A case of ulcerative colitis presenting with cerebral venous thrombosis

Patients with inflammatory bowel disease (IBD) have been reported to have an increased risk of thromboembolism. Cerebral venous thrombosis (CVT) is a rare but serious extraintestinal manifestation of IBD. Due to its highly variable manifestation and low incidence, CVT is not usually readily recognized by physicians. Herein, we report a case of a 35-year-old male presenting with CVT associated with ulcerative colitis (UC). The patient was admitted with chief complaints of bloody diarrhea that had started 3 days prior. Sigmoidoscopy showed hyperemic and edematous mucosa, friability, and shallow ulcers from the sigmoid colon to the rectum suggestive of IBD. Three days later, the patient started complaining of a headache, and gradually developed a decreased level of consciousness. Magnetic resonance imaging of the brain revealed CVT with hemorrhagic infarctions. An angiogram was obtained to evaluate the extent of CVT, and anticoagulation therapy was initiated with intravenous heparin. During hospitalization, he was diagnosed with UC and treated with 5-aminosalicylic acid. After discharge, the patient was recovered without neurological deficit, and remission of UC was also obtained. The presence of headache or acute worsening of neurological status in a patient with IBD should alert the health professionals about the possibility of CVT. (Intest Res 2018;16:306-311)
Gastrocolocutaneous Fistula: An Unusual Case of Gastrostomy Tube Malfunction with Diarrhea

A gastrocolocutaneous fistula is a rare complication of percutaneous endoscopic gastrostomy (PEG). We report a case of agastrocolocutaneous fistula presenting with intractable diarrhea and gastrostomy tube malfunction. A 62-year-old woman with ahistory of multiple system atrophy was referred to us because of PEG tube malfunction. Twenty days prior to presentation, the patientstarted developing sudden diarrhea within minutes after starting PEG feeding. Fluoroscopy revealed that the balloon of the PEG tubewas located in the lumen of the transverse colon with the contrast material filling the colon. Subsequently, the PEG tube was removedand the opening of the gastric site was endoscopically closed using hemoclips. Clinicians should be aware of gastrocolocutaneousfistula as one of the complications of PEG insertion. Sudden onset of diarrhea, immediately after PEG feedings, might suggest thiscomplication, which can be effectively treated with endoscopic closure.

MPAs(Massively Parallel Architectures) should address two fundamental issues for scalability: synchronization and communication latency. Dataflow architecture faces problems of excessive synchronization overhead and inefficient execution of sequential programs while they offer the ability to exploit massive parallelism inherent in programs. In contrast, MPAs. based on von Neumann computational model may suffer from inefficient synchronization mechanism and communication latency. DAVRID(DAtaflow/Von Neumann RISe hybrID) is a massively parallel multithreaded architecture which takes advantages of von Neumann and dataflow models. It has good single thread performance as well as tolerates synchronization and communication latency. In this paper, we describe the DAVRID architecture in detail and evaluate its performance through simulation runs over several benchmarks.

<P>Electrical contacts between the nanomaterial and metal electrodes are of crucial importance both from fundamental and practical points of view. We have systematically compared the influence of contact properties by dc and EIS (Electrochemical impedance spectroscopy) techniques at various temperatures and environmental atmospheres (N<SUB>2</SUB> and 1% O<SUB>2</SUB>). Electrical behaviors are sensitive to the variation of Schottky barriers, while the activation energy (<I>E</I><SUB>a</SUB>) depends on the donor states in the nanowire rather than on the Schottky contact. Equivalent circuits in terms of dc and EIS analyses could be modeled by Schottky diodes connected with a series resistance and parallel RC circuits, respectively. 이동 가입자(mobile station)에 대한 위치정보의 신속한 처리는 이동 통신망에 있어 필수적이다. 특히, 서울과 같은 대도시 지역에서의 이동 가입자의 빈번한 위치 이동은 트래픽의 폭주를 낳을 수 있기 때문에 위치정보의 신속한 처리를 위해 위치 등록기(location register)의 적절한 위치 선정이 중요하다. 본 논문에서는 도로를 중심으로한 정방형 형태의 도심지에 있어 이동 가입자의 트래픽 이동 형태를 반영한 위치 등록 발생률의 분석과 하나의 위치 등록기에 수용 가능한 위치 영역(location area)수의 산정과 이에 따른 위치 등록 발생률을 분석하였다. 분석 결과 본 연구에서 제안된 방법은 대도시 밀집지역에 있어 위치 등록기의 설치 위치 선정과 위치 등록 발생률의 예측에 매우 유용함을 알 수 있었다.

Objectives: Endoscopic submucosal dissection (ESD) is a widely used approach for the resection of superficial gastric neoplastic lesions. Endoscopic mucosal resection (EMR) is acceptable for lesions <10–15 mm in size. Herein, we compared the clinical outcomes of ESD with those of EMR after circumferential precutting (EMR-P) for gastric adenomas ≤15 mm. Methods: We retrospectively analyzed the data of 213 patients with 228 gastric adenomas ≤15 mm in size who were treated endoscopically at a single tertiary hospital in Korea between November 2018 and October 2022. We evaluated the complete endoscopic resection rate, recurrence rate, procedure-related complications, and procedure time according to the procedure used. Results: Among the 228 gastric adenomas, 49 were treated with EMR-P and 179 with ESD. The histological complete resection rate was higher in the ESD group than in the EMR-P group (87% vs. 57%, p<0.001). No significant between-group differences were observed in endoscopic en bloc resection rates (ESD vs. EMR-P, 96% vs. 90%; p=0.081). The procedure time was significantly shorter in the EMR-P group than in the ESD group (28.9±19.7 min vs. 8.8±5.9 min, p<0.001). The local recurrence rate in patients with histologically incomplete resection did not differ between the two groups (ESD vs. EMR-P, 8.7% vs. 9.5%; p=0.924). Conclusions: For gastric adenomas ≤15 mm, EMR-P may be the preferable treatment method considering the en bloc resection rate, procedure time, and local recurrence rate. However, considering the complete resection rate, ESD is recommended as the treatment of choice for high-grade adenomas and early gastric cancer lesions.
