The measurement of D, CII and CIII line spectral profiles and the CD band spectrum from the vicinity of a test limiter surface is performed in Heliotron J test limiter experiments. With deep insertion of the limiter, there is a localized intense region of the D, CII and CIII line intensity in the limiter surface, and the CD band spectrum is observed at this region. It is considered that the deuterium ion flux and heat load concentrate at this region and enhance carbon impurity generation.r_che

Background: We evaluated decreases in glycosylated hemoglobin (HbA1c) achieved by reducing carbohydrate from various sources in type 2 diabetes mellitus patients. Methods: We followed up 138 male and 107 female outpatients on a moderate low-carbohydrate diet without diabetic medication for 6 months. Changes in carbohydrate sources (Δcarbohydrate) were assessed from 3-day dietary records at baseline and 6 months, and associations with changes in HbA1c (ΔHbA1c) were examined with Spearman’s correlation coefficients (rs) and multiple regression analysis. Results: ΔHbA1c was –1.5%±1.6% in men and –0.9%±1.3% in women, while Δtotal carbohydrate was –115.3±103.7 g/day in men and –63.6±71.1 g/day in women. Positive associations with ΔHbA1c were found for Δtotal carbohydrate (rs=0.584), Δcarbohydrate from soft drinks (0.368), confectionery (0.361), rice (0.325), bread (0.221), Chinese soup noodles (0.199) in men, and Δtotal carbohydrate (0.547) and Δcarbohydrate from rice (0.376) and confectionery (0.195) in women. Reducing carbohydrate sources by 50 g achieved decreases in HbA1c of 0.43% for total carbohydrate, 1.33% for soft drinks, 0.88% for confectionery, 0.63% for bread, 0.82% for Chinese soup noodles and 0.34% for rice in men and 0.45% for total carbohydrate, 0.67% for confectionery and 0.34% for rice in women, although mean reductions in carbohydrate from these sources were much smaller than that from rice. Conclusion: Decreases in HbA1c achieved by reducing carbohydrate from soft drinks, confectionery, bread and Chinese soup noodles were 2- to 4-fold greater than that for rice. Our results will enable patients to decrease HbA1c efficiently (UMIN000009866).

Objective: To examine the association between the mobilization level during intensive care unit (ICU) admission and independence in activity of daily living (ADL), defined as Barthel Index (BI)≥70. Methods: This was a post-hoc analysis of the EMPICS study involving nine hospitals. Consecutive patients who spend >48 hours in the ICU were eligible for inclusion. Mobilization was performed at each hospital according to the shared protocol and the highest ICU mobility score (IMS) during the ICU stay, baseline characteristics, and BI at hospital discharge. Multiple logistic regression analysis, adjusted for baseline characteristics, was used to deter-mine the association between the highest IMS (using the receiver operating characteristic [ROC]) and ADL. Results: Of the 203 patients, 143 were assigned to the ADL independence group and 60 to the ADL dependence group. The highest IMS score was significantly higher in the ADL independence group than in the dependence group and was a predictor of ADL independence at hospital discharge (odds ratio, 1.22; 95% confidence interval, 1.07–1.38; adjusted p=0.002). The ROC cutoff value for the highest IMS was 6 (specificity, 0.67; sensitivity, 0.70; area under the curve, 0.69). Conclusion: These results indicate that, in patients who were in the ICU for more than 48 hours, that patients with good function in the ICU also exhibit good function upon discharge. However, prospective, multicenter trials are needed to confirm this conclusion.
Influences of the Electroless Nickel Film Condition on the Electroless Au/Pd/Ni Wire Bondability

Electroless Au/Ni plating process is intensively applies to high density printed boards. In this process, the local corrosion is often occurred between the deposited nickel and the deposited gold. Generally, nickel is tends to diffuse from the local corroded areas to the deposited gold surface due to the strong affinity of oxygen after thermal treatment. These areas cause the surface mounting failure. Recently, electroless Au/Pd/Ni plating process is studied actively as the substitution of electroless Au/Ni plating process because of suppressing the nickel corrosion reaction. The wire bonding strength is increased with increasing the palladium and gold film thickness. However, thin gold, palladium films are required to the electronics devices for cost reduction. In this study, we investigated the influence of nickel microstructure for the wire bondability. The deposited nickel microstructures are also influenced the wire bonding properties after thermal treatment. It was confirmed that the good wire bondability can be achieved by using nickel film with layered microstructure than the columnared microstructure. From the AES analysis, we confirmed that preparation of uniform layered microstructure of nickel film is key factor to keep the fold concentration of the gold film surface after thermal treatment

In this study, we examined the feasibility of using subzonal cell injection with electrofusion for interspecies somatic cell nuclear transfer (iSCNT) to produce sei whale embryos and to improve their developmental capacity by investigating the effect of osmolarity and macromolecules in the culture medium on the in vitro developmental capacity. Hybrid embryos produced by the electrofusion of fetal whale fibroblasts with enucleated porcine oocytes were cultured in modified porcine zygote medium-3 to examine the effects of osmolarity and fetal serum on their in vitro developmental capacity. More than 66% of the whale somatic cells successfully fused with the porcine oocytes following electrofusion. A portion (60∼81%) of the iSCNT whale embryos developed to the two- to four-cell stages, but no embryos were able to reach the blastocyst stage. This developmental arrest was not overcome by increasing the osmolarity of the medium to 360 mOsm or by the addition of fetal bovine or fetal whale serum. Our results demonstrate that sei whale-porcine hybrid embryos may be produced by SCNT using subzonal injection and electrofusion. The pig oocytes partly supported the remodeling and reprogramming of the sei whale somatic cell nuclei, but they were unable to support the development of iSCNT whale embryos to the blastocyst stage.

