Total Knee Arthroplasty is a surgery of replacing a knee joint with artificial implants. The objectives of the surgery is to restore and correct the lower limb mechanical axis and alleviate pain in the knee joint. Various robotic systems have been proposed to accomplish high accuracy of the alignment of the lower limb mechanical axis and therefore precise cutting of the femur and tibia. These robotic systems for TKA have drawbacks such as increased surgery time and limitation of surgeon’s direct intervention. These drawbacks mainly arises from using the end mill for the cut and adopting an automated scheme of the process. We propose a novel surgical robotic platform that is able to effectively guide the surgeons during TKA. The robotic platform accurately locates the planes of cut and positions the cutting jig. Surgeons use the oscillating saw which they have been using in the conventional surgery, thus the user experiences the familiar force feedback when making the cut, while greatly reducing the load of human resources and surgery time.

<P>Cl-amidine, which is a small-molecule inhibitor of PAD, has therapeutic potential for inflammation-mediated diseases. However, little is known regarding the manner by which PAD inhibition by Cl-amidine regulates inflammatory conditions. Here, we investigated the effects of PAD inhibition by Cl-amidine on the functioning of DCs, which are pivotal immune cells that mediate inflammatory diseases. When DC maturation was induced by TLR agonists, reduced cytokine levels (IL-6, IL-1 beta, and IL-12p70) were observed in Cl-amidine-treated DCs. Cl-amidine-treated, LPS-activated DCs exhibited alterations in their mature and functional statuses with up-regulated antigen uptake, down-regulated CD80, and MHC molecules. In addition, Cl-amidine-treated DCs dysregulated peptide-MHC class formations. Interestingly, the decreased cytokines were independent of MAPK/NF-kappa B signaling pathways and transcription levels, indicating that PAD inhibition by Cl-amidine may be involved in post-transcriptional steps of cytokine production. Transmission electron microscopy revealed morphotypical changes with reduced dendrites in the Cl-amidine-treated DCs, along with altered cellular compartments, including fragmented ERs and the formation of foamy vesicles. Furthermore, in vitro and in vivo Cl-amidine treatments impaired the proliferation of naive CD4(+) and CD8(+) T cells. Overall, our findings suggest that Cl-amidine has therapeutic potential for treating inflammation-mediated diseases.</P>

This study evaluated the Monte Carlo method for determining the dose calculation in fluoroscopy by using a realistic human phantom. The dose was calculated by using Monte Carlo N-particle extended (MCNPX) in simulations and was measured by using Korean Typical Man-2 (KTMAN-2) phantom in the experiments. MCNPX is a widely-used simulation tool based on the Monte-Carlo method and uses random sampling. KTMAN-2 is a virtual phantom written in MCNPX language and is based on the typical Korean man. This study was divided into two parts: simulations and experiments. In the former, the spectrum generation program (SRS-78) was used to obtain the output energy spectrum for fluoroscopy; then, each dose to the target organ was calculated using KTMAN-2 with MCNPX. In the latter part, the output of the fluoroscope was calibrated first and TLDs (Thermoluminescent dosimeter) were inserted in the ART (Alderson Radiation Therapy) phantom at the same places as in the simulation. Thus, the phantom was exposed to radiation, and the simulated and the experimental doses were compared. In order to change the simulation unit to the dose unit, we set the normalization factor (NF) for unit conversion. Comparing the simulated with the experimental results, we found most of the values to be similar, which proved the effectiveness of the Monte Carlo method in fluoroscopic dose evaluation. The equipment used in this study included a TLD, a TLD reader, an ART phantom, an ionization chamber and a fluoroscope.
경추근막통증증후군에 대한 초음파 유도하 약침 치료 3례 증례보고

