      • 여성에서 Dehydroepiandrosterone sulfate와 인슐린저항성증후군의 연관성

        연구배경: DHEA는 안드로겐 전구체로서 연령 증가에 따라 감소한다. 남성에서 DHEA는 인슐린저항성 및 심혈관질환에 대한 보호 작용을 하는 것으로 알려져 있으나 여성에서는 인슐린저항성과 심혈관질환에 대한 DHEA의 역할은 정립되어 있지 않다. 본 연구는 여성에서 인슐린저항성과 심혈관질환에서 DHEA의 역할을 규명하고자 시행하였다. 방법: 지역사회에서 무작위 추출된 471명의 여성을 대상으로 DHEA보다 반감기가 길고 일중 변동이 적은 것으로 알려져 있는 DHEAS의 농도를 방사면역측정법으로 측정하였다. DHEAS 농도와 인슐린저항성증후군 및 이 증후군을 구성하는 인자들과의 연관성을 관찰하였다. 결과: 1. 인슐린저항성증후군 인자의 빈도는 비만 25.3%, 내당능장애 8.5%, 고혈압 21.9%, 이상지질혈증 6.2%이었으며 인슐린저항성증후군의 빈도는 16.5%이었다. 2. DHEAS는 연령(r=-0.47, p<0.001), 수축기혈압(r=-0.18, p<0.001), 이완기혈압(r=-0.10, p<0.05), 공복혈당(r=-0.10, p<0.05), 포도당부하 2시간 혈당(r=-0.12, p<0.01), 및 중성지방(r=-0.16, p<0.01)과 의미있는 음의 상관관계가 있었다. 3. DHEAS 농도를 사분위수로 나눈 후 연령을 보정한 후 인슐린 저항성증후군 구성요소의 빈도를 관찰하였을 때 DHEAS 농도 감소에 따라 고혈압의 빈도가 유의하게 증가하였다.(p<0.05). 4. 연령(p<0.0001) 및 체질량지수(p<0.05)가 DHEAS에 영향을 주는 인자로 작용하였다. 5. 연령을 보정한 후 낮은 DHEAS농도는 인슐린저항성증후군의 위험인자로 작용하지 않았다. 결론: 여성에서 혈청 DHEAS 농도는 혈압, 혈당 및 혈청 지질 농도와 음의 상관관계가 있었으며, 연령을 보정한 후 DHEAS 농도가 감소함에 따라 고혈압의 빈도가 증가하는 것으로 보아 최소한 DHEAS는 인슐린저항성증후군에 대해 위해 작용을 가지지는 않는 것으로 생각된다. 그러나 DHEAS의 인슐린저항성증후군 및 심혈관질환에 대한 보호 작용을 증명하기 위해서는 이들 대상자를 전향적으로 추적 관찰하여 인슐린저항성증후군 및 심혈관질환의 발생과 DHEAS의 관계를 규명해야 할 것이다. Background: Dehydroepiandosterone (DHEA) is an androgen precursor, and is known to be decreased by the aging process. DHEA has been known to have a protective effect on insulin resistance and cardiovascular disease in men, but remains controversial in women. The aim of this study was to elucidate the role of DHEA on insulin resistance, and the risk for cardiovascular disease, in women. Methods: We analyzed the relationship between DHEA sulfate (DHEAS), known to have a longer half-life and less diurnal variation than DHEA, and insulin resistance syndrome (IRS) in 471 non-diabetic women from an urban community diabetes prevalence study. Serum DHEAS concentrations were measured using a commercially available radioimmunoassay kit. Results: 1. the frequencies of obesity, impaired glucose tolerance, hypertension and dyslipidemia were 25.3, 8.5, 21.9 and 6.2%, respectively, and the frequency of IRS was 16.5%. 2. DHEAS was significantly inversely correlated with age (r=-0.47, p<0.001), systolic blood pressure (r=-0.18, p<0.001), diastolic blood pressure (r=-0.10, p<0.05), fasting serum glucose (r=-0.10, p<0.05), postchllenge 2 hour glucose (r=-0.12, p<0.01) and triglycerides (r=-0.16, p<0.01). 3. As serum DHEAS concentrations, by quartiles, were decreased, the age-adjusted frequency of hypertension was significantly increase (p<0.05). 4. A Multiple linear regression analysis revealed that DHEAS was significantly associated with age (p<0.0001) and BMI (p<0.05). 5. A Logistic regression analysis showed that DHEAS was not associated with IRS after adjustment for age. Conclusion: DHEAS is inversely associated with age. DHEAS has no harmful effect, and may even have a protective role, on insulin resistance syndrome. Prospective examinations of DHEAS and insulin resistance syndrome in women are needed to confirm the mechanism for the association between DHEAS and the development of cardiovascular disease (J Kor Soc Endocrinol 17:675∼684, 2002).

