      • 만성 화농성 중이염의 세균학적 고찰

        The chronic suppurative otitis media is one of the most common disease in the otologic field. And the works on bacterial distribution of chronic suppurative otitis media were studied by many otologist, but there is some difference. The bacteriologic study was done on 126 cases of chronic suppurative otitis media who visited the Department of Otolaryngology, Chosun University Hospital between January 1989 and December 1989 and the following conclusions were obtained. 1) In sex distribution, the male had higher incidence rate than female (M: 56.3%, F:43.7%). 2) In age distribution, 3rd decade (34.1%), 2nd decade (34.1%), were most prevalent. 3) In duration of the disease, the hightest frequence was below 5 years duration 4) Among 126 cases, 98 cases(77.8%) were unilateral (right ear was 47 cases(37.3%) and left ear was 51 cases(40.5%))and 28 cases(22.2%) were bilateral. 5) The 136 cases(88.3%) showed the positive results under culture, but 18 cases(11.7%) didn't growthed. Among 136 cases 95 cases(61.7%) had single infection and the 41 cases(26.6%) had mixed infection. 6) The most frequently isolated bacteria was staphylococcus(38 strains, 27.9%) and next were proteus(27 strains, 19.9%) and pseudomonas (22 strains, 16.2%). 7) The Staphylococcus was sensitive to cephalothin(94.7%), amikacin(78.9%) and resistant to penicaIin(63.2%). 8) The Proteus was sensitive to amikacin(92.6%), cephalothin(88.9%) and resistant to minocycline(81.5%) and gentamicin(70.4%). 9) The Pseudomonas was sensitive to amikadn(86.4%), gemtamicin(59.1%) and resistant to minocycline(90.9%), ampicillin(86.4%) and cephalothin(86.4%).

      • Feasibility and safety of nerve stimulator attachment to energy-based devices: A porcine model study

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Recently, several energy-based devices (EBDs) have been developed and applied in the context of thyroid surgery. EBDs can reduce operation time, blood loss, and postoperative pain. Compared to conventional electrocautery, EBDs operate at a relatively lower temperature and produce minimal lateral tissue damage. Yet, during device operation, the tip of the EBD is hot enough to cause thermal nerve damage, increasing the need for surgeons to be cautious about EBD application. To increase the safety of EBDs, we attached nerve stimulators to the tips of two EBDs and compared them to conventional monopolar nerve stimulation using a porcine model.</P> <P><B>Methods</B></P> <P>Three piglets (30–40 kg) underwent total thyroidectomy after orotracheal intubation with a nerve integrity monitor (NIM) electromyography (EMG) endotracheal tube. Nerve stimulators were attached to two EBDs (Harmonic Focus<SUP>®</SUP>+ and LigaSure™). After dissection and identification of six recurrent laryngeal nerves in the three piglets, both of the EBDs with attached nerve stimulators and a conventional monopolar nerve stimulator were applied near the nerve and EMG parameters were recorded using the NIM 3.0 system. The stimulus intensity was varied from 5 mA to 1 mA and the maximum distance and amplitude at which nerve detection was achieved were measured.</P> <P><B>Results</B></P> <P>There were no statistically significant differences between the maximum distance or mean amplitude obtained from nerve stimulators attached to EBDs and those obtained from the conventional nerve stimulator. Additionally, there were no adverse EMG events related to the use of nerve stimulators attached to EBDs.</P> <P><B>Conclusions</B></P> <P>Attachment of a nerve stimulator to an EBD for nerve detection during thyroidectomy was as safe and effective as attachment of a conventional nerve stimulator. Use of a nerve stimulator attachment may reduce the likelihood of EBD-associated nerve damage during thyroid surgery.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Nerve stimulator-EBDs showed efficacy and safety comparable to those of conventional monopolar probes. </LI> <LI> Nerve stimulator-EBD has the potential to reduce the likelihood of EBDs related thermal RLN injury. </LI> <LI> Nerve stimulator-EBD would have particular utility for endoscopic or robotic thyroid surgery. </LI> </UL> </P>

      • Realization of Human Interactive Control for Fish Aquarium Robotic

        The proposed study is the continuation of the previous study that aims to control the robotic fish which had researched on bio-mimetic ornamental aquarium robotic. Recently, our research held on human interactive control based on hand gesture recognition. From the real time video, users are able to control the motion of the robotic fish by using two hands where the left hand deciding which fish will be controlled and the right hand deciding which motion of the robotic fish will be controlled. In the other case, user utilized one hand, which decides to select the tracking control algorithms such as color mark, stop zone and lead-lag tracking control algorithms. Hand gesture recognition consists of hand segmentation, and gesture recognition from the hand features. Our result shows that an interactive human control, which is in this study, is hand gesture, successfully control the motion of the robotic fish in an interactive manner.