Objectives : Cervical myofascial pain syndrome (C-MPS), a key aspect of Visual Display Terminals (VDT) Syndrome, leads to acute and chronic pain due to muscle shortening, potentially causing headaches and movement disorders. This study examines the effectiveness of Pharmacoacupuncture at the intersection of the Splenius Capitis (SC), Levator Scapulae (LS), and Serratus Posterior Superior(SPS) muscles under ultrasound guidance, known as the SLS triangle in treating C-MPS. Methods : Three patients (aged 30 to 37) with acute C-MPS presented at two Korean medicine clinics with severe neck pain and restricted cervical rotation. The evaluation of symptoms before treatment, immediately after treatment, and one day after treatment was conducted using the cervical rotation Range of motion (ROM) and the Numeric Rating Scale (NRS) for cervical pain. Treatments included Acupuncture, Cupping therapy, Chuna manipulation, Physical Therapy, and Pharmacopuncture (Jaha-geo extract, 2cc at the SLS triangle). Results : All three patients showed significant improvement post-treatment. Immediately after treatment, Patient 1’s NRS improved from 7 to 2, Patient 2’s NRS improved from 4 to 1, and Patient 3’s NRS improved from 6 to 2. Directly after the procedure, there were no significant adverse reactions except for one patient experiencing temporary dizziness. When the patients were followed up by phone 24 hours later, no adverse reactions were reported. Conclusions : This study demonstrates that ultrasound-guided pharmacopuncture can significantly reduce pain and improve cervical rotation in patients with cervical myofascial pain syndrome. The findings suggest this non-invasive treatment is both safe and effective, warranting further research through larger-scale clinical trials to validate its efficacy and establish standardized protocols.

<P><B>Abstract</B></P><P><B>Introduction:</B></P><P>Assessments of the range of motion (ROM) in human joints have been widely used to evaluate the joint condition. Although maker based motion capture system is the most popular and practical method in the clinical field, there have been limitations such as the relatively long time required in procedure or influence of attached markers on natural movement. Recently, markerless motion capture systems have been actively developed due to their relatively lower cost and convenience for installation. The POM-Checker (Team Elysium Inc., Seoul, Rep of Korea), a markerless motion capture system, was developed with new algorithms to assess the ROM in human joints. However, the measure of the POM-Checker has not been compared with a golden-standard device in evaluating the ROM in the human joints. So we developed a protocol to evaluate the validation and reliability of the POM-Checker in measuring the shoulder ROM. This study will also provide a standard procedure to measure the shoulder ROM with the POM-Checker and 3D motion analysis system simultaneously.</P><P><B>Methods/design:</B></P><P>This protocol is for a single institution comparative study to evaluate the validity and reliability of POM-Checker. Six participants will be recruited. We will measure the angles of shoulder abduction and flexion with POM-Checker and 3D motion analysis system simultaneously. The primary outcome is the angles of shoulder abduction and flexion.</P><P><B>Discussion:</B></P><P>This will be the first study to analyze the validity and reliability of POM-Checker in measuring shoulder ROM. Although the sample size of this study is small, it may not influence on the results conclusively, because the measures are very precise numerical angles. Furthermore, the angles of shoulder movements will be measured with both devices simultaneously.</P><P><B>Conclusion:</B></P><P>The results of the study will be helpful to find out the validity and reliability of a new developed ROM measure device, POM-Checker, by comparison with a golden standard system, 3D motion capture system, in measuring the shoulder ROM. It will also contribute to use of the POM-Checker in measuring the ROMs in many human joints</P>

The aim of this study was to report the effect of Korean medicine treatments and vestibular rehabilitation practice on an stroke patient with dizziness. The patient received Korean medicine treatments, such as acupuncture and herbal medication, and vestibular rehabilitation practice. The effects of treatments were assessed with the Korean dizziness handicap inventory, Korean activities-specific balance confidence scale, Korean vestibular disorders activities of daily living scale. Researchers conducted gait analysis for evaluation patient’s gait disturbance’s improvement during treatments. The intensity of dizziness were significantly reduced after the complete treatment. After treatment with traditional Korean medicine and vestibular rehabilitation practice, the patient showed significant improvements in symptoms and gait parameters. This case proved that combined therapy of Korean medicine treatment and vestibular rehabilitation practice can be useful for stroke patients who suffered from dizziness. however, studies of larger populations are required in the future. The purpose of A/C system in the vehicle is to have a driver comfortable by adjusting temperature and humidity. To increase this A/C system efficiency, it is good way to increase compressor’s performance. But, if compressor"s performance is increased with noise, it will not be good design. The purpose of this study is, by optimizing Rear Head’s refrigerant path, to increase compressor’s performance and decrease noise value in the vehicle. In this paper, to satisfy compressor’s performance and noise we conducted CFD analysis, performance test, pulsation test, bulk noise test and real car noise test. Through these experiments, we could design optimization discharge system of scroll compressor.
