The purpose of this study is to examine educational needs for mothers of premature infants and to develop educational multimedia for mothers of premature and high risk infants. This research was designed as a descriptive study. Data were collected for four months from April 2002 to July 2002. One hundred and one subjects in one general hospital in Seoul participated in the study. Measurement tools used in this study was educational needs scale developed by Cho. Kyoul Ja et at. It asks mothers to rate each item on a four-point Likert type scale. The collected data were analyzed using SPSS win 8.0. All variables were listed as frequency, mean, standard deviation and percentiles. The results were as follows : The educational needs of the mothers of primi pregnancy were higher than the mothers of multi pregnancy. The educational needs on management of diseases such as prevention of infection, symptoms of disease, mental development, attachment promotion, congenital metabolism test, management of convulsion, care of vomiting and fever were higher than general care of infants such as immunization, measurement of temperature, hiccough care, follow up care. From the results above, I developed the educational multimedia for premature and high risk infant care. I think it will help to increase understanding of parents and to reduce stress level of mothers of premature and high risk infants.

Aim(s): This study aims to develop and evaluate the problem-based learning (PBL) simulation program for nursing students. Four times of PBL group sessions were applied before the simulation so that the student participants can pre-learn about simulation context and contents. The simulation program reflected home visiting healthcare programs for vulnerable households by public health centers in Korea. Method(s): The scenario was developed for students to provide comprehensive nursing care for the frail elderly in the community as a home visiting nurse in the public health center. The draft was developed by the researchers and revised after experts’ reviews (3 academic experts and 2 clinical experts). We measured the effect quantitatively and qualitatively. A total of 29 students completed survey questionnaires after the simulations. Additionally, 2 focus group interviews were conducted with 10 students who participated in the program. Verbatim transcripts were analyzed using Giorgi’s phenomenological approach. Result(s): The Simulation Design Scale mean scores were in the order of feedback/guided reflection (4.81 ± 0.40), problem-solving (4.76 ± 0.41), fidelity (4.76 ± 0.46), objectives and information (4.59 ± 0.58) and support (4.48 ± 0.50). The Educational Practices in Simulation Scale mean scores were in the order of active learning (4.78 ± 0.35), collaboration (4.76 ± 0.53), diverse ways of learning (4.71 ± 0.54), and high expectation (4.69 ± 0.56). Three major themes emerged from the interview: immersive learning experiences, enhancing nursing competency, and changes in perspectives on nursing. Conclusion(s): This PBL simulation module was effective to inspire active learning in the student participants. The program was evaluated as a systematic learning process where students could be self-directed learners interacting and collaborating with other colleagues, instructors, and environments. It also provided opportunities to obtain holistic view of nursing, communication skills, and confidence. More diverse PBL simulations need to be developed in nursing education.
Caspase-8 controls the secretion of inflammatory lysyl-tRNA synthetase in exosomes from cancer cells

<P>Aminoacyl-tRNA synthetases (ARSs), enzymes that normally control protein synthesis, can be secreted and have different activities in the extracellular space, but the mechanism of their secretion is not understood. This study describes the secretion route of the ARS lysyl-tRNA synthetase (KRS) and how this process is regulated by caspase activity, which has been implicated in the unconventional secretion of other proteins. We show that KRS is secreted from colorectal carcinoma cells within the lumen of exosomes that can trigger an inflammatory response. Caspase-8 cleaved the N-terminal of KRS, thus exposing a PDZ-binding motif located in the C terminus of KRS. Syntenin bound to the exposed PDZ-binding motif of KRS and facilitated the exosomic secretion of KRS dissociated from the multi-tRNA synthetase complex. KRS-containing exosomes released by cancer cells induced macrophage migration, and their secretion of TNF-α and cleaved KRS made a significant contribution to these activities, which suggests a novel mechanism by which caspase-8 may promote inflammation.</P>
Effects of a low-FODMAP enteral formula on diarrhea on patients in the intensive care unit

