Assessing clinical reasoning abilities of medical students using clinical performance examination

Purpose: The purpose of this study is to investigate the reliability and validity of new clinical performance examination (CPX) for assessing clinical reasoning skills and evaluating clinical reasoning ability of the students. Methods: Third-year medical school students (n=313) in Busan-Gyeongnam consortium in 2014 were included in the study. One of 12 stations was developed to assess clinical reasoning abilities. The scenario and checklists of the station were revised by six experts. Chief complaint of the case was rhinorrhea, accompanied by fever, headache, and vomiting. Checklists focused on identifying of the main problem and systematic approach to the problem. Students interviewed the patient and recorded subjective and objective findings, assessments, plans (SOAP) note for 15 minutes. Two professors assessed students simultaneously. We performed statistical analysis on their scores and survey. Results: The Cronbach α of subject station was 0.878 and Cohen κ coefficient between graders was 0.785. Students agreed on CPX as an adequate tool to evaluate students’ performance, but some graders argued that the CPX failed to secure its validity due to their lack of understanding the case. One hundred eight students (34.5%) identified essential problem early and only 58 (18.5%) performed systematic history taking and physical examination. One hundred seventy-three of them (55.3%) communicated correct diagnosis with the patient. Most of them had trouble in writing SOAP notes. Conclusion: To gain reliability and validity, interrater agreement should be secured. Students clinical reasoning skills were not enough. Students need to be trained on problem identification, reasoning skills and accurate record-keeping.

<P>Upon screening synthetic small molecule libraries with the infectious hepatitis C virus (HCV) cell culture system, we identified a benzothiazepinecarboxamide (BTC) scaffold that inhibits HCV. A structure-activity relationship (SAR) study with BTC5 was performed, and modifications that led to nanomolar antiviral activity and improved the selective index (CC50/EC50) by more than 1000-fold were identified. In addition, a pharmacophore modeling study determined that the tricyclic core and positive charge on the piperidine moiety were essential for antiviral activity. Furthermore, we demonstrated that BTC interferes with HCV glycoprotein E1/E2-mediated viral entry and the generation of infectious virions by using HCV pseudoparticle and cell culture supernatant transfer assays, respectively. BTC showed potent antiviral activity against HCV genotype 2 (EC50 = 0.01 +/- 0.01 mu M), but was less potent against a genotype 1/2 chimeric virus (EC50 = 2.71 +/- 0.05 mu M), which expressed the structural proteins of HCV genotype 1. Objectives: The purpose of this study was to suggest methods to investigate continuous monitoring of concentration levels and assess the exposure of individuals considering the actual time activity of residents for formaldehyde and particulate matter (PM10, PM2.5) in the indoor and outdoor air of a house, assess the health risks of children and adults based on the results of the exposure assessment, and provide basic data on studies for assessing exposure and health risks in Korea in the future. Methods: The concentration levels of formaldehyde and particulate matter were measured in a family home in Gyeonggi-do Province from April 25 to July 31, 2019, using electrochemical sensors (formaldehyde) and light scattering sensors (PM10, PM2.5). Risk assessment by the duration of exposure by time activity was performed by dividing between weekdays and weekends, and indoors and outdoors. Results: The greatest level of carcinogenic risk from inhaling formaldehyde was indoors during the weekdays for both children and adults. For children, the risk was at 7.5 per approximately 10,000 people, and for adults, the risk was at 4.1 per approximately 10,000 people. PM10 and PM2.5 also showed the greatest values indoors during the weekdays, with children at 1.7 people and 1.4 per approximately 100 people, respectively, and adults at 8.2 per approximately 1,000 and 1.8 per approximately 100 people, respectively. Conclusions: The risks of formaldehyde, PM10 and PM2.5 were shown to be high indoors. Therefore, consideration of exposure assesment for each indoor pollutant and management of indoor air quality is necessary.

Background and Objects : The environment of hospital have been changed rapidly. To adapt changes, it is needed to boost customer satisfaction. This study aims to investigate satisfaction of customer satisfaction program participants Method : After the one week customer satisfaction program was completed, we surveyed 50 participants. The questionnaire method was used in this study. The questionnaires item were sex, age, department, satisfaction, positive change, necessity and interval in customer satisfaction program. Results : The major results of the analysis were as follows Positive change after customer satisfaction program were 6.90±1.94. Necessity further customer satisfaction program were 7.74±2.09. Conclusions : Customer program should be needed and keep teaching to the staff.

Objectives : The study was aimed to survey participant's characters in stroke prevention & Gigong program Methods : The data were collected 34 participants in stroke prevention & Gigong program. The questionnaires to survey characters in stroke prevention & Gigong program composed demographic item, past history, family history, stroke presymptoms, etiology, prevention, why join of Gigong program, etc. Results : Half of participants didn't know anything presymptoms, prevention for stroke. Conclusion : From these results, stroke education program have been improvement and it is needed further study to survey efficacy to stroke education program.
