Most local recurrences following mastectomy with flap reconstruction develop in the superficial skin and subcutaneous fat layer or at the mastectomy site. However, there is a possibility of local recurrence in the reconstructed tissue with lymphangiogenesis into the reconstructed flap if the flap was harvested from a distant organ. We herein report a case of a patient with invasive carcinoma that exhibited extensive lymphovascular invasion and tumor emboli at the initial diagnosis and repeatedly recurred not only in the original breast skin and tissue, but also in the reconstructed flap.

An 82-year-old man diagnosed with supraglottic cancer sought a consultation for percutaneous endoscopic gastrostomy (PEG) placement. Preoperative chest radiography (posterior-anterior [PA] view) revealed no abnormalities, and PEG tube placement was performed using the “pull” method. Chest radiography (PA view) performed 3 days postoperatively showed free air that was not observed immediately after PEG tube placement; therefore, the patient was diagnosed with pneumoperitoneum. Abdominal computed tomography confirmed that the PEG tube was appropriately positioned within the stomach; however, the colon was observed between the abdominal wall and stomach, which indicated that the PEG tube had passed through the colon. Review of preoperative chest radiographs (PA views) confirmed that the colon was visualized in the area wherein usually stomach gas should have been observed. The patient was diagnosed with a gastro-colo-cutaneous fistula that occurred postoperatively, following a procedure that was performed without confirmation of anatomical variations. The PEG tube was removed surgically, and we performed percutaneous gastrostomy.
Human DJ-1 and its homologs are novel glyoxalases.

<P>Human DJ-1 is a genetic cause of early-onset Parkinson's disease (PD), although its biochemical function is unknown. We report here that human DJ-1 and its homologs of the mouse and Caenorhabditis elegans are novel types of glyoxalase, converting glyoxal or methylglyoxal to glycolic or lactic acid, respectively, in the absence of glutathione. Purified DJ-1 proteins exhibit typical Michaelis-Menten kinetics, which were abolished completely in the mutants of essential catalytic residues, consisting of cysteine and glutamic acid. The presence of DJ-1 protected mouse embryonic fibroblast and dopaminergically derived SH-SY5Y cells from treatments of glyoxals. Likewise, C. elegans lacking cDJR-1.1, a DJ-1 homolog expressed primarily in the intestine, protected worms from glyoxal-induced death. Sub-lethal doses of glyoxals caused significant degeneration of the dopaminergic neurons in C. elegans lacking cDJR-1.2, another DJ-1 homolog expressed primarily in the head region, including neurons. Our findings that DJ-1 serves as scavengers for reactive carbonyl species may provide a new insight into the causation of PD.</P>

<P><B>Abstract</B></P> <P>To achieve efficient photodynamic activity, substantial effort has been dedicated to precise control of the intracellular localization of current photosensitizers (PSs). Given the extremely small radius of action of singlet oxygen, the direct targeting of PSs to the mitochondria is expected to greatly enhance the photodynamic therapy (PDT) activity. Here, we report mitochondria-targeting 6-(furan-2-yl)- and 6-(thiophen-2-yl) indolizino[3,2<I>-c</I>]quinolines (IQs) as novel PSs. IQ derivatives containing 5-membered heterocyclic aromatic rings were synthesized, and their photophysical properties as PSs were characterized. The anticancer potentials of <B>2a</B>-<B>2f</B> were investigated using various cancer cell lines, and they exhibited dose-dependent and light exposure time-dependent cytotoxicity. Among the synthesized compounds, <B>2b</B>, which contains a furan ring, showed dual functions as an imaging probe as well as a PS. Real-time confocal fluorescence images revealed the mitochondrial localization of <B>2b</B> as a primary site of photodamage in live cells. Targeted reactive oxygen species (ROS)-generation capabilities and the photoinduced DNA cleavage of IQs led to mitochondrial dysfunction and photoinduced apoptosis via the intrinsic pathway. 3D RI tomograms of individual live HeLa cells treated with <B>2b</B> showed that the progress of photoinduced apoptosis was affected by the PS concentration and light irradiation time.
