        치근단 낭으로 오인한 단낭성 법랑모세포종

        Unicystic ameloblastomas are different from conventional ameloblastomas in clinical, radiographic and pathological characteristics. Clinically, the unicystic ameloblastomas tend to occur at an earlier age, and they are mostly located in posterior mandible. The lesions appear as well-circumscribed radiolucent unilocular lesion in radiographical studies. When removing the lesions, however, unicystic ameloblastomas share similar features with odontogenic cysts that the diagnosis may only become evident when the entire specimen is available for pathologic examination. Vickers and Gorin represented the early histological characteristics of ameloblastic change, and Robinson et al. described a distinct variant of ameloblastomas in which the response to enuleation was good, showing a low recurrent rate. We report a case of two identical featured radiolucent lesions on the periapical region of the left mandible. The lesions were thought to be periapical cysts clinically, but diagnosed as unicystic ameloblastoma based on their histologic findings.

      • 들기자세 유형별 L₄-L?의 근전도 분석 : At the Base Knee Angle 무릎각도를 중심으로

        This research is to find out the most suitable lifting posture and to provide fundamental data for preventing usual injuries. To do this, we select five male graduates and five female graduates of P university, analyze their muscular actions in terms of posture types, measure the limit value of weight lifting by comparing each lifting posture type(knee angle 40˚, 80˚, 180˚) with action amplitudes of two parts of erector spinae muscle, and draw our conclusion as follow ; 1. In case of every lifting posture(that is, knee angle 40˚, 80˚, 180˚), females have higher muscular conductivity value in L⁴-L? of right erector spinae muscle than males(p<.001), but in L⁴-L? of left erector spinae males has higher muscular conductivity value than females(p<.001). 2. Males show lower degree of change in muscular conductivity value from 10kg to 25kg weight lifting. 3. Females show gradually increasing degree of change in muscular conductivity value according as weight lifting is heavier. 4. Males show the constant muscular conductivity action of average 50∼110㎶ in the L⁴-L? in case of each lifting posture, knee angle 40˚, 80˚, and 180˚. 5. Females also show the constant muscular conductivity action of average 100∼200㎶ in the L⁴-L? in case of each lifting posture, knee angle 40˚, 80˚, and 180˚. But they show more active muscular conductivity in more than 15kg weight lifting, IOSHIC permission load value. In short, in case of males, weight lifting up to 25kg has little effect on erector spinae muscle but in case of females, weight lifting from more than 15kg has gradual effect on erector spinae muscle. Therefore, the muscular conductivity values of various posture types(by knee angle)are to be identified according to weight lifting steps.

        검도 머리치기 유형에 따른 상지의 근전도 비교 분석

Korean Journal of Sport Biomechanics, Vol. 14, No. 3, pp. 219-233, 2004. The purpose of this study was to compare the EMG activities of four-forearm muscles during Kumdo head striking. The four skilled and unskilled Kumdo club members were selected from D university in B city. Investigated muscles were left brachioradialis, right brachioradialis and left flexor carpi radialis, right flexor carpi radialis. Raw EMG data were collected during the head striking motions and the average EMG were calculated by the frame width of 0.05s, and then the average %MVC were calculated. The average %MVC values of each muscle in each group were compared. The results are as follows. 1) In each group, there were no significant statistical differences between every muscle over the all phases. 2) There were significant differences, however, between skilled group and unskilled group. The former got higher average %MVC at left flexor carpi radialis and the latter at right brachioradialis in the ready phase and in the impact phase.

        표준화된 수술 전 교육이 위암수술 후 환자의 자가간호지식, 수행 및 신체회복에 미치는 효과

        Purpose: The purpose of this study was to identify the effects of a standardized preoperative education program on self-care knowledge, performance, satisfaction, and physical recovery in the stomach cancer patients undergoing gastrectomy. Methods: A nonequivalent control group non-synchronized design was utilized and 63 participants who underwent gastrectomy (31 for experimental group, 32 for control group) were recruited at a university hospital from May to August 2015. Results: There were significant differences between the groups in self-care knowledge (F=17.63, p<.001), performance (F=-9.25, p<.001) and satisfaction (F=-6.91, p<.001). Although the pain levels (F=974.57, p<.001) showed significant differences in each group and 3 time intervals (F=18.26, p<.001), there was no interaction of group and time (F=0.09, p=.917). The highest body temperature at 48 hours after surgery (F=1.32, p=.192), as well as presence of atelectasis (F=2.23, p=.213) indicating a chance of pulmonary complications, and the time of first gas pass (F=-1.05, p=.299), presence of paralytic ileus (F=0.13, p=.719) were not significantly differ. Conclusion: The preoperative education program developed in this study can be utilized as a part of nursing interventions and be beneficial to patients who undergo stomach cancer surgery for their thorough understanding.

        Effects of Change in the Height of Therapy Tables on the Fatigue of Splenius Capitis Muscle and Trapezius Muscle

        The purpose of this study is to analysis of muscle fatigue in the upper trapezius and splenius capitis muscles according to therapy table height variation. The subjects were consisted of 15 healthy adults(10 males, 5 females) who had no medical history of neurological and musculoskeletal problems. In experiment, wireless electrode EMG system was measured for each the upper trapezius and splenius capitis muscles during the treatment performed on table. the differences in the muscle fatigue was compared for 4 types of table height(-6cm, -3cm, 0, +3cm from elbow in 90° flexion position). Muscle fatigue according to therapy table height were significant difference except for left upper trapezius. And muscle fatigue of right upper trapezius and splenius capitis showed significant decrease in +3cm table height compared to -6cm table height(p<.05). Muscle fatigue of right upper trapezius and splenius capitis were the highest in -6cm table height, but those were the lowest in +3cm table height. This study propose to change therapy table height higher than +3cm from elbow in 90° flexion position, if you hope to reduce muscle fatigue.

