Primary Dysmenorrhea: Pathophysiology, Diagnosis, and Treatment Updates

Primary dysmenorrhea (PD) is a common, disregarded, underdiagnosed, and inadequately treated complaint of both young and adult females. It is characterized by painful cramps in the lower abdomen, which start shortly be-fore or at the onset of menses and which could last for 3 days. In particular, PD negatively impacts the quality of life (QOL) of young females and is the main reason behind their absenteeism from school or work. It is suggested that increased intrauterine secretion of prostaglandins F2α and E2 are responsible for the pelvic pain associated with this disorder. Its associated symptoms are physical and/or psychological. Its physical symptoms include headache, lethargy, sleep disturbances, tender breasts, various body pains, disturbed appetite, nausea, vomiting, constipation or diarrhea, and increased urination, whereas its psychological symptoms include mood disturbances, such as anxiety, depression, and irritability. While its diagnosis is based on patients’ history, symptoms, and physical exam-ination, its treatment aims to improve the QOL through the administration of nonsteroidal anti-inflammatory drugs, hormonal contraceptives, and/or the use of non-pharmacological aids (e.g., topical heat application and ex-ercise). Patients must be monitored to measure their response to treatment, assess their adherence, observe poten-tial side effects, and perform further investigations, if needed.
Application of Multi-Valued AdaBoost to Control of Autonomous Mobile Robot

The autors proposed a tuning method, which made use of good generality of rule teaching and high accuracy of playback-type teaching. This method utilized a feature of boosting algorithm that constructed a strong classifier by combining multiple weak classifiers. However, the obtained controller had a limit in accuracy because this method was based on two-valued classifiers. This paper presents a modified boosting algorithm which is capable of aggregating mult-valued weak classifiers, and of identifying accurate and general control rules. Simulations of pass passage problem of a mobile robot are carried out to test the proposed method. It is shown that this method acquires control rules more accurate than those obtained by the conventional two-valued boosting algorithm, and in the limited taught space, generalization of the final hypothesis by the proposed method is demonstrated.
Capital Regulations, Risk and Efficiency Nexus in Emerging Economies

This research examines the dynamic linkage among capital regulations, risk, and efficiency in the Pakistani and Indian banking industries. This paper has used the Generalized Method of Moments (GMM) during the period between 2009–2016. The study covers findings on both countries collectively and separately as well. The results of the study reveal that while capital requirements have a positive impact on the overall risk, they proved to have also a negative impact on the overall efficiency of both countries. The results unveil that Basel capital regulations have increased the banks’ risk-taking; however, these capital regulations decrease the collective banks’ efficiency in both countries. Furthermore, individual capital regulations in Pakistan and Indian banks have a positive effect on the risks of these banks. These capital regulations have no effect on Pakistani banking efficiency while these regulations decrease the efficiency in Indian banks. The results have shown that these regulations have an identical positive effect on the risk of each country but that is not the case of bank efficiency of each country. Basel III capital regulations on both countries’ banks neither increase nor reduce the bank risk.
Pulmonary Embolism Secondary to Hydatid Cyst of the Right Ventricle: A Case Report

Cardiac hydatid cyst is a rare disease that accounts for 0.5–2% of all hydatid disease. It most commonly involves the left ventricle (60%), followed by the right ventricle (15%), interventricular septum (5–9%), and right atrium (3–4%). A 54-year-old female, who presented with dyspnea was diagnosed with pulmonary embolism and was found to have cardiac hydatid cyst of the right ventricle. The patient was successfully treated with surgery plus albendazole therapy pre- and post-surgery.
The Potential of Rural Farm Families in Resource Management in Tanzania

Natural environment and socio-economic dimensions are interdependent because people get their basic needs from the environment, and inversely, for the integrity of the environment to be sustained, livelihood practices must become environmental friendly. Deterioration of environment and livelihoods interests in the Matengo highlands in Mbinga district triggered farmer-based initiatives through the facilitation of external based development partners. However, while farmers have organised themselves into groups in attempt to address the experienced livelihood-environment challenges since 2002, scant information exists on the role of farmers’ groups in that regard and hence this is the rationale for the present article. Using satellite data the article first substantiates the contention on deterioration of the natural environment by estimating the change in forest cover. Then, the role of farmers’ groups in improving the livelihood and conserving the environment is unveiled through focus group discussion, key informant interviews, informal interviews and observation methods. Consequently, recommendations are given as a way forward for improving the harmony between livelihood integrity and the sustainability of the natural environment.

In order to characterize the transit output response of a 0.2-μm dual-gate InGaAs/InAlAs fieldeffect transistor (HEMT), we monolithically integrate a 0.2-μm interdigitated InAlAs-based photoconductive switch (PCS), a 0.2-μm dual-gate InGaAs/InAlAs HEMT, and coplanar waveguide transmission lines on an InP substrate. The impulse response of the 0.2-μm interdigitated InAlAsbased PCS is estimated to be as fast as 550 fs at full-width at half maximum (FWHM), corresponding to a 3-dB bandwidth of approximately 800 GHz. The transit output response of the 0.2-μm dual-gate InGaAs/InAlAs HEMT is estimated to be 7.3 ps at FWHM, indicating the cutoff frequency (ft) of approximately 136 GHz. This demonstrates that the electro-optical sampling technique is useful for accurately estimating the transit output response of ultrafast devices.

Objective: This study aimed to assess gynecologic oncologists (GOs)’ perceptions and attitudes toward cancer survivorship to help improve survivor care. Methods: We conducted a web-based questionnaire survey about survivorship issues for the GOs belonging to the Japan Gynecologic Oncology Group. We analyzed the proactiveness of the participants toward addressing 25 survivor issues. In addition, the practice patterns and barriers to care for survivors’ long-term health issues, such as second primary cancer (SPC) and lifestyle-related diseases (LSRD), and return-to-work (RTW) support were assessed. Results: We received 313 responses. The respondents had a mean of 22 years of physician experience. The ratio of men to women was approximately 7:3, and 84.7% worked at facilities for multidisciplinary cancer treatment. The respondents’ proactiveness for addressing psychosocial problems was significantly lower than physical and gynecological issues (p<0.01 by χ2 test). However, most GOs tried to contribute to such issues according to patients’ demands. Women GOs were more proactively involved in some survivorship issues than the men (p<0.05 by logistic regression analysis). The rates of the respondents who proactively discussed SPC, LSRD, and RTW were unexpectedly high (60.7%, 36.1%, and 52.4%, respectively). However, the GOs only provided verbal support for these issues in many cases. Conclusion: The Japanese GOs were enthusiastic about survivorship care. However, their tendency to deal with survivors’ problems through their own knowledge and judgments raises concerns about the quality of care. Therefore, creating survivorship care guidelines and enhancing multidisciplinary collaboration should be prioritized.