      • 임부의 연령에 따른 보완대체요법에 관한 인식 태도 경험 비교 연구

        Purpose: This study aims to test if the age of pregnant women affects the perception, experience, and attitude towards complementary alternative therapy (CAT), by investigating the perception, experience, and attitude of pregnant women under the age of 35, and above 35 (Advanced Maternal Age). The study will 1) find the general characteristics of pregnant women under and over the age of 35 2) compare the perception, behavior, and experience of pregnant women under and over the age of 35, and 3) find the comparison of the perception, behavior, and experience according to the general characteristics of pregnant women under and over the age of 35. Method: The subjects of this study are composed of 44 women under the age of 35 and 44 women above the age of 35, totaling 88 pregnant women visiting midwiferies, obstetrics and gynecology, and baby fairs in Seoul and Gyeong-Gi area. Data for this study was collected from November 28, 2015 to January 6, 2016 for 40 days. The collected data analyzed mean, standard deviation, t test, ANOVA, and chi-square by using SPSS 23.0. Result Results show that there is a significant difference in the perception, attitude, and experience of the two-sample groups in the use of CAT. The question of “what is most necessary when applying CAT in a hospital clinical environment” for the perception criteria came out with the results of t=9.980, p<.05. For the attitude, results showed that the answers of pregnant women under the age of 35 displayed a more positive attitude towards CAT. (t=2.421, p<.05) Lastly, in the experience section, pregnant women under the age of 35 showed more CAT use than the other sample group. Conclusion Results have shown that there is a difference in the perception, behavior, and experience between the two sample groups. Therefore further research on the efficacy and the adverse effects of CAT needs to be done.

        Bortezomib과 Dexamthasone으로 치료한 골수외 형질세포종 4예

        Despite the use of aggressive local and systemic treatment including autologous stem cell transplantation in multiple myeloma, extramedullary recurrences are common and the prognosis of these patients is poor. Many novel drugs such as thalidomide, lenalidomide and bortezomib improve the response of treatment of multiple myeloma, but some reports failed to describe thalidomide has effect in extramedullary plasmacytoma. Recent data report on the successful treatment plasmacytomas with bortezomib in patients with advanced multiple myeloma. We treated 4 relapsed or refractory extramedullary plasmacytomas with bortezomib at our institution. We recognized all these extramedullary plasmacytomas decreased and showed more than partial response. This report lends support to the efficacy of bortezomib in the treatment of plasmacytoma and describes the safe use of bortezomib. Responses may, however, be of short duration. Therefore, despite our limited experience, we propose that bortezomib may be considered a therapeutic option for such patients who have risk of radiation therapy

        Actinomycosis is a chronic suppurative infection, whitch infiltrates mucosa-associated tissues. Thoracic, pelvic and cervicofacial infections of actinomyces are not uncommon, but endobronchial actinomycosis is rarely reported. We report a case of a 29 year old man who presented with a recurrent pneumonia and chronic cough. Fiberoptic bronchoscopy revealed occlusion of the right. lower medio-basal segment bronchus with exophytic endobronchial mass. The diagnosis was confirmed histologically by demonstration of sulfur granules in the bronchoscopic biopsy of the mass. Intravenous administration of amoxacillin and clindamycin followed by oral amoxacillin and clindamycin therapy resulted in complete recovery.

