      • Interaction of <i>Veratrum nigrum</i> with <i>Panax ginseng</i> against Obesity: A Sang-ban Relationship

        <P>Obesity has become a major health threat in developed countries. However, current medications for obesity are limited because of their adverse effects. Interest in natural products for the treatment of obesity is thus rapidly growing. Korean Medicine (KM) is characterized by the wide use of herbal formulas. However, the combination rule of herbal formulas in KM lacks experimental evidence. According to <I>Shennong's Classic of Materia Medica</I>, the earliest book of herbal medicine, <I>Veratrum nigrum</I> (VN) has antagonistic features against <I>Panax ginseng</I> (PG), and the PG-VN pair is strictly forbidden. In this study, we have shown the effects of PG, VN, and their combination on obesity in high-fat (HF) diet-induced obese mice and in 3T3-L1 cells. PG, VN, and PG-VN combination significantly reduced weight gain and the fat pad weight in HF diet-induced obese mice. They also significantly decreased lipid accumulation and the expressions of two major adipogenesis factors, PPAR<I><I>γ</I></I> and C/EBP<I><I>α</I></I>, in 3T3-L1 cells. In addition, the PG-VN combination had synergistic effects compared with the mixture of extracts of PG and VN on inhibition of PPAR<I><I>γ</I></I> and C/EBP<I><I>α</I></I> expressions at lower doses. These results indicate a new potential anti-obese pharmacotherapy and also provide scientific evidence supporting the usage of herbal combinations instead of mixtures in KM.</P>

        CASE REPORT : Natural Course of an Untreated Metastatic Perirectal Lymph Node After the Endoscopic Resection of a Rectal Neuroendocrine Tumor

        Lymph node metastasis is rare in small (i.e., <10 mm) rectal neuroendocrine tumors (NETs). In addition to tumor size, pathological features such as the mitotic or Ki-67 proliferation index are associated with lymph node metastasis in rectal NETs. We recently treated a patient who underwent endoscopic treatment of a small, grade 1 rectal NET that recurred in the form of perirectal lymph node metastasis 7 years later. A 7-mm-sized perirectal lymph node was noted at the time of the initial endoscopic treatment. The same lymph node was found to be slightly enlarged on follow-up and finally confirmed as a metastatic NET. Therefore, the perirectal lymph node metastasis might have been present at the time of the initial diagnosis. However, the growth rate of the lymph node was extremely low, and it took 7 years to increase in size from 7 to 10 mm. NETs with low Ki-67 proliferation index and without mitotic activity may grow extremely slowly even if they are metastatic. (Intest Res 2015;13:175-179)

      • Clinical Impact of Hepatic Steatosis in Patients with Chronic Hepatitis B Infection on Tenofovir Therapy

        Aims: To further observe and identify the effect of nonalcoholic fatty liver disease (NAFLD) on the response to tenofovir disoproxil fumarate (TDF) treatment in patients with chronic hepatitis B (CHB). Methods: From 2012-2014, consecutive antiviral-naïve CHB patients who started TDF and underwent controlled attenuated parameter (CAP) measurement were enrolled from two tertiary medical center. Based on CAP score, patients were divided into groups with NAFLD (CAP score ≥ 250) and non-NALFD (CAP score < 250). The demographic, anthropometric and clinical data was investigated at baseline, 24wk, 48wk, and 96wk. Results: A total of 159 patients (mean age 47.23±11.61 years, 90 (56.6%) males) were involved in the study, with overall percentages of NAFLD as 35.8% (n=57). The two groups were not significantly different in terms of age, sex, biochemical parameters and the status of compensated liver cirrhosis. Moreover, HBV DNA level and the status of HBeAg positive were equally distributed in both two groups. However, the NAFLD group showed a higher level of BMI compared with non-NAFLD group (p < 0.05). Although the HBV DNA undetectable rate increased gradually after treatment with TDF in both two groups, non-NAFLD group showed a higher virological response rate compared to NAFLD group (54.4% vs. 41.1%, 80.6% vs. 69.6%, 95% vs. 72.7%; week 24, week 48, week 96, respectively). However, there was significant difference between two groups only at week 96 (p<0.001). The rate of ALT normalization was higher in non-NAFLD group throughout the whole time spot, but reached no statistical significance. Multivariate analysis indicated that a higher HBV DNA level was associated with early virological non-response and that hepatic steatosis may influence long-term virological non-response. Conclusions: NAFLD significantly affected long-term virological response in CHB patients treated with TDF. Further investigation is needed in the future, which calls for a specific antiviral strategy in CHB patients with NAFLD.

