        유기용제노출이 직업적 청력손실에 미치는 영향

        목적 : 산업 장에서 노출되는 유기용제가 청력역치에 어떠한 영향을 미치는가를 5년 동안의 추적조사를 통하여 파악하고자 하였다. 방 법 : 유기용제와 소음에 동시에 노출된 근로자 43명과 소음노출수준, 연령, 근무경력을 짝짓기 한소음노출군을 대상으로 문진과 이경검사, 소음노출 수준, 유기용제노출수준을 조사하였다. 결과 : 1. 전체 근로자의 각 주파수별 청력역치 변화양상에서 500Hz∼2000Hz에서는 추적기간 동 소음동안 현저한 변화가 없었고, 4000Hz 및 8000Hz에서는 역치가 증가하는 경향을 나타내었다. 2.유기용제와 소음에 동시에 노출된군에서 소음 노출군에 비해 4000Hz 이상에서 청력역치가 높게 나타났으나, 통계적인 유의성은 없었다(p(0.05). 3.반복측정 분산분석 법으로 각 주파수별 평균청력 역치의 연차적 변화에 미치는 영향을 분석한 결과 유기용제 노출은 통계적으로 유의한 영향이 없었으나(p)0.05), 소음노출수준은 250Hz, 2000Hz 및 4000Hz에서, 연령은 250Hz와 500Hz에서 통계학적으로 유의한 영향을 미치는 것으로 나타났다(p(0.05). 4.소음노출군파 동시노출군 각각에서의 청력역치 변화량을 paired t-test로 비교한 결과 250Hz∼4000Hz에서는 통계학적 유의성이 없었으며(p(0.1), 8000Hz에서만 경계치 (p=0.0884)의 유의성을 나타내었다. 5.요인들의 영향을 보정한 상태에서 관찰하기 위해 관찰기간동안의 청력역치변화량을 종속변수로 두고 일반선형모형을 이용하여 주파수별로 관찰한 결과 8000Hz에서만 모델에 통계학적 유의성이 있었으며 (p(0.05), 여기서 소음노출수준이 통계학적으로 유의한 영향을 미치는 것으로 나타났고(p(0.05), 유기용제노출은 경 계수준이 었다(p=0.087). 결론 : 청력에 대한 유기용제의 영향이 소음과 연령에 비해 상대적으로 미미하여 명확한 결론에 이르지는 못하였으나, 다소 영향이 있는 것으로 생각되어 유기용제의 영향에 대한 추가연구가 필요할 것으로 판단된다. 또한 보다 효과적인 근로자들의 청력관리를 위해서는 소음 외 산업 장에서 동시에 노출되는 여러 유해물질들을 고려하여야 할 것이다. Objectives : This study was to evaluate the impacts of simultaneous exposure to noise and mixed solvents on workers'hearing threshold level (HTL) over five-year period. Methods : The study was conducted by interview and annual audiometric test on workers in ship building industry from 1994 to 1998. The cases(workers who exposed to noise and mixed solvent simultaneously) were 43 male workers and controls (workers who exposed to nolle) were selected by matching method with regard to age, sex, carrier, and noise exposure level. To assess the impacts of solvent exposure on hearing threshold level, with considering other factors, general liner model was used. Results : 1. The audiogram of all subjects showed typical sensorineural hearing loss pattern. The mean HTLe were increased at high frequency (4000HB and 8000Hz) for study period. 2. The HTLs of cases were more increased than those of controls at high frequencies (4000Hz and 8000Hz), but there was not statlstical significance. 3. The impact of age on the HTL was statistically significant at 250Hz and 500Hz (p(0.05), and the impact of noise on the HTL was statistically significant at 250Hz, 2000Hz and 4000Hz (p(0.05), but the impact solvent exposure on the HTL was not significant. 4. The changes in HTLs of cases were higher than those of controls at high frequency, there were not statistical significance at 4000Hz, but only at 8000Hz (p=0.087). 5. Statistical analysis of the general linear model implicated that the changes in HTL was impacted by noise exposure bevel significantly (p=0.031) and Impacted by solvent exposure weakly (p=0.087) at 8000Hz. Conclusions : The results of this study suggest that workers who simultaneously exposed to noise and mixed solvent were at risk for more affected HTL than those exposed to noise exposure only, but we could not find definitive evidence. Further detailed studies must be made in large number of workers.

