        부산·울산·경남지역 직업병 감시체계

        Objective: Occupational medicine specialists in the Busan, Ulsan and Kyung-Nam areas established an area-based occupational disease surveillance system and used this system to collected case information for the purpose of preventing occupational diseases Methods: l l hospital participated in this system. The authors selected five main diseases, which were hand-arm vibration syndrome (HAVS), work-related musculoskeletal disorder (WRMSD), occupational asthma, occupational skin disease and occupational and occupational lung cancer and established their case definitions. All cases were reported on the web, and real time analysis was conducted. Results: Between April 2001 and April 2003, 192 cases of HAVS, 118 cases of WRMSD, 33 cases of occupational asthma, 17 cases of occupational skin disease and 17 cases of occupational lung cancer (for a total of 377 cases of these five main diseases) were reported. most of the HAVS cases came from shipbuilding, and 172 of them (92.7%) were associated with grinding. Of the four main types of WRMSD, carpal tunnel syndrome (CTS) was the most prevalent with 46 cases and shipbuilding was also the main industry involved (83.9%). The ergonomic risk factors involved mainly associated with the hand. In 19 (57.6%) and 4 (12.1%) cases of occupational asthma, the agents involved. The causative agents of occupational lung cancer included Cr, welding fume, PAH, Ni, etc. 10(58.8%) of the cases were diagnosed as squamous cell carcinoma and 5(29.4%) as adenoma. Conclusion: This result showed that an area-based occupational disease surveillance system might provide an effective method of evaluating the prevalence of such diseases, however the Busan, Ulsan and Kyung-Nam provinces are too large to be treated as individual areas. Therefore, the authors suggest that each province should establish its own surveillance system.

      • 조혈모세포이식 후 발생한 주폐포자층 폐렴에 대한 고찰

        Background : Pneumocytitis cainii pneumonia (PCP) can occur in immunocompromised hosts especially such as AIDS or cancer patients. Although recent research had focused on PCP in AIDS patients, few studies have described the clinical presentations of PCP in recipients of stem cell transplantation (SCT). We evaluated the clinical manifestations of PCP in SCT patients admitted at St. Mary's hospital, Seoul, Korea. Methods : The medical records of 17 PCP patients undergoing SCT between Feb. 1998 and Feb. 2000 were reviewed. The diagnosis of PCP was confirmed through the demonstration of Pneumocytitis cainii via either cytology of brochoalveolar lavage (BAL) or histological technique of lung biopsy. CMV disease and CMV infection were confirmed by BAL culture and antigenemia respectively . Results : Seventeen patients were all recipients of allogeneic SCT and 7 of 17 patients were performed non-sibling SCT. Patients presented with symptoms including brief period (4 ∼23 days) of fever (76%), dyspnea (70%), cough (64%), and signs such as rare(58.8%), Sixteen patients (94%) had been receiving immunosuppressive agent such as cyclosporine A (64%) or Fk506 (35%) without PCP prophylaxis. Eleven patients (64%) were treated with corticosteroid with mean dose of 16 mg/day prednisolone and mean duration of 4.6 months after post-SCT period. Twelve patients were co-infected with CMV. Another co-infected miCroorganisms were Pseudomonas aeruginosa, Mycobacterium tuberculosis, herpes simplex virus, parainfluenza virus, Average duration of treatment with trimethoprim-sulfamethoxazole (TMP/SMX) was 21 ±9 days. Four patients died, and three of them were related with PCP. Conclusion : PCP developed frequently in patients who were taking immunosuppressive drug due to graft versus host disease or were not taking TMP/SMX prophylaxis. High risk patients showing fever, cough, or dyspnea should be considered to take early bronchoscopic intervention for detection of PCP. When treat for PCP, it also be considered to the possibility of coinfection such as CMV. (Korean J Infect Dis 33:273∼279, 2001)

      • The LC-MS/MS Based Multi-omics Analysis to Investigate Clostridioides difficile Infection Using in vitro Coculture Device, The Mimetic Intestinal Host-Microbe Interaction Coculture System, MIMICS

        Clostridioides difficile is a gram-positive bacterium and is considered as an enteric pathogen. C. difficile colonizes the host with intestinal dysbiosis and produces virulence factors, Toxin A and B. Although the cytotoxicity of C. difficile has been well elucidated, the exact pathogenic mechanisms and biological pathways of C. difficile infection (CDI) are not yet clear. Here, we developed a CDI model using MIMICS to investigate C. difficile-host interactions throughout the early (12 hr) and later (48 hr) stages of infection. We also apply an LC-MS/MS-based multi-omics approach to study the physiological changes in CDI-induced gut epithelial cells at the molecular and cellular levels. The proteomic analysis confirmed that proteins involved in cellular responses such as stress, immune responses and DNA damage responses are upregulated at early stages of infection. In addition, a representative CDI phenotype has been identified in the late stage of the infection. Our in vitro CDI model and multi-omics approach not only provide a better understanding of host-anaerobic pathogen interactions but may also aid in the treatment of C. difficile and other anaerobic infections.