        Seroprevalence of SARS-CoV-2 antibodies in the community based on participants in the 2020 Korea National Health and Nutrition Examination Survey

        OBJECTIVES: The Korea National Health and Nutrition Examination Survey (KNHANES) is a nationwide cross-sectional surveillance system that assesses the health and nutritional status of the Korean population. To evaluate the occurrence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in the community, we investigated the prevalence of antiSARS-CoV-2 antibodies in the sera of KNHANES participants. METHODS: Subjects were recruited between April 24 and December 12, 2020. In total, 5,284 subjects aged 10-90 years from 17 regions participated. SARS-CoV-2 antibodies were screened using the Elecsys Anti-SARS-CoV-2 assay. Positive samples were verified using 4 different SARS-CoV-2 antibody assays and the plaque reduction neutralizing test. The final seropositivity criteria were a positive screening test and at least 1 positive result from the 5 additional tests. RESULTS: Almost half (49.2%; 2,600/5,284) of participants were from metropolitan areas, 48.9% were middle-aged (40-69 years), and 20.5% were in their 20s or younger. The seropositivity rate was 0.09% (5/5,284). Three of the 5 antibody-positive subjects had a history of infection, of whom 2 were infected abroad and 1 was infected in a local cluster outbreak. CONCLUSIONS: The low SARS-CoV-2 antibody seroprevalence in Korea indicates that there have been few coronavirus disease 2019 (COVID-19) cases due to successful COVID-19 management measures (e.g., diagnostic tests for overseas arrivals, national social distancing, and strict quarantine measures). Moreover, asymptomatic infections were uncommon due to active polymerase chain reaction testing. However, hidden infections may exist in the community, requiring the continuation of quarantine and vaccination measures.

        Prevalence of Atopic Dermatitis among Korean Adults Visiting Health Service Center of the Catholic Medical Center in Seoul Metropolitan Area, Korea

        <P>The point prevalence of atopic dermatitis (AD) among Korean adults visiting the Health Service Center for health check-ups was assessed. AD was diagnosed based on the questionnaires filled by patients and through physical examination by dermatologists. A total of 3,563 persons completed the questionnaires, and 2,032 persons were examined by the dermatologists. According to the questionnaires, the prevalence of AD was 7.1%, and from the dermatologists' examination, it was 2.6% overall, which decreased with age significantly. The prevalence in men was higher than that of women. Grading the severity of AD according to their eczema area and severity index scores, 70.6% were classified as being mild, 25.5% moderate, and 3.9% severe. Interestingly, concomitance of psoriasis and AD was found in 0.5% of those examined by the dermatologists. Our results suggest that AD is one of the most common skin diseases not only in children but also in adults in Korea.</P>

      • Extent and Divergence of Heteroplasmy of the DNA Barcoding Region in Anapodisma miramae (Orthoptera: Acrididae)

        A partial sequence of the mitochondrial cytochrome oxidase subunit I (COI) gene is widely used as a molecular marker for species identification in animals, also termed a DNA barcode. However, the presence of more than one sequence type in a single individual, also known as heteroplasmy, is one of the shortcomings of barcode identification. In this study, we examined the extent and divergence of COI heteroplasmy, including nuclear-encoded mitochondrial pseudogenes (NUMTs), at the genomic-DNA level from 13 insect species, including four individuals of orthopteran Anapodisma miramae. Furthermore, a long fragment of mitochondrial DNA (~13.5 kb) and cDNA from A. miramae were used as a template for COI PCR to compare the patterns of heteroplasmy between DNA sources and to investigate a possible way to avoid ambiguity in DNA barcoding. When multiple numbers of clones originated from genomic DNA were sequenced, heteroplasmy was prevalent in all species (3~16 heteroplasmic copies), with a varying degree of maximum sequence divergence (<1% in 7 species, <4% in 3 species, <6% in 2 species and 2.15-8.03% in four A. miramae individuals). In five species, NUMTs also were observed when genomic DNA was used as a template. Long fragment DNA also is a source of heteroplasmic amplification, but the divergent haplotypes and NUMTs obtained in the genomic DNA-based PCR were not detected in A. miramae. On the other hand, cDNA was heteroplasmy-free, without NUMTs when multiple numbers of clones were sequenced. Consistently, one dominant haplotype was always obtained from the genomic DNA-origin clones in all species and also from the long fragment- and cDNA-origin clones of A. miramae. Furthermore, the dominant haplotype was identical in sequence, regardless of the DNA source. Thus, one possible solution to avoid the barcoding problem in relationship to heteroplasmy could be the acquisition of multiple numbers of barcoding sequences to determine a dominant haplotype that can be assigned as barcoding sequence for a given species.

