        출산에 따른 그레이브스병의 재발 및 관련이자

        Background: Pregnancy affects the course of Graves' Disease (GD), and patients who initially maintain euthyroid function into their middle trimester with minimum doses of antithyroid drugs become exacerbated after delivery. Even patients who are completely cured, requiring no treatment during pregnancy, can relapse after delivery. In this study, we examined the postpartum changes in the thyroid functions of patients with GD, and attempted to determine the factors contributing to these changes. Methods: The study subjects were recruited from pregnant women visiting our outpatient clinic for routine prenatal evaluations. 45 women previously diagnosed with GD, who had been treated and cured with hyperthyroidism, and were no longer taking any thyroid medications, were evaluated for 1 year post delivery. Results: Among 45 patients, 20 (44.4%) developed thyroid disorders following delivery. Postpartum thyroiditis (PPT) developed in 8 patients (17.8%), and GD developed in 12 (26.0%). The onset of the PPT disease 3.1 ± 1.4 months following delivery, which was significantly earlier than the 6.7 ± 2.7 months required for the post delivery onset of GD (p=0.003). The TBII values, measured during the thyrotoxic state in each womaen, were negative in women with PPT and positive in 71.4% of women with GD (p=0.030). The duration of treatment for hyperthyroidism prior or pregnancy, the number of recurrences, and the time interval without treatment, were not associated with the development of postpartum thyroid disorders. Whereas, the mean number of past pregnancies for women who developed PPT was 3.9 ± 2.1, and was significantly higher than the 2.2 ± 1.7 for women developing no thyroid dysfunctions (p=0.044). In 13 women their initial onset of GD occurred within one year postpartum, 7 (53.8%) having had a recurrence, which was significantly higher than in women whose disease onset occurred unrelated to delivery (5 of 32 women: 15.6%). Conclusion: Women with GD developed postpartum thyroid dysfunctions in 44.4% of cases. Women whose initial disease onset occurred within one year postpartum had higher recurrences of GD, and women who developed PPT had a history of higher gravidity compared to the euthyroid women postpartum. Therefore, if women with GD develop postpartum thyroid dysfunctions, the diagnosis should be made, and a treatment modality planned, following careful considerations of the patients' past obstetric history, changes in clinical manifestations and the TBII values

        Overexpression of Insulin Degrading Enzyme could Greatly Contribute to Insulin Down-regulation Induced by Short-Term Swimming Exercise

        Exercise training is highly correlated with the reduced glucose-stimulated insulin secretion (GSIS), although it enhanced insulin sensitivity, glucose uptake and glucose transporter expression to reduce severity of diabetic symptoms. This study investigated the impact of short-term swimming exercise on insulin regulation in the Goto-Kakizaki (GK) rat as a non-obese model of non-insulin-dependent diabetes mellitus. Wistar (W/S) and GK rats were trained 2 hours daily with the swimming exercise for 4 weeks, and then the changes in the metabolism of insulin and glucose were assessed. Body weight was markedly decreased in the exercised GK rats compare to their non-exercised counterpart, while W/S rats did not show any exercise-related changes. Glucose concentration was not changed by exercise, although impaired glucose tolerance was improved in GK rats 120 min after glucose injection. However, insulin concentration was decreased by swimming exercise as in the decrease of GSIS after running exercise. To identify the other cause for exercise-induced insulin down-regulation, the changes in the levels of key factors involved in insulin production (C-peptide) and clearance (insulin-degrading enzyme; IDE) were measured in W/S and GK rats. The C-peptide level was maintained while IDE expression increased markedly. Therefore, these results showed that insulin down-regulation induced by short-term swimming exercise likely attributes to enhanced insulin clearance via IDE over-expression than by altered insulin production.

        To evaluate the specificity and historical trends of the anesthesia in Chungnam National University Hospital, anesthetic experiences of 83,572 in total performed at Chungnam National University Hospital from January 1967 to December 1996 were analyzed statistically and clinically according to age, sex, surgical department, physical status, elective and emergency surgery, premedicants, IV anesthetics, anesthetic methods, inhalational anesthetics, muscle relaxants. The results were as follows : 1. The number of cases has been steadily increased year by year. 2. The ratio of male to female was 52.8% : 47.2%. 3. According to the age, There were 44,905 cases (54%) in the group of 13-45 years. 4. According to the ASA classification of physical status, most of the cases were belonged to the class 1 & 2 (72.5%). 5. The ratio of elective to emergency was 79.4% to 20.6%. 6. Recently, glycopyrrolate has been used increasingly. 7. Thiopental sodium has been mainly used for intravenous induction agent. 8. General anesthesia has been used mainly. 9. Enflurane is the most common inhalational anesthetic agent. but Isoflurane has been used increasingly. 10. Most of muscle relaxants are Pancuronium & Vecuronium(84.5%).

