        성인의 천미추부 종괴

        We developed an animal model to recreate the condition of an open fracture in communication with the maxillary sinus. We then studied wound healing of the sinus wall structures following fracture in the presence of autogenous bone and alloplastic implant. This model is designed to simulate the repair of an orbital floor fracture in humans. The New Zealand White rabbit was used as the animal model. Standardized 8mm defects were made bilaterally in the maxillary sinuses to include bone and mucosa in 36 rabbits. Two different implants and autogenous calvarial bone graft were placed in the soft-tissue pockets to obturate the defects, exposing one surface of the implant to the open sinus. Medpor porous polyethylene, silicone and calvarial bone implant were compared. Animals were killed at 1, 2 and 8 weeks after implantation. Gross examination of the specimens for the amount of mucosal closure and implant tissue fixation was performed. Histological sections were evaluated for bone and soft-tissue morphology juxtaposed to the implant. Complete closure of the mucosal defect was demonstrated with each type of implant. Medpor implants showed both vascular and soft-tissue ingrowth into pores by week 1. Bone ingrowth was seen by week 2. Closure of the Medpor obturated defects occurred more rapidly than in the silicone group. The Medpor implants and calvarial bone demonstrated bone and soft-tissue fixation, callus formation and maturation, while mature overlying mucosa was reconstituted over the defects. Silicone implants demonstrated a fibrous tissue reaction within 1 week of implantation and they never became fixed to bone or soft tissue. Maxillary sinus wall regeneration occurred in all defects. This study supports clinical observations of maxillary sinus wall regeneration in humans. Although sacrococcygeal mass is rare and usually found in infants or children, adolescent or adult patients with protruding mass in sacrococcygeal region occasionally come to us simply for a cosmetic problem. In this situation, even though there is no definite neurological deficit, it should be evaluated whether or not the underlying bony pathology or dural defect exists. Few cases about the sacrococcygeal mass have been reported in adults. We reviewed our cases including preoperative evaluation methods and postoperative diagnosis. From March, 1993 to February, 1997, we experienced 6 adult patients with sacrococcygeal mass and no neurological abnormality. Preoperative evaluation were made by plain X-ray, myelogram, computed tomography(CT), and magnetic resonance imaging (MRI), as needed. Postoperative diagnoses were 2 meningoceles, 2 lipomyelomeningoceles, 1 desmoid tumor, and 1 teratoma. From our experiences, CT or MRI is essential to evaluate the sacrococcygeal mass preoperatively. These methods can visualize the precise anatomic location and extent of the mass, its relation to the spinal cord, and associated bony abnormalities. MRI is superior to CT, especially in defining the nature of the mass and involvement of the spinal cord. Conclusively, even a simple mass in the sacrococcygeal region in adults needs MRI or CT evaluation, and MRI is the most valuable method of evaluating the mass preoperatively and provides important information to establish a treatment plan.

        아동행동 조사표에 의한 입원한 행동장애 청소년 분류

        To make a suchclassification of conduct disorder by using CBCL. the authors investigated 37 conduct disorders adolescents in Seoul National Mental Hospital during the period from June 1, 1987 to June 30, 1991. The results were as follows : 1) The total numbers of conduct disorder adolescents are 37 patient, and 4 clusters are subclassified. 2) In one way ANOVA. the scores of externalizing and internalizing showed statistically significance. 3) To compare the score of subscale factor, in parent form CBCL, delinquent, aggressive, hyperactive factor showed statistically significance, and the remainder factors are not significant. 4) In comparison between clinical data and clusters, the age of symptom onset showed statistically significance, and orders are cluster 4, cluster 2, cluster 3 and cluster 1. The X2 test, the sex showed statistically significant the ratio of females 60.0% in cluster 2, 14.3% in cluster 3, 12.5% in cluster 1 and 0% in cluster 4. 5) i) In the main and interaction effects between 4 clusters and 2 forme of CBCL in each matched CBCL subscale, external, internal. aggressive(P<0.001) and deliniquent(P<0.01) factor showed statistically significant. ii) In the two forms of CBCL, social, school and total factor showed statistically significant. The remainder factor showed statistically no significance. iii) In the clusters and two forms of CBCL. the external and aggressive and internal factor showed statistically significance and in the cluster 1, the score of YSR of external and aggressive factor are higher than parent form CBCL, and in cluster 3 the scores of parent form and higher. In cluster 4, the score of internal factor of parent form CBCL is higher than the self form.