We constructed seven intensity-modulated radiation therapy (IMRT) treatment plans for prostate cancer (49 irradiation fields which contained seven randomly-sampled patients and seven fields) and evaluated the dose distributions by using a radiochromic film (EBT3 film) and a 2D detector. We superposed the calculated dose distribution of the IMRT treatment plan on EBT3 film and the 2D detector results and then compared those with the -analysis pass rate. The relative positions of the beam and the detector were varied; the results of the analysis of the superior-inferior (SI) direction potentially differed, depending on the detector position, under an irradiation beam with the same fluence map. The detector was moved over a range of ±8 mm in the SI direction in 1-mm step increments, measurement were made at each position, and the results were analyzed. The -analysis compared the dose distributions from EBT3 film and the radiation treatment planning system (RTPS) for each patient and field; the pass rate with the -analysis from 98 to 100% was 2.04%. When we compared the dose distributions of the 2D detector and the RTPS, the pass rate from 98 to 100% was 63.2%. The mean values for the -analysis pass rates for EBT3 film and the 2D detector were 94.2 and 97.6%, respectively. Volume averaging of the data indicated a mean pass rate and standard deviation of 98.6 and 0.91%, respectively, and a pass rate of more than 96% for all positions. A 2D detector can, therefore, be used as an alternative apparatus for IMRT dose verification.

Glioblastoma (GBM) is the most common and aggressive type of primary brain neoplasm. The current standard therapy for GBM consists of maximal surgical resection within safe limits, followed by radiation therapy (RT) and chemotherapy with temozolomide. Despite advances in treatment, the prognosis of GBM remains poor. Epileptic seizure is one of the most common symptoms in patients with GBM. Valproic acid (VPA), a histone deacetylase inhibitor, is often used as an anti-epileptic drug in patients with brain neoplasms due to its effectiveness and low toxicity profile. Several in vivo and in vitro studies have indicated that VPA has radiosensitizing effects for gliomas and radioprotective influence on normal brain tissue or hippocampal neurons. The results of several retrospective studies have also indicated potential benefit to improve survival of patients with GBM. Moreover, the promising treatment results of a phase 2 trial of concurrent radiation therapy, temozolomide, and VPA for patients with GBM have been recently reported. The use of VPA in patients with GBM has thus recently receiving more attention. In this article, we review the role of VPA in radiation therapy for GBM, focusing on the clinical evidence.

The persistent and ubiquitous environmental contaminant, tributyltin chloride (TBT), induces not only imposex in gastropods but also the differentiation of adipocytes in vitro and increases adipose mass in vivo in vertebrates. TBT is a nanomolar affinity ligand for retinoid X receptor (RXR) in the rock shell(Thais clavigera) and for both the RXR and the peroxisome proliferator activated receptor $\gamma(PPAR\gamma)$ in the amphibian (Xenopus laevis), mouse, and human. The molecular mechanisms underlying induction of imposex by TBT have not been clarified, though several hypotheses are proposed. TBT promotes adipogenesis in the murine 3T3-L1 cell model and perturbs key regulators of adipogenesis and lipogenic pathways in vivo primarily through activation of RXR and $PPAR\gamma$. Moreover, in utero exposure to TBT leads to strikingly elevated lipid accumulation in adipose depots, liver, and testis of neonate mice and results in increased adipose mass in adults. In X. laevis, ectopic adipocytes form in and around gonadal tissues following organotin, RXR or $PPAR\gamma$ ligand exposure. TBT represents the first example of an environmental endocrine disrupter that promotes adverse effects from gastropods to mammals.
Diagnosis of Lumbar Foraminal Stenosis using Diffusion Tensor Imaging

Diagnosis of lumbar foraminal stenosis remains difficult. Here, we report on a case in which bilateral lumbar foraminal stenosis was difficult to diagnose, and in which diffusion tensor imaging (DTI) was useful. The patient was a 52-year-old woman with low back pain and pain in both legs that was dominant on the right. Right lumbosacral nerve compression due to a massive uterine myoma was apparent, but the leg pain continued after a myomectomy was performed. No abnormalities were observed during nerve conduction studies. Computed tomography and magnetic resonance imaging indicated bilateral L5 lumbar foraminal stenosis. DTI imaging was done. The extraforaminal values were decreased and tractography was interrupted in the foraminal region. Bilateral L5 vertebral foraminal stenosis was treated by transforaminal lumbar interbody fusion and the pain in both legs disappeared. The case indicates the value of DTI for diagnosing vertebral foraminal stenosis.
