        필리핀 여행 후 발생한 쯔쯔가무시병 1예

        Infectious diseases imported from other countries have increased as more and more Koreans are going abroad for various purposes. Tsutsugamushi disease from other endemic area such as Southeast Asia is important, because it can occur in any season and eschar may be absent. We report a case of imported tsutsugamushi disease acquired in the Philippines. A patient presented with fever, headache, and maculopapular skin rash. However, eschar was absent. Polymerase chain reaction (PCR) for 56-kDa gene of Orientia tsutsugamushi using buffy coat was positive. Serum indirect immunofluorescent antibody assay was initially negative but became positive with a titer of 1:320 at follow-up, Sequencing analysis revealed the strain to be 100% identical to the TW73R strain identified in Taiwan. After the patient received doxycycline, body temperature normalized in 12 hours. Tsutsugamushi disease is one of the differential diagnoses that should be included for patients with fever who have recently returned from Southeast Asian countries. PCR for O. tsutsugamushi using patient's buffy coat was useful for early diagnosis.

        초분광센서를 활용한 이산화질소 농도 추정식에 관한 연구

        국내 산업단지에서 배출되는 대기오염물질의 모니터링을 위해 굴뚝원격감시시스템이 운영되고 있으나 대상 시설이 한정적이어서, 시스템이 설치되지 않은 시설은 단속 요원이 직접 모니터링 및 단속을 수행하고 있다. 그래서 효율적인 산업단지에서 배출되는 대기오염물질의 모니터링을 위해 다양한 센서를 활용한 연구들이 수행되고 있다. 이에 따라 본 연구에서는 초분광센서로 측정할 수 있는 분광복사량을 활용하여 대기오염물질 중 이산화질소의 농도를 추정할 수 있는 공식을 개발하고 검증하였다. 농도 추정식 개발을 위해 다양한 농도의 이산화질소를 대상으로 태양천정각, 관측천정각, 상대방위각을 다르게 하여 분광복사량을 관측하였다. 관측된 분광복사량에서 특정 파장 간의 값의 차이를 흡수 깊이로 하였으며, 흡수 깊이와 이산화질소 농도와의 관계를 이용하여 농도 추정식을 개발하였다. 그리고 개발된 농도 추정식들의 검증을 위해 이산화질소와 아황산가스가 혼합된 가스를 대상으로 측정한 분광복사량을 이용하였다. 그 결과, 추정식의 형태에 따라 결정 계수와 RMSE가 0.71~0.88, 72~323 ppm으로 나타났으며, 지수 형태의 농도 추정식의 결정 계수가 가장 높게 나타났다. 추정식의 형태에 따라 농도의 변화에 따른 추정 농도의 정확도가 일정하지 않지만, 향후 농도 추정식의 고도화가 이루어진다면 초분광 센서를 활용하여 산업단지 배출되는 이산화질소의 모니터링에 사용 가능할 것으로 판단된다. The CleanSYS(Clean SYStem) is operated to monitor air pollutants emitted from specific industrial complexes in Korea. So the industrial complexes without the system are directly monitored by the control officers. For efficient monitoring, studies using various sensors have been conducted to monitor air pollutants emitted from industrial complex. In this study, hyperspectral sensors were used to model and verify the equations for estimating the concentration of NO2(nitrogen dioxide) in air pollutants emitted. For development of the equations, spectral radiance were observed for NO₂ at various concentrations with different SZA(Solar Zenith Angle), VZA(Viewing Zenith Angle), and RAA(Relative Azimuth Angle). From the observed spectral radiance, the calculated value of the difference between the values of the specific wavelengths was taken as an absorption depth, and the equations were developed using the relationship between the depth and the NO₂ concentration. The spectral radiance mixed gas of NO₂ and SO₂(sulfur dioxide) was used to verify the equations. As a result, the R2(coefficient of determination) and RMSE(Root Mean Square Error) were different from 0.71~0.88 and 72~323 ppm according to the form of the equation, and R<SUP>2</SUP> of the exponential form was the highest among the equations. Depending on the type of the equations, the accuracy of the estimated concentration with varying concentrations is not constant. However, if the equations are advanced in the future, hyperspectral sensors can be used to monitor the NO₂ emitted from the industrial complex.

      • Catalytic conversion of 1,2-dichlorobenzene over mesoporous materials from zeolite.

