Malignant peritoneal mesothelioma is a rare neoplasm that arises from the mesothelium of a serosal cavity and is a rapidly fatal disease with a median survival of 4 to 12 months for untreated cases. Recently, we experienced a case with malignant peritoneal mesothelioma who was suspected hepatocelluar carcioma by abdominal CT scan and was confirmed by biopsy including immunohistochemical stain(calretinin) after surgery. We performed tumor excisions and wedge resection of the liver(segment Ⅷ)and inserted Tencoff catheter in abdominal cavity at 25th day of post-operation. We treated with intraperitoneal paclitaxel(25mg/m^(2)/day for 5 days) six courses monthly. She was well tolerable and is still living without any evidence of recurrence for 14th month of post-operation.
자가 면역성 간염에서 자가항체의 발현 양상과 스테로이드 치료의 효과

목적 : 최근 8년간 본원에서 경험한 자기면역성 간염의 혈액학적 특징과 스테로이드의 치료효과를 알아보고자 본 연구를 시행하였다. 방법 : 1996년 이후부터 2002년까지 충남대학교 병원에서 1999년 International Autoimmune Hepatitis Group(LAHG)에서 제시한 수정 진단 기준안에 의거하여 총 진단점수가 probable 이상에 해당하는 환자들을 대상으로 임상적 특징, 검사실 및 조직 소견과 스테로이드와 azathioprine의 치료 효과를 후향적으로 조사하였다. 결과 : 9예의 환자들은 여성이 78%, 40대가 많았고, 급성 발병이 33.3%, 피로감, 식욕부진 및 황달 등이 나타났으며, 항핵항체가 55.6%에서 그리고 항평활근 항체가 77.8%에서 양성이었고, 동반 질환은 류마치스 관절염이 2예, 특발성 혈소판 감소증 1예, Henoch-Sch nlein 자반증 및 그레브스병이 각각 1예 씩이었다. 7예에 대해서 스테로이드와 azathioprine으로 치료하여 생화학적 임상적 완해가 이루어져서 유효한 치료 결과를 얻었다. 결론 : 자가면역성 간염은 우리 나라에서 원인을 찾을 수 없는 만성 간질환의 경우, 특히 중년여성에서 다른 자가면역성 질환이 동반되었을 때 자가면역성 간염을 의심하여 자가항체 선별검사를 시행해 보아야 할 것으로 생각된다. 향후 다기관 연구를 통하여 좀 더 많은 증례를 모으고 전향적인 연구를 시행하여야 할 필요가 있다. Although autoimmune hepatitis(AIH) is common in western contries, some studties have been reported in Korea. The aim of the study was to analyze the clinical and histological features and expression pattern of the autoantibody and to examine clinical course of AIH in Korea. For 9 patients diagnosed as having AIH in our hospital since 1996, we reviewed medical records and analyzed clinical and laboratory findings retrospectively. They had a probable or definite AIH according to the revised criteria of International AIH group. The mean age was 42.7(21∼68)years and 7(77.8%) were female. Three of them(33.3%) showed acute hepatitis and five of them showed cirrhosis. Fatigue and anorexia were the most frequently complained symptoms(55.6%). Four patients(44.4%) were accompanied with other autoimmune diseases. Antinuclear antibody was detected in 5(55.6%) and anti-smooth muscle antibody was detected in 7(77.8%). Three patients showed focal picemeal necrosis and four showed periportal fibrosis. Among 7 patients who received immunosuppressive therapy, six patients(85.7%) showed initial response. and biochemical and serologic remission were achived in 5(71.4%) and 3(42.8%) patients, AIH seems to be partly responsible for chronic liver disease of unknown etiology in Korea. AIH should be suspected in female patients with unknown chronic liver disease of unknown etiolgy, especially accompanied with other autoimmune disease. The clinical characteristics of AIH may not be quite different from those of Western countries.

A combined treatment with conventional chemotherapies can enhance the effectiveness of chemotherapeutic agents against cancers. Here, we have shown that the naturally occurring triterpenoids synergistically enhance the response of cervical cancer cells to taxol. Of the triterpenoid compounds, pristimerin enhanced the anticancer effect of taxol with the highest efficiency by combination. Pristimerin synergizes with taxol to inhibit clonogenic survival and tumor growth in nude mice, and to enhance cell death in cervical cancer cells. A combined treatment with taxol and pristimerin induced cervical cancer cell death by increasing intracellular reactive oxygen species levels, upregulation of death receptor death receptor 5 (DR5), activation of Bax, and dissipation of mitochondrial membrane potential. Treatment with N-acetyl-L-cysteine, a thiol-containing antioxidant completely blocked combined treatment-induced Bax translocation as well as DR5 upregulation. Moreover, inhibition of Jun N-terminal kinase/c-Jun pathway attenuated cell death by blocking DR5 upregulation and Bax activation. These results indicate that the triterpenoid, pristimerin, synergistically enhances taxol response of cervical cancer cells through DR5 expression and Bax activation. Furthermore, the reactive oxygen species-dependent activation of the Jun N-terminal kinase/c-Jun pathway is required for the DR5 upregulation and Bax activation. The molecular mechanism revealed by this study may aid in the design of future combination cancer therapies against cells with intrinsically reduced sensitivity to taxol.

