        A Systematic Review and Meta-Analysis of Arthroscopic Meniscus Repair in Young Patients: Comparison of All-Inside and Inside-Out Suture Techniques

        Purpose: The purposes of the present study were to review published studies that investigated arthroscopic meniscus repair to treat meniscus injury in young patients and to compare all­inside and inside­out suture techniques.Methods: Various electronic databases were queried for published articles, and this search was updated in August 2017 for evaluating the outcomes of arthroscopic meniscus surgery in young patients. Data search, extraction, analysis, and quality assessment were performed according to the Cochrane Collaboration guidelines, and the clinical outcomes were evaluated using various outcome values in young patients according to suture techniques.Results: Three randomized controlled trials and three prospective comparative studies were included in this systematic review and meta­analysis. There were no significant differences in clinical outcomes such as meniscus healing rate (risk ratio [RR], 1.11; 95% confidence interval [CI], 0.90 to 1.37; I2=39%) and perioperative complications (RR, 0.62; 95% CI, 0.23 to 1.72; I2=43%) between all­inside and inside­out techniques for meniscus repair.Conclusions: The present study shows favorable results for clinical outcomes such as meniscus healing rate and perioperative complications in young patients. Furthermore, based on our results, both all­inside and inside­out meniscal suture techniques are equally effective in these patients.

      • Equaatiorial-Skew [Co(TRDTRA)(OH₂)] 착물과 CN, NO₂그리고 NCS 이온간의 치환반응

        Equatorial-Skew형[Co(TRDTRA)(OH₂)]착물과 ??,?? 그리고 ?? 이온 간의 치환반응결과 생성된 차물의 치환반응성 및 구조를 전자흡수 스펙트럼과 Yamatera 이론에 따라 조사하였다. 속도론적 자료로부터 착물과 ??,?? 그리고 ?? 이온의 반응차수는 각각 1차이며 총괄 반응차수는 2차임을 알 수 있었다. ?? 및 ?? 이온에서 생성된 착물의 구조는 이성질화 반응이 동반된 Polar-Chair 형이며, ?? 이온의 경우에는 이성질화 반응이 동반되지 않은 Equatorial-Skew 형임을 알 수 있었다. The substitution reaction and structrue of products obtained from the reaction of Equatorial-Skew-[Co(TRDTRA)(OH??)](TRDTRA=trimethylenediaminetriacetate) with ??,??, and NCS ions have been investigated by means of electronic absorption spectroscopy and theoretical calculation based on the Yamatera`s theory. According to kinetic data, the substitution reaction order for the complexes such as ??,?? and ?? was the first order, respectively, and overall reaction order was second order. It has been determined that the structure of products having ?? and ?? ions was Polar-Chair type complexes which were accompanying with isomerization and having ?? ion was Equatorial-Skew type complex which was not accompanying with isomerization.

        The aim of this study is to report 2 cases of acute promyelocytic leukemia who died from intracranial hemorrhage following leukapheresis and to provide proper preventive measures against hemorrhage following leukapheresis. From 1994 to 1997, a total of twenty-six patients with leukemia underwent leukapheresis to control hyperleukocytosis at Ajou University Hospital. Two patients with acute promyelocytic leukemia received all-trans retinoic acid but developed drug-induced hyperleukocytosis. Shortly after leukapheresis, they died from intracranial hemorrhage. The risk factors for fatal hemorrhage are thought to be coexisting disseminated intravascular coagulation(DIC), thrombocytopenia aggravated by leukapheresis, exacerbated coagulopathy related to mechanical trauma through leukapheresis and the excess use of citrate during leukapheresis. To reduce the risk of the bleeding associated with leukapheresis, it is necessary to replace platelet sufficiently before and after leukapheresis and to give calcium to correct coagulopathy induced by excess citrate which is used as anticoagulant as well as the correction of disseminated intravascular coagulation.

        Cystic Medullary Thyroid Carcinoma: A Case of Undergoing Endoscopic Thyroid Lobectomy

        On ultrasonography, medullary thyroid carcinoma (MTC) shows hypoechogenicity, an irregular margin, a predominantly solid composition, and microcalcifications, similar to those observed in papillary thyroid carcinoma (PTC). MTC presenting as a cystic lesion is rare, and endoscopic thyroidectomy can be performed for benign thyroid masses and early stage PTC, however it is inappropriate for MTC regardless of cystic change. The authors report a case of cystic MTC found after endoscopic thyroid lobectomy and provide a review of the literature on this topic.