      • Calculation of Effective Material Constant for PolySilicon Material by BEM

        This study is with respect to calculation of the efficient material constant of poly-silicon material using Boundary Element Method. As the MEMS field is gradually developed, reliability and safety of MEMS parts are an important factor in its industrial application. Generally, most of the MEMS parts consist of multi-grain poly-silicon of the orthotropic with random material direction. So it is important to analyze stress and know the material constant for multi-grain orthotropic material so that these parts are used for industrial field. Lattice model is used for micro structure evolution of the multi-grain. A system of numerical method is proposed to extract the effective young's modulus and poisson's ratio through the analysis of stress within the program. And the results analyzed by FEM are compared with those of FEM to verify that the program is sufficiently valid. Finally, we examined the true that it is possible for condition to create grains slowly or quickly to affect the stiffness and its distribution through repeated analysis.

        Characteristics Evaluation on Welding Metal Zones Welded with Inconel 625 Filler Metal to Cast Steel for Piston Crown Material

        Since the oil price has been significantly jumped for several years, a heavy oil of low quality has been mainly used in the diesel engine of the merchant ship. Thus, a combustion chamber of the engine has been often exposed to severely corrosive environment more and more because temperature of the exhaust gas of the combustion chamber has been getting higher and higher with increasing of using the heavy oil of low quality. As a result, wear and corrosion of the engine parts such as exhaust valve, piston crown and cylinder head surrounded with combustion chamber are more serious compared to the other parts of the engine. Therefore, an optimum repair welding for these engine parts is very important to prolong their lifetime in a economical point of view. In this study, Inconel 625 filler metal were welded with GTAW method in the cast steel which would be generally used with piston crown material. And the corrosion properties of weld metal, heat affected and base metal zones were investigated using electrochemical methods such as measurement of corrosion potential, anodic polarization curves, cyclic voltammogram and impedance etc. in 35% $H_2SO_4$ solution. The weld metal and base metal zones exhibited the highest and lowest values of hardness respectively. Furthermore, the corrosion current density of the weld metal zone revealed the lowest value, having the highest value of hardness. The corrosive products with red color and local corrosion like as a pitting corrosion were considerably observed at the base metal zone, while these morphologies were not wholly observed in the weld metal zone. In particular, the polarization characteristics such as impedance, polarization curve and cyclic voltammogran associated with corrosion resistance property were well in good agreement with each other. Consequently, it is suggested that the mechanical and corrosion characteristics of the piston crown can be predominantly improved by repair welding method using the Inconel 625 electrode.

      • Capillary malformation on midline of forehead: nevus simplex or medial fronto-facial capillary malformation ?

        Background: Nevus simplex, a part of capillary malformation (CM), is one of the common birthmark. Nevus simplex known as salmon patch or angel’s kiss is located on glabella, upper eyelid, and nape and tends to fade around the ages of 1-4 years. However, medial fronto-facial capillary malformation differs from typical nevus simplex by (1) wider location on forehead, (2) more intense color, and (3) incomplete involution. Objectives: The aim of this study was to investigate clinical features and natural history of CM on midline of forehead. Methods: We prospectively recruited patients with CM on midline of forehead from 2009 to 2016. We checked clinical features including age, date of visit, birth history, family history, shape, location, size, concurrent anomalies, and natural course. Results: Sixty-four patients (41 girls; 65.0%) were included. The mean age at initial visit was 7.5 years (20 days to 45 years). Lesions on the forehead were band or linear shaped (54.0%), V-shaped (25.4%) and ill-defined oval to geographic (15.9%). Family history of CM was checked in 9.4% (6/64). 28.1% (18/64) of patients accompanied infantile hemangioma. Neurological abnormality was observed in 4.6%. In 36 infantile patients who were regularly followed up until the ages of 4 years, incomplete resolution were found in 94.4% (60/64) and unchanging in 5.6% (4/64). Conclusion: We found CM on midline of forehead is different from nevus simplex considering its peculiar presentation and incomplete resolution.

      • Detection of MicroRNAs Associated with EMT through Regulation CD44/TGF-β1 in Hepatocellular Carcinoma