        실리콘 겔에 활성화된 복강 대식세포의 interleukin-6 및 tumor necrosis factor-α에 의한 섬유모세포 중식 자극

        Silicone gel breast implants may induce local(fibrous capsular contracture) or systemic(rheumatoid arthritis, systemic sclerosis, etc) complications. The exact mechanism of fibrous capsular contracture has not been fully understood. In the present study, we tried to find out the effect of silicone gel on the fibroblast proliferation which has been known as a major contributing factor in fibrous capsular contracture formation. In vitro, activated macrophages are known to secrete monokines which affect fibroblast proliferation and collagen synthesis. And tumour necrosis factor-α(TNF-α) and interleukin-6(IL-6), which were released by macrophages, were reported as potent stimulator of fibroblast proliferation. The goal of this study is to investigate the role of macrophages and tumour necrosis factor-αor interleukin-6 in the interaction of fibroblasts and silicone gel. We designed four groups, two experimental and two control, using Institute for Cancer Research(ICR) mouse peritioneal macrophage and silicone gel. For the preparation of the conditioned medium of macrophages, peritoneal macrophages were prepared and cultured for 24 hours on the silicone gel-coated and naked (not coated) surface [silicone gel-macrophage conditioned medium(SCM; experimental group) and normal polystyrene-macrophage conditioned medium(NCM; control group) respectively]. To correct the effect of 10% fetal bovine serum which was included in Rapid Prototyping and Manufacturing Institute (RPMI) 1640 medium and draw the effect only by macrophages, the RPMI 1640 medium with 10% fetal bovine serum was cultured by the same method on the silicone gel-coated and naked surface (silicone gel-macrophage free conditioned medium; SFM and normal polystyrene-macrophage free conditioned medium; NFM respectively). Each conditioned medium was added onto NIH 3T3 fibroblasts culture at a final 25% concentration of total culture medium and followed by the cultivation for 24 hours. For antibody neutralizing experiments, each conditioned medium was preincubated with polyclonal rabbit anti-mouse TNF-α antibody or polyclonal rat anti-mouse IL-6 antibody for 1 hour and then, conditioned medium with antibody was added to the culture medium of NIH 3T3 fibroblasts by the same method. After 24 hours cultivation, total number of viable fibroblast(cell growth), DNA synthesis and collagen synthesis of fibroblasts with each medium were measured by sulforhodamine B(SRB) assay, 3H-thymidine and 3H-proline incorporation respectively. The results were as follows: 1. In the experiment about the effect of the conditioned medium on the fibroblast activity, the experimental group(SCM), compared with the control group(NCM), showed a significant increase of the cell growth (p<0.01), a significant decrease of DNA synthesis(p<0.001), but no significant difference in the collagen synthesis. 2. In the experiment about the effect of polyclonal rabbit anti-mouse TNF-α antibody on the fibroblast activity, after the addition of antibody the experimental group, compared with the control group, showed a significant decrease of the cell growth(p<0.001), a significant increase of DNA synthesis(p<0.01), but no significant difference in the collagen syn thesis. 3. In the experiment about the effect of polyclonal rat anti-mouse IL-6 antibody on the fibroblast activity, after the addition of antibody the experimental group, compared with the control group, showed a significant decrease of the cell growth(p<0.001), a significant increase of DNA synthesis(p<0.0001), but no significant difference in the collagen synthesis. In conclusion, culture supernatants (conditioned medium) of peritoneal macrophages, activated by silicone gel, stimulate the NIH 3T3 fibroblast proliferation. TNF-α and IL-6, products of macrophage, are involved in the stimulation of NIH 3T3 fibroblast proliferation in an in vitro condition.

        Human sparganosois was reported more than 100 cases in Korean population. However no case reported yet in Korea as a patient was infected by sparganum from other geographical area, such as Thailand. The case we are reported here is a 38 year-old Korean male who had been suffered from mass being a small, round and soft consistance in upper portion of his right thigh. The patients stated that he had an opportunity to eat living snake in Bangkok, Thailand during trip. As therapy praziquantel (75㎎/㎏/day) was given p.o. for 14 days, but in vain to cure. Following medical treatment, surgical exicision was performed to remove a alive, soft, whitish-milky, mobile and elongated sparganum. Serlogical test by micro-ELISA for sparganum-specific antibody revealed a high serum antibody level of absorbance 0.67 (positive criterion: absorbance 0.22).

        In this study is behavior of crack propagation and fracture under static tensile load, shear load, rotary bending fatigue load in friction welded dissimilar materials. Crack was observed in fine ferrite zone of SM15C and heat affected zone of STS304. Plastic flow was occured by friction welding process, so direction of crack is inclined with welded surface. Fatigue fracture most occurred in SM15C heat affected zone around interface. Fibrous zone with the view of macroscope and dimple pattern with the view of microscope were observed at fracture surface. Result is more metal micrstructure dependence than stress dependence by mechanical and metallic mechanics.

