Background: Psoriasis is a chronic T17 cells-driven immune-mediated inflammatory disease. However, psoriasis patients may have elevated serum total immunoglobulin E (IgE), which is a hallmark of Th2 inflammation. In previous case reports, patients with psoriasis with high serum IgE levels did not respond well to treatment or exacerbated eczema lesions. Objectives: We sought to investigate the clinical characteristics of psoriasis patients with high serum total IgE levels. Methods: This is a retrospective chart review of 130 patients with plaque psoriasis who had done serum total IgE level test from November 1, 2009, to October 31, 2019. We compared the demographics, clinical characteristics, severity of the disease, and treatment regimen for each high IgE group (>214 U/mL), and normal IgE group (≤214 U/mL). Results: Among 130 patients with psoriasis, 41 patients (31.5%) had high serum IgE level. High IgE levels were positively associated with the severity of disease and psoriasis lesions including scalp, hands, feet, and flexures which are difficult-to-treat were observed significantly higher in patients with high serum IgE level. There was no correlation between age, gender, disease duration, or presence of pruritus, and serum total IgE level. Conclusion: High serum IgE levels in psoriasis may be a prognostic factor which is related with more severe and difficult-to-treat traits requiring systemic treatment.

A few studies strongly suggest that pernicious anemia is a risk factor for osteoporosis, but the mechanism is unknown. Recently we experienced a 65-year-old female with pernicious anemia and osteoporosis. She had multiple compression fractrues of thoracolumbar vertebrae and low bone mineral density of spine and femur by dual photon absorptiometry. Pernicious anemia was confirmed with low serum vitamin B12 level and characteristic bone marrow findings. She did not have other secondary causes of ostoeporosis other than pernicious anemia. Although we co not believe pernicous anemia is solely responsible for osteoporosis of this patient, it must have contributed some to the development of the disease. In summary; pernicious anemia is a risk factor for osteoporosis and optimal evaluation and management of bone loss should be done in patients with pernicious anemia, especially in postmenopausal state(J Kor Soc Endocrinol 8:351~355, 1993).

In this study, we analyzed the annual heating and cooling energy requirements of reference building with 97 certificated windows using TRNSYS 17, to review the applicability of the “Window Energy Consumption Efficiency Rating System” to the office building. The analyzed model was a medium-size office building with lightweight curtain walls and a window-to-wall ratio of 52%. The results are as follow : First, the annual total thermal load was 60,261 to 111,616 kWh and the ratio of heating load to cooling load was 1:9 showing that cooling was the main factor of the thermal load. Second, the total thermal load by SHGC showing an almost perfect linear relationship. The F-test significance probability in the regression model was 0.000 meaning statistical significance and the coefficient of determination was 0.9656 showing high correlation. In contrast, the F-test significance probability in the regression model by U-value was 0.00484 showing statistical significance but the coefficient of determination was 0.0805 showing low correlation. Third, the SHGC of window was the main factor of the total thermal load in the office building. It showed the limitation of subject in “Window Energy Consumption Efficiency Rating System” which was limited to insulation performance and airtight performance. Thus, in to evaluate the window sets in terms of energy saving, the “Window Energy Consumption Efficiency Rating System” with SHGC should be considered.
Clinical Application of Proteomics in Skin Diseases

The term proteome is rooted from the combination of two words; ``protein`` and ``ome``. It refers to the total proteins that are expressed by the entire genome. The proteomics, therefore, is the study of the total proteins. About 20,000 genes in human are transcribed into 100,000 RNAs, and these RNAs are translated into 1,000,000 proteins. Then the proteins undergo posttranslational modifications, such as phosphorylation and glycosylation, to gain their proper functions in our body. Therefore, the proteomics can reflect the functional aspects of the molecular biology better than the genomics and the transcriptomics, the study of DNAs and RNAs. The recent developments in the proteomics have led to the increased application of its concept not only in the fundamental molecular biology but also in the clinical medicine, including the identification of the biomarkers or investigation of pathogenesis in certain diseases. Our paper is a review for the topics that should be considered when performing the proteomics, and its application in studying the dermatologic diseases.

Although topical steroid is the first choice of drug in many skin diseases, some patients, such as prurigo nodularis (PN) patients, do not sufficiently respond to strong steroids, possibly due to low penetration rate of drugs in thickened skin. This study was performed to investigate if applying biodegradable microneedle (MN) patch after topical steroid increases the penetration of topical steroids and treatment efficacy in PN patients. The penetration rate was measured by Franz diffusion cell system using 3D skin. Patients who have bilateral symmetrical PN lesions were included. Both right and left sides were treated with topical steroid twice a day and only right side was applied with biodegradable MN patch after topical steroid once a week. The penetration of topical steroid was significantly higher in biodegradable MN applied 3D skin equivalent than non-applied 3D skin equivalent. Area and height of PN decreased after 4 weeks of treatment in both sides; however, biodegradable MN patch applied side showed more decrease in area and height of PN compared to non-applied side (p < 0.05). Visual analog scale was also significantly lower in biodegradable MN applied side (p < 0.05). Our results demonstrated that applying biodegradable MN patch after the use of topical steroid can enhance the penetration of topical steroid and increase the treatment efficacy. Therefore we suggest that this novel patch can be used efficiently in PN treatment.
Case Report : A Case of a Cutaneous Angiomyolipoma

A cutaneous angiomyolipoma, which is also known as a cutaneous angiolipoleiomyoma, is a rare benign mesenchymal tumor. Only 18 cases have been reported in the English literature. We describe a case of an angiomyolipoma presenting on the right ear helix of a 26-year-old female. The histopathologic examination revealed a typical form of an angiomyolipoma with a proliferation of mature adipocytes. As with all previously reported cases, our patient did not present with the stigmata of tuberous sclerosis. This is the 20th reported case of cutaneous angiomyolipoma and the 3rd reported case in Korea. (Ann Dermatol 21(2) 217~220, 2009)
Case Report : A Case of Hand-foot-mouth Disease in an Immunocompetent Adult

Hand-foot-mouth (HFM) disease is primarily a disease of children, although it can be seen in immunocompromised adults. We describe a case of HFM disease in a 35-year-old immunocompetent male. He presented with multiple, 1~2 mm sized, erythematous vesicular papules on both hands and feet. Histopathological findings of a skin biopsy showed superficial perivascular lymphocytic infiltration, focal keratinocyte necrosis, and reticular degeneration. Because delayed diagnosis can cause spread of the disease to children, dermatologists should be aware that HFM disease can occur in adults with intact immune systems.