        ‘건국’을 바라보는 두 가지 시선

        In commemoration of the 60th anniversary of the foundation of the Republic of Korea, the oral statements from people who actually lived and experienced the process of foundation will be examined in this article, and of course the historical meaning will be assessed as well. For the past few years, a reactionary trend against the existing historical studies of the Korean contemporary period has been formed. This trend could be labelled as a 'reinterpretation of the Korean contemporary history', and it is particularly emphasizing the role of the elites during the process which started from liberation and then led to the government's foundation, and also to the Korean war. In the meantime, historical studies using oral statements have begun to be noticed as well. Such studies complimented and also enhanced the existing studies with their new methods, and they ended up presenting a fundamentally critical analysis of the aforementioned trend of 'reinterpretation'. During the three years period under U.S. military government and also the First Republic period, the people of the Republic of Korea were protected neither by the founding ideology of the nation nor the nation's own constitution. On the other hand, they were busy protecting themselves from governmental bodies such as the police, the youth alliance and the military, as we can hear from the oral statements they made. Individual's freedom of speech and the principle of fair competition, which are usually regarded as the essence of free democracy and market-based economy, were being considerably threatened during the process of the nation's foundation. Especially the very process of determining the constitutional system for the Republic of Korea, in other words the May 10th election, was a party for the elites alone, as the free voices of the community members or debates discussing major agendas were no where to be found. Oral statements from the people who lived that time period truly reveals how such 'party for elites' was an empty one, and how much the Republic of Korea owes to the sacrifice and efforts of its own people, who were excluded from the foundation process itself.

        한국인에서 당화혈색소를 통한 당뇨병 진단의 적절성 평가

        Objectives: An HbA1c test result ≥ 6.5% has recently been recommended as the defining criterion for diabetes by the American Diabetes Association (ADA). The aim of the study was to assess the screening test properties of HbA1c for undiagnosed diabetes mellitus (DM) according to the ADA criteria. Methods: Oral glucose tolerance test (OGTT) and HbA1c were performed among 163 subjects without previously diagnosed diabetes at Haenam General Hospital, Korea between January, 2005 and June, 2010. The OGTT is considered to be the gold standard for diagnosing diabetes. Based on a ROC curve, optimal sensitivity and specificity were derived for identifying the HbA1c threshold. The level of agreement (ĸ statistics) between OGTT and HbA1c were calculated. Results: The prevalence of pre-diabetes and diabetes were 27.6% and 56.4%. The area under the receiver operating characteristic curve for undiagnosed DM was low in HbA1c (0.814 [95% CI: 0.750-0.879]) compared to fasting plasma glucose (0.924 [0.884-0.964]) and 2-h plasma glucose (0.924 [0.884-0.964]). HbA1c of 6.5% gave a sensitivity of 70.7%, a specificity of 76.1%. The level of agreement between two diagnostic criteria was low (ĸ = 0.448). Conclusion: The findings in this study demonstrate that the discordance between the OGTT and HbA1c criteria in the diagnosis of diabetes in some Koreans is large. The measurement of HbA1c alone may be inefficient to screen undiagnosed DM. Further large-scale, population-based studies are needed to evaluate the HbA1c threshold for diagnosing diabetes in Korea.

        Melatonin Protects Chronic Kidney Disease Mesenchymal Stem/Stromal Cells against Accumulation of Methylglyoxal via Modulation of Hexokinase-2 Expression

        Treatment options for patients with chronic kidney disease (CKD) are currently limited; therefore, there has been significant interest in applying mesenchymal stem/stromal cell (MSC)-based therapy to treat CKD. However, MSCs harvested from CKD patients tend to show diminished viability and proliferation due to sustained exposure to uremic toxins in the CKD environment, which limits their utility for cell therapy. The application of melatonin has been demonstrated to improve the therapeutic efficacy of MSCs derived from and engrafted to tissues in patients suffering from CKD, although the underlying biological mechanism has not been elucidated. In this study, we observed overexpression of hexokinase-2 (HK2) in serum samples of CKD patients and MSCs harvested from an adenine-fed CKD mouse model (CKD-mMSCs). HK2 upregulation led to increased production levels of methylglyoxal (MG), a toxic metabolic intermediate of abnormal glycolytic processes. The overabundance of HK2 and MG was associated with impaired mitochondrial function and low cell proliferation in CKD-mMSCs. Melatonin treatment inhibited the increases in HK2 and MG levels, and further improved mitochondrial function, glycolytic metabolism, and cell proliferation. Our findings suggest that identifying and characterizing metabolic regulators such as HK2 in CKD may improve the efficacy of MSCs for treating CKD and other kidney disorders.

