Background: The C1561T variant of the glutamate carboxypeptidase II (GCPII) gene is critical for natural methylfolylpolyglutamte (methylfolate) absorption, and has been associated with perturbations in folate metabolism and disease susceptibility. However, little is known on C1561T-GCPII as a risk factor for colorectal cancer. Therefore, this study examined whether C1561T-GCPII influences folate metabolism and adenomatous polyp occurrence. Materials and Methods: 164 controls and 38 adenomatous polyp cases were analysed to determine blood folate and plasma homocysteine (Hcy) level, dietary intake of natural methylfolate, synthetic pteroylglutamic acid (PteGlu), vitamin C and C1561T-GCPII genotype. Results: In controls and cases, 7.3 and 18.4 percent of subjects respectively, were found to have the CT genotype, increasing the risk for adenomatous polyp occurrence 2.86 times (95% CI:1.37-8.0, p=0.035). Total dietary folate, methylfolate and PteGlu intake and the level of erythrocyte folate and plasma Hcy did not predict the occurrence of an adenomatous polyp. However, dietary natural vitamin C intake was associated with adenomatous polyp risk within C1561T-GCPII CT genotype subjects (p=0.037). Conclusions: The findings suggest that C1561T-GCPII variation may be associated with risk for adenomatous polyp, and vitamin C may modify risk by interacting with the variant gene, its expression product and/or folate substrates.
Dietary Patterns and Risk for Metabolic Syndrome in Korean Women : A Cross-Sectional Study

<P><B>Abstract</B></P><P>Dietary patterns are a risk factor for metabolic syndrome (MetS). The prevalence of MetS has increased in Korea, and this condition has become a public health issue. Therefore, the present cross-sectional study aimed to identify the associations between dietary patterns and the risk of MetS among Korean women.</P><P>The data of 5189 participants were analyzed to determine dietary intake and lifestyle. A principal components analysis was employed to determine participant dietary patterns with regard to 106 food items. MetS was diagnosed using the National Cholesterol Education Program, Adult Treatment Panel III. Logistic regression analyses were applied to evaluate the associations between dietary pattern quintiles and MetS and to generate odds ratios (ORs) and 95% confidence intervals (CIs) after adjusting for potential confounders.</P><P>Three dietary patterns were identified: “traditional,” “western,” and “prudent.” The “prudent” dietary pattern consisted of a high intake of fruits and fruit products as well as nuts, dairy, and a low consumption of grains; this pattern was negatively associated with the risk of MetS. The highest quintile of the “prudent” dietary pattern was significantly less likely to develop MetS (OR: 0.5, 95% CI: 0.36–0.68, <I>P</I> for trend <0.001) compared with the lowest quintile. This pattern was also negatively associated with all of the MetS diagnostic criteria: abdominal obesity (OR: 0.52, 95% CI: 0.41–0.65), blood pressure (OR: 0.72, 95% CI: 0.59–0.87), triglycerides (OR: 0.67, 95% CI: 0.52–0.85), fasting glucose (OR: 0.64, 95% CI: 0.43–0.95), and high-density lipoprotein cholesterol (OR: 0.53, 95% CI: 0.42–0.68). However, the “traditional” and “western” dietary patterns were not associated with the risk of MetS.</P><P>The “prudent” dietary pattern was negatively associated with the risk of developing MetS among Korean women.</P>

The incidence and mortality of gastric cancer have been steadily decreased over the past few decades. However, gastric cancer is still one of the leading causes of cancer deaths across many regions of the world, particularly in Asian countries. In previous studies, nutrition has been considered one of significant risk factors in gastric cancer patients. Especially, malnourished patients are at greater risk of adverse clinical outcomes (e.g., longer hospital stay) and higher incidence of complications (e.g., wound/ infectious complications) compared to well-nourished patients. Malnutrition is commonly found in advanced gastric cancer patients due to poor absorption of essential nutrients after surgery. Therefore, nutritional support protocols, such as early oral and enternal feeding, have been proposed in many studies, to improve unfavorable clinical outcomes and to reduce complications due to delayed application of oral nutritional support or parental feeding. Also, the supplied with enternal immuneenriched diet had more benefits in improving clinical outcomes and fewer complications compared to a group supplied with control formula. Using nutritional screening tools, such as nutritional risk index (NRI) and nutritional risk screening (NRS 2002), malnourished patients showed higher incidence of complications and lower survival rates than non-malnourished patients. However, a long-term nutritional intervention, such as nutritional counseling, was not effective in the patients. Therefore, early assessment of nutritional status in patients using a proper nutritional screening tool is suggested to prevent malnutrition and adverse health outcomes. Further studies with numerous ethnic groups may provide stronger scientific evidences in association between nutritional care and recovery from surgery in patients with gastric cancer.
의사의 환자 지향적 커뮤니케이션과 만성질환자의 삶의 질과의 관계: 건강행태의 매개효과를 중심으로

의사의 환자 지향적 커뮤니케이션은 진료에 대한 환자의 만족도, 의사에 대한 신뢰도를 증가시킬 수 있고, 이를 통해 환자의 건강상태와 건강관련 행동을 변화시킬 수 있으며, 나아가 환자의 삶의 질에도 긍정적인 영향을 미칠 수 있다고 보고되고 있다. 이와 관련하여 국외에서는 활발히 연구되고 있으나, 국내에서는 관련 연구가 부족한 실정이다. 이에 본 연구는 의사의 환자 지향적 커뮤니케이션이 만성질환자의 삶의 질에 미치는 영향을 파악하고, 이 과정에서 음주, 흡연, 신체활동 등 건강행태의 매개효과를 확인하고자 하였다. 이를 위해 국민건강영양조사(2015)에서 고혈압, 당뇨, 비만, 고콜레스테롤 혈증 중 한 가지 이상의 질환을 가진 1,680명의 성인을 대상으로 매개회귀분석을 실시하였다. 연구 결과, 환자 지향적 커뮤니케이션은 만성질환자들에게 건강지향적인 행동을 유도하는 효과가 있었으며, 건강증진 행위는 그들의 삶의 질에 긍정적인 영향을 미치는 매개요인으로 작용하였다. 본 연구는 만성질환자들의 삶의 질 향상을 위해 질병을 관리하고 치료하는데 있어서 의료인의 환자 지향적 커뮤니케이션의 중요성을 확인하였다는 점에서 의의가 있다고 하겠다. It is reported that physicians’ patient-centered communication improves patients’ satisfaction and trust in their physicians, which can affect their health-related behaviors and, in turn, their quality of life. However, patient-centered communication has not been actively researched in Korea. The purpose of this study was to investigate the effects of patient-centered communication on the quality of life of chronically ill patients and to verify the mediating role of health-related behaviors such as drinking, smoking, and physical activity. To that end, we conducted a mediated regression analysis for 1,680 patients who were identified in the 2015 Korea National Health and Nutrition Examination Survey as having hypertension, diabetes, obesity, and/or hypercholesterolemia. According to the results, patient-centered communication contributed to inducing health-oriented behaviors in chronically ill patients, and health-promoting behaviors played the role of mediators that positively influenced their quality of life. This study confirmed the importance of patient-centered communication in managing and treating chronic diseases and in improving the quality of life of the patients.
