In acute pulmonary embolism (PE), circulatory failure and systemic hypotension are important clinically for predicting poor prognosis. While pulmonary artery (PA) clot loads can be an indicator of the severity of current episode of PE or treatment effectiveness, they may not be used directly as an indicator of right ventricular (RV) failure or patient death. In other words, pulmonary vascular resistance or patient prognosis may not be determined only with mechanical obstruction of PAs and their branches by intravascular clot loads on computed tomography pulmonary angiography (CTPA), but determined also with vasoactive amines, reflex PA vasoconstriction, and systemic arterial hypoxemia occurring during acute PE. Large RV diameter with RV/left ventricle (LV) ratio > 1.0 and/or the presence of occlusive clot and pulmonary infarction on initial CTPA, and clinically determined high baseline PA pressure and RV dysfunction are independent predictors of oncoming chronic thromboembolic pulmonary hypertension (CTEPH). In this pictorial review, authors aimed to demonstrate clinical and serial CTPA features in patients with acute massive and submassive PE and to disclose acute CTPA and clinical features that are related to the prediction of oncoming CTEPH.

<P><B>Abstract</B></P><P>Gold(I)‐catalyzed intramolecular 6‐<I>endo</I>‐<I>dig</I> cyclization of <I>tert</I>‐butyl ynoates afforded α‐pyrone cores of violapyrones. Moreover, this reaction was successfully applied to the stereospecific syntheses of (+)‐ and (–)‐violapyrone C, which allowed the absolute configuration of natural (+)‐violapyrone C to be assigned by comparison of the optical rotations. This first total synthesis, which proceeded in 22 % yield over 10 steps from (<I>S</I>)‐(–)‐2‐methylbutanol, features silver(I) oxide promoted monobenzylation of 1,4‐butanediol, Wittig olefination, Claisen condensation, Corey–Fuchs reaction, and gold(I)‐catalyzed α‐pyrone synthesis.</P>

<P>The strongly correlated thermoelectric properties have been a major hurdle for high-performance thermoelectric energy conversion. One possible approach to avoid such correlation is to suppress phonon transport by scattering at the surface of confined nanowire structures. However, phonon characteristic lengths are broad in crystalline solids, which makes nanowires insufficient to fully suppress heat transport. Here, we employed Si–Ge alloy as well as nanowire structures to maximize the depletion of heat-carrying phonons. This results in a thermal conductivity as low as ∼1.2 W/m-K at 450 K, showing a large thermoelectric figure-of-merit (ZT) of ∼0.46 compared with those of SiGe bulks and even ZT over 2 at 800 K theoretically. All thermoelectric properties were “simultaneously” measured from the same nanowires to facilitate accurate ZT measurements. Spatial Multiplexing of OFDM Signals With QPSK Modulation Over ESPAR

<P>An electronically steerable parasitic array radiator (ESPAR) transmitter, employing one active and one parasitic antenna element, which can simultaneously transmit two orthogonal frequency division multiplexing (OFDM) signals with quadrature phase shift keying (QPSK) modulation via spatial multiplexing, is developed. In particular, a minimum mean square error (MMSE) scheme for quantizing the parasitic load impedance is developed to reduce the number of distinct load impedance values that should be realized for spatial multiplexing over ESPAR. To this end, the probability density function of the parasitic load impedance is derived, and the realizable region of the load impedance is analyzed. It is observed that the resistance of the active antenna element should be large enough to transmit OFDM signals. The characteristics of two existing ESPAR antennas with different mutual coupling matrices are examined, and it is observed that the one with the larger active antenna resistance is suitable for OFDM transmission. The bit error rate (BER) and the receive error vector magnitude performances of the proposed ESPAR systems with 6/8-bit quantizers and zero-forcing (ZF) receiver are compared with those of the corresponding ideal multiple-input multiple-output (MIMO) system through computer simulation. The results demonstrate that the performances of the proposed ESPAR with the 8-bit quantizer and the ideal MIMO systems can be almost identical, whereas the proposed ESPAR with the 6-bit quantizer exhibits some performance loss.</P>

Objective-The Korean Brain Aging Study for the Early Diagnosis and Prediction of Alzheimer’s disease (KBASE) aimed to recruit 650 individuals, aged from 20 to 90 years, to search for new biomarkers of Alzheimer’s disease (AD) and to investigate how multi-faceted lifetime experiences and bodily changes contribute to the brain changes or brain pathologies related to the AD process. Methods-All participants received comprehensive clinical and neuropsychological evaluations, multi-modal brain imaging, including magnetic resonance imaging, magnetic resonance angiography, [11C]Pittsburgh compound B-positron emission tomography (PET), and [18F]fluorodeoxyglucose-PET, blood and genetic marker analyses at baseline, and a subset of participants underwent actigraph monitoring and completed a sleep diary. Participants are to be followed annually with clinical and neuropsychological assessments, and biannually with the full KBASE assessment, including neuroimaging and laboratory tests. Results-As of March 2017, in total, 758 individuals had volunteered for this study. Among them, in total, 591 participants-291 cognitively normal (CN) old-aged individuals, 74 CN young- and middle-aged individuals, 139 individuals with mild cognitive impairment (MCI), and 87 individuals with AD dementia (ADD)-were enrolled at baseline, after excluding 162 individuals. A subset of participants (n=275) underwent actigraph monitoring. Conclusion-The KBASE cohort is a prospective, longitudinal cohort study that recruited participants with a wide age range and a wide distribution of cognitive status (CN, MCI, and ADD) and it has several strengths in its design and methodologies. Details of the recruitment, study methodology, and baseline sample characteristics are described in this paper.