<P>This study investigates the possible causes for low development of blastocysts in vitrified immature oocytes by evaluating the changes of mitochondrial membrane potential and reactive oxygen species (ROS) production and finds a recovery mechanism for these conditions in vitrified immature oocytes. To recover from the cryoinjury, we cultured vitrified immature oocytes in milrinone containing medium for 1, 3, and 5 hours and then extended the culture for oocyte maturation. There was no difference in in vitro maturation and fertilization rate between fresh and vitrified/warmed oocytes. However, the development rate of blastocysts in vitrified/warmed oocytes was significantly lower than that in fresh oocytes (p<0.05). The development rate of blastocysts was recovered if these oocytes were cultured for 3 hours in milrinone. Vitrified/warmed oocytes incubated in milrinone for 0 and 1 hour showed a significantly higher level of ROS (p<0.05) and a significantly lower mitochondrial membrane potential (p<0.05) than fresh oocytes. However, there was no significant difference (p>0.05) between vitrified oocytes incubated in milrinone for 3 hours and fresh oocytes in terms of ROS level and mitochondrial membrane potential. In conclusion, alteration of highly polarized mitochondria distribution in vitrified oocytes may have an effect on mitochondrial activity, including ROS production during fertilization and further development. Preincubation in milrinone before in vitro maturation of immature vitrified/warmed oocytes may help the redistribution of highly polarized mitochondrial inner membrane potential and in reducing ROS and enhance the further embryonic development after fertilization.</P>

호남고속철도구간에 부설되는 테스트베드는 400km/h 급 고속차량의 운행에 적합하도록 개발된 인프라의 핵심기술을 실열차 운행시 성능평가를 통하여 실용화 기반을 구축하기 위한 것으로 2014 년까지 구축될 예정이다. 테스트베드에는 국가연구개발사업으로 개발 중인 환경소음 저감장치(방음벽 상단장치, 흡음블럭), 전차선로 시스템의 개발품과 이의 성능을 측정하기 위한 계측시스템, 그리고 400km/h 이상의 고속대역에서 노반, 궤도, 교량의 성능을 평가하기 위한 모니터링시스템이 설치된다. 본 실행계획은 테스트베드를 구축하기 위한 액션플랜으로‘400km/h 급 고속철도 인프라 시범기술 개발’과제에서 수행되었다. 이 과제는 한국철도기술연구원을 주관기관으로 2010 년 12 월 착수하여 2014 년 10 월 종료 예정이며 현재는 3 차년도 과제가 진행 중에 있다. This execution plan, made an issue of development of high-speed railroad infrastructure testing technology of 400km/h class action plan for building a test-bed. This challenge in charge of the Korea Railroad Research Institute it started in December 2010 in the institution, it is scheduled to end in October 2014, issue of the third year is in progress now.

최근 국내의 도심지에서 지하굴착공사 및 수도관 파열 등으로 토사가 유실되고 있으며 비근한 예로 도시철도 지하철 석촌 지하철역 및 인천 2호선 인근에서 대규모 공동이 발생하였다. 지하철은 일일 이용자수가 높으므로 사고발생시 커다란 인명피해가 발생할 수 있어 안정성 확보가 필요하며 이를 위해서는 도시철도에 영향을 미치는 위험인자에 대한 평가가 반드시 이루어져야 하므로 본 연구에서는 도시철도 인접지역에서 공동이 발생한 경우 차량 하중을 고려한 철도 노반의 안전성을 평가하기 위하여 삼차원 유한차분해석 프로그램인 FLAC3D를 이용하였다. 궤도중심으로부터 이격거리 20m에서 발생한 다양한 공동의 깊이, 폭, 길이에 따른 노반 침하량을 산정하여 노반 안정성을 평가하였다. In recent days, underground excavations and water pipeline damages have caused a lot of soil losses especially in urban areas. For instance, large-scale cavities were found at the roads adjacent to both Seokchon subway station and Incheon subway line No.2. Since a lot of people use urban subway every day, a big accident leads fatal life loss. Therefore, urban railway should be secured and the risk factors influencing on the railway stability should be examined. In this study, a three-dimensional finite difference model using the commercial program FLAC3D is adopted to estimate the stability of railway roadbed adjacent to cavity as a train loading is applied. Roadbed settlements were estimated, with respect to various depths, widths, and lengths of the cavities located at a 20-m distance from the center of railway, to examine the stability of the roadbed.