      • LC-MS/MS Based Multi-omics Approaches for Prebiotic Inulin Utilization on Faecalibacterium prausnitzii

        The human gut commensal bacteria Faecalibacterium prausnitzii is well known for its anti-inflammatory effects that improve host intestinal health. Although several studies reported that inulin, one of the well-known prebiotics, increases the abundance of F. prausnitzii in the intestine, the mechanism under this effect of inulin remains unclear. Here, we applied LC-MS/MS-based multi-omics approaches to confirm the effects of inulin on F. prausnitzii. Interestingly, the proteomic analysis revealed that the putative proteins involved in sucrose utilization of F. prausnitzii are upregulated in the presence of inulin. To investigate the function of the proteins, we cloned the target genes, and observed the ability for sucrose degradation. In addition, we demonstrated that the sucrose degradation activity in F. prausnitzii culture media is enhanced by using inulin as a carbon source compared to glucose. Taken together, this study suggests that the reduction of sucrose induced by inulin could potentially improve host health and prevent gut dysbiosis like type 2 diabetes which is associated with sucrose.

        Questionnaire-based Survey of Demographic and Clinical Characteristics, Health Behaviors, and Mental Health of Young Korean Adults with Early-Onset Diabetes

        Background: The incidence of early-onset diabetes is increasing among young adults. However, there are limited data on the characteristics and management of young Korean adults with diabetes. This study assessed the clinical and demographic characteristics, health behaviors, and mental health among young Korean adults with diabetes mellitus. Methods: This cross-sectional study included young Korean adults with diabetes (n = 225) with an onset age of 20–39 years from four university hospitals. Demographic characteristics, management of diabetes, and mental health were assessed using a questionnaire survey. Results: Type 2 diabetes was the most common type (73.3%), and 13.8% of participants were classified as other types or unknown. Approximately, 64.7% of participants had a strong family history of diabetes, and 76% had treatment within three months of diagnosis. Approximately, 11.1% of participants had diabetic complications; 39.1% of participants received insulin injections, including oral anti-diabetic medications. Additionally, 30.4% were smokers, and only 28% had active physical activity; 26.5% of participants had >3 hours of screen time. One third of participants never had breakfast, and 60.5% went out to eat at least three times a week. Half of the participants showed moderate to severe stress perception, and 21.4% of patients had moderate to severe levels of depression based on the Korean version of Beck Depression Inventory score. Conclusion: Early-onset diabetes was associated with a strong family history and early insulin treatment. Young adults with diabetes had poor health behaviors and frequent mental depression. These findings suggest the necessity of health policies for improving health behaviors and mental distress.