BACKGROUND/OBJECTIVES: A dietary restriction on the intake of fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAPs) has been reported to be effective in the treatment of gastrointestinal (GI) tract complications. Enteral nutrition (EN) is widely used for patients who cannot obtain their nutritional requirements orally, but many studies have reported EN complications, especially diarrhea, in up to 50% of patients. SUBJECTS/METHODS: We performed a single-center, non-randomized, controlled trial to determine the effects of a low-FODMAP enteral formula on GI complications in patients in intensive care units (ICUs). Patients in the ICU who needed EN (n = 66) were alternately assigned to the low-FODMAP group (n = 33) or the high-FODMAP group (n = 33). RESULTS: Anthropometric and biochemical parameters were measured, and stool assessment was performed using King"s Stool Chart. We excluded patients who received laxatives, GI motility agents, proton pump inhibitors, antifungal agents, and antibiotics other than β-lactams. There were no differences in GI symptoms during 7 days of intervention, including bowel sound, abdominal distension, and vomiting between the 2 groups. However, diarrhea was more frequent in the high-FODMAP group (7/33 patients) than the low- FODMAP group (1/33 patients) (P = 0.044). CONCLUSIONS: Our results suggest that a low-FODMAP enteral formula may be a practical therapeutic approach for patients who exhibit enteral formula complications. Our study warrants further randomized clinical trials and multicenter trials.
The effect of neutral-surface iron oxide nanoparticles on cellular uptake and signaling pathways

<P>In recent years, iron oxide nanoparticles (IONPs) have been applied widely to biomedical fields. However, the relationship between the physicochemical properties of IONPs and their biological behavior is not fully understood yet. We prepared 3-methacryloxypropyltrimethoxysilane (MPS)-coated IONPs, which have a neutral hydrophobic surface, and compared their biological behavior to that of Resovist (ferucarbotran), a commercialized IONP formulation modified with carboxymethyl dextran. The rate of MPS-IONP uptake by human aortic endothelial cells (HAoECs) was higher than ferucarbotran uptake, indicating that the neutral hydrophobic nature of MPS-IONPs allowed them to be absorbed more readily through the plasma membrane. However, the signaling pathways activated by MPS-IONPs and ferucarbotran were comparable, suggesting that surface charge is not a key factor for inducing changes in HAoECs. In vivo fate analysis showed that MPS-IONPs accumulated for longer periods in tissues than hydrophilic ferucarbotran. These findings could enlarge our understanding of NP behavior for advanced applications in the biomedical field.</P>

Objective Enhancing subjective well-being is an effective way to improve mental health. This study aimed to validate a virtual realitybased interactive feedback program as an intervention tool for promoting subjective well-being. Methods Thirty-six males participated in this program, consisting of three tasks constructed based on the theories of positive psychology: ‘Experience-based problem recognition task’, ‘Future self-based success story expression task’, and ‘Strength expression task’. Participants rated visual analog scores associated with each of the tasks’ contents. The concurrent validity of task scores was evaluated by correlations with the psychological scale scores. Results The total task score was positively correlated with scores of Mental Health Continuum-Short Form (MHC-SF) emotional wellbeing and psychological well-being, Rosenberg’s Self-Esteem Scale, Dispositional Hope Scale agency dimension and pathways dimension, and Life Orientation Test Revised, but not with MHC-SF social well-being scores. After controlling for the effects of the other task scores, the task scores had linear relationships with certain psychological assessments. Conclusion Since the task scores are closely related to indicators of well-being, self-esteem, hope, and optimism, the program contents are well associated with certain aspects of subjective well-being and thus may be available for training that improves subjective well-being through interactive feedback.
Effect of a Parasite Game on the Hand-Washing Behavior of Children with Disabilities

Self-hygiene is considered one of the most essential self-management skills necessary for children with disabilities to acquire and perform independently. With this in mind, we developed an Android-based mobile application intended to teach children how to eliminate parasites. To assess its effectiveness, three children with developmental disabilities were tested prior to and after playing the game. The results indicated that the children’s knowledge of health behaviors and health-related self-efficacy improved after playing the game. Implications for health education and management are discussed.