        Clinical Significance of Epstein-Barr Virus and Helicobacter pylori Infection in Gastric Carcinoma

        Background/Aims: Epstein-Barr virus (EBV) and Helicobacter pylori (HP) coinfection may synergistically induce severe inflammatory responses in the stomach tissue, increasing the risk of developing gastric cancer. We aimed to analyze the effect of EBV and HP coinfection on the clinicopathologic features and prognosis of gastric cancer, as well as to evaluate the role of EBV infection in non-gastric carcinoma with lymphoid stroma (non-GCLS). Methods: Overall, 956 patients who underwent surgery for gastric cancer between September 2014 and August 2015 were eligible and divided into groups, according to GCLS morphology, EBV infection, and HP infection. Clinicopathologic characteristics and oncologic outcomes were analyzed retrospectively. Results: EBV and HP coinfection was significantly associated with male sex, proximal location, GCLS morphology, and equivocal p53 expression (p<0.001). Multivariate analysis revealed that EBV infection alone (hazard ratio [HR], 0.362; 95% CI, 0.131 to 0.996; p=0.049) and lower third location (HR, 0.624; 95% CI, 0.413 to 0.943; p=0.025) were inversely correlated with overall survival. During median follow-up period of 72 months, overall survival rate was not significantly different between the EBV and HP coinfection group and others (97.6% vs 86.8%, log-rank p=0.144). In non-GCLS patients (n=920), overall survival rate was not significantly different between the EBV infection group and others (96.9% vs 86.4%, log-rank p=0.126). Conclusions: EBV and HP coinfection is not an independent prognostic factor for gastric cancer. EBV infection status, regardless of HP infection, affects the clinicopathologic features of all types of gastric cancer. However, it does not lead to a significant difference in overall survival of non-GCLS patients.

      • Minimally invasive treatment for gastric cancer: Approaches and selection process

        <P>Minimally invasive treatment of gastric cancer has emerged as a result of the technical advances, better understanding of gastric physiology, and more knowledge of the biologic behavior of gastric cancer. This treatment results in improved quality of life embodied by smaller incisions, reduced length of hospital stay, and a faster return to productive life. However, minimally invasive treatment for gastric cancer must take into consideration the potential effects of these techniques on tumor dissemination at the time of the treatment procedure, as well as the rates of recurrence and overall survival. Several technical treatment approaches to gastric cancer have now become possible, utilizing endoscopy, laparoscopy, or an open method. Endoscopic mucosal resection (EMR), limited resection, and laparoscopic surgical resection are the currently practiced modalities as the minimally invasive treatment. Lymph node dissection with the minimally invasive techniques is a barrier to its wide application. Although it is not commonly performed in Western countries, the use of minimally invasive treatment for gastric cancer is growing, especially in Korea and Japan. Minimally invasive treatment for early gastric cancer (EGC) has already been shown to be safe and effective in many retrospective series though no prospective randomized studies comparing it to open resection have been performed.       • Optical properties and visible light-induced photocatalytic activity of bismuth sillenites (Bi<sub>12</sub>XO<sub>20</sub>, X = Si, Ge, Ti)

        <P><B>Abstract</B></P> <P>Three bismuth sillenite compounds with the general composition, Bi<SUB>12</SUB>XO<SUB>20</SUB> (X = Si, Ge, Ti), were prepared by a conventional solid state synthesis route and their local crystal structure, light absorption and visible-light induced photocatalytic activities were systematically investigated. The prepared powder samples show comparable particle size distribution (~ 0.5–5µm) and BET surface area (~ 0.8m<SUP>2</SUP>/g). The optical band gap energies were estimated from diffuse reflectance spectra to be in the range 2.6–2.9eV. The local crystal structure variation and its influence on the electronic band structure was elucidated in the three sillenite compounds by Raman analysis and density functional theory (DFT). The visible-light induced photocatalytic activity was evaluated by monitoring the degradation of rhodamine B dye solution under visible-light irradiation (> 420nm). We found that the Bi<SUB>12</SUB>TiO<SUB>20</SUB> powder shows the highest photocatalytic activity, which is attributed to the higher visible light absorption and the smaller band gap resulting from its distinct local crystal structure (longer Bi-O distance).</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Cerebral Arteriovenous Malformation Associated with Moyamoya Disease

        The coexistence of moyamoya disease (MMD) with an arteriovenous malformation (AVM) is exceedingly rare. We report two cases of AVM associated with MMD. The first case was an incidental AVM diagnosed simultaneously with MMD. This AVM was managed expectantly after encephalo-duro-arterio-synangiosis (EDAS) as the main feeders stemmed from the internal carotid artery, which we believed would be obliterated with the progression of MMD. However, the AVM persisted with replacement of the internal carotid artery feeders by new external carotid artery feeders from the EDAS site. The AVM was eventually treated with gamma knife radiosurgery considering an increasing steal effect. The second case was a de novo AVM case. The patient was initially diagnosed with MMD, and acquired an AVM eight years later that was slowly fed by the reconstituted anterior cerebral artery. Because the patient remained asymptomatic, the AVM is currently being closely followed for more than 2 years without further surgical intervention. Possible differences in the pathogenesis and the radiologic presentation of these AVMs are discussed with a literature review. No solid consensus exists on the optimal treatment of MMD-associated AVMs. Gamma knife radiosurgery appears to be an effective treatment option for an incidental AVM. However, a de novo AVM may be managed expectantly considering the possible risks of damaging established collaterals, low flow characteristics, and probably low risks of rupture.