        Upregulation of receptor tyrosine kinase-like orphan receptor 2 in idiopathic pulmonary fibrosis

        Background/Aims: Receptor tyrosine kinase-like orphan receptor 2 (ROR2) is a major regulator of Wnt signaling, which is involved in fibroblast dysfunction. Because its role has not been evaluated in idiopathic pulmonary fibrosis (IPF), we examined the clinical implications of ROR2 expression. Methods: ROR2 mRNA expression was measured using reverse transcription polymerase chain reaction in lung tissue-derived fibroblasts from IPF patients (n = 14) and from controls (n = 10). ROR2 protein was measured using enzyme-linked immunosorbent assay in primary fibroblasts from IPF patients (n = 14) and controls (n = 10), and in bronchoalveolar lavage (BAL) fluids obtained from normal controls (NC; n = 30). IPF patients (n = 84), and other patients with interstitial lung diseases, including nonspecific interstitial pneumonia (NSIP; n = 10), hypersensitivity pneumonitis (HP; n = 10), and sarcoidosis (n = 10). Results: ROR2 mRNA and protein levels were significantly higher in IPF fibroblasts than in controls (p = 0.003, p = 0.0017, respectively). ROR2 protein levels in BAL fluids from patients with IPF were significantly higher than in those from NC (p < 0.001), and from patients with NSIP (p = 0.006), HP (p = 0.004), or sarcoidosis (p = 0.004). Receiver operating characteristic curves showed a clear difference between IPF and NC in ROR2 protein level (area under the curve, 0.890; confidence interval, 0.829 to 0.950; p < 0.001). ROR2 protein levels were significantly higher in GAP stage III than in GAP stages I and II (p = 0.016). Conclusions: ROR2 may be related to the development of IPF, and its protein level may be a useful and severity-dependent candidate marker for IPF.

        High-resolution ultrasonography is a more useful primary diagnostic tool than magnetic resonance imaging for subungual glomus tumors: a single-center retrospective study

        Purpose: Ultrasonography (US) and magnetic resonance imaging (MRI), which has better resolution, have been suggested as appropriate diagnostic tools for digits; however, comparative studies of both modalities are scarce. This study compared the diagnostic performance of high-resolution US and MRI in characterizing subungual glomus tumors. Methods: We retrospectively analyzed the data of consecutive patients who were examined with high-resolution US and MRI before surgical exploration for subungual tumors from January 2017 to April 2020. The patients’ clinical data and radiological findings were collected. Historical reports for diagnostic confirmation and measurements of mass size were reviewed. We conducted the McNemar test and evaluated the degree of agreement between the size measurements made using both techniques. Results: Overall, 22 patients (age range, 19–72 years) underwent surgical exploration and had subungual glomus tumors. The mean duration from initial symptom presentation to diagnosis was 7 years (range, 5–30) years. Only nine patients (40.9%) presented with the symptomatic triad of pain, cold sensitivity, and tenderness. MRI detected lesions in 19 cases (86.4%); three cases (13.6%) were undetected. Contrastingly, US was able to detect all lesions. Nine patients (40.9%) had visibly rich blood flow in tumor tissues on Doppler US. MRI and US showed bone erosion in eight and 15 cases, respectively. Conclusion: The detection rate of high-resolution US for subungual glomus tumors was as high as that of MRI. US can be applied easily and quickly and can be more useful as a primary diagnostic tool.

      • 두개인두종 수술시 뇌하수체경이 절단된 환자에서 요붕증의 자연해소 -증례보고-

        A 27-year-old man visited to our institute suffered from diabetes insipidus(DI) symptoms and progressively decreasing visual acuity. The brain magnetic resonance imaging(MRI) showed 4.4×4.3×3.8 cm sized brain tumor situated on the suprasella area and his symptoms and laboratory findings were compatible with panhypopituitarism. Surgery was scheduled and craniotomy via right pterional, trans-lamina terminalis approach was performed and total removal was achieved. However, in the last stageght procedure, proximal pemoval was ach, which was the origin site lasttr, was the origin site for that tumor, was transected. The pathology was confirmed for craniopharyngioma and postoperatively, headache was improved 3 days after the surgery. Panhypopituitarism needed continuous hormonal replacement therapy but interestingly, DI symptoms were improved spontaneously, 5 days after surgery through administration of 6 times of vasopressin for 4 days. With review of the literatures, the authors reports the case of craniopharyngioma that the DI symptoms were resolved spontaneously, despite of transectioning of the pituitary stalk during surgery.
