      • 필라델피아 염색체 양성 급성골수성백혈병에서 Imatinib Mesylate로 분자학적 관해를 유도한 후 성공적인 자가 말초혈액 조혈모세포이식이 시행되었던 1예

        저자들은 필라델피아 염색체 양성 급성골수성백혈병 환자에서 복합항암화학요법으로 혈액학적 완전관해를 유도하고, imatinib mesylate를 이용하여 분자학적 관해를 유도한 후, 성공적으로 자가 말초혈액조혈모세포이식을 시행하였던 1예를 경험하였기에 이를 보고하는 바이다. Philadelphia chromosome (Ph¹)-positive acute myeloid leukemia (AML) is very rare disease, comprising 3~4% of the AML, and is a clinically distinct entity with poor clinical outcome with conventional chemotherapy. We report a case of Ph¹-positive AML who underwent successful autologous peripheral blood stem cell transplantation after achieving a molecular remission with imatinib mesylate.

      • A Study of the Collision Detection of Robot Manipulator without Torque Sensor

        In this study, we proposed the collision observer for real-time detection of collisions between service robot and unknown obstacle. In case of a mobile equipped extra manipulator such as a service robot the safety of the systemin service field is very important because a collision between the manipulator and a person may damages against the one. Therefore it is required to detect the collision against the unknown obstacle and to make them safe. This paper proposes current based method to detect unexpected collision without using extra sensor such as the force/torque(F/T), tactile sensor or the visual sensor. Instead of these sensors, the collision is detected by the observer for collision. To detect the collision of the manipulator without force/torque sensor, two parameters are only required for calculation of collision observer. One is current value, the other is control input value. Because the current from the servo amplifier is generated to follow the controller command input, the current can be used for detecting the collision.After detecting the collision of robot manipulator, we can control the robot to make them safe or reduce damage. The proposed collision detection method is implemented in a light weight robot manipulator. It is confirmed that the proposed method is valid by experimental results.

        Current based Compliance Control Method for Minimizing an Impact Force at Collision of Service Robot Arm

        A motion of a robot manipulator under environment for human and robot to coexist should be able to quickly sense external force and react for minimizing a damage due to physical contacts. In the absence of sensing external force, relative motions between robot and human are not predictable and unexpected collisions may occur at some position during motion of the robot arm. This paper proposes a real-time collision detection method and a compliance control based on detecting abnormal current value to minimize an impact force at the moment of collision between service robot arm and unknown obstacle. In the introduced method, the extra sensors such as the Force/Toque sensor or the visual sensor to detect the collision are not necessary. Instead of these sensors, the collision detection and the safety motion are achieved by a simple method based on the current characteristics in according to operating of each joint motor of robot manipulator. In order to evaluate the performance for minimizing an impact force, the proposed method is applied to the developed light weight robot arm for a service robot.

        Differences in the survival rates of older patients with colorectal cancers in 2003 and 2009

        Purpose: The aim of this study was to investigate survival in patients aged ≥70 years who underwent colorectal cancer surgery in 2003 and 2009. In addition, we aimed to identify the factors that could affect survival in these patients. Methods: In a cross-sectional study, a retrospective review of the data for 878 patients ho underwent colorectal cancer surgery with curative intent in the years 2003 and 2009 was performed. The primary outcome was the 5-year overall survival rate (5-OSR), and he clinicopathologic factors that could affect overall survival were analyzed. Results: The 5-OSR was 77.8% and 84.9% in 2003 and 2009, respectively (P = 0.013). Age, American Society of Anesthesiologists physical status classification, stage, type of surgery, and length of hospital stay possibly affected survival per the univariate and multivariate analyses. In patients aged ≥70 years, the 5-OSR in 2009 was 75.9%, which showed improvement compared to 53.7% in 2003 (P = 0.027). The stage, type of surgery, and hospital stay were the variables that possibly affected survival in patients aged ≥70 years per the univariate analysis, whereas the stage (III; hazard ratio [HR], 2.188; P = 0.005) and length of hospital stay (>12 days; HR, 2.307; P = 0.004), were the variables that showed statistical significance on the multivariate analysis. Conclusion: We found that early stage and shortening the length of hospital stay could affect survival in older patients with colorectal cancers. Because of limited evidence on the influence of shortening the length of hospital stay on survival in older patients, further investigations are warranted.

      • Sensorless Control of Brushless DC Motors With Torque Constant Estimation for Home Appliances

        <P>A new sensorless control algorithm for brushless dc motors (BLDCMs) is proposed in this paper. The torque constant of a BLDCM is used as a reference signal for position detection because it is constant during the entire speed range and can be estimated by calculating the ratio of the back electromotive force (EMF) to the rotor speed. By using both a disturbance observer and the torque constant estimation error, the rotor speed can be obtained. The back EMF can be easily obtained from the voltage equation of the BLDCM. The estimated back EMF decreases simultaneously with the estimated torque constant at the commutation point. By using this phenomenon, the commutation of the phase currents can be done automatically at the drop point of the estimated torque constant. Unlike conventional back-EMF-based methods, the proposed method provides highly accurate sensorless operation even under low speeds because only the drop of the torque constant is used for position detection and current commutation. Therefore, the position accuracy is not affected by the electric parameter errors of the BLDCM. Also, this algorithm does not require an additional hardware circuit for position detection. The validity of the proposed algorithm is verified through several experiments.</P>

