We report a case of a 59-year-old man with testicular germ cell tumor who showed new hypermetabolic lesions at the left axillary lymph nodes on a post-treatment positron emission tomography-computed tomography (PET-CT) scan. The hypermetabolic lesions were found to be caused by an influenza vaccination 10 days prior to the PET-CT scan and disappeared without additional treatment. <P>Insulin resistance occurs early in the disease process, preceding the development of type 2 diabetes. Therefore, the identification of molecules that contribute to insulin resistance and leading up to type 2 diabetes is important to elucidate the molecular pathogenesis of the disease. To this end, we characterized gene expression profiles from insulin-sensitive tissues, including adipose tissue, skeletal muscle, and liver tissue of Zucker diabetic fatty (ZDF) rats, a well characterized type 2 diabetes animal model. Gene expression profiles from ZDF rats at 6 weeks (pre-diabetes), 12 weeks (diabetes), and 20 weeks (late-stage diabetes) were compared with age- and sex-matched Zucker lean control (ZLC) rats using 5000 cDNA chips. Differentially regulated genes demonstrating > 1.3-fold change at age were identified and categorized through hierarchical clustering analysis. Our results showed that while expression of lipolytic genes was elevated in adipose tissue of diabetic ZDF rats at 12 weeks of age, expression of lipogenic genes was decreased in liver but increased in skeletal muscle of 12 week old diabetic ZDF rats. These results suggest that impairment of hepatic lipogenesis accompanied with the reduced lipogenesis of adipose tissue may contribute to development of diabetes in ZDF rats by increasing lipogenesis in skeletal muscle. Moreover, expression of antioxidant defense genes was decreased in the liver of 12-week old diabetic ZDF rats as well as in the adipose tissue of ZDF rats both at 6 and 12 weeks of age. Cytochrome P450 (CYP) genes were also significantly reduced in 12 week old diabetic liver of ZDF rats. Genes involved in glucose utilization were downregulated in skeletal muscle of diabetic ZDF rats, and the hepatic gluconeogenic gene was upregulated in diabetic ZDF rats. Genes commonly expressed in all three tissue types were also observed. These profilings might provide better fundamental understanding of insulin resistance and development of type 2 diabetes.</P>
Determination of the source rate released into the environment from a nuclear power plant

<P>A study was carried out to determine the source rate from a nuclear power plant using the tracer experimental data conducted at the Yeoung-Kwang nuclear site. The least squares method optimises the agreement between the released source rate and the calculated source rate by minimising the errors between the measured concentrations and the calculated ones using the Gaussian plume model. The least squares estimator generally estimates the source rate to be within a factor of 2. The forecasting ability of the source rate is improved by applying the modified dispersion coefficients that are calculated using the experimental data. Determination of the source rate in an early phase nuclear emergency will be helpful for the decision making when taking appropriate and prompt countermeasures in the case of a radiological emergency.</P>

Objective: This study aimed to compare the efficacy between no-touch (NT) radiofrequency ablation (RFA) and conventional RFA using twin internally cooled wet (TICW) electrodes in the bipolar mode for the treatment of small hepatocellular carcinomas (HCC). Materials and Methods: In this single-center, two-arm, parallel-group, prospective randomized controlled study, we performed a 1:1 random allocation of eligible patients with HCCs to receive NT-RFA or conventional RFA between October 2016 and September 2018. The primary endpoint was the cumulative local tumor progression (LTP) rate after RFA. Secondary endpoints included technical conversion rates of NT-RFA, intrahepatic distance recurrence, extrahepatic metastasis, technical parameters, technical efficacy, and rates of complications. Cumulative LTP rates were analyzed using Kaplan-Meier analysis and the Cox proportional hazard regression model. Considering conversion cases from NT-RFA to conventional RFA, intentionto- treat and as-treated analyses were performed. Results: Enrolled patients were randomly assigned to the NT-RFA group (37 patients with 38 HCCs) or the conventional RFA group (36 patients with 38 HCCs). Among the NT-RFA group patients, conversion to conventional RFA occurred in four patients (10.8%, 4/37). According to intention-to-treat analysis, both 1- and 3-year cumulative LTP rates were 5.6%, in the NT-RFA group, and they were 11.8% and 21.3%, respectively, in the conventional RFA group (p = 0.073, log-rank). In the as-treated analysis, LTP rates at 1 year and 3 years were 0% and 0%, respectively, in the NT-RFA group sand 15.6% and 24.5%, respectively, in the conventional RFA group (p = 0.004, log-rank). In as-treated analysis using multivariable Cox regression analysis, RFA type was the only significant predictive factor for LTP (hazard ratio = 0.061 with conventional RFA as the reference, 95% confidence interval = 0.000–0.497; p = 0.004). There were no significant differences in the procedure characteristics between the two groups. No procedure-related deaths or major complications were observed. Conclusion: NT-RFA using TICW electrodes in bipolar mode demonstrated significantly lower cumulative LTP rates than conventional RFA for small HCCs, which warrants a larger study for further confirmation.