        <P>In this study, 1,2-dichlorobenzene (DCB), an important precursor of PCDDs and PCDFs, was chosen as a suitable model compound for the catalyzed deep oxidation of dioxin. The recently developed mesoporous materials from zeolites (MMZ) were used for the first time as a support for an oxidation catalyst. The catalytic oxidation of 1,2-dichlorobenzene over Pt/MMZ was carried out, and the catalytic activity was compared with that of Pt/gamma-Al2O3, Pt/Al-MCM-41 and Pt/Beta catalysts. Pt/MMZ showed the highest catalytic activity among the catalysts tested. Interestingly, the catalytic activity of Pt/MMZ was maintained (> 40%) at low temperatures (250 degrees C) at which the other catalysts showed extremely low activity (< 5%). The high catalytic activity of Pt/MMZ was attributed to both the sufficient acidity and mesoporosity of the MMZ support.</P>

        평면 디스플레이 기판 운송용 롤러 컨베이어 주위의 유동장에 관한 수치해석 연구

        본 논문에서는, 청정지역내에서 다수의 유리 기판을 적재하여 반송하는 롤러 컨베이어 시스템에서, 롤러와 카세트 사이의 기계적 마찰로 인해 발생하는 오염입자에 의한 유리 기판의 오염여부를 판단하기 위해, 시스템 내부의 유동장을 전산유체역학 기법을 통해 해석하였다. 수치해석 결과 일정 속도로 이동하는 카세트 하부에 큰 와류가 생성 되는 것을 확인하였다. 그러나 이 와류는 후면에서 유입되는 강한 기류와, 상부 FFU에서 공급되어 하부로 배기되는 기류로 인해, 카세트의 하부 영역에만 갇히게 되는 것으로 나타났다. 따라서 카세트와 롤러 사이의 기계적 마찰로 인해 오염입자가 발생하여도 상부로 이동할 수 없어 유리 기판의 오염 가능성은 낮았다. 또한 이동속도가 빨라짐에 따라 유리 평판 사이의 공기 유동의 방향이 평판의 점성 전단력에 의해 역방향으로 바뀌게 됨을 확인하였다. Flow field in a roller conveyor system, induced from the movement of a cassette in which glasses for flat panel display are loaded, is numerically studied in this paper. Contamination on the glass surface by dust particles produced from mechanical friction between roller and cassette is predicted from the analysis results of flow fields. Results show that a large swirl flow is formed under the moving cassette with constant speed. This swirl flow is confined only under the cassette because two main streams from the backward and the fan filter unit on the top ceiling are sufficiently strong. Therefore, it can be said that possibility of the contamination by the particles originated from the friction is relatively low. It is also revealed that flow direction between glass plates is changed according to the speed of the cassette movement due to the shear force of glass plates.

      • 만성 위축성 위염및 구강상피 위축을 동반한 악성빈혈 1례

        악성빈혈은 가장 흔한 비타민 ??의 결핍질환으로 위점막 위축으로 인한 내인자의 분비저하로 발생되며, 근래 환자의 혈청, 타액, 위분비액내의 내인자와 벽세포에 대한 항체를 증명해 냄으로써 위점막의 위축을 자가면역기전으로 설명하려는 경향이 있다. 저자들은 최근 설통 및 흉부 압박감을 주소로 내원한 60세 여자환자가 만성 위축성 위염을 동반한 거적아구성빈혈의 소견을 보이면서 항내인자와 하위벽세포항체가 검출되고, 비타민 ??의 비경구 투여후 급속한 회복을 보여 악성빈혈로 진단하고 이에 문헌고찰과 함께 보고하는 바이다. Pernicious anemia is the most common vitamin ?? deficiency, in which intrinsic factor secretion ceases owing to atrophy of gastric mucosa. On the basis of incomplete evidence, recently pernicious anemia thought to be caused by an autoimmune reaction against gastric parietal cell. A 60 year old female had been admitted to the Department of Internal Medicine, Dong guk University Hospital with chief complaints of sore-tongue and chest pressure. Microscopic examination of peripheral blood and bone marrow disclosed macro-ovalocytic, anisocytic red cell apperance, hypersegmented neutrophils, and hypercellular marrow with megaloblastoid erythropoiesis. Gastroendoscopy with biopsy shows the finding of chronic atrophic gastritis over body to fundus. By using indirect immunofluorescent techniques, we detected the antiparietal cell antibody and antiintrinsic factor antibody. Based on the above findings, a case of pernicious anemia associated with chronic atrophic gastritis could be diagnosed.

        ORiginal Article : Endoscopic Experience Improves Interobserver Agreement in the Grading of Esophagitis by Los Angeles Classification ; Conventional Endoscopy and Optimal Band Image System