      • Enhanced Photosensing and Photodynamic Treatment of Colon Cancer Cells Using Methoxy Poly(ethylene glycol)-Conjugated Chlorin e6

        <P>Nanophotosensitizer composed of methoxy poly(ethylene glycol) (MePEG) and chlorin e6 (Ce6) (abbreviated as Pe6) was synthesized for efficient delivery of Ce6 to the colon cancer cells. Pe6 nanophotosensitizer has small diameter less than 100 nm with spherical shape and core-shell structure. They were activated in aqueous solution while Ce6 was quenched due to its poor aqueous solubility. They showed no intrinsic cytotoxicity against normal cells and colon cancer cells. Pe6 nanophotosensitizers showed enhanced cellular uptake, phototoxicity, and reactive oxygen species (ROS) generation at in vitro cell culture experiment. Furthermore, they showed improved tumor tissue penetration and accumulation in vivo animal studies. We suggested Pe6 nanophotosensitizers as an ideal candidate for PDT of colon cancer.</P>

      • 급성 전골수구성 백혈병의 변이형과 감별이 어려운 골수성/Natural Killer 세포 급성 백혈병

        연구배경: 골수성/NK 세포 급성 백혈병은 형태학적 및 면역표지자 검사에서 급성 전골수구성 백혈병의 미세과립성 변이형(M3v)과 유사하여 흔히 M3v로 오진할 가능성이 높은 새로운 백혈병 아형 중의 하나로 알려져 있다. 이에 저자들은 M3와 감별이 어려웠던 골수성/NK 세포급성 백혈병의 임상적 및 혈액학적 특징을 분석하여 보았다. 방법: 전남대학교병원 혈액·종양내과에서 FAB 분류상 골수성/NK 세포 급성 백혈병에 해당되는 환자 3예를 대상으로 세포 형태학적 특성, 면역표지자검사. 염색체 핵형분석, FISH 분석 및 임상적인 특징 등을 후향적으로 알아보았다. 결과: 평균 연령은 37.7세(범위, 18-58) 였으며, 여자가 2예였다. 혈액응고검사는 모두 정상이었고, 1예에서 경막에 과립구성 육종이 동반되었다. 형태학적으로 골수모구는 깊이 함몰된 핵막과 미세한 아주르친화성 과립을 보이면서 소량의 세포질을 보였고, myeloperoxidase와 sudanblack-B에 강양성을 보였다. 면역표지자 검사에서 CD13+, CD56+, HLA-DR-, CD14-, CD19-,을 보였다. 염색체 핵형 분석상 2예에서 t(8;21)에 양성을 보였으며, t(15;17)는 모두 음성이었고, PML-RARα 는 모두 음성이엇다. 3예중 2예에서 완전 관해에 도달하였으며, 이들 중 1예는 12개월만에 재발하였다. 결론: 골수성/NK 세포급성 백혈병은 M3v와는 달리 출혈성 경향이 동반되니 않고, 면역표지자 검사에서 CD56+을 보이며, t(15;17) 및 PML-RARα는 음성을 보이기 때문에 감별 진단이 가능하였다. 향후 이러한 NK 세포 아형들의 명확한 정의와 임상적인 의의가 규명되어야 할것으로 사료된다. Background: The myeloid/Natural Killer (NK) cell acute leukemia is a unique subtype of acute leukemia potentially misdiagnosed as the microgranular variant of acute promyelocytic leukemia (M3v) by morphologic and immunophenotypic finding. In this study, we have conducted a retrospective analysis to evaluate clinical and hematological characteristics of myeloid/NK cellacute leukemia patients who were diagnosed in Chonnam University Hospital. Methods: Three patients with myeloid/NK cell acute leukemia were reviewed by cytomotphology and cytochemical reactions, immunophenotype, cytogentic analysis, fluorescence in situ hydridization (FISH) analysis and clinical characteristics. Results: There was no evidence of disseminated intravascular coagulopathy in there patients. One patient had granulocytic sarcoma in meninges. The blasts on marrow had deply invaginated nuclear membranes and scanty cytoplasms with fine azurophilic granules and high positivity for myeloperoxidase and sudan black-B. Immumophenotypic analysis showed HLA-DR, CD13+, CD56+, CD2-, CD5-, and CD19-. In chromosomal analyses, 2 cases had t (8;21) and 1 case had normal karyotype. However, all patients did not show both t (15;17) and PML-RARα gene rearrangement. Of 3 patients, 2 patients achieved complete remission and one patient relapsed after 12 months. Conclusion: Although the morphology of myeloid/NK cell acute leukemia were similar to that of M3v, myeloid/NK cell acute leukemia showed distinct clinical and hematological findings.

        Disruptive behavioral disorders [oppositional defiant disorder (ODD) and conduct disorder (CD)] are common comorbid disorders in children and adolescents with attention-deficit/hyperactivity disorder (ADHD). ODD/CD may be comorbid up to 60% of children and adolescents with ADHD. Comorbid ODD and CD symptoms can have a significant impact on the course and prognosis o f patients with ADHD. Stimulant medications have shown effectiveness in treating both core ADHD symptoms and oppositional symptoms. Assessment and management of comorbid disruptive behavioral symptoms is an essential part in evaluation and treatment of ADHD patients. Further research is needed to investigate the efficacy and safety of various behavioral and medication regimens for ADHD, comorbid ODD and CD and to examine the relationships between ADHD, comorbid ODD and CD.
