        최근 발병한 정신분열병 환자의 증상 관해 및 정신사회적 기능 : 1년 전향적 추적 관찰 연구

        Objectives : The long-term outcome of schizophrenia is still considered vahable and inconclusive. We perfonned a one-year prospective observational study to investigate the longitudinal outcomes in patients with recent-onset schizophrenia. The primary purpose was to determine the descriptive outcomes in tenns of symptom remission and psychosocial function. The secondary purpose was to identify predictor variables associated with the outcomes. Methods : Patients experiencing their first episode ofpsychosis or hospitalization within the past 2 years with a diagnosis of DSM-IV schizophrenia were included. Clinical symptoms were assessed monthly using Brief Psychiatric Rating Scale (BPRS), Scale for the Assessment of Positive Symptoms (SAPS), Scale for the Assessment of Negative Symptoms (SANS) and Clinical Global Impression (CGI) scale. Psychosocial function was measured using PSYCH-UP (Psychosocial symptoms you currently have, follow-up version) and Heinrichs' Quality of Life Scale (QLS) at baseline, 6, and 12 months and Global Assessment of Functioning (GAF) monthly. Remission was defined on the basis of overall, and psychotic core symptoms as having 1) BPRS total score ≤ 36, and 2) Each individual item score of core psychotic items (conceptual disorganization, suspiciousness, hal-lucinatory behavior, unusual thought content) ≤3, and 3) CGI severity score ≤3, and 4) Duration of remission at least for 2 consecutive months. To identify the predictive variables for the outcome, relationships between time to remission and various clinical variables were examined using Cox proportional hazards model. Results : A total of 49 patients with schizophrenia were enrolled in this study. The proportions of patients with symptom re-mission were 54.5% (24/44, drop outs=5) at 3 months, 55.3% (21/38, drop outs=11) at 6 months, and 64.3% (18/28, drop outs=21) at 12 months. Mean time to remission was 3.76±2.43 (s.d.) months, and the mean duration of remission was 7.54±2.99 months. Significantly longer time to remission was predicted by higher SANS total score at baseline in various predictor variables (p=0.01). Impairments of psychosocial function measured using PSYCH-UP and QOL were significantly improved at the end of this study, but still remained at mild to moderate level. SANS total score and GAF score significantly correlated with most ofthe items related to psychosocial fimction at 12 months. The proportion of the patients with GAF score ≥60 increased from 6.1% at baseline to 85.8% at 12 months. Of the patients who were in remission at 12 months, 95.5% obtained GAF score ≥60. Conclusion : This study showed 64.3% of symptom remission rate in patients with recent-onset schizophrenia. Symptom re-mission was accompanied by significant improvement ofglobal function. The severity of negative symptom at baseline appeared to be a significant predictor for time to remission. Psychosocial function was improved at the end of this study, but impainnents still remained at mild to moderate level. GAF score and negative symptoms significantly correlated with psychosocial function.

        Numerical Modelling of Temperature Distribution and Pressure Drop through the Layered Burden Loading in a Blast Furnace

        Analysis of the internal state of the blast furnace is necessary to predict and to control the operating conditions. Especially, it is important to develop models of the blast furnace to predict the cohesive zone because shape of the cohesive zone influences overall operating conditions of blast furnace such as gas flow, chemical reactions and temperature. Because many previous blast furnace models have assumed cohesive zone to be fixed, it was not possible to evaluate the shape change of cohesive zone in relation with operating conditions such as PCR, blast condition, and production rate. In this study, an axi-symmetric 2-dimensional steady state model is proposed to simulate blast furnace processes. In this model, cohesive zone is determined by the solid temperature. Finite volume method is employed for numerical simulation. To find location of the cohesive zone, entire calculation procedure is iterated until converged. Through this approach, shape of the cohesive zone, velocity and temperature within the furnace are predicted from the model.