        소수민족문학사 서술의 전략

        ‘문학사 쓰기’가 상상으로서 민족 혹은 국가를 실체로 파악하도록 추동하는 근대적 기획이라는 점에서, 조선족문학사 서술 역시 ‘조선족’이라는 특수한 공동체가 자기정립을 해나가는 과정으로서 이해할 필요가 있다. 더불어 ‘중국조선족’ 이라는 자기인식이 중국이라는 국가를 구성하는 하나의 소수민족(국민)으로서의 자기와 조선민족이라는 기원적 자기의 길항속에서 중첩적으로 구성되는 것과 마찬가지로, ‘중국조선족문학사’는 중국문학사의 부분으로서 자기를 정립해가는 동시에 남북한문학사라는 타자와의 동질과 차이를 구현함으로써 자기를 만들어가는 방식이 된다. 이는 문학사 서술의 행위 속에 자기정체성 정립의 욕망이 기입되어 있음을 의미한다. 이 글은 이러한 인식적 전제를 바탕으로 중국조선족문학사 서술의 특수성을 자기정립의 욕망을 실현하는 전략으로 파악하고 그 구체상을 들여다보았다. 특히, 2007년 발간된 오상순 외의 『중국조선족문학사』에 주목하였다. 최초의 조선족문학사인 조성일 외의 『중국조선족문학사』를 참조항으로 삼으면서도 자기인식의 수정을 시도하는 자기성찰적 서술태도를 가지고 있기 때문이다. 이 문학사를 기점으로 조선족문학의 특수성은 ‘디아스포라’로 규정되고 있으며, 이는 조선족문학의 존재론적 위기를 극복하는 기재로 작용하면서 현재적 의미를 획득하고 있다. It is necessary to understand the narrative throughout the history of Korean Chinese literature as a process by which a specific community of minorities identified as ‘Korean Chinese’ is establishing its identity wherein writing a history of literature is in and of itself a modern project to promote the recognition of an ‘imagined nation’ as a real nation. Just as the self-awareness of ‘Korean Chinese’ ethnicity is constructed in a superimposed manner whereby antagonism manifests in the conflict to identify oneself based on one’s origin from the nation of Korea or the current state of being a part of a minority nation constituting the state of China, Korean Chinese literary history has also been established by categorizing itself as being a part of Chinese literary history and by revealing the commonalities and differences compared to North and South Korean literary history. This means that the desire of establishing self-identity is in the act of writing literary history. Based on this cognitive premise, this paper considers the distinctiveness of description in Korean Chinese literature as a narrative strategy for realizing the desire of self-identity and seeks to analyze the detailed aspects of narrative strategy in Korean Chinese literary history. particularly, this paper focuses on the “The History of Korean Chinese Literature” written by O sang sun (etc.) and published in 2007. From this work, the identity of the Korean Chinese and their literary history is defined as a ‘Diaspora’, which serves as a basis for overcoming the ontological crisis of Korean Chinese literature.

      • 작두콩(Canavalia gladiata (Jacq.) DC) 및 작두콩깍지의 추출물에 관한 기능성 평가

        본 연구는 작두콩(도두, MDC302)과 작두콩깍지(MDC303)의 물 추출물(MDC302W, MDC303W)과 에탄올 추출물(MDC302E, MDC303E)의 항산화 및 항염증 효과에 대한 것이다. 항산화 효과는 DPPH과 SOD activity 측정을 통하여 확인하였고, 항염증 효과는 족부 부종 실험동물 모델과 Raw 264.7 세포주를 이용하여 PGE<sub>2</sub> 및 NO의 생성을 통해 확인하였다. 작두콩과 작두콩깍지의 SOD 활성은 작두콩 물 및 에탄올 추출물보다 작두콩깍지 물 및 에탄올 추출물에서 더 높았고 물 추출물(MDC303W)보다 에탄올 추출물(MDC303E)에서 활성이 더 높게 나타났다. 가장 활성이 높은 추출물은 MDC303E였고, 비타민 C(IC<sub>50</sub> 0.022)보다 높은 항산화력(IC<sub>50</sub> 0.003)을 보였다. DPPH 역시 작두콩 추출물보다 작두콩깍지 추출물에서 보다 높은 항산화력을 보였다. MDC302W 및 MDC302E 추출물들을 각각 단회 투여했을 때, 흰쥐 족부에서의 부종억제 효과는 관찰되지 않았다. 그러나, MDC303W의 경우 200 mg/kg과 400 mg/kg의 농도에서는 유의성 있게 부종이 억제됨을 관찰할 수 있었다. 400 mg/kg의 농도에서는, 4시간 경과 후 부종 억제율은 61%로 나타났다. MDC303E의 경우 200 mg/kg와 400 mg/kg의 농도에서 4시간 경과 후 부종억제율은 각각 47%와 45%로 유의적인 결과를 보였다. Raw 264.7 세포주에서 MDC303W로 독성을 평가하였을 때 50% 세포 성장 억제농도(IC<sub>50</sub>)는 3355.02 ㎍/㎖였다. LPS로 염증을 유발시켜 PGE<sub>2</sub> 생성을 촉진시키고 세포 성장을 억제하지 않는 최고농도인 2,000 ㎍/㎖까지 MDC303W 추출물을 농도별로 처리하였을 때, PGE<sub>2</sub> 생성 억제는 250~500 ㎍/㎖ 범위에서는 LPS 처리군보다 감소되었으나 그 외의 농도에서는 정상 수준으로 회복되지는 않았다. LPS에 의해 활성화된 Raw 264.7 세포는 nitrite를 분비하게 된다. MDC303W를 2,000 ㎍/㎖까지 농도별로 처리한 결과, 농도 의존적으로 NO 생성 활성을 저해함을 확인하였다.
