연구목적: 스마트폰에 접속하여 혈당을 측정할 수 있는 소형 혈당 모듈을 개발하여 그 유용성을 실험 적으로 검증하고자 하였다. 대상 및 방법: 혈액이 전극에 주입되어 생성되는 전류가 전압의 형태로 변환되어 혈당값으로 변환되는 전기화학적 측정 원리를 적용하여 소형 혈당 모듈을 개발하였다. 혈당 모듈의 최소한의 측정 혈액 양은 0.5 μL이며, 5초 이내로 측정이 가능하도록 설계 및 제작하였다. 스마트폰과의 통신은 frequencyshift keying(FSK) 모뎀 방식을 적용하였다. 혈당 모듈의 정확도 실험을 위해 시판되는 혈당계와 인 공혈액으로 비교 실험을 수행하였다. 결과: 시판되는 제품과의 성능 비교 실험에서 시판 제품은 391 mg/dL를 나타내었으며, 소형 혈당 모 듈은 401 mg/dL를 나타내었다. 혈당 모듈의 크기는 50×30×7 mm로서 휴대의 편리성을 높였다. 결론: 스마트폰 기반의 소형 혈당 모듈을 개발하였으며, 기존 제품과의 혈당 측정 상대오차가 약 2.6%로서 정확한 혈당 측정이 가능함을 실험적으로 검증하였다.
One-step synthesis of gene carrier via gamma irradiation and its application in tumor gene therapy

<P><B>Introduction</B></P><P>Although numerous studies have been conducted with the aim of developing drug-delivery systems, chemically synthesized gene carriers have shown limited applications in the biomedical fields due to several problems, such as low-grafting yields, undesirable reactions, difficulties in controlling the reactions, and high-cost production owing to multi-step manufacturing processes.</P><P><B>Materials and methods</B></P><P>We developed a 1-step synthesis process to produce 2-aminoethyl methacrylate-grafted water-soluble chitosan (AEMA-g-WSC) as a gene carrier, using gamma irradiation for simultaneous synthesis and sterilization, but no catalysts or photoinitiators. We analyzed the AEMA graft site on WSC using 2-dimensional nuclear magnetic resonance spectroscopy (2D NMR; 1H and 13C NMR), and assayed gene transfection effects in vitro and in vivo.</P><P><B>Results</B></P><P>We revealed selective grafting of AEMA onto C6-OH groups of WSC. AEMA-g-WSC effectively condensed plasmid DNA to form polyplexes in the size range of 170 to 282 nm. AEMA-g-WSC polyplexes in combination with psi-hBCL2 (a vector expressing short hairpin RNA against <I>BCL2</I> mRNA) inhibited tumor cell proliferation and tumor growth in vitro and in vivo, respectively, by inducing apoptosis.</P><P><B>Conclusion</B></P><P>The simple grafting process mediated via gamma irradiation is a promising method for synthesizing gene carriers.</P>

N/A To determine the clinical significance of the fourth heart sound (S.) in hypertensive heart, 31 patients with essential hypertension aged between 32 to 77 years old and 15 normal controls were examined by physical examination, phonocardiography and echocardiography, All hypertensive patients had normal sinus rhythm and no history of coronary artery disease nor COPD and most of them had moderate to severe hypertension. Depending upon the presence of audible S4, hypertensive patients were divided into 3 groups; those without audible S, (group I, 15 cases), those with audible S, (group II, 12 cases) and those with questionably audible S, (group III, 4 cases). M-mode echocardiograms of the left ventricle and pulsed wave Doppler echocardiograms of the mitral valve were performed to determine the left ventricular thickness/dimension ratio (T/D), left ventricular mass index (LVMI) and A/E ratio of mitral flow pattern. T/D and LVMI were significantly increased in both hypertensive group I and group 11 (p<0.01) and T/D of hypertensive group II was more increased than that of hypertensive group I (p<0,05), but there was no significant difference between hypertensive group I and group II in LVMI. In contrast, the A/E ratio of mitral Doppler flow pattern was increased only in hypertensive group II compared to normal controls (p<0.01), and there was a significant difference between hypertensive group I and group II (p<0.01). Audible S, was found in hypertensive patients with an A/E ratio greater than 1.10 and in a normal person with an A/E ratio of 1.27, and all 10 hypertensive patients above an A/E ratio of 1.20 except one had an audible fourth heart sound. In conclusion, audible fourth heart sound is a useful bedside parameter for detecting left ventricular diastolic dysfunction in the hypertensive heart.

The Arg/N-degron pathway, which is involved in the degradation of proteins bearing an N-terminal signal peptide, is connected to p62/SQSTM1-mediated autophagy. However, the impact of the molecular link between the N-degron and autophagy pathways is largely unknown in the context of systemic inflammation. Here, we show that chemical mimetics of the N-degron Nt-Arg pathway (p62 ligands) decreased mortality in sepsis and inhibited pathological inflammation by activating mitophagy and immunometabolic remodeling. The p62 ligands alleviated systemic inflammation in a mouse model of lipopolysaccharide (LPS)-induced septic shock and in the cecal ligation and puncture model of sepsis. In macrophages, the p62 ligand attenuated the production of proinflammatory cytokines and chemokines in response to various innate immune stimuli. Mechanistically, the p62 ligand augmented LPS-induced mitophagy and inhibited the production of mitochondrial reactive oxygen species in macrophages. The p62 ligand-mediated anti-inflammatory, antioxidative, and mitophagy-activating effects depended on p62. In parallel, the p62 ligand significantly downregulated the LPS-induced upregulation of aerobic glycolysis and lactate production. Together, our findings demonstrate that p62 ligands play a critical role in the regulation of inflammatory responses by orchestrating mitophagy and immunometabolic remodeling.

