        부산·울산·경남지역 직업병 감시체계

        Objective: Occupational medicine specialists in the Busan, Ulsan and Kyung-Nam areas established an area-based occupational disease surveillance system and used this system to collected case information for the purpose of preventing occupational diseases Methods: l l hospital participated in this system. The authors selected five main diseases, which were hand-arm vibration syndrome (HAVS), work-related musculoskeletal disorder (WRMSD), occupational asthma, occupational skin disease and occupational and occupational lung cancer and established their case definitions. All cases were reported on the web, and real time analysis was conducted. Results: Between April 2001 and April 2003, 192 cases of HAVS, 118 cases of WRMSD, 33 cases of occupational asthma, 17 cases of occupational skin disease and 17 cases of occupational lung cancer (for a total of 377 cases of these five main diseases) were reported. most of the HAVS cases came from shipbuilding, and 172 of them (92.7%) were associated with grinding. Of the four main types of WRMSD, carpal tunnel syndrome (CTS) was the most prevalent with 46 cases and shipbuilding was also the main industry involved (83.9%). The ergonomic risk factors involved mainly associated with the hand. In 19 (57.6%) and 4 (12.1%) cases of occupational asthma, the agents involved. The causative agents of occupational lung cancer included Cr, welding fume, PAH, Ni, etc. 10(58.8%) of the cases were diagnosed as squamous cell carcinoma and 5(29.4%) as adenoma. Conclusion: This result showed that an area-based occupational disease surveillance system might provide an effective method of evaluating the prevalence of such diseases, however the Busan, Ulsan and Kyung-Nam provinces are too large to be treated as individual areas. Therefore, the authors suggest that each province should establish its own surveillance system.

      • 조혈모세포이식 후 발생한 주폐포자층 폐렴에 대한 고찰

        Background : Pneumocytitis cainii pneumonia (PCP) can occur in immunocompromised hosts especially such as AIDS or cancer patients. Although recent research had focused on PCP in AIDS patients, few studies have described the clinical presentations of PCP in recipients of stem cell transplantation (SCT). We evaluated the clinical manifestations of PCP in SCT patients admitted at St. Mary's hospital, Seoul, Korea. Methods : The medical records of 17 PCP patients undergoing SCT between Feb. 1998 and Feb. 2000 were reviewed. The diagnosis of PCP was confirmed through the demonstration of Pneumocytitis cainii via either cytology of brochoalveolar lavage (BAL) or histological technique of lung biopsy. CMV disease and CMV infection were confirmed by BAL culture and antigenemia respectively . Results : Seventeen patients were all recipients of allogeneic SCT and 7 of 17 patients were performed non-sibling SCT. Patients presented with symptoms including brief period (4 ∼23 days) of fever (76%), dyspnea (70%), cough (64%), and signs such as rare(58.8%), Sixteen patients (94%) had been receiving immunosuppressive agent such as cyclosporine A (64%) or Fk506 (35%) without PCP prophylaxis. Eleven patients (64%) were treated with corticosteroid with mean dose of 16 mg/day prednisolone and mean duration of 4.6 months after post-SCT period. Twelve patients were co-infected with CMV. Another co-infected miCroorganisms were Pseudomonas aeruginosa, Mycobacterium tuberculosis, herpes simplex virus, parainfluenza virus, Average duration of treatment with trimethoprim-sulfamethoxazole (TMP/SMX) was 21 ±9 days. Four patients died, and three of them were related with PCP. Conclusion : PCP developed frequently in patients who were taking immunosuppressive drug due to graft versus host disease or were not taking TMP/SMX prophylaxis. High risk patients showing fever, cough, or dyspnea should be considered to take early bronchoscopic intervention for detection of PCP. When treat for PCP, it also be considered to the possibility of coinfection such as CMV. (Korean J Infect Dis 33:273∼279, 2001)

      • LC-MS/MS Based Multi-omics Approaches for Prebiotic Inulin Utilization on Faecalibacterium prausnitzii

        The human gut commensal bacteria Faecalibacterium prausnitzii is well known for its anti-inflammatory effects that improve host intestinal health. Although several studies reported that inulin, one of the well-known prebiotics, increases the abundance of F. prausnitzii in the intestine, the mechanism under this effect of inulin remains unclear. Here, we applied LC-MS/MS-based multi-omics approaches to confirm the effects of inulin on F. prausnitzii. Interestingly, the proteomic analysis revealed that the putative proteins involved in sucrose utilization of F. prausnitzii are upregulated in the presence of inulin. To investigate the function of the proteins, we cloned the target genes, and observed the ability for sucrose degradation. In addition, we demonstrated that the sucrose degradation activity in F. prausnitzii culture media is enhanced by using inulin as a carbon source compared to glucose. Taken together, this study suggests that the reduction of sucrose induced by inulin could potentially improve host health and prevent gut dysbiosis like type 2 diabetes which is associated with sucrose.