본 연구는 A7075재의 피로강도 및 수명을 개선하기 위해 Shot peening전후, UNSM(표면나노개질처리)전후로 나누어 실험하였다. 피로시험 시편은 야적 및 질적 데이터를 얻기위한 피로시험 기계에 의해 수행되었고, 파면해석 및 파괴역학적으로 자료를 분석하여 효율적인 Shot Peening의 적용 가능성을 연구하였다. This research is conducted to improve the fatigue strength and life of the Al7075 as follows ‘before SP’ and ‘after SP’, ‘before UNSM’ and ‘after UNSM’. The fatigue test specimen was performed by the fatigue test machine to acquire the quantitative and qualitative data. And the surface of these fractures in fracture mechanics was observed to confirm the effect and possibility of Shot Peening and UNSM.
Analysis of Piled Piers Considering Riverbed Scouring

본 연구에서는 지반말뚝, 말뚝-말뚝캡, 그리고 말뚝-유체간의 상호작용 해석을 수행하여 교각세굴을 고려한 말뚝기초의 거동을 해석하였다. 지반-말뚝의 상호작용은 비선형 하중전이곡선(p-y, t-z, 그리고 q-z 곡선)을, 말뚝-말뚝캡의 상호작용에서는 군말뚝의 배열과 말뚝-말뚝캡 사이의 구속조건을 고려하였다. 말뚝유체의 상호작용은 세굴에 의한 지반의 강성 저하를 고려하여 지반-말뚝의 상호작용에 포함하여 해석하였다. 그 결과 세굴심이 깊어질수록 말뚝에 발생하는 최대 휨모멘트의 값이 증가함을 알 수 있었으며, 이를 바탕으로 세굴에 따른 군말뚝의 안정성 평가에서는 지반-말뚝 및 말뚝-말뚝캡의 상호작용을 고려한 해석을 수행하는 것이 바람직함을 알 수 있었다. This paper describes a simplified numerical procedure for analyzing the response of bridge pier foundations due to riverbed scouring. A computationally efficient algorithm to analyze the behavior of a pile group is proposed by considering soil-pile, pile-cap, and pile-fluid interactions. The complex phenomenon of the pile-soil interaction is modeled by discrete nonlinear soil springs (p-y, t-z and q-z curves). The pile-cap interaction is considered by geometric configuration of the piles in a group and connectivity conditions between piles and the cap. The pile-fluid interaction is incorporated into the procedure by reducing the stiffness of the soil-pile reactions as a result of nonlinearity and degradation of the soil stiffness with river bridge scouring. Through the numerical study, it is shown that the maximum bending moment increases with increasing scour depth. Thus it is desirable to check the stability elf pile groups based on soil-pile and pile-cap interactions by considering scouring depth in the riverbed.

