Streptozotocin유도 당뇨병이 백서 악하선에 미치는 영향에 관한 병리조직학적 연구

The purpose of this study was to observe the microscopicchange of salivary gland tissues, which is the cause of xerostomia in diabetic condition; for this target, the author injected STZ 0.1mlIlOOgm b.w. on rat to produce diabetes, and then observed microscopic change in submandibular gland through the histopathologic method, obtaining as follows 1. All of the experimental specimens suffered diabetes after injection of STZ, but the blood glucose Ievel was irregular. 2. There were not interrelationship between the blood glucose level and microscopic change on salivary gland tissues. 3. The salivary gland changed after diabetes initiation in lapse of times; after 14 days, suffered severe destruction, however after 17 days, it was regenerated. 4. Salivary glands showed congested, destructive acim cells, and hyperplastic ductal cells as well as salivary gland duct-like structures. 5. There were accumulation of fat granules within, the cytoplasm of the acini cells on mucous gland in diabetic condition. 6. According to insulin injection, there were no more changes on salivary gland tissues, even in the accumulation of fat granules. 7. Histological changes of the serous gland were obvious more than the mucous gland in this experimental condition.

<P><B>Abstract</B></P> <P>It is thought that vitamin C has protective roles on stress-induced heart damage and the development of cardiovascular diseases, but its precise role and mechanisms are unclear. In the present study, we investigated the specific mechanisms by which vitamin C leads to protecting the heart from stress-induced damage in the <I>Gulo(</I>−/−<I>)</I> mice which cannot synthesize vitamin C like humans. By exposure to stress (1h/day), the heartbeat and cardiac output in vitamin C-insufficient <I>Gulo(</I>−/−<I>)</I> mice were definitely decreased, despite a significant increase of adrenaline (ADR) and noradrenaline (NA) production. A change of cardiac structure caused by the death of cardiomyocytes and an increased expression of matrix metalloprotease (MMP)-2 and -9 were also found. Moreover, lipid peroxidation and the production of tumor necrosis factor-alpha (TNF-α) in the heart were increased. Finally, all vitamin C-insufficient <I>Gulo(</I>−/−<I>)</I> mice were expired within 2 weeks. Interestingly, all of the findings in vitamin C-insufficient <I>Gulo(</I>−/−<I>)</I> mice were completely prevented by the supplementation of a sufficient amount of vitamin C. Taken together, vitamin C insufficiency increases the risk of stress-induced cardiac damage with structural and functional changes arising from the apoptosis of cardiomyocytes.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Vitamin C insufficiency accelerates the stress-induced death of <I>Gulo(</I>−/−<I>)</I> mice. </LI> <LI> Oxidative stress in the heart is increased by stress and vitamin C insufficiency. </LI> <LI> TNF-α, ADR, and NA are excessively produced by stress and vitamin C insufficiency. </LI> <LI> Structural/functional cardiac changes are induced by stress and vitamin C insufficiency. </LI> <LI> Vitamin C supplementation prevents cardiac changes and death of <I>Gulo(</I>−/−<I>)</I> mice. </LI> </UL> </P>

Background/Aims: Obscure gastrointestinal bleeding (OGIB) is defined as recurrent acute or chronic bleeding for which no sources have been identified by routine endoscopic and contrast studies. The patients with OGIB are frequently difficult to manage and often undergo extensive investigation and even laparatomy without diagnosis. The aim of this study was to review our overall experiences of the investigation and treatment of OGIB. Methods: We reviewed, retrospectively, medical records of 41 cases with OGIB from June 1996 to July 2002. Results: Of the 41 patients with OGIB, there were 21 men and 20 women and their mean age was 47.8 years (range, 18 to 81 years). Diagnosis was possible in 13 patients (31.7%) and no specific causes were identified in the remaining 28 patients (68.3%). The principal etiologies were gastrointestinal stromal tumor of the small bowel in 3 cases, duodenal diverticulum in 2 cases. Other causes are as follows: adenocarcinoma of the duodenal 3rd portion, duodenal varix at the 4th portion, splenic artery pseudoaneurysm, Crohn’s disease, jejunal ulcer, ileal ulcer, ileal angiodysplasia, and anastomosis site bleeding after hemicolectomy. Conclusions: We conclude that further and logical diagnostic investigations such as small bowel enteroscopy and wireless capsule endoscopy will promise better results. <연구목적> 간경변증 환자에서 식도정맥류의 유병률은 25~75%로 다양하게 보고되고 있고, 정맥류가 있는 환자의 1/3은 일생 중에 정맥류 출혈을 경험하게 되며 이는 간경변증 환자의 중요한 사인 중 하나이다. 이러한 식도정맥류 출혈의 위험인자로 문맥압 항진증의 정도(WHPG>12 mmHg)와 정맥류의 크기 등이 제시되고 있다. 하지만 거대 식도정맥류의 존재를 예견할 수 있는 비내시경적 예측인자에 대한 연구는 드물며, 특히, 국내 연구는 거의 없는 실정이

<서론> 결절성 재생증식(nodular regenerative hyperplasia)은 간조직의 재생 결절이 간에 미만적으로 퍼져있으면서 각 결절이 간경변과는 달리 섬유성 중격에 둘러싸이지 않은 드문 질환이다. 이는 간내 혈류의 균일하지 않은 분포에 대한 이차성 또는 비특이적인 적응 상태로 원인은 아직 밝혀지지 않았지만 자가면역질환, 류마티스 질환, 골수증식질환, 만성적 정맥울혈 등과 관계된다는 보고가 있다. 정확한 유병율에 대해서는 알려진 바가 없지만