      • The LC-MS/MS Based Multi-omics Analysis to Investigate Clostridioides difficile Infection Using in vitro Coculture Device, The Mimetic Intestinal Host-Microbe Interaction Coculture System, MIMICS

        Clostridioides difficile is a gram-positive bacterium and is considered as an enteric pathogen. C. difficile colonizes the host with intestinal dysbiosis and produces virulence factors, Toxin A and B. Although the cytotoxicity of C. difficile has been well elucidated, the exact pathogenic mechanisms and biological pathways of C. difficile infection (CDI) are not yet clear. Here, we developed a CDI model using MIMICS to investigate C. difficile-host interactions throughout the early (12 hr) and later (48 hr) stages of infection. We also apply an LC-MS/MS-based multi-omics approach to study the physiological changes in CDI-induced gut epithelial cells at the molecular and cellular levels. The proteomic analysis confirmed that proteins involved in cellular responses such as stress, immune responses and DNA damage responses are upregulated at early stages of infection. In addition, a representative CDI phenotype has been identified in the late stage of the infection. Our in vitro CDI model and multi-omics approach not only provide a better understanding of host-anaerobic pathogen interactions but may also aid in the treatment of C. difficile and other anaerobic infections.

      • 작두콩[Canavalia gladiata (jacq) DC] 및 작두콩깍지 추출물의 특성

        본 연구는 작두콩(도두) 작물의 부가가치를 향상시킬 목적으로 작두콩과 작두콩깍지를 물과 에탄올로 추출하여 일반성분, 미량무기질, 인지질, phenolic compound, flavonoid, isoflavone, saponin 함량을 분석한 결과이다. 작두콩과 작두콩깍지 물 추출물의 일반성분으로 수분은 작두콩 3%, 작두콩깍지 4%, 지방은 작두콩 0.1%, 작두콩깍지 0.2%, 단백질은 작두콩 40.0%, 작두콩깍지 7.0%, 탄수화물은 작두콩 49.1%, 작두콩깍지 64.1%, 회분은 작두콩 7.9%, 작두콩깍지 24.4%, 열량은 작두콩 357 kcal/100g, 작두콩깍지 286 kcal /100g,이었다. 작두콩의 미량 무기질 함량을 비교한 결과 작두콩에는 P, 작두콩깍지에는 Al, Ca, Fe, K 의 함량이 많았다. 작두콩과 작두콩깍지에 대한 물 및 에탄올 추출물의 아세톤 불용물로서 인지질 함량은 작두콩는 1.92, 17.4%, 작두콩깍지는 3.66, 13.84% 이었다. 작두콩과 작두콩깍지의 물 및 에탄올 추출물을 열풍건조로 얻은 파우더의 량으로 수율을 계산한 결과 작두콩의 물 추출물 및 에탄올 추출물는 21.77, 5.18%, 작두콩깍지의 물 추출물 및 에탄올 추출물는 23.89, 3.11% 이었다. 작두콩과 작두콩깍지의 물 추출물을 HPLC로 분석한 결과 작두콩는 3,4-DHBA, chlorogenic acid, genistein, daidzein, daidzin, 작두콩깍지는 3,4-DHBA, caffeic acid, ferulic acid를 확인하였다. 작두콩과 작두콩깍지의 에탄올 추출물을 HPLC로 분석한 결과 작두콩는 8종(3,4-DHBA, hesperidin, chlorogenic acid, genistein, genistin, daidzein, daidzin, ginsenoside Rb1)이 확인되어 물 추출물보다 3종(hesperidin, genistin, ginsenoside Rb1)의 화합물이 더 확인되었고, 작두콩깍지는 7종(3,4-DHBA, hesperidin, caffeic acid, gallic acid, ferulic acid, daidzin, ginsenoside Rb1)이 확인 되어 물 추출물 보다 4종(hesperidin, gallic acid, daidzin, ginsenoside Rb1)의 화합물이 더 확인되었다. 결론적으로 작두콩와 작두콩깍지의 함유성분 추출에는 물 보다 에탄올이 효과적이었다.