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>We investigated the efficacy and safety of fulvestrant plus goserelin (F + G) versus anastrozole plus goserelin (A + G) in comparison with goserelin (G) alone in premenopausal women with hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2-), tamoxifen-pretreated metastatic breast cancer (MBC).</P> <P><B>Patients and methods</B></P> <P>In this multicentre, open-label, randomised phase II study, premenopausal women aged ≥18 years with HR+, HER2–, tamoxifen-pretreated MBC were randomly assigned (1:1:1) to F + G, A + G or G alone. The primary end-point was time to progression (TTP). Secondary end-points included overall survival, overall response rate, clinical benefit rate and toxicity.</P> <P><B>Results</B></P> <P>Of 138 eligible patients, 44 were randomly assigned to receive F + G, 47 to A + G and 47 to G alone. The median follow-up duration was 32.2 months (interquartile range: 23.69–40.86) and the median age was 43.0 years (range 23.0–55.0). The median TTP was 16.3 months (95% confidence interval [CI] 7.5–25.1) for F + G, 14.5 months (95% CI 11.0–18.0) for A + G and 13.5 months (95% CI 10.3–16.8) for G alone. Compared with G alone, the hazard ratios were 0.608 for F + G (95% CI, 0.370–0.998; p = 0.049) and 0.982 for A + G (95% CI, 0.624–1.546; p = 0.937). In terms of visceral metastasis, a stratification factor, there were no TTP differences according to treatment arm. Grade III or IV toxicities were rarely observed. Of the common adverse events, grade I arthralgia and joint stiffness were more frequently observed in the F + G than in the A + G or G-alone groups (p < 0.05, respectively).</P> <P><B>Conclusions</B></P> <P>F + G provides a promising new option for the treatment of premenopausal women with HR+, HER2-, tamoxifen-pretreated MBC.</P> <P><B>Trial registration</B></P> <P>ClinicalTrials.gov number NCT01266213 and Korean Cancer Study Group (KCSG) Breast cancer protocol number BR10-04.</P> <P><B>Highlights</B></P> <P> <UL> <LI> In premenopausal women with hormone receptor-positive metastatic breast cancer, hormone treatment similar to that used in postmenopausal women is the standard treatment option after tamoxifen treatment failure. </LI> <LI> In this study, fulvestrant plus goserelin (G) has better clinical outcome than G alone in premenopausal women, especially those younger than 40 years. </LI> <LI> Aromatase inhibitor with G is not superior to G alone in tamoxifen-pretreated premenopausal women. </LI> </UL> </P>

      • Phylogeny of Apis with the description of newly sequenced mitochondrial genomes of Apis species (Hymenoptera: Apidae)

        We sequenced complete mitochondrial genomes (mt genomes) of four Apis species, such as A. cerana, A. dorsata, A. laboriosa, and A. mellifera ligustica. The A. m. ligustica used in this study was originated from United States and selected for a high hygienic behavior during several years of breeding program. Considering current phylogenetic hypothesis of Apis the positive relationships between phylogeny and gene rearrangement was not supported. Phylogenetic reconstruction using available Apoidea species has shown the relationships ((Apidae + Colletidae) + Crabronidae). Within Apis, a strong support for three groups was obtained: A. florea and A. andreniformis as a basal group to the other A. cerana and A. mellifera group and A. dorsata and A. laboriosa group. The test to find out suitable single mt gene for the phylogeny at the genus level and below-genus level of Apis provided an identical topology from ND4L and ND6 in the ML analysis and ND3 and NF4L in the BI analysis to that from whole mt genome, with relatively strong support, indicating that these individual genes can potentially be utilized for within-species level phylogeny of Apis.