        Numerical methods that are known as Monte as Monte Carlo methods can be loosely described as statistical simulation methods, where statistical simulation is defined in quite general terms to be any method that utilizes sequences of random numbers to perform the simulation. Monte Carlo methods are now used routinely in many problems, the simulation of the esoteric subnuclear processes in high energy physics experiments, cellular automata, self-organized critical phenomena, the Ising spin glass, the simulation of a Bingo game, etc. The physical processes are simulated directly, and there are no needs to even write down the differential equations that describe the behaviors of the systems. The only requirement is that the physical systems be described by probability density functions. We consider with the Monte Carlo variational method and the Monte Carlo path integral method(MCPI). These methods are used to calculate the energy of the ground states of H2 molecule by the formal similarity between the Schroedinger equation and the multi-dimensional diffusion equation.

        Effects of solutions of 0.5% bupivacaine, containing different concentrations of dextrose (0%, 0.5%, 0.8%, 1%, 3%, 5%, 8%) were studied after intrathecal injection in seven (Group A-Group G) groups of 15 patients. The results are summarized as follows: 1. The mean specific gravity of solutions was 1.0065, 1.0087, 1.0092, 1.0094, 1.0152, 1.0198 and 1.0314 (Group A-Group G). These values are isobaric (Group A, B), hyperbaric (Group E, F, G), and slightly hyperbaric (Group C, D) compared to cerebrospinal fluid at 25℃. 2. The mean maximum level of sensory block was increased according to the increment of specific gravity of the solutions. But no significant differences between Group A and B, Group C and D, Group E, F, and G. 3. The mean time of sensory block to T11 was decreased according to the increment of specific gravity of the solutions. 4. The mean time to the expected level (T4) of sensory block at given dosage of each solution was 25.7, 18.4, 9.7 minutes(Group E, F, G, respectively). 5. Complete motor block of the legs was rapid according to the increment of specific gravity of the solutions.

        Direct methods and indirect methods for the adaptive control of linear plant, DC motor, are currently well known. Recently, methods for combining direct and indirect methods have also been suggested. In this note the latter method is combined with the variable structure method. New adaptive laws are suggested which overcome the principal draw-backs of the variable structure method. While the direct and indirect components of the new method assure stability, the variable structure component improves the transient response of the system. Simulation results are presented to show that the overall system is asymptotically stable in the ideal case, and exhibits robustness and improved performance in the presence of perturbations.

      • 연속회분식 반응기를 이용한 축산 폐수의 생물학적 고도 처리 운전 인자 도출에 관한 연구

        축산폐수는 고농도의 유기탄소 뿐만아니라 영양염류를 포함하는 대표적인 고농도 폐수이며 적절히 처리되어지지 못할 경우 심각한 담수의 부영양화와 연안의 적조를 야기할 수 있다. 이러한 축산폐수를 보다 경제적이며 기술적으로 처리하기 위해 본 연구에서는 연속회분식반응기(SBR)을 이용한 축산폐수의 고도처리에 적용할 수 있는 운전인자와 운전기법을 연구하였다. 초기 암모니아와 유기탄소부하를 달리하며 정해진 주기로 운전하는 네 가지 전형적인 회분식 실험과 원수를 연속으로 주입하면서 간헐 폭기로 운전하는 세 가지의 변형회분식 반응 실험을 통하여 전형적인 SBR에서 비 질산화 속도에 가장 많은 영향을 미치는 인자가 초기 pH 와 유리 암모니움 농도이며 최적 초기 암포니아성 비 질소 부하율은 0.1 ~ 0.15 kg NH_4^+-N /kg VSS 근처이며 이때 비 질산화율은 0.06 kgNH_4-N/ kg MLVSS·day로 관찰되었으며, 변형 SBR로 운전하는 경우 최소 35%에서 최대 65% 까지 탈질용 외부기질 주입량을 절감할 수 있었다. Piggery wastewater is a representative of high strength wastewater which contents not only high concentration organic carbon but also nutrients. From this reason piggery wastewater could be cause a serious eutrophication of fresh water and red tide in coastal areas if not properly treated. In this study to treat these piggery wastewater technically and more economically, the operation parameters and methods for advanced treatment using Sequencing Batch Reactor (SBR) were investigated. Four sets of experiments were conducted on laboratory scale typical SBR (Fill in a time) with fixed cycle and different till ratio to evaluate most proper initial loading of organic carbon and nitrogen and three sets of experiments on laboratory scale inodified SBR (Fill continuously) with intermittent aeration. As results the nitrification rate was most sensitively affected by intial pH and free ammonia concentration and in typical SBR operation most profit initial specific nitrogen loading were investigate around 0.1~0.15kg NH_4^+-N /kg VSS and in this range the average specific nitrification rate was 0.06 kg NH_4-N/ kg MLVSS·day. And in modified SBR operation with most profit specific nitrogen loading rate the cost of external denitrification cost were saved minimum 35% to maximum 65% each experimental sets.