        탈북작가의 `몸` 형상화와 윤리적 주체의 가능성 - 김유경의 소설을 중심으로 -

        본 연구는 탈북작가 김유경의 장편소설 『청춘연가』와 『인간모독소』를 대상으로 삼아 탈북작가의 자기서사적 형상화 양상을 살폈다. 김유경의 소설은 이전의 탈북작가와는 차별화된 글쓰기 방식으로 주목받으며 문학적 성취를 이루었다는 평가를 받고 있다. 이러한 평가의 배경에는 그의 소설이 탈북자의 현실을 충실히 재현하며 탈북자의 의식을 총체적이고 객관적으로 보여준다는 인식이 놓여 있다. 현실의 재현과 집단성 표출이라는 문학적 인식은 일면 타당하지만 문학과 현실의 관계를 `반영`이라는 일차원적 관계로 한정해 이해한다는 점에서 한계를 노정한다. 본 연구는 이러한 지점을 연구의 출발점으로 삼아 현실을 재현하는 동시에 극복하려는 작가적 욕망의 구체성에 주목하였다. 두 소설이 보여주는 탈북작가의 자기서사화 과정은 폭력적 현실에 저항하며 윤리적 주체되기의 여정과 다름없다. 폭력의 대상에서 윤리적 주체로 이행이 가능한 것은 `몸`이라는 물질성에 주목하기 때문이다. 몸의 물질성은 자연적 소요나 고정적 본질로 주어진 것이 아니다. 물질성은 주체가 끊임없이 변화할 수 있는 운동성 있는 장으로 몸이 존재하도록 만든다. 물질성이 충만한 장인 몸을 통해서 주체는 주거하며 삶을 향유하고 노동하고 소유한다. 또한 몸을 통해 타자와의 만남이라는 사건을 쉼 없이 경험한 주체는 타자에게 자신을 열어젖힘으로써 윤리적 주체로 나아간다. 본고는 두 소설이 그려내는 이러한 주체되기의 여정을 레비나스의 사유에 기대어 충실히 따라가며 작품에 내재된 윤리적 주체의 가능성을 해명하고자 하였다. 이를 통해 작품의 재현양상에 주목한 기존연구에서 한걸음 더 나아가 탈북작가의 글쓰기를 자기서사화라는 관점에서 새롭게 읽어내고자 하였다. This study analyzes the aspects of self-narratives presented in the novels by North Korean defector novelist Kim Yu Kyung. Her novels have received critical acclaim for its distinguished writing and have been praised for its aesthetic achievement comparable to that of South Korean literature at the time. Reason being, her novel realistically reproduced the reality and effectively revealed the holistic consciousness of North Korean defectors from an objective point of view. However, these broad connections limit the perspective on literature as it creates a one-dimensional understanding that literature is simply a reflection of reality. Therefore, this research focuses on revealing the author`s desire to not just simply reflect reality but to overcome it. By analyzing Kim Yu Kyung`s works < Love Song for Youth > and < Place of Human Desecration >, this paper provides evidence that the self-narratives presented in these novels represent how the protagonists not only expose their violent reality but in the process of revealing their circumstances and actions as the objects of aggression, become the ethical subjects of the narrative. The reason for this transcendence is due to the materiality of the body. Although the body is a recipient of pain when violence is acted upon it, it can also become the source of healing due to its materiality. By referencing Emanuel Levinas` ethics of the Other, this paper demonstrates how the subject, through meeting and interacting with others, becomes the ethical subject beginning from the body where the subject first exists.

        LPS로 유도된 RAW 264.7 대식세포에서 닭발, 오가피, 두충 혼합 추출물의 항염증 활성

        This study was to investigate the anti-inflammatory effect of chicken-feet, Acathopanax, and Eucommia ulmoides Oliver mixture (CAEM) extract against lipopolysaccharide (LPS) in RAW 264.7 macrophage cells. We examined cytotoxicity of CAEM extract on RAW 264.7 cells, and their anti-inflammatory properties by measuring nitric oxide (NO) and pro-inflammatory cytokine production. In addition, inflammation-related gene expression was analyzed by real time-PCR in LPS-stimulated RAW 264.7 macrophage cells. Treatment of CAEM extract at concentrations of 0.31-2.5 mg/mL did not have any cytotoxic effect in RAW 264.7 cells. Treatment of LPS significantly increased the NO production, whereas CAEM extract (1.25, 2.5mg/mL) significantly decreased the levels of NO, compared to only LPS-treated cells. Furthermore, treatment with CAEM extract significantly suppressed the LPS-induced interleukin (IL)-6 and tumor necrosis factor-α (TNF-α) production. In order to confirm whether CAEM extract regulates inflammation-related gene expression, we analyzed the mRNA levels of IL-6, TNF-α, IL-1β, and inducible NO synthase (iNOS) by real time-PCR. Exposure of RAW 264.7 cells on LPS upregulated the mRNA levels of IL-6, TNF-α, IL-1β, and iNOS, compared to control group. However, those of expression were downregulated by treatment with CAEM extract. These findings suggest that CAEM extract may contribute to the prevention and treatment of inflammatory-related diseases.