        Aims: CD44 have known as important modulators of epithelial-mesenchymal transition (EMT) together with transforming growth factor beta1 (TGF-β1). Moreover, CD44 and TGF-β1 double positive more enhanced cancer stem cell characteristics acquisition, EMT, and metastasis. In cancer, miRNAs functions as tumor suppressors or oncogenes. This study aimed to investigate the identify microRNAs regulating the EMT with CD44/ TGF-β1 in HCC cells. Methods: We sorted CD44<sup>-</sup> and CD44<sup>+</sup> liver cancer stem cells by fluorescence-activated cell sorting (FACs) in TGF-β1-positive SNU-368 cells and TGF-β1-negative SNU-354 cells. The miRNA profiles of CD44<sup>-</sup> and CD44<sup>+</sup> HCC cells were analyzed by next-generation sequencing (NGS). To investigate synergy effect of CD44 and TGF-β1, we induced EMT by TGF-β1 treatment or inhibited EMT by TGF-β1 inhibitors. The expression level of mRNA and protein were detected by quantitative real-time PCR (qRT-PCR) and western blot. Results: miRNA NGS array data were compared among CD44 or TGF-β1 single expression HCC cells and CD44/TGF-β1 double positive HCC cells. The results showed that four miRNAs (miR-96-5p, miR-221-5p, miR-186-3p and miR-152-3p) were up-regulated and two miRNAs (miR-296-3p, miR-10a-5p) was down-regulated in CD44<sup>+</sup>/TGF-β1<sup>+</sup> cells than an expression of either one alone. These results were confirmed by qRT-PCR. TGF-β1-stimulated SNU-354 (CD44+/TGF-β1<sup>+</sup>) cells up-regulated three miRNAs (miR-96-5p, miR-221-5p, miR-186-3p) than SNU-354 (CD44<sup>+</sup>/TGF-β1<sup>-</sup>) cells and also, down-regulated the two miRNAs (miR-296-3p, miR-10a-5p). Inhibition of TGF-β1 in SNU-368 cells reduced miR-221-5p and increased miR-296-3p. Furthermore, TGF-β1-stimulated SNU-354 (CD44<sup>+</sup>/TGF-β1<sup>+</sup>) cells induced EMT and TGF-β1 inhibitor-treated SNU-368 (CD44<sup>+</sup>/ TGF-β1<sup>-</sup>) cells inhibited EMT. Overexpression of miR-221-5p in SNU-354 (CD44<sup>+</sup>/TGF-β1<sup>-</sup>) cells exhibited lower E-cadherin and higher β-catenin. Next, the loss of miR-221-5p in SNU-368 (CD44<sup>+</sup>/TGF-β1<sup>+</sup>) cells showed increased E-cadherin. Conclusions: We identified CD44/TGF-β1-related miRNAs and confirmed regulation of EMT by miRNAs in HCC cells. The results would suggest a possible application that CD44/TGF-β1- regulated miRNAs may serve as specific biomarkers and therapeutic targets for hepatocellular carcinoma.

      • The clinical analysis for metastases in patient with cutaneous squamous cell carcinoma: a single center experience

        Background: Cutaneous squamous cell carcinoma (cSCC) constitutes the second most common skin cancer in Korea. cSCC rarely metastasized but it has ability to metastasize to any organ of the body. However, reports about potential risk factors of metastases of cSCC were limited to English literature. Objectives: The aim of this study is to analyze the rate and clinical characteristics of metastases occurred in patients with cSCC and to identify predictive factors of metastatic cSCC. Methods: We retrospectively reviewed the patients with metastatic cSCC who visit at Pusan National University Hospitals (Busan and Yangsan) between 1998 and 2016. Results: Among total 290 patients with primary cSCC, metastases were observed in 19 patients (6.6%). In patients with metastases, median age at diagnosis was 74 years (range, 49-96). Median clinical diameter was 32.8 mm (range, 4-45). Cheek (26.5%, 5/19) was the most common primary site, followed by the eyebrow (15.8%, 3/19), thigh (15.8%, 3/19), and hand (10.5%, 2/19). Most common metastatic site were LNs (100%, 19/19), followed by lung (10.6%, 2/19), and kidney (5.3%, 1/19). In contrast to previous reports, ear and lip were not high risk metastatic area in our study. (p=0.392 and 0.533, respectively). Conclusion: This study identified the incidence and clinical features of metastatic cSCC. We should examine the sentinel LN first for early detection of metastasis when we meet the cSCC.

        IoT Open-Source and AI based Automatic Door Lock Access Control Solution

        Recently, there was an increasing demand for an integrated access control system which is capable of user recognition, door control, and facility operations control for smart buildings automation. The market available door lock access control solutions need to be improved from the current level security of door locks operations where security is compromised when a password or digital keys are exposed to the strangers. At present, the access control system solution providers focusing on developing an automatic access control system using (RF) based technologies like bluetooth, WiFi, etc. All the existing automatic door access control technologies required an additional hardware interface and always vulnerable security threads. This paper proposes the user identification and authentication solution for automatic door lock control operations using camera based visible light communication (VLC) technology. This proposed approach use the cameras installed in building facility, user smart devices and IoT open source controller based LED light sensors installed in buildings infrastructure. The building facility installed IoT LED light sensors transmit the authorized user and facility information color grid code and the smart device camera decode the user informations and verify with stored user information then indicate the authentication status to the user and send authentication acknowledgement to facility door lock integrated camera to control the door lock operations. The camera based VLC receiver uses the artificial intelligence (AI) methods to decode VLC data to improve the VLC performance. This paper implements the testbed model using IoT open-source based LED light sensor with CCTV camera and user smartphone devices. The experiment results are verified with custom made convolutional neural network (CNN) based AI techniques for VLC deciding method on smart devices and PC based CCTV monitoring solutions. The archived experiment results confirm that proposed door access control solution is effective and robust for automatic door access control.