        Role of p38 MAPK on the Down-Regulation of Matrix Metalloproteinase-9 Expression in Rat Astrocytes

        In spite of their pathophysiological importance in neuro-inflammatory diseases, little is known about the signal transduction pathways that lead to the induction of matrix metalloproteinases(MMPs) in the central nervous system. We reported previously that lipopolysaccharide (LPS) induced MMP-9 expression through ERK1/2 pathway in rat primary astrocytes (Glia 41:15-24, 2003). Here, we investigated the role of other MAPK pathways, including p38 and JNK/SAPK, on the regulation of MMP-9 expression in LPS-stimulated rat primary astrocytes. LPS activated both p38 and JNK in astrocytes. Treatment with a specific p38 MAPK inhibitor SB203580, but not JNK inhibitor Sp600f 25, increased the LPS-stimulated MMP-9 expression in a concentration-dependent manner. Anti-inflammatory cytokines, including IFN-${\gamma}$ and 1L-4,activated p38 MAPK and decreased MMP-9 production in LPS-stimulated astrocytes. When p38 MAPK activation was blocked by SB203580, the inhibitory effects of these cytokines on MMP-9 induction were abolished. Finally, direct injection of SB203580 into the lateral ventricle of rat brain increased the LPS-induced MMP-9 activity in cerebral cortex. Altogether, these results suggest that p38 activation down-regulates the inflammatory stimulation-induced over-expression of MM P-9, both in primary astrocytes and in cerebral cortex. The elaborate inter-play between ERK1/2 and p38 pathways provides a more sophisticated mechanism for regulating MMP-9 activity in neuroirflammatory diseases.

      • Thermal Management System in Electric vehicle Battery Pack Using Phase Change Material

        Thermal Management in the Electric Vehicel is essential to improve the battery performance. This paper aims at the investigating the performance of Phase Change Material to prevent the thermal runway in the EVs. The better phase change materials are identified through simulation, based on heat transfer rate through the PCM. The design of the battery module was done in auto cad and the analysis done in ANSYS CFD software. The three pcms like paraffin wax, RT and Expanded Graphite were selected for the comparison. From the simulation result it was identified that the battery pack temperature maximum is not beyond the 60℃ (safe operating temperature range) in all test condition with the Expanded Graphite pcm material. The results were validated with numerical calculation and shown good agreement based on data collected at different temperatures.

        Passive Sampler 를 이용한 대도시의 대기질 평가

        Filter badge type sampler has not been widely used to evaluate air quality over large cities in Korea while it can be successfully used for multi-point sampling and analysis. We evaluated the passive sampler as a new tool to monitor air quality over large cities. We latticed Metropolitan Seoul into 2 × 2Km to give 136 points. NO₂ concentrations were measured at all the points in the Spring and Summer of 1997. According to the passive sampler data, natural green zones generally recorded lower NO₂ concentrations than major streets and traffic congestion areas. Passive samplers with abundant 136 points gave more detailed picture of NO₂ distribution while auto-monitoring network did not clearly provide the characteristics of local land use. Also, passive samplers gave 15% higher values than auto-monitoring network. The correlation between the two values appears very high judging from the regression slope of 0.92 and correlation coefficient of 0.91. This study clearly demonstrates the effectiveness of the passive sampler as a tool to monitor air quality over large cities.