        Analysis of the internal state of the blast furnace is needed to predict and control the operating condition. In previous study, generalized concept of solid fuel combustion was introduced for analysis of iron-making processes such as coking process and iron ore sintering process. Blast furnace would be modeled by similar approach with counter-current moving bed reactor. In this study, an axi-symmetric 2-dimensional steady state model is proposed to simulate blast furnace process. In this model, two phases such as solid (coke and ore) and gas are considered. Layer structure and shape of deadman are assumed to be fixed. Finite volume method is used for numerical simulation. Because CZ is changed by solid temperature range, whole calculation procedure is iterated until CZ is converged. Through this approach, shape of CZ, velocity, composition and temperature within the furnace are predicted by model.

      • A Numerical Analysis of the Thermo-fluid Flow Change in a Blast Furnace Due to Distribution of the Burden

        A blast furnace can be modeled in a similar approach using a counter-current bed reactor. In the present study, an axi-symmetric two-dimensional steady state model is proposed to simulate the blast furnace process. It is assumed that only two phases (solid and gas phases) exist in the furnace. The charging material is treated as porous media constructed by coke and ore layers. The internal gas flow of the furnace is controlled by burden distribution owing to the different permeability of the two layers. For an understanding of the influence of the burden distribution on the operating conditions, the entire layer structure is predicted from the measured top layer structure and the solid flow is assumed according to the potential flow. Using this burden distribution, the flow, energy, and chemical species conservation equations are considered for each phase. In addition, the mass generation/consumption according to the reaction, heat transfer between the gas and the solid phases, and the reaction heat are reflected by source terms in the governing equations. For several Lo/Lc (layer thickness ratio of ore and coke layer) cases, layer structures are constructed and numerical simulations are conducted. The finite volume method was used for the numerical simulations. Through this approach, the flow, composition and temperature within the furnace are predicted by the model.

        <P>In a previous study, we demonstrated that pcDNA3.1/hNIS (human sodium iodide symporter) vaccination generated hNIS-associated CD8+IFN-gamma+ (interferon-gamma) T cells, which are known to be involved in antitumor immunity. However, the immune response induced was insufficient to control tumor growth in vivo, which required a novel approach to potentiate hNIS vaccination effects. In the present study, we administered 131I radioiodine therapy prior to hNIS vaccination in CT26/hNIS tumor-bearing mice to facilitate the vaccine-induced immune response. We characterized hNIS-associated cytotoxic T-cell immune response and the antitumor effects induced by this 131I + hNIS combination therapy. The survival rates of CT26/hNIS tumor cells were significantly reduced by 131I treatment compared with the parental CT26 cells in vitro. 131I + hNIS combination therapy stably suppressed tumor growth below or near the original tumor size level of initial treatment, achieving 100% survival rates. Specifically, 131I + hNIS therapy enhanced IFN-gamma production, hNIS-associated antitumor cytotoxic T-lymphocyte (CTL) response, and induced more dendritic cells but reduced T-regulatory cells in tumor masses. Collectively, these results suggest that combined therapy effectively enhances hNIS-associated antitumor immune response, leading to CT26/hNIS tumor growth inhibition and complete survival in Balb/C mice. These findings provide a novel and effective means of treating cancer.</P>

