inductance is only 13.3% of the conventional shunt peaking case. The prototypes are fabricated in a 65-nm complementary metal-oxide-semiconductor process. Electrical measurement results show that Driver 1 exhibits a differential output swing of 5 V-PP, a data rate of 20 Gb/s, and a power consumption of 534 mW. The measured performance of Driver 2 is 3.8 V-PP and 348 mW at 25 Gb/s. The active areas of the proposed modulator drivers are only 0.068 mm(2) and 0.038 mm(2).</P>

        Aerodynamic pitch control design for reversal of missile’s flight direction

        <P>This paper proposes a purely aerodynamic pitch control design scheme for agile missiles that need to reverse their flight directions for tracking a target. The main challenge in performing this kind of manoeuvre is that the missiles may enter the high angle of attack domain in which aerodynamic actuation is ineffective. Assuming that reliable aerodynamic data are available for the flight envelope, a systematic search algorithm is employed to find an aerodynamic control parameter and the motor thrust activation time, so as to enable the missile's required manoeuvre. The strength of the proposed design scheme is that no third control mechanism such as thrust vectoring is required for the missile control even in the high angle of attack domain. The proposed design scheme is tested on aerodynamic data available in literature, and is shown to be promising via simulations.</P>

      • Static Output Feedback Stabilization of Interconnected Systems

        This paper is concerned with the celebrated static output feedback control problem (SOFP) subject to linear constraints on control input K, e.g. bounds on control magnitude, zeros in some elements of K, etc. These constraints typically arise in the control of resource-limited systems interconnected to each other, where the local control for each system makes use of its own and neighbouring systems’ outputs only, and its magnitude is bounded. This control problem can be approached by a spectral-norm minimization technique (q-SNM), whose preliminary version was previously introduced with little mathematical justification but shown to be promising for the regular SOFP without control constraints. This paper mathematically justifies q-SNM by showing its explicit relationship with spectral radius, and extends q-SNM to accommodate various linear control constraints.

        노인 혈액투석 환자의 약물순응도 및 관련 요인 연구

        Purpose: The purpose of this study was to investigate medication adherence and related factors in elderly hemodialysis patients. Method: The convenience samples were used in 123 elderly hemodialysis patients who were being treated in seven outpatient hemodialysis rooms in Seoul and Kyonggi-do. Data collection took place from October 7th 2010 to November 12th 2010. The structured questionnaires included content of medication adherence, expectation for medication, resistance to medication, knowledge about medication, and attitude toward medication. The data were analyzed using SPSS version 17.0. Specifically, the mean, standard deviation, Chi-square test, Fisher’s exact test were used. Result: There was a statistically significant difference between medication adherence and drug discomfort(χ<sup>2</sup>=5.297, p=.026). Conclusion: Nurses caring for the elderly hemodialysis patients should take into account the patient’s drug discomfort to increase the patient’s medication adherence.