        抗精神病藥物에 依한 無顆粒細胞症 治驗 1例

        The authors experienced a female schizophrenic patient who revealed severely decreased leucocytes (350/㎣) with the manifestation of high fever, chillness and sore throat after the oral administration of neuroleptics. Agranulocytosis induced with the use of neuroleptic drugs has been well known complication in psychiatric practice although it is quite rare. In this case, following the combined use of neuroleptics (thioridazine halopondol and chlorproonazine), severe decrement of leucocytes (mainly lymphocytes with few neutrophils) was noticed on peripheral blood and marked decrement of erythroid series, especially of myeloid series, with decrement of total cellular elements were confirmed through the biopsy of bone marrow. She did not responded at all to the massive administration of antibiotics, but dramatic improvement was noticed both clinically and in the follow-up biopsy findings of bone marrow after two times trial of leukapheresis.

        Midfacial degloving approach을 이용한 양측성 상악골절제술의 치험례

        Maxillary cancer accounts for only 0.3% of all cancers and tumors of the nose and paranasal sinuses are 80% of maxillary neoplasms. The complexes of midfacial anatomical landmarks make the extension of the maxillary cancer more easy. In 1826 since Lazar first made unilateral maxillectomy for the maxillary cancer, various developments were evoluated and in the surgical approach of these lesions most oral & maxillofacial surgeons have used the vestibular approach or Weber-Fergusson approach and also unilateral lesions are common but bilateral lesons are rare. In 1992 Sjovall introduced the bilateral maxillectomy and for the preventions of the facial scar, In 1927 Portmann and Retrouvey reported the sublabial approach and In 1979 Price reported the midfacial degloving approach and in 1981 Allen and Siegel used the term "Midfacial degloving approach". Unlike the previous used Weber-Fergusson approach and Caldwell-Luc method, midfacial degloving method can make the intraoral approach of midfacial structure exposure without facial incision and has populated gradually for the removal of tumors and facial plastic surgery. In this case, two male pt. with bilaterally extensive malignant neoplasm on maxillary sinus was treated with pre or post op. radiotherapy and then underwent the bilateral subtotal maxillectomy via midfacial degloving approach and relatively satisfactory results were obtained.

        This study was performed to estimate the organic matter removal efficiency when apply to natural purification method with variable media. We used gravel, concrete, tire, scrubber as media of contact oxidation reactor. And when scrubber, gravel, the average SS, BOD, COD removal efficiency was very high(when scrubber, 96.3, 78.1, 59.0 % and gravel, 82.1, 73.8, 56.5 %, respectively). If the physical and chemical operations are added to contact oxidation process for the removal of nitrogen, phosphorous, the water quality of urban small stream which is polluted by municipal waste water will be purified.

        Laser cladding processing allows rapid transfer of heat to the material being minimum conduction into base metal. The effects of CO_2 laser cladding with high ?? powder were investigated. High viscosity bronze powder consists of bronze powder a?? used at a high temperature. The material has a high viscosity so that it can be e?? substrate. Therefore, Laser cladding can be processed on a curved or slope surface. CO_2 laser cladding was designed. It consists of the high viscosity bronze powder f?? the shielding gas system and the preheating system. The high viscosity powder properly at 0.35g/s and 0.50g/s. Because of the metallic bond between bronze powder the hardness of dilution layer was suddenly increased. Experimental as results in?? viscosity mixed powder can be a useful cladding material.

      • 複合組織鋼의 摩擦 熔接部의 疲勞破壞特性에 관한 硏究

        This study has been carried out to investigate the characteristics of fatigue crack of welded zone and heat affected zone of friction welded dual phase steel. The results obtained from the experiments are summarized as follows. Microstructure of welded zone is changed to ferrite and pearlite from ferrite and martensite by wel-ding heat. Micro-vickers hardness of welded zone is higher than that of base metal zone and heat affec-ted zone. Micro-vickers hardness of heat affected zone is lower than that of welded zone and microstructure of heat affected zone is changed to fine martensite and ferrite by heating cycle of welding. Fatigue limits of heat affected zone is a little higher than that of welded zone and base metal zone. There are no difference of fatigue limit of base metal zone and welded zone. Fatigue strength of welded zone is similar to that of base metal zone. At high △K region(△K>19MPa·??), fatigue crack propagation velocity of heat affected zone is slower than that of welded zone and base metal zone, and the difference of fatigue crack propagation velocity is smaller than that of high △K region. The crack occurs to the notch in heat affected zone first, but the final fracture surface is the surface of welded zone. It is abvious that heat affected zone of friction welded dual phase steel superior to welded zone and base metal zone in fatigue strength and resistance of fatigue crack propagation. The above mentioned facts are contrary to theory that heat affected zone of general metals is weaker than welded zone and base metal zone.