      • 냉온수배관을 이용한 여름딸기 국소냉난방기술 개발

        본 연구는 여름철에 수경재배베드에 의한 딸기 생산을 위하여 근권부 및 관부를 냉수배관을 이용하여 냉방시험을 하였으며 근권부, 근권부와 관부, 관부냉방에 대한 세가지 요인으로 시험하였다. 지름이 16mm인 PE관을 냉수배관으로 설치하였으며 지하수 20L/min 를 순환시켜 오전 9시부터 오후 7시까지 국소부 냉방을 연속적으로 실시하였다. 지하수의 수온은 14.8℃이고 베드상부 20cm 의 온실내부온도가 35.0.℃ 인 경우 근권부냉방에 의하여 근권부의 온도가 20.0 ~ 21.0℃를 유지하였고 그중에서도 근권부 및 관부를 동시에 냉각시키는 베드의 근권부온도가 20.0℃로 가장 낮은 것으로 나타났다. 관부냉각인 경우 근권부와 관부를 동시에 냉각시키는 베드상부에서 2cm, 10cm, 20cm의 온도를 측정한 결과 33.5, 34.5, 35.0℃ 로 나타났으나 지속적인 모니터링과 수확량 조사 등 냉방효과에 대한 보다 정밀한 분석이 필요하다 하겠다. This study wast carried out to investigate the effects of zone cooling in cultivating strawberry in a nutriculture bed during summer by installing cold water pipe in root zone and around the crown area. The zone cooling for summer strawberry was tested by three cases which were cooling root zone, root zone plus crown area and crown area. Polyethylene cold water pipe with external diameter of 16mm and groundwater with a temperature of 14.8℃ were used for the cooling system. Zone cooling circulation was continuously performed at a flowrate of 20 L/min from 9:00 a.m. to 7:00 p.m. When air temperature at 20cm above the nutriculturebed was 35.0℃, the temperatures of root zone in three cases were maintained at 20.0 ~ 21.0 ℃ and the root zone temperature of root zone plus crown cooling bed was the lowest at 20℃. In zone cooling of crown area, air temperatures measured at 2cm, 10cm, 20cm above the nutriculturebed were 33.5℃, 34.5℃, 35.0 ℃ at the root zone plus crown cooling bed. However, ongoing monitoring and research including more precise analysis for cooling effect are necessary afterward.

      • 냉수배관을 이용한 여름딸기재배의 국소냉방효과

        여름딸기 고설재배시 고온기에 대한 국소부 냉방효과를 평가하기 위해 강원도 평창 여름딸기 온실에 국소냉난방시스템을 설치하고 적용하였다. 냉수의 온도는 공기대물 히트펌프에 의해 15~20℃.에서 조절되었으며 냉수배관을 근권, 근권과 관부, 관부에 설치하고 세가지 요인으로 시험하였다. 지름이 16mm인 백색연질PE관을 관부에 설치하였으며 근권부에는 같은 크기의 스테인레스 주름관을 설치하였다. 냉수의 순환량은 생장부는 400L/hr 를 순환시켰으며 근권부는 240L/hr 로 8월에 주야간 구분없이 계속 순환시켰다. 효과분석을 위하여 베드별 생산량을 조사하였으며 관부와 근권부를 동시에 냉방한 베드가 대조구에 비해 37% 증가율을 보였고 생장부를 냉방한 베드가 34%, 근권부만을 한 것이 20%의 증가율을 보였다. 근권부 및 관부를 동시에 냉각시킨 베드의 근권부 온도는 17~24℃로 유지되었고 관부의 온도는 18~29℃ 사이에서 유지되었다. The effects of spot cooling on growing strawberry in a nutriculture bed during summer by spot cooling system was estimated at the summer strawberry plastic greenhouse in Pyeongchang Gangwondo. The temperature of cooling water was controllded by air to water heat pump and maintained at the range of 15~20℃. Cooling pipes were installed in root zone and put that very close to crown spot. Spot cooling effect was tested in three cases which were cooling root zone, root zone plus crown spot and crown spot only. White low density polyethylene pipe in external diameter of 16mm was installed on crown spot, and Stainless steel flexible pipe in same size was installed in root zone. Crown zone cooling water circulation was continuously performed at a flowrate of 400 L/hr and Root zone was 240L/hr all day long. Strawberry production per bed was surveyed during August. Production increasing rate of root zone plus crown cooling bed was 37% and crown cooling bed was 34% and Root zone cooling bed was 20%. The temperatures of root zone in root zone plus crown cooling bed was maintained at 17.0 ~ 25.0 ℃ and crown spot was maintanied at 18~29℃ during August 2013.