        무갑상선 쥐에서 중식성 반흔의 새로운 실험모델

        Proliferative scarring in the form of keloids and hypertrophic scars continues to be a clinical problem for some patients. The lack of an animal model for such scarring has been an obstacle to studying the biology and effective therapy of these entities. Consequently we created an accurate reproductive animal model to systematically study them. Human proliferative scars were explanted into flaps based on isolated vascular pedicles in congenitally rats. We compared the procollagen type Ⅲ peptide levels of proliferative scar tissue before and after explanting. The procollagen type Ⅲ peptide levels of explanted proliferative scar tissue remained increased as before explanting. Histological analysis of the explanted proliferative scar tissue revealed that all explants retained their original histotypic character even after 1 year. We could also retain the volume of implanted proliferative scar for 1 year and studied in vitro cellular proliferation. Fibroblast cultures from explanted scars demonstrated less aggressive growth characteristic than those from original surgical specimens. The advantages of this animal model are as follows: 1. The explants retain their histotypical character for a long period. 2. Placement of the explants outside the dorsum of a nude rat makes serial observation and measurement easier. 3. Agents under test can be injected into the explants through a catheter inserted into a single pedicle of island flap without the possibility of spreading systematically.

      • Characteristics of Bird Inhabitation and Management in World Heritage Changdeok Palace Rear Garden

        The Changdeok Palace (Changdeokgung) has been registered as a world heritage site in December of 1997 for its combination of well-preserved architecture and surrounding nature. The palace functions as a representative tourist location of Seoul as well as a forest within a city which shows an ecosystem within a cityscape. The Changdeok Palace's rear garden was opened to the public in 2004 and thus required suitable management and stable management of the forest in the garden. Study of the wild avian fauna of the forest showed 161 individuals (Peak count) of 41 species, and dominant species included the magpies (Pica pica), brown-eared bulbul (Hypsipetes amaurotis), dusky thrush (Turdus migratorius), tree sparrow (Passer montanus), vinous-throated parrotbill (Paradoxornis webbianus fulvicauda) and the long-tailed tit (Aegithalos caudatus), in descending order. Since the study area is located near the city, magpies was the most dominant species, but the location was also providing habitat for various endangered and government protected bird species such as birds of prey and the mandarin duck. The effect of the visitors to the rear garden of the Changdeok Palace on its avian fauna was minimal, and the study has proposed future plans for management of wild avian habitat, of visitors and users, of trails and natural enemies.

      • Variation of Bird community and Habitat use after Developing Hangang river Ecological Learning Center

        The Hangang (Han River) Ecological Learning Center is a river ecosystem park which was completed in December of 2002. The river ecosystem park was created based on the understanding of the problems of standard park-formed river which centers around convenient facilities instead of on the wish to accentuate the innate unique characteristics of a river. Therefore, the study was conducted in order to analyze the changes and habitation usage of the avifauna in the Hangang Ecological Learning Center to provide basic information for the improvement of bird species diversity in an ecosystem park. The study was conducted in July and October of both 2003 and 2005 to study the avian colonies of the same time period, and the habitat usage of four different areas were examined. Main dominant species includes species which preferred waterside and grassland, and 2005 showed higher levels of species, bird count and number of migratory birds. Furthermore, among the four regions used as habitats, the confluence region showed the highest level of increase, and this is predicted to have caused by the shallow depth of the confluence and the stability of the waterside plant colonies.

      • Inhabiting Condition and Preservation of Black-faced Spoonbill around Ganghwa County

        Regular monitoring on the Black-faced Spoonbill and survey on their breeding were performed in the vicinity of Ganghwa County from August 2002 till July 2003 in order to understand the inhabiting conditions of the endangered species of the bird (EN) defined by IUCN. Maximum 230 individuals of the Black-faced Spoonbill around the County were observed at the southern tidal flat of Ganghwa, which was the largest and 51 at Gyodongdo Island, 55 at Boreumdo Island while one at Seokmodo Island. No Black-faced Spoonbill was observed at Jumundo Island. For the inhabiting status of the bird as per the time of survey, 203 individuals were observed in August 2002, which was recorded as the largest of its kinds, on the other hand, only 5 individuals were found in March, which was the least. No Black-faced Spoonbill was observed from November till February of the following year. For the areas of survey, more individuals of the endangered bird were observed at the southern tidal flat of Ganghwa during the period of survey than other areas and it is because the bird took a rest at the southern tidal flat of Ganghwa and fed their foods. Breeding of the bird was verified at Yudo Island, Seokdo Island, Bido Island and Suribongdol Island and the breeding status as per each island is 100 individuals at Yudo Island, 10 at Seokdo Island, 8 at Bido Island and 5 at Suribongdol Island. On the other hand, the Black-faced Spoonbill found at the southern tidal flat of Ganghwa fed their foods at the nearby rice paddy during May and June. Heavy rainfall in 2007 and the pressure on developing the area surrounding Ganghwdo Island may force the endangered birds to breed at the artificial island of the Incheon Songdo International City or the main inhibiting place is in danger of vanishing. In order to protect the Black-faced Spoonbill, therefore, development and use of not only the tidal flat but also the rice paddy that is the wetland with fresh water and shallow tidal flat should be minimized. Also, converting to the eco-friendly development plan of the reclaimed land and collecting information to restore the inhabiting place of Black-faced Spoonbill is essential.
