A case of peritoneal dialysis-associated peritonitis caused by Brevundimonas vesic ularis

Brevundimonas vesicularis are a non-lactose fermenting, gram-negative bacillus that is assigned to a group IV Pseudomonas species and considered to have low virulence. There are sporadic reports of the organism causing endocarditis, meningitis, septicemia, and pneumonia; however, it is an unusual pathogen in cases of peritoneal dialysis (PD)-associated peritonitis. Furthermore, recommendations for treatment of PD-associated refractory peritonitis induced B. vesicularis were not included in the International Society for Peritoneal Dialysis guidelines. One day, a 71-year-old Korean woman who had been treated with CAPD for 2 years was admitted to our hospital because of turbid peritoneal effluent accompanied by constant abdominal pain that began 5 days prior to her admission. The underlying causes of herend- stage renal disease were hypertension and diabetes mellitus. On admission, her blood pressure was 119/74 mmHg, heart rate was 94 beats/min, respiratory rate was 20/min, and body temperature was 37.7℃. Her abdomen was diffusely tender with normal bowel sounds, and no evidence of infection was identified around the catheter exit site. The laboratory findings showed PD peritonitis: the white blood cell (WBC) count of the peritoneal effluent was 2,800/mm3 with a neutrophil predominance (95%). Her WBC count was 6.4 × 109/L, and C-reactive protein was 4.32 mg/dL. Bacteria were not observed on Gram stain examination After peritoneal fluid was sent for bacterial culture, a single 1-g dose of cefazolin and a single 1-g dose of ceftazidime were given intraperitoneally per day. Culture of the peritoneal dialysate revealed B. vesicularis which was susceptible to cefepime, cefotaxime, imipenem, piperacillin, and gentamicin. Despite the use of intraperitoneal antibiotics for 5 days, the patient’s abdominal pain and intermittent fevers persisted, and the WBC count of the peritoneal effluents was elevated to 430/mm3. In the end, the PD catheter had to be removed on the 5th day after starting intraperitoneal antibiotics. We conclude this is the first refractory case of PD-associated peritonitis due to B. vesicularis, which resolved following catheter removal in South Korea.

Strongyloides stercoralis is endemic to tropical and subtropical regions, and infections are usually asymptomatic. However,immunocompromised patients, such as those receiving immunosuppressive therapy, high-dose steroids, or chemotherapy, can developfatal hyperinfections. An 84-year-old man without any symptoms was diagnosed with strongyloidiasis during a regular screeningcolonoscopy. His medical history only involved a gastric endoscopic submucosal dissection for early gastric cancer 6 months previously. Few cases have been published about asymptomatic strongyloidiasis diagnosed in an immunocompetent host via endoscopic mucosalresection with characteristic colonoscopic findings. We report a case of colon-involved asymptomatic strongyloidiasis with specificcolonic findings of yellowish-white nodules. This finding may be an important marker of S. stercoralis infection, which could preventhyperinfections.

<P>Since one of the main drawbacks of Li<SUB>4</SUB>Ti<SUB>5</SUB>O<SUB>12</SUB> as a negative-electrode material is its low electronic conductivity, several researchers have attempted to improve the conductivity by narrowing the band gap through transition-metal doping. Herein, we report another, more significant effect of doping in addition to the band gap narrowing, namely, an anomalous decrease in the structural disorder in Li<SUB>4</SUB>Ti<SUB>5</SUB>O<SUB>12</SUB> upon Cr<SUP>3+</SUP>-ion doping. Although it is generally recognized that doping with heterogeneous elements increases the structural disorder, the Cr<SUP>3+</SUP>-ion doping in Li<SUB>4</SUB>Ti<SUB>5</SUB>O<SUB>12</SUB> demonstrated an unexpected structural phenomenon. From the results of various structural analyses using a synchrotron beam, such anomalous structural changes were revealed to originate from charge redistribution at nearby Ti<SUP>4+</SUP> ions. Finally, the capacity was markedly enhanced, especially at high <I>C</I>-rates (125 mA h g<SUP>−1</SUP> for 10<I>C</I> charge/10<I>C</I> discharge, 145 mA h g<SUP>−1</SUP> for 1<I>C</I> charge/50<I>C</I> discharge) because of both the band gap narrowing and the increased ionic diffusivity due to the decreased structural disorder, but was decreased instead for too-high doping levels.</P> <P>Graphic Abstract</P><P>Cr doping into Li<SUB>4</SUB>Ti<SUB>5</SUB>O<SUB>12</SUB> lattice unexpectedly led to a decrease in structural disorder and eventually a drastic capacity enhancement at high <I>C</I>-rate (125 mA h g<SUP>−1</SUP> for 10<I>C</I> charge/10<I>C</I> discharge, 145 mA h g<SUP>−1</SUP> for 1<I>C</I> charge/50<I>C</I> discharge). <IMG SRC='http://pubs.rsc.org/services/images/RSCpubs.ePlatform.Service.FreeContent.ImageService.svc/ImageService/image/GA?id=c2ee22734g'> </P>

