        The purposes of this study was subjected a scheme to improve technical highschcol “2+lsystem” curriculum. The method of the study for carrying out the study is as follows. First, the realities of management were analyzed through the preceding study related with technical highschool “2+lsystem” and the plan of school education of technical highschool “2+1 system” management school. Second, the conference of the teacher in charge of technical highschool management school and the person in charge of the department related with Education City and Province Hall was held. Third, the question search was practiced to all school subjects in the whole country through the electronic document with the help of ministry of education & human resources development (N=930). The effect of the study is as follows. First, the thing which is formed in the schools and industries of technical highschool “2+lsystem” curriculum formation must be made to manage according to the seventh curriculum. Second, the field practice of the present type must be made to carry out in technical highschool as well during the third grade (mid term, final term). Third, the curriculum formation by system improvement and the completed unit (216 unit) are drawn up according to the seventh curriculum.

      • Transplantation of autologous perichondrium with amniotic membrane for progressive scleral necrosis

        <P><B>Abstract</B></P> <P><B>Purpose</B></P> <P>Scleral necrosis with severe ischemia is refractory to conventional treatment because of avascular progressive necrosis. We assessed the therapeutic efficacy and safety of autologous perichondrium transplantation in patients with progressive scleral necrosis (PSN) and analyzed the clinical effects.</P> <P><B>Methods</B></P> <P>This study was a prospective, interventional, and noncomparative case series. Reconstructive surgery using autologous perichondrium and amniotic membrane (AM) was performed in patients with PSN who showed progressive ischemic scleral melting with impending perforation state and/or broad avascular area larger than 10 mm in diameter. The primary outcome was restoration of scleral integrity with healthy vascularized epithelium over the graft at six months after surgery. The secondary outcome was complication rate associated with autologous perichondrium graft use.</P> <P><B>Results</B></P> <P>Eighteen eyes of 14 patients underwent reconstructive surgery using autologous perichondrium patch and AM grafts. Observations indicated the graft provided the eyeball with successful structural integrity in 17 out of the 18 cases (94.4%) at six months after surgery. One eye showed a small scleral defect due to wound dehiscence at four month after the surgery. Additional surgery using perichondrium and AM stabilized the eye. The scleral necrosis healed completely after perichondrium and AM transplantation, even in cases with full-thickness scleral defect. The scleral integrity was maintained until the last follow-up session. There were no serious complications of endophthalmitis or graft infection.</P> <P><B>Conclusions</B></P> <P>Reconstructive surgery using autologous perichondrium and AM is an effective method for restoration of scleral integrity and vascularization of the episclera and conjunctiva in eyes with PSN. Therefore, autologous perichondrium can be considered as an appropriate new biologic tissue for PSN.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Progressive scleral necrosis is refractory to conventional therapy because of its broad ischemic nature. </LI> <LI> Revascularization and epithelialization over the scleral defect are essential for healing the scleral ischemia and ulceration. </LI> <LI> Perichondrium autograft is new effective therapy for progressive scleral necrosis, even for full-thickness scleral necrosis. </LI> </UL> </P>

        Background: Reactive oxygen species (ROS) is associated with cancer progression such as cell proliferation and survival in cancers including melanoma, and also affect a transcription factor, Specificity protein (Sp1). The flavonoid morin is known to inhibit growth of cancer cells such as lung and breast cancer. For these reasons, we hypothesized that morin may inhibit cancer activities in melanoma by altering ROS generation. Objectives: The aim of this study is to figure out the effects and mechanisms of morin in melanoma cell line, G361 cells. Methods: The effects of morin on cell proliferation and apoptosis were determined using standardized assays. The changes in pro-apoptotic and anti-apoptotic proteins were analysed by western blots. The level of cellular ROS was evaluated by measuring DCF-DA fluorescence and rhodamine-123 fluorescence intensities. Results: We found that morin treatment induced ROS levels and apoptosis, as signified by increased proportion of cells with Annexin V-PE(+) staining and a sub-G0/G1 peak in the cell cycle, and this was accompanied by downregulation of Sp1, Mcl-1, bcl-2 and upregulation of cleaved caspase-3, Bax and PUMA in western blot. Conclusion: Our data suggest that morin may contribute to treatment of melanoma by inducing cell apoptosis through regulating ROS, pro-apoptotic and anti-apoptotic proteins such as sp1 and Mcl-1.

