      • Antiulcer activity of Trichosanthes cucumerina linn. against experimental gastro-duodenal ulcers in rats

        Trichosanthes cucumerina Linn. (cucurbitaceae) is widely used in Indian folk medicine for variety of disease conditions. The aim of present study was to evaluate the antiulcer activity of 50% ethanolic extract of fruits of Trichosanthes cucumerina Linn. (TCFE) using various experimental models of gastric and duodenal ulceration in rats. Oral administration of 50% ethanolic extract of fruits of Trichosanthes cucumerina Linn. was evaluated in rats against ethanol, aspirin and pylorus ligated gastric ulcers as well as cysteamine-induced duodenal ulcers. In all the models studied, the antiulcer activity of TCFE compared with that of cimetidine (100 mg/kg, p.o.), an $H_2$ receptor antagonist. TCFE showed significant antiulcer activity in ethanol-induced and aspirin-induced gastric ulcer models. In 19 h pylorus ligated rats, significant reduction in ulcer index, total acidity and pepsin activity was observed with TCFE, when compared with the control group. Mucosal defensive factors such as pH, mucin activity and gastric wall mucous content was found to be increased with TCFE. TCFE was also, afforded remarkable protection in cysteamine-induced duodenal lesions. The antiulcer activity of TCFE was comparable with that of cimetidine. Thus, TCFE possess significant antiulcer activity against both gastric and duodenal ulcers in rats. The antiulcer activity may be attributed to its cytoprotective action and inhibition of acid secretary parameters.

        Successful Endovascular Treatment of a Ruptured Giant Lumbar Arteriovenous Malformation

        Arteriovenous malformations (AVMs) are rare congenital vascular anomalies where rupture can be fatal. This report describes a case of a 69-year-old woman who presented with sudden severe lower back pain radiating to her left buttock and thigh and an inability to move her left leg. Computed tomography angiography (CTA) showed an extensive ruptured retroperitoneal AVM on the left paravertebral side with active bleeding and a large hematoma. Cardiopulmonary resuscitation with considerable blood transfusion was required. The AVM was effectively treated with coil embolization of the feeding lumbar arteries. Shortly after the intervention, the patient developed abdominal compartment syndrome. Urgent laparotomy with evacuation of the retroperitoneal hematoma was required. The patient was discharged 16 days after admission in a good physical condition and has been free of symptoms for more than a year after intervention. However, follow-up CTA showed residual contrast enhancement in the AVM and further surveillance is required.

        Impact of Job Satisfaction on Greek Nurses' Health-Related Quality of Life

        Background: Employee job satisfaction and its relationship with health and quality of life has been an issue of major concern over the past decades. Nurses experience difficult working conditions that affect their job satisfaction, health, and quality of life. Methods: A cross-sectional study was undertaken in three general hospitals and their respective health centers. Stratified random sampling by level of education was used, and 508 nurses and nursing assistants were included. A self-administered anonymous questionnaire, which included the Measure of Job Satisfaction, the 36-item Short Form Health Survey, as well as demographic details, education, and work conditions data, was used. Results: Greek nurses were found to be dissatisfied with their job according to the total score of the job satisfaction scale, although personal satisfaction and satisfaction with support had had higher scores. Their general health was reported as average, because of physical and mental health problems, low vitality, low energy, and increased physical pain. Multivariate linear regression analysis revealed that males and those wishing to stay in the job had higher physical and mental health. Increased job satisfaction was related to increased physical and mental health. Conclusion: Although Greek nurses are not satisfied with their work, those with high levels of job satisfaction had better health-related quality of life. The findings suggest that improvement of the work environment would contribute to a healthier and more satisfied nursing workforce.

        Total Endovascular Therapy of Abdominal Aortic Aneurysm, Peripheral Artery Disease, and Chronic Mesenteric Ischemia: A Challenging Case

        We presented a challenging case of a patient diagnosed with abdominal aortic aneurysm (AAA), peripheral artery disease, and chronic mesenteric ischemia (CMI). Herein, we describe the treatment in this high-risk patient diagnosed with CMI who also had critical limb ischemia and his AAA had rapidly expanded. First we performed angioplasty and celiac arterial stenting. Afterwards, we proceeded to perform balloon angioplasty of the iliac arteries and chimney endovascular aneurysm repair (Ch-EVAR) preserving the inferior mesenteric artery (IMA). The patient was discharged three days later and his IMA remained patent eighteen months post-operation. A thorough pre-operative assessment is essential in such challenging cases. Minimally invasive procedures like endovascular therapy and the chimney technique extend the prognoses in high-risk patients.

        Proinflammatory cytokines in irritable bowel syndrome: a comparison with inflammatory bowel disease

        Background/Aims: Irritable bowel syndrome (IBS) is a common disease often considered as a functional intestinal disorder. Inflammation in IBS is a quite intriguing theory. The aim of this study was to investigate tumor necrosis factor (TNF)-α, interleukin (IL)-1β and IL-6 expression in inflammatory bowel disease (IBD) patients, IBS patients and normal controls. Methods: IBS and IBD patients along with normal controls were recruited in the study. In all groups, 2 pinch biopsies were taken at each of 3 anatomical sites (terminal ileum, cecum, and rectum). IBS patients were also subcategorized according to the syndrome clinical manifestations. Two monoclonal antibodies (mAb), TNF-α mAb and IL-6 mAb, and one polyclonal antibody IL-1β mAb were applied for immunohistochemical analysis. Results: In IBD patients intensity of TNF-α and IL-1β were lower than in IBS patients or controls, while IL-6 was significantly increased comparing to the aforementioned groups. In IBS patients TNF-α was increased comparing to IBD patients or controls, while IL-6 and IL-1β were similar to controls. In IBS subgroups, TNF-α was lower in diarrhea predominant IBS patients and higher constipation predominant IBS patients. Differences among IBS subgroups regarding IL-6 and IL-1β were nonsignificant. Conclusions: IL-6 seems to be the most important proinflammatory cytokine in IBD patients, while TNF-α could play a more significant role in IBS pathogenesis. (Intest Res 2020;18:115-120)