<P>We assessed the accuracy of yeast bloodstream isolate identification performed over a 1-year period at 10 South Korean hospitals, using the matrix-assisted laser desorption ionization–time of flight (MALDI-TOF)-based Vitek MS system. The overall phenotypic misidentification rate was 3.4% (18/533), with considerable variation between hospitals (0.0% to 19.0%), compared to 1.1% (6/533) for the Vitek MS system.</P>

<P><B>Abstract</B></P> <P>Effects of the crystalline tungsten carbide (WxC) phases on an ordered mesoporous bimetallic Rh-Mn/WxC, which were prepared by changing carbon source to tungsten (C/W) ratios of the WxC support using a hard-template of an ordered mesoporous SBA-15, were investigated for a direct synthesis of mixed alcohols by CO hydrogenation from syngas. The C/W ratios on the mesoporous Rh-Mn/WxC showed a significantly different catalytic activity, especially on the C<SUB>1</SUB> - C<SUB>3</SUB> alcohol productivity. The Rh-Mn/WxC prepared at C/W molar ratio of 10 having a metastable W<SUB>2</SUB>C main phase (Rh-Mn/WxC(10)) revealed a higher CO conversion of 8.1% and selectivity to higher alcohols of 54.4% compared to other catalysts having a main crystalline phases of WO<SUB>3</SUB> or WC. The enhanced catalytic activity and selectivity to mixed alcohols on the Rh-Mn/WxC(10) were attributed to the largely exposed smaller active Rh nanoparticles with its stronger interactions with the metastable W<SUB>2</SUB>C phases. The superior activity was originated from the intimate interactions of Rh nanoparticles with Mn promoter by maintaining proper oxidation states confirmed by surface ratios of the metallic Rh to oxidized Rh<SUP> <B>n+</B> </SUP> species. The stable preservation of the ordered mesoporous structures of the W<SUB>2</SUB>C phase in the amorphous carbon matrixes significantly altered the chemical states of the small Rh nanoparticles below 2 nm in size by preferentially existing on the outer surfaces of the W<SUB>2</SUB>C support, which resulted in showing an enhanced productivity of higher C<SUB>1</SUB> –C<SUB>3</SUB> alcohols with 171.8 g/(kg<SUB>cat</SUB>·h).</P> <P><B>Highlights</B></P> <P> <UL> <LI> WxC phases on mesoporous Rh-Mn/WxC changed C<SUB>1</SUB>-C<SUB>3</SUB> alcohol selectivity from direct CO hydrogenation. </LI> <LI> Metastable W<SUB>2</SUB>C phase is beneficial for a higher productivity of mixed alcohols. </LI> <LI> Different oxidation states of strongly interacted small Rh nanoparticles are well correlated with catalytic activity. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Background: Primary testicular diffuse large B-cell lymphoma (DLBCL) is a kind of rare but aggressive extranodal lymphoma. Relapse in central nervous system (CNS) is majorconcerns in the treatment of primary testicular DLBCL. However, the role of intrathecal prophylaxis is still controversial issue. Methods: We retrospectively reviewed the medical records of 14 patients with primary testicular DLBCL diagnosed between November 2000 and June 2012. We analyzed CNS relapse rate in the patients treated without intrathecal prophylaxis. Survival curves were estimated using the Kaplan-Meier method. Results: The median age at diagnosis was 57 years (range, 41-79 years). Unilateral testicular involvement was observed in 13 patients. Nine (64. 3%) patients were in stage I, 1 in stage II, and 4 in stage IV. The international prognostic index was low or low-intermediate risk in 12 patients, and high-intermediate risk in 2 patients. Thirteen patients underwent orchiectomy. All of the patients received systemic chemotherapy without intrathecal prophylaxis. And, prophylactic radiotherapy was administered to the contralateral testis in 12 patients. The median follow-up period of surviving patients was 39 months (10-139 months). Median overall survival had not been reached and median progression-free survival was 3. 8 years. Four patients experienced relapse, but CNS relapse was observed in only one patient (7. 1%) with stage IV disease at 27 months after a complete response. Conclusions: Even without intrathecal prophylaxis, the rate of relapse in the CNS appears not to be high in Korean patients with primary testicular DLBCL compared with prior reports.