        Background/Aims: Interobserver variation by experience was documented for the diagnosis of esophagitis using the Los Angeles classification. The aim of this study was to evaluate whether interobserver agreement can be improved by higher levels of endoscopic experience in the diagnosis of erosive esophagitis. Methods: Endoscopic images of 51 patients with gastroesophageal reflux disease (GERD) symptoms were obtained with conventional endoscopy and optimal band imaging (OBI). Endoscopists were divided into an expert group (16 gastroenterologic endoscopic special-ists guaranteed by the Korean Society of Gastrointestinal Endoscopy) and a trainee group (individuals with fellow-ships, first year of specialty training in gastroenterology). All endoscopists had no or minimal experience with OBI. GERD was diagnosed using the Los Angeles classification with or without OBI. Results: The mean weighted paired κ statistics for interobserver agreement in grading erosive esophagitis by conventional endoscopy in the expert group was better than that in the trainee group (0.51 vs 0.42, p<0.05). The mean weighted paired k statistics in the expert group and in the trainee group based on conventional endoscopy with OBI did not differ (0.42, 0.42). Conclusions: Interobserver agree-ment in the expert group using conventional endoscopy was better than that in the trainee group. Endoscopic experience can improve the interobserver agreement in the grading of esophagitis using the Los Angeles classification. (Gut Liver 2014;8:154-159)

        A Multicenter, Randomized, Comparative Study to Determine the Appropriate Dose of Lansoprazole for Use in the Diagnostic Test for Gastroesophageal Reflux Disease

        Background/Aims: The diagnostic proton pump inhibitor test (PPI test) is a method used in diagnosing gastroesophageal reflux disease (GERD). This study aimed to determine the appropriate dose of lansoprazole for use in the diagnostic test for GERD. Methods: This study was a randomized, controlled, multicenter trial in the Daegu-Gyeongbuk area. Patients with typical reflux symptoms such as regurgitation and heartburn for at least three months were enrolled in this study. Patients were divided into two groups, the erosive reflux disease (ERD) group and the non-erosive reflux disease (NERD) group, and randomized to 14 days of treatment with lansoprazole at a dose of 15 mg, 30 mg or 60 mg once daily. The PPI test was considered positive if the patient`s symptoms improved by more than 50%. Results: A total of 218 patients were enrolled, and analysis was performed on the 188 patients who completed the study. The PPI test was positive in 93.2% of the ERD group and 87.2% of the NERD group. A positive PPI test was observed in 91.7%, 89.4%, and 87.2% of the 15 mg, 30 mg, and 60 mg groups, respectively. Significant symptom score changes were observed starting on day 8 for the 15 mg, 30 mg, and 60 mg groups. Conclusions: In this multicenter, randomized study of Korean patients, the standard dose of lansoprazole was as effective as a high dose of lansoprazole in relieving the symptoms of GERD, regardless of the presence of ERD, by day 14 of treatment.         미국흰불나방 (Hyphanria cunea)에 대한 곤충병원성선충 Steinernema carpocapsae의 병원성

        Environmentally sound control of fall webworm, Hyphanria cunea (Drury) with entomopathogenic nematode, Steinernema carpocapsae Pocheon strain was evaluated in the laboratory and pot. Pathogenicity of S. carpocapsae Pocheon strain was different depending on larval stage, i.e., mortality of the 2nd instar and the 3~4th instar was 100% with > 20 infective juveniles (Ijs)/larva in 3 days, but the 5th instar was 34% with 80Ijs/larva in 3 days. Pathogenicity of S. carpocapsae Pocheon strain was higher with increasing nematode concentration. Mortality of Hyphanria cunea larva by S. carpocapsae Pocheon strain was not significantly different (more than 70%) between nematode concentration on treated trees (Malus alba and Platanus orientalis) and in pot.

      • 방진고무의 피로특성에 미치는 열노화손상의 영향

        천연가황고무로 제작된 자동차 냉각모듈 방진고무는 엔진룸의 온도상승으로 인한 열스트레스와 냉각모듈 팬회전에 의한 진동피로하중을 동시에 받고 있다. 특히 열스트레스에 의한 손상은 피로특성에 직접적인 영향을 미친다. 본 연구에서는 70℃, 100℃와 120℃에서 1주, 2주, 3주, 4주 동안 선노화된 인장시편과 2 주, 4 주 동안 선노화된 방진고무부품으로 인장실험과 진동피로실험을 수행하였다. 인장실험결과를 통해서 정적인 기계적특성의 변화와 각 온도에 따른 노화메커니즘을 분석하였다. 진동피로실험 후, 열노화손상에 따른 피로특성을 분석한 결과 노화온도와 노화시간이 증가함에 따라 피로특성의 감소가 뚜렷하였다. 또한 피로특성에 미치는 열스트레스로 인한 손상을 지지하중과 하중이력의 변화로 설명하였다. Cooling module rubber mounts which is manufactured by natural vulcanized rubber is experienced by temperature stress and vibration fatigue load. Specially, damage by temperature stress causes direct effects in fatigue property. In this research, to conduct a vibration fatigue experiment by rubber mount to do heat aging for 2 and 4 weeks in 70℃, 100℃ and 120℃. When vibration fatigue property for heat aging temperature was analyzed and temperature increases, decrease of fatigue property showed remarkably. Also, the damage by temperature stress in fatigue property was explained this by change of support load, of load history.