We developed the educational purpose mobile application, named "Wild Flowers of Bukhansan National Park (version 1.0)", aiming for easy identification of wild flowers for students and visitors in the park. When visitors find a flower or part of plant in the park, visitors can search for its name utilizing the pictures and characters provided in their own smartphone mobile devices or tablet PCs. The application provides pictures of wild flowers in the park and character-based searching system based on 12 diagnostic features (e.g., growth form, leaf arrangement, flower symmetry, petal color, petal number, sepal number, etc). We adopted the complete floristic survey of Chung and Lee (1962) and added species that we confirmed their distribution in the park during the development of this application. In summary, number of vascular plants in this park was estimated to be 428 taxa including 100 families, 280 genera, 327 species, 1 subspecies, 50 varieties, and 5 formas. We provided a total of 588 pictures representing 358 taxa and each taxon includes multiple pictures in many cases. Included identification quizzes can be an efficient educational tool as well as fun activity for students and visitors who are learning plant species in Korea. Our next step will include GPS function in the application for indicating visitor's location and for providing previously reported sites of the species that we interested in the map of the park. The future application which includes GPS function will be a valuable tool for the monitoring of rare plants, plant researches related to the climate changes, etc. We currently provide Korean iPhone version only, and English version and both of android versions will be serviced soon.

Springing is the resonance phenomenon of a ship hull girder with incoming waves having the same natural frequency of the ship. In this study, a simple and reliable calculation method was developed based on quadratic strip theory using the Timoshenko beam approach as an elastic hull girder. Second-order hydrodynamic forces and Froude-Krylov forces were applied as the external force. To improve the accuracy of the strip method, the variation in the added mass along the ship hull longitudinal direction, so called tip-effect, was considered. The J-factor was also employed to compensate for the effect of three-dimensional fluid motion on the two-node vibration of the ship. Using the developed method, the first- and second-order vertical bending moments of the Flokstra ship were compared. A comparative study was also carried out for a uniform barge ship and a 10,000 TEU container ship with the respective methods including the J-factor and tip-effect.

Activity recognition using smartphone accelerometer suffers from the user dependency problem that acceleration patters of one user differ from those of others for the same activity. Moreover, it also suffers from the position dependency problem since a smartphone may be placed in any pockets or hands. In order to overcome these problems, this paper proposes an effective activity recognition method which is less dependent with both specific users and specific positions of the smartphone. Based on the proposed method, we implement a real-time activity recognition system working on an Android smartphone. Throughout some experiments with 6642 examples collected from different users and different positions, we investigate the performance of our activity recognition system.

We propose an accelerometer-based gesture recognition method for smartphone users. In our method, similarities between a new time series accelerometer data and each gesture exemplar are computed with DTW algorithm, and then the best matching gesture is determined based on k-NN algorithm. In order to investigate the performance of our method, we implemented a gesture recognition program working on an Android smartphone and a gesture-based teleoperating robot system. Through a set of user-mixed and user-independent experiments, we showed that the proposed method and implementation have high performance and scalability.

Objectives This study was aimed to evaluate clinical practice guidelines (CPGs) of traffic injuries, which has already been developed at domestic or outside of country, and to explore the Korean medical treatments included in the CPGs. Methods Twelve electronic databases (PubMed, Cochrane library, China National Knowledge Infrastructure [CNKI {Chinese Academic Journals, CAJ}], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS], KoreaMed, Korean Medical Guideline Information [KoMGI), National Guideline Clearinghouse [AHRQ], Core Outcome Measures in Effectiveness Trials Initiative Website [COMET], Turning Research into Practice [TRIP], The National Institute for Health and Care Excellence [NICE], and Medical Research Information Center [MedRIC]) up to July 2021 were searched. Only systematically developed CPGs for traffic injuries were selected and appraised. The appraisal was conducted by using Appraisal of Guidelines for Research & Evaluation (AGREE) II tool. Results Six CPGs were included and evaluated. All CPGs were appraised as highly recommended because they exceeded 60% in more than 4 among 6 domains of AGREE II, including domain of ‘rigor of development’, and 30% in the rest. Recommendations related to Korean medicine treatments such as on manual therapy related to Chuna were given in 6 CPGs, 4 for acupuncture, 1 for Qigong and 1 for Taping. Conclusions The 6 CPGs were developed up to now through a systematic development methodology and there were many recommendations for Korean medical treatments related to manual (Chuna) treatment, acupuncture and Qigong. However, the evidence for the side effects and risk factors of Korean medical treatments was scantly reflected in CPGs. Therefore, it is considered that balanced CPG with benefits and risks should be developed, covering Korean medical diagnosis, treatment and prognosis. (J Korean Med Rehabil 2021;31(4):129-143)