        CO2 레이저 사용시 마취관리에 관한 임상적 고찰

        The first laser was developed by Maiman in 1960 using a ruby crystal as an active medium. The word laser is an acronym for Light Amplification by Stimulated Emission of Radiation and this light beam can be focused to a small spot, resulting in precisely controlled coagulation, incision, or vaporization of tissue. The two types of lasers commonly used for airway surgery are the carbon dioxide laser and the neodymium-yttrium-aluminum-garnet (Nd YAG) laser. The light of the former will easily vaporize tissue and can be used to make a precise incision with minimal damage to adjacent tissue and the light of the latter can be conducted through fiberoptic instruments and permits successful endoscopic resection of untreatable recurrent or persistent malignant diseases of the major airways. Airway and endotracheal tube fire are the most feared hazard during laser surgery of the airway. The risk of fire depends on the nature of the tube material, the gaseous milieu, the beam wattage, and its mode of operation. The surgeon should use the laser intermittently at moderate wattage (15W) in a pulsed mode to prevent excessive heat field buildup and tissue dessication. For safe anesthetic management, nitrous oxide should be avoided, and a mixture of oxygen (25%) and air should be used or helium, which is a known flammability quencher may be used during airway laser surgery at a 60 percent He 40 percent 0, mix. Due to the increase in CO₂laser treatments, we reviewed the clinical records of 150 patients in whom CO₂laser surgery was performed and the anesthetic management was evaluated from April 1987 to December 1988. 1) A total of 150 patients were evaluated, including 81 males and 69 females ranging in age from 10 months to 74 years and averaging 41 years. 2) The operations performed were vocal nodule (26.0%), vocal polyp (16.0%), laryngeal papilloma (16.0%), etc. 3) The methods for keeping the airway open during general anesthesia were orotracheal intubation (67.5%) and intubation through the tracheostomy stoma (32.5%). 4) Thmain anesthetic agents were halothane, enflurane, and fentanyl. The anesthesia and surgery lasted 60.9+-24.0minutes and 44.5+-28.5 minutes on average respectively. 5) Laryngoscopic laser surgery was carried out in 127 patients (84.7%), bronchoscopic surgery in 14 patients (9.3%) and surgery using a handpiece in only 8 patients (6.0%). 6) Complications were found in 3 cases, including endotracheal cuff ignition and pneumomedias-timum.

        Naringenin Derivative Diethyl (5,4'-dihydroxy flavanone-7-yl) Phosphate Inhibits Cell Growth and Induces Apoptosis in A549 Human Lung Cancer Cells

        Anti-cancer effects of naringenin derivative diethyl (5,4'-dihydroxy flavanone-7-yl) phosphate were evaluated in human lung cancer cells. The effect of diethyl (5,4'-dihydroxy flavanone-7-yl) phosphate (dEdHF-7-p) on A549 cell viability was measured using MTS assay and cell counting. Morphological changes were detected using phase-contrast microscopy. Apoptosis was analyzed using Hoechst staining. The influence of dEdHF-7-p on cell cycle distribution was determined using propidium iodide (PI) staining, and protein expression was determined by Western blot analysis. A newly synthesized naringenin derivative dEdHF-7-p suppressed cell growth of A549 though mechanisms including inhibition of cell cycle and increased apoptosis. Apoptotic and cell cycle modulators were changed by dEdHF-7-p in A549 cells; cyclins, ppRB, and anti-apoptotic factor Bcl-2 were down-regulated, whereas apoptotic factor Bax and cyclin-dependent kinase inhibitors p21 and p53 were enhanced, thereby releasing cytochrome c into the cytosol of dEdHF-7-p-treated-A549 cells. dEdHF-7-p treatment processed caspases-3/-8/-9 and cleavage of poly ADP-ribose polymerase. The dEdHF-7-p treatment enhanced Fas expression and decreased expression of cell survival factors such as PI3K and p-Akt in a dose-dependent manner. Taken together, dEdHF-7-p induces apoptosis by inhibiting the PI3K/Akt survival signaling pathway and modulating mitochondria-emanated intrinsic and Fas extrinsic pathways in A549 cells.