        태권도 아래막기 동작의 운동역학적 분석

        This research is for providing taekwondo masters and learners with statistical basic data by analyzing mechanically the movement of arae makki, which is very basic defensive skill. To meet the purpose of this research, with 6 taekwondo masters, I analyzed the time required, the shift of physical center, speed, the range and speed of left fist, the angle and speed of the joint of left arm, angle of body twisting, and the repulsive force of floor. Through these, I could conclude like following. The first, the average required time for arae makki was 0.68±0.11 s, and it was shorter than for Preliminary motion. The second, the shift of physical center was not big in horizontal and vertical direction, however was big in back and forth direction. The speed of the shift was the fastest when the fist reached the highest. The third, the range of fist moved from the right to the left, from up to down, and from inside to outside of body. The highest speed was 6.80 m/s. The fourth, the change of the angle of arm was the biggest in elbow joint. bending was shown in preparing moves, and extension was shown in defending moves. The angle speed was the highest when checking the offense. The fifth, the angle of body twisting was the biggest when the left fist was in the highest. At the moment of checking the offense, the twisted body recovered and the checking moves were completed. The repulsive force was 0.857±0.154 %BW.

        저에너지 손상에 의한 고관절 골절 환자에서 골밀도와 골대사 지표의 분석

        목적: 골다공성 골절군과 정상 대조군을 남, 여 성별에 따라 분류하고, 각각의 골밀도와 골대사 지표를 비교, 분석하여 골밀도의 감소와 골교체의 이상이 골다공성 골절의 한 원인으로 작용하는지를 분석하였다. 대상 및 방법: 1999년 11월부터 2003년 8월 까지 교통사고 및 추락 등 고에너지 손상이 아닌 낙상 등 저 에너지 손상으로 인한 고관절부 골절로 본원에서 치료한 76명과 대조군 83명을 대상으로 하였다. 골절군은 남자가 22예, 여자가 54예이었고 평균나이는 남자가 67.2 (51-79)세이었고, 여자가 71.1 (55-87)세이었다. 대조군은 남자가 22예, 여자가 61예이었고, 평균나이는 남자가 67.9 (57-85)세, 여자가 70.1 (60-80)세이었다. 골밀도는 이중에너지 방사선 흡수계측기를 이용하여 측정하였고, 골대사 지표로는, 혈청 골 특이성 알칼리 인산분해 효소와 혈청 osteocalcin을 골 형성지표로 측정하였고, 골 흡수 지표로 요 deoxypyridinoline을 측정하였다. 결과: 혈청 골 특이성 알칼리 인산분해 효소의 평균값은 남녀 모두 골절군에서 통계학적으로 대조군보다 유의하게 낮았다(p<0.05). 혈청 osteocalcin의 평균값도 남녀 모두 골절군에서 통계학적으로 대조군보다 유의하게 낮았다(p<0.05). 요디옥시피리디놀린의 평균값은 남녀 모두 골절군과 대조군 사이에 의미있는 차이는 없었다. DEXA를 이용하여 측정한T-score의 평균값은 남녀 모두 골절군에서 유의하게 낮았다(p<0.001). 결론: 골다공성 골절군에서 남녀 모두 대조군보다 골밀도 및 골형성이 감소되는 것으로 나타나, 골밀도의 감소와 골형성의 저하가 골다공성 골절의 원인으로 작용하는 것으로 분석되었다. Purpose: This study was conducted to compare bone mineral density (BMD) and biochemical markers of bone metabolism of an osteoporotic hip fracture group with those of a control group. Materials and Methods: The fracture group included 76 patients with hip fractures who were injured by low energy trauma from November, 1999 to August, 2003. The control group consisted of 83 patients recruited over the same period. There were 22 men and 54 women in the fracture group, with an average age of 67.2 years for men and 71.1 years for women. In the control group, there were 22 men and 61 women, of average age 67.9 years for men and 70.1 years for women. There were no siginificant differences in age, height, weight, or body mass index. We analyzed BMD by a dual energy X-ray absorptiometry (DEXA). Bone specific alkaline phosphatase (B-ALP) and osteocalcin were used as markers of bone formation and urine-deoxypyridinoline (DPD) as a maker of bone resorption. Results: B-ALP showed a significant decrease in the fracture group (p<0.05). Also, average serum osteocalcin showed a significant decrease in the fracture group (p<0.05). There was no significant difference in urine-DPD between the two groups. The mean values of BMD in men and women were significantly decreased in the fracture group. Conclusion: The results of this study suggest that a decreased BMD contributes to hip fractures in elderly people and that B-ALP and serum osteocalcin rather than urine-DPD are reliable biochemical markers of bone metabolism in osteoporotic hip fractures.
