Objectives : Lumbar disc herniations at the L1-L2, L2-L3, and L3-L4 level are rare. The purpose of this study is to observe the variable clinical finding of upper lumbar disc herniations and to provide proper treatment modality. Patients and Methods : Between 1998 Jan. and 1999 Dec., seventeen patients with upper lumbar disc herniations who were undergone surgery in our institute were retrospectively evaluated. Patients were evaluated based on following factors : age, sex, aggravating factor, symptoms and signs type of disc herniation, type of surgery, and outcome of post-surgery. Results : The incidence of lumbar disc herniation at the level of L1-2, L2-3 and L3-4 is 6.1% with declining frequency as the level ascends. The peak age incidence is 6th decade in both sex. Preoperative symptomes and signs are variabe. In sixteen cases, posterior approach was done. In these cases, twelve cases were performed microscopic partial hemilaminectomy, and four cases with spinal stenosis were performed total laminectomy and posterior screw fixation with bone fusion. In one case, lateral extracavitary approach was done. In results of operation, fourteen cases showed more than good grade(82%). Conclusion : The age incidence of lumbar disc herniations at the level of L1-2, L2-3 and L3-4 older than lumbar disc herniations at the lower level of L4-5 and L5-S1. The signs and symptoms are variable. In our cases, most of the patients were performed posterior approach with microscopic partial hemilaminectomy except the patients combined spinal stenosis. One case was performed a lateral extracavitary approach because the risk of the cord and cauda equina injuries. The prognosis of upper lumbar disc herniations after treatment with only microscopic partial hemilaminectomy and diskectomy is comparable with the prognosis of lumbar disc herniations at the lower level.
전립선암과 전립선비대증 환자에서 혈청 유리 및 총전립선특이항원 측정의 의의

Purpose: This study was peformed to investigate whether the free to total PSA ratio(F/T PSA ratio) offers a better discriminant for the detection of patient with prostatic cancer(CaP) and those with benign prostatic hyperplasia(BPH) than serum total PSA alone. Materials and Methods: From September 1996 to February 1999, free and total PSA levels were measured on frozen sera samples of 154 men with BPH and 46 with CaP using immunoradiometric assay. Results: In all subjects, the mean total PSA was significantly higher in CaP group(72.57±66.58ng/ml) than BPH group(3.12±4.10ng/ml). The mean PSAD was significantly higher in CaP group(1.64± 1.86) than BPH group(0.08±0.01). The mean F/T PSA ratio was significantly lower in CaP group(0.19± 0.07) than BPH group(0.30±0.13). Among 41 subjects whose total PSA were between 4-20ng/ml, the mean PSAD was significantly higher in CaP group(0.37±7.16) than BPH group(0.17± 0.07), but the mean F/T PSA ratio was not significantly different between CaP(0.19± 0.10) and BPH group(0.25±7.13). By comparing the sensitivity and specificity of total PSA alone, PSAD and F/T PSA ratio on receiver operating characterislic (ROC) Cutie in all subiects and subiects with total PSA between 4-20ng/ml, F/T PSA ratio had no superiority than total PSA alone and PSAD for discrimination between CaP and BPH. Conclusions: We suggest that the F/T PSA ratio is not superior to total PSA alone and FSAD in the detection of prostatic cancer, and further evaluation of the usefulness of F/T PSA ratio should be required.

Arteriovenous fistula of uterus is a very rare cause of massive uterine bleeding. However, it is important to make a rapid and precise diagnosis when it does occur, because life-threatening massive vaginal bleeding may occasionally be a symptom of this disease. Uterine curettage or surgical trauma can cause vascular abnormalities, including pseudoaneurysms, acquired ateriovenous malformations, ateriovenous fistulas, and rupture of vessels. The diagnosis is made by angiography, and gray scale ultrasonography, color and duplex doppler ultrasonography, computed tomography, magnetic resonance imaging is helpful. Ultrasonography is the most commonly performed initial imaging examination for evaluation of abnormal uterine bleeding. Color and duplex Doppler US is an appropriate modality for the detection and diagnosis of uterine ateriovenous malformations and for follow-up after embolization. Transcatheter embolization has replaced hysterectomy as the treatment of choice in women wish to retain thier fertility. A case of arteriovenous malformation of the uterus which was diagnosed by ultrasonography. So reported this case with a brief review of the literature.

Advancement of the digital media has brought, upon requests from users, the evolvement of space into various forms. Stories, information, and events in such diverse forms of space have evolved in parallel. Interactions between senses of sight, touch, and hearing and the sophisticated movements of the media serve as triggers for responses in the spatial environment. The thesis focuses on analysis on the expression and language of spatial design in accordance with expressional types of the spatial environment. The responsive environmental space means the space where reacts and adapts to the interaction between humans and nature naturally, rather than to mechanical signals or pre-set configurations. The responsive space, in that sense, could be considered a space with artificial intelligence (IA). Space with real intelligence would cause unexpected, complicated issues and it would be only humans, after all, that could respond to such issues. This means the users of the responsive space should always be humans.

A numerical simulation has been performed to estimate the transient pressure variation in the tunnel when C7 test train passes through the test tunnel in the Kyoeng-Bu high-speed railway. A modified patched grid scheme is developed to handle the relative motion between a train and a tunnel. Also, a hybrid dimensional approach is proposed to calculate the train-tunnel interaction problem efficiently. An axi-symmetric unsteady Euler solver using the Roe's FDS is used for analyzing a complicated pressure field in tunnel during the test train is passing through the tunnel. Usually, this complex phenomenon depends on the train speed, train length, tunnel length, blockage ratio between train and tunnel cross-sectional area, relative position between train and tunnel, etc. Therefore, numerical simulation should be done carefully in consideration of these factors. Numerical results in this study would be good guidance to make test plans, test equipments selection and to decide their measuring locations. They will also supply important information to the pressurization equipment for high-speed train.

본 논문은 자율주행을 위한 Camera, LiDAR, GPS와 같은 센서들을 모바일 플랫폼에 배치해 인지, 판단, 제어로 자율주행 시스템을 제안한다. 제안된 시스템은 각 상황 프로세스를 병렬적으로 구성해 실시간으로 다양한 상황에 대비하고, K-City(한국형 자율주행 실험도시)에서 도심 환경 속 모의실험을 통해 검증하였다. 본 연구를 통해 개발한 자율주행 시스템이 실제 도심 환경에서 적용될 수 있길 기대한다.
