분자생물학적 방법에 의한 Trichophyton rubrum의 동정 및 형별 판정

Background: Trichophyton rubrum is one of the major pathogens causing dermatophytoses on human. The identification of this species by mycological methods are sometimes difficult and time-consuming. Moreover, suitable methods for subtyping of this species are not established yet. Objective: This study was performed to identify and subtype T. rubrum by molecular biological methods. Methods: Total 65 clinical isolates of T. rubrum were included and classified according to the results of 8 mycological tests. Their identification were done by random amplified polymorphic DNA (RAPD) analysis. Subtyping of this species was performed by analyzing the DNA band patterns produced by amplifying the non-transcribed spacer (NTS) area of ribosomal DNA. Results: The 65 strains of T. rubrum could be classified into 5 phenotypic varieties according to the results of mycological tests. All clinical isolates produced identical band pattern with those of standard strains of T. rubrum by RAPD analysis. Amplification of NTS area produced 13 PCR patterns. Conclusion: The confirmative identification of T. rubrum could be done by RAPD analysis regardless of their phenotypic variations. Subtyping of T. rubrum was successfully performed by amplifying NTS area but these PCR patterns were not correlated with their phenotypic characteristics. Background: Treatment of alopecia totalis and alopecia universalis is often challenging and unsatisfactory. Recently, Janus kinase inhibitor has shown promising results. Objectives: The aim of this study is to compare the efficacy and tolerability between oral tofacitinib and conventional modalities for treating refractory alopecia totalis/universalis. Methods: A total 74 patients (18, treated with tofacitinib; 26, treated with conventional oral treatment (steroid ± cyclosporine); 30, treated with diphenylcyclopropenone) were included and their medical records were reviewed retrospectively. Results: After 6 months, 44.4% of patients in the tofacitinib group, 37.5% in the conventional oral treatment group, and 11.1% in the diphenylcyclopropenone group achieved 50% SALT change. During the treatment, 10% of patients in the tofacitinib group, 73.1% in the conventional oral treatment group, and 10% in diphenylcyclopropenone group experienced adverse drug reactions. Conclusion: In conclusion, oral tofacitinib was more effective than diphenylcyclopropenone immunotherapy and more tolerable than conventional oral treatment after 6 months of treatment.

Background: Melasma shows characteristic histological features of photoaged skin. Objective: We evaluated the effect of dermal rejuvenation using a temperature-controlled continuous non-invasive radiofrequency (RF) device on melasma. Methods: Continuous skin heating at the temperature of 43℃ for 20 minutes was performed in ten subjects with melasma who underwent 3 tri-weekly RF sessions. Pigmentation was evaluated with Mexameter<sup>®</sup> and investigator’s global assessment (IGA). Immunohistochemical staining and image analysis was performed to evaluate biopsies from melasma skin before and after the treatment. Results: The lesional melanin index was decreased by 13.7% at week 9. IGA score was improved from 3.50 at baseline to 2.95 at week 9. No significant adverse event was reported. Histologic analysis revealed reduced melanin and increased collagen density and thickness. The expression of procollagen-1 and type IV collagen was increased after the treatment. The number of p16<sup>INK4A</sup>-positive senescent fibroblasts was reduced after the treatment, while the expression of heat shock protein 70 and 90 was increased. Stromal derived factor-1, a senescence-associated anti-melanogenic factor secreted from the fibroblasts, was up-regulated after the treatment, while the level of c-kit was not changed. Conclusion: Thermal skin stimulation by the temperature-controlled continuous RF device improved melasma through dermal rejuvenation.
A preliminary study in melasma following INMODE forma procedure

Background: Melasma is a common hypermelanosis that frequently appears on the face. In recent studies, it is suggested that dermal structural alteration, including solar elastosis and basement membrane disruption, may have an important role in the pathogenesis of melasma. Forma device (InMode Inc, Richmond Hill, ON, Canada, North America) is a radiofrequency dermal heating device used on facial wrinkles and rhytides. Objectives: The aim of this study was to evaluate the effects of Forma device on melasma in a small group of volunteers. Methods: All volunteers were treated 4 times at 3 weeks intervals. Every visit, melanin index and erythema index was measured to assess the improvement in hyperpigmentation and erythema. Clinical photos were evaluated to estimate the degree of skin changes. Results: Clinical and instrumental evaluation showed that melasma improved after INMODE forma treatment. Conclusion: We suggest that treatment with Forma device may have beneficial effects on melasma by dermal remodeling and direct heat delivery on melanocytes.
Treatment of alopecia areata with tofacitinib

Background: Treatment of alopecia areata (AA) is often empiric and not yet satisfactory. Recently, Janus kinase inhibitor showed promising results in Caucasians. Objectives: To investigate efficacy, tolerability, and hair growth trajey of oral tofacitinib monotherapy for Korean AA patients over extended period. Methods: This is a retrospective study of adult AA patients treated with tofacitinib monotherapy for least 4 months. Results: Thirty-six Korean patients with median initial SALT score of 98.9 were included; 91.7% had refractory AA. Twenty-nine (80.6%) patients demonstrated more than 5% SALT change. Twenty patients (55.6%) achieved 50% SALT change at median 7.5 months with 5 mg twice dosage. There were significant differences between SALT 50 achievers and non-achievers: duration of current episode (ρ = 0.009), duration of disease since first onset (ρ = 0.026) and age at onset of first episode (ρ = 0.011). Tofacitinib was well tolerated and there was no any serious adverse effect such as malignancy or tuberculosis reactivation. Conclusion: Oral tofacitinib monotherapy may be effective and tolerable in patients with AA. SALT 50 achievement was associated with followings: duration of current episode, duration of disease since first onset, and age at onset of first episode.
