        위절제술 환자의 표준진료지침 개발 및 적용 효과

        Background : Gastric cancer is the most common malignant tumor in Korea. surgical operation is one of the major treatment modalities for gastric cancer patients. Therefore, gastrectomy is one of the most common procedures in General Surgery. There were variation in length of hospital stay and medical treatment for gastrectomy between three surgeons at Asan Medical Center. Clinical pathways have received considerable attention as a tool for recucing the medical practice variation, increasing the efficiency of care process, and improving the quality of care. The aim of this study was to evaluate the effect of a clinical pathway for gastrectomy in gastric cancer patients. Methods : The clinical pathway for gastrectomy was developed and implemented by a multidisciplinary group in Asan Medical Center. A computerized clinical pathway program was developed and revised after a pilot test. A total of 145 patients underwent gastrectomy by three surgeons at Asan Medical Center. We compared the length of hospital stay, patient satisfaction, and unplanned readmission rate between the pre-pathway group(n=67) and the post-pathway group(n=78). We also investigated the degree of satisfaction among the physicians and nurses who were main end-users of the clinical pathway. Results : The clinical pathway was applied to all target patients. The average length of hospital stay was shortened from 12.7days to 10.6days(p<0.01). The degree of patient satisfaction with the care process changed from 90.3% to 89.2% after the implementation of the clinical pathway, but the difference was of satistically significant(p=0.761). Unplanned readmission rate was 2.9% in the pre-pathway group. More than 90% of physicians and nurses answered that the clinical pathway had been a useful tool in their medical practice. Conclusions : The findings of the study demonstrated that implementation of the clinical pathway for gastrectomy produced substantial reduction in the length of hospital stay while improving the quality of patient outcomes. The computerized clinical pathway program can be used as one of the powerful patient management tools for reducing the practice variations and increasing the efficiency of care process in Korea hospital settings.

        중학교 가정 교과 수행평가 방법 및 도구 개발

        The purpose of the study is to develop a Performance Assessment Method & Tools for Home Economics at middle school. Through the content analyses of middle school Home Economics curriculum and 8 textbooks of lst, 2nd, and 3rd grade respectively, teching elements for each content area were drawn and each specific learning objectives were idntified accordingly. In addition, assessment element for student's performance in each teaching element were specified for analysis. Analytic framework for the development of Performance Assessment Method and Tools were proposed by each teaching element. Beased on the analytic framework proposed, a model method and tools were developed by 10 different test measures of description written test, statement written test, oral test, pro and con discussion, performance test, experiment, interview, observation, self-test report, and portfolio. The model proposed in the study has been modified and improved through the feasibility studies of experts' examination and field research application. The model assessment method and tools for the study has been compleeted finally by proposing 45 performance assessment tools for the content areas of food, clothing, housing, resource management and consumerism, human development and family relationship.

      • 원전 고등제어 알고리즘 설계를 위한 소규모 시뮬레이터 개발

        There has been a difficulty in implementing control algorithms (including even proportional-integral-derivative control algorithms) designed to verify and test to many small-scale simulators. In particular, it was almost impossible to implement advanced control algorithms to them. Therefore, it is required to develop a new simulator that facilitates the implementation of advanced control algorithms and the interface between different kinds of application programs. The objective of this work is to improve the existing small-scale simulator (FISA-2/WS) for the Kori unit 2 nuclear power plant and thus enable advanced control algorithms to be tested and verified by being applied to it. The developed simulator consists of FORTRAN, Visual C++, and MATLAB programs and an advanced control method using a receding horizon control method was applied to it to control the steam generator water level.

        The purpose of this study were to develop Knowledge Management System for the nurse and to identify its effectiveness. The program was developed in development of the contents and programing. And the system was operated to 85 nurse for three weeks. Data collection were performed during the period of May 1, 2005 to December 30, 2005. The data was collected from 85 nurses using questionnaires. In order to ascertain the validity of clinical application of the system, Prior experience of the computer mediated communication, user satisfaction and utilization were analyzed. The instruments for the study were satisfaction(Kim, 2000), utilization(Kim, 2002). The collected data were analysed by descriptive statistics, using SPSS WIN 11.0 program. The results showed average score which is the satisfaction(6.09) showed middle satisfaction scores. It is concluded that if Nursing Knowledge Management System is operated considering all the above factors, It wound be one of the best qualified continuing clinical method for nurses.