With the progress of image-guided localization, body immobilization system, and computerized delivery of intensity-modulated radiation delivery, it became possible to perform spine radiosurgery. The next question is how to translate the high technology treatment to the clinical application. Clinical trials have been performed to demonstrate the feasibility of spine radiosurgery and efficacy of the treatment in the setting of spine metastasis, leading to the randomized trials by a cooperative group. Radiosurgery has also demonstrated its efficacy to decompress the spinal cord compression in selected group of patients. The experience indicates that spine radiosurgery has a potential to change the clinical practice in the management of spine metastasis and spinal cord compression.

최근 배달 서비스에서는 배달비 인상에 대한 불만 해소를 위해 노선이 비슷한 이용객의 주문을 함께 배송하는 대신, 배달비를 줄이는 ‘알뜰배달’을 도입했다. 하지만 예정 배달 시간보다 더 지연되는 상황에서 사용자에게 즉각적인 안내와 사과 메시지가 없는 것이 문제로 제기되었다. 이를 개선하기 위하여, 안내 팝업의 UX 라이팅 측면과 이모지 유무에 따른 사용자 경험의 차이를 비교하고자 했다. 실험 결과, 배달 지연 상황에서 간접화행과 이모지를 함께 활용한 경우가 가장 긍정적으로 평가되었으며, 위 유형과 다른 유형들의 차이는 유의미한 것을 확인했다.

<P>We demonstrate a method to realize vertically oriented Ge nanowires on Si(111) substrates. Ge nanowires were grown by chemical vapor deposition using Au nanoparticles to seed nanowire growth via a vapor–liquid–solid growth mechanism. Rapid oxidation of Si during Au nanoparticle application inhibits the growth of vertically oriented Ge nanowires directly on Si. The present method employs thin Ge buffer layers grown at low temperature less than 600 °C to circumvent the oxidation problem. By using a thin Ge buffer layer with root-mean-square roughness of ∼ 2 nm, the yield of vertically oriented Ge nanowires is as high as 96.3%. This yield is comparable to that of homoepitaxial Ge nanowires. Furthermore, branched Ge nanowires could be successfully grown on these vertically oriented Ge nanowires by a secondary seeding technique. Since the buffer layers are grown under moderate conditions without any high temperature processing steps, this method has a wide process window highly suitable for Si-based microelectronics. </P>

<P>A study by S. Yoon, J. Lee, and co‐workers on the improvement of rate performance highlights an important issue for pseudocapacitor electrode materials. On page 3747, the synthesis of an ordered mesoporous tungsten oxide–carbon nanocomposite is presented via a ‘one‐pot’ soft‐template method. The ordered mesoporous structure, partial reduction of the metal oxide, and the nanosized mixing of the metal oxide/carbon result in both high power and energy density. </P>
A case report of continuous veno-venous hemodialysis (CVVHD) in acute methanol poisoning

Methanol is a common ingredient in many products such as illicit liquors, windshield washer fluids, and homemade alcoholic beverages. Whether accidental or intentional, the ingestion of methanol can be potentially fatal if not diagnosed and treated promptly. Methanol is oxidized first by hepatic alcohol dehydrogenase to formaldehyde then to formic acid, which results in acidosis, neurotoxicity and death in severe poisoning. Here reported is the case of a 46-year-old Korean woman who survived methanol intoxication. She was presumed to have ingested methanol six hours before arriving at the emergency department. She was drowsy with a Glasgow Coma Scale score of E2V4M5. She was afebrile with a blood pressure of 103/73 mmHg, a pulse rate of 77 beats/min and a respiratory rate of 24 breaths/min. Initial arterial blood gas showed a pH of 7.180, bicarbonate of 12.3 mmol/L, PaCO2 of 33.0 mmHg and base excess of -14.9. Her serum osmolar gap was 249 mOsm/kg H2O and the serum anion gap was 21.7 mmol/L. Based on the history and the blood test, we diagnosed the methanol intoxication and promptly initiated continuous veno-venous hemodiafiltration (CVVHDF). CVVHDF was performed for 39 hours using the Prismaflexi system (Gambro Lundia AB, Sweden) on the first day of admission at the following settings: blood flow, 150 mL/min; replacement volume, 1,000 mL/h; and dialysate, 1,000 mL/h. Two days after hospitalization, we stopped CVVHDF after the anion gap normalized. On day 5, the patient was discharged without any complications. We received the result of methanol concentrations in serum and urine from our reference laboratory when the patient discharged. The analysis detected high concentrations of methanol in blood and urine and high concentrations had decreased in both serum and urine during CVVHDF. In conclusion, our case supports that CVVHDF enhances the elimination of serum methanol by a decrease in half-life. Keywords: Methanol; Hemodiafiltration; Acidosis