        C-shaped root canals of mandibular second molars in a Korean population: a CBCT analysis

        Objectives: The purpose of this study was to investigate the C-shaped root canal anatomy of mandibular second molars in a Korean population. Materials and Methods: A total of 542 teeth were evaluated using cone-beam computed tomography (CBCT). The canal shapes were classified according to a modified version of Melton's method at the level where the pulp chamber floor became discernible. Results: Of the 542 mandibular second molars, 215 (39.8%) had C-shaped canals, 330 (53%) had 3 canals, 17 (3.3%) had 2 canals, 12 (2.2%) had 4 canals, and 8 (1.7%) had 1 canal. The prevalence of C-shaped canals was 47.8% in females and 28.4% in males. Seventy-seven percent of the C-shaped canals showed a bilateral appearance. The prevalence of C-shaped canals showed no difference according to age or tooth position. Most teeth with a C-shaped canal system presented Melton's type II (45.6%) and type III (32.1%) configurations. Conclusions: There was a high prevalence of C-shaped canals in the mandibular second molars of the Korean population studied. CBCT is expected to be useful for endodontic diagnosis and treatment planning of mandibular second molars.

        걸음마기 아동의 어머니를 대상으로 한 부모존경-자녀존중 부모교육프로그램의 효과

        Objective: This study used a pre-test and post-test quasi-experimental design to evaluate the effectiveness of the Respected Parents & Respected Children parent education program developed for mothers of toddlers. Methods: A total of 40 mothers were assigned to two groups, an intervention group (n = 20) and a waiting group (n = 20). Mothers in the intervention group participated in six weekly sessions of the parent education program. After the termination of the program, differences between the preand post-test in terms of mothers’ parenting characteristics (i.e., parenting behaviors, parenting efficacy, and parenting stress) and toddlers’ social and emotional characteristics were compared in both groups. Results: Mothers in the intervention group showed significant improvement in their parenting behaviors and parenting efficacy and decreased parenting stress, whereas mothers in the waiting group did not show any significant differences between pre- and post-test scores. Mothers in the intervention group also reported a significant improvement in their toddlers’ level of imitation/play. Conclusion: These findings indicate that the Respected Parents & Respected Children parent education program is effective in bringing about positive changes in both mothers’ and toddlers’ characteristics, which can be crucial for the future developmental outcomes of children.

        직장 내 성희롱과 근로감독제도 : 근로감독관의 신고사건 처리 경험을 중심으로

        최근 몇 년간 지속되어 온 미투 운동(#me too)은 ‘직장 내 성희롱’을 다시 한번 사회적으로 크게 공론화시켰고, 성희롱 규제와 예방을 위한 중요한 제도로서 근로감독제도에 대한 관심과 논의를 확대시켰다. 이 글은 근로감독관의 성희롱 신고사건 처리 경험을 중심으로 근로감독제도의 운영 현황과 문제점을 살펴보았다. 각 지방고용노동관서(지방고용노동청)에 성희롱 사건에 대한 충분한 이해와 사건 처리 경험을 가진 인력이 충분하지 않은 상황에서 근로감독관들에게 성희롱 사건은 부담스럽게 느껴졌고, 성희롱 신고사건은 소수의 사건 처리 경험이 있는 근로감독관이나 몇몇 근로감독관에게 순번제로 배정되었다. 2018년부터 고용평등전담 근로감독관제도를 도입하고 운영하고 있지만, 각 지방고용노동관서마다 규모, 인력, 사건 수 등이 상이해 전담 인력을 지정하고 운영하는 방식은 단일하지 않았다. 성희롱 사건 처리 과정에서 근로감독관의 전문성이나 역량의 문제가 대두되고 있지만, 여전히 관련 교육이나 참고할 수 있는 자료가 불충분했다. 성희롱 · 성차별은 여전히 사회적 문제로 대두되고 있으나, 성희롱 · 성차별을 포함하는 고용평등 업무는 ‘여성’ 업무로 인식되거나 여성 근로감독관에게 집중되면서 주변화되어 있었다. 여성 근로감독관이라는 위치는 사건에 깊이 몰입되거나 성희롱의 위험에 노출되게 하는 등 또 다른 변수로 작용하고 있었다. 기대보다 낮은 처벌수준이나 실질적으로 가해자 징계를 강제할 수 없는 현실적 한계를 마주하면서 피해자들은 진정 결과에 분노하거나 좌절하기도 했다. 성희롱 규제와 예방을 위한 제도로서 근로감독제도가 효과적으로 작동하기 위해서는 전담 근로감독관의 전문성과 역량을 강화할 수 있는 방법을 조직적 차원에서 고민하고 지원할 필요가 있으며, 사업장 근로감독 권한을 효율적으로 집행하면서 성희롱 발생 사업장의 사후 점검과 예방적 조치 등을 강화할 필요가 있다. The #MeToo movement, which has continued in recent years, has socially raised the issue of workplace sexual harassment. This is increasing the discussion about labor inspections as a means by which to regulate and prevent sexual harassment in the workplace. This paper analyzes the reality and feasibility of labor inspections, focusing on the process of petitions concerning sexual harassment through in-depth interviews with labor inspectors and sexual harassment victims. Local labor administration offices experience difficulties when dealing with sexual harassment cases due to the lack of both skilled manpower and material resources. Despite the introduction of the employment equality exclusive labor inspection system in 2018, it is not enough to cover problems. Employment equality work, including sexual harassment and sex discrimination in employment, is mainly assigned to female labor inspectors as such work mainly involves women. Female labor inspectors tend to experience more stresses and are exposed to the risks of sexual harassment as well when dealing with sexual harassment cases. Petitioners who are sexual victims are annoyed or frustrated when they face the limitations on realistic sanctions for sexual misconduct. It is essential for labor inspectors to enhance professionalism and capabilities to deal with sexual harassment cases. Continuous training and systematic management are needed. Labor inspections should also be strengthened to implement stipulated measures in responses to sexual harassment cases and prevent (re)occurrence of sexual harassment.

        교육훈련프로그램특성이 기업의 활성화를 매개로 교육훈련 만족도에 미치는 영향에 관한 연구

        A survey was performed on employees of a small and medium-sized company how had participated in an education & training program, in order to identify the relationship between the enterprise revitalize and its characteristics, and to know how the program and its satisfaction was related with each other. The results showed that education & training program characteristics had influence on the enterprise revitalize and then the latter made effect on its satisfaction. It is suggested that a plan should be developed to promote self-improvement needs of employees when an education & training program is to be made, and by changing the state of facility or training conditions, the efficiency of the program should also be pursued. In addition, it is imperative for the participants to have strong motivation for self-improvement in the program, and it seems that the effectiveness of the program is expected to work in accordance with the will of the company as well as its growth and developmental standards.

        모바일 SNS속성이 관계형성과 정보공유를 매개로 사용자 만족도에 미치는 영향

        This research conducted a survey targeting mobile SNS users in order to see whether mobile SNS attribute(accessibility, usefulness, newness, connectivity) through the medium of information share and relationship formation had a positive influence on user satisfaction. The results of the analysis showed that usefulness and accessibility had a significant influence on information share, while accessibility and connectivity had a significant influence on relationship formation. Information share and relationship formation both had a significant influence on user satisfaction. Based on this research, an angle of approach different from the business transactions and information services offered in existing mobile networks is necessary when corporations provide SNS through mobiles. When considering that the most important part in the use of mobile SNS is the social component, this can be actively used for the development and marketing of SNS services. Therefore, the SNS provider must focus its efforts on finding a method to stimulate user sensibilities in order to increase customer satisfaction and draw out continuous usage.
