Purpose: For the atypical cases of fine needle aspiration (FNA) cytology of thyroid nodules, ultrasonographic findings are a primary guideline for the surgical treatment. However, they have the intrinsic risk of overtreatment, as well. In this study we examined whether the Bethesda system could provide a real effect on the diagnostic rate of atypical cytology, and thereby reduce the number of cases diagnosed as atypical from FNA cytology. Methods: We reviewed 166 cases diagnosed as atypical by FNA cytology at this institute between the years 2005 to 2010. We classified these cases on the basis of ultrasonographic and cytological findings and compared them with the histological results. Results: Ultrasonographically, findings suspicious for malignancy and indeterminate were associated with 83.7% and 47.2% of malignancy rates, respectively. Cytopathologically, the malignancy rates varied according to the main cytological features and the highest malignancy rate was 77.3%. Based on the Bethesda system, 39.2% of the cases diagnosed as atypical could be grouped into the category of suspicious for malignancy and yielded a malignancy rate of 76.9%. Conclusion: Although ultrasonography provides an excellent guideline for the surgical treatment of atypical cases, it also showed considerable risk of overtreatment. The Bethesda system did not offer definitive effects on the rate of atypical cytology, but this system seemed to provide stricter boundaries for the atypical cytology and to aid in reducing the rates thereof. This in turn may permit that more limited cases are allotted to ultrasonographic decision making.
Pneumatic driven pulsatile ECMO in vitro evaluation with oxygen tanks

Extracorporeal membrane oxygenation device is a procedure in which mechanical systems circulate blood and supplyoxygen to patients with impaired cardiopulmonary function. Current venoarterial systems are associated with low patientsurvival rates and new treatments are needed to avoid left ventricular dilation, which is a major cause of death. In thisstudy, a new mobile pulsatile ECMO with a pump structure that supplies pulsatile flow by using an oxygen tank as a powersource is proposed. In vitro experiments conducted under mock circulation system as like patient conditions demonstratedthat 2.8 L oxygen can sustain the outflow of 1 L/min of pulsatile blood flow for 53 min, while a 4.6 L tank was able tosustain the same flow for 85 min. The energy equivalent pressure evaluation index of the pulsatile blood pump showsthat the mobile pulsatile ECMO could supply sufficient pulsatile blood flow compared to the existing pulsatile ECMO. Through in vitro experiments performed under mock circulation conditions, this new system was proven to supply sufficientoxygen and pulsatile blood flow using the pressure of an oxygen tank even while transporting a patient.

Mesenteric fibromatoses occur as sporadic lesions or as parts of familial adenomatous polyposis, are clonal lesions prone to locally aggressive behavior, but lack metastatic capacity. Fibromatoses harbor somatic β-catenin or adenomatous polyposis coli (APC) mutations, leading to intranuclear accumulation of b-catenin. We report a case of mesenteric fibromatosis presenting as a 5.4x4.8 cm submucosal tumor of the jejunum in a 42-year-old woman. Histologically, the tumor consisted of proliferation of bland, elongated-spindle fibroblasts with collagen deposition and a characteristic prominent vasculature. Immunohistochemically, the tumor cells showed nuclear staining for β-catenin. Mesenteric fibromatosis should be included in the differential diagnosis of submucosal spindle cell tumors of the gastrointestinal tract, and nuclear immunoreactivity for β-catenin may help distinguish these tumors form histological mimics.

Gangliocytic paragangliomas are rare benign neuroendocrine tumors that arise mainly in the duodenum. A total of 3 cases of duodenal gangliocytic paraganglioma have been reported in Korea. The authors encountered another case of a gangliocytic paraganglioma in the duodenum. A 48-year old man presented with a mass in the duodenum that had been found incidentally in a medical checkup. The endoscopic examination revealed a 3.5×2.2 cm sized polypoid mass in the second portion of the duodenum. The patient underwent a polypectomy in the form of a wedge resection under general anesthesia. Microscopically, the tumor was composed of epithelial cells forming paraganglioma like patterns, spindle cells and ganglion cells. Gangliocytic paragangliomas are believed to be benign tumors but regional lymph node metastasis and recurrence can occur in rare cases. During the 27-month follow-up after treatment, there was no evidence of recurrence or lymph node metastasis. We report this case to help improve the understanding of this rare disease.

This study presents a finite element fluid-structure interaction model for the horizontal canal system of the inner ear. The fluid? structural interaction problem is treated rigorously with a strong coupling between the fluid flow and the structural displacements. The endolymph is modeled as a slightly compressible Newtonian fluid and the cupula partition is represented by a linearly elastic solid. There are two models, Simple model and Real model. It is compare between simple model and real model. We estimate the shape of semi-circular canal. The real model has been got the pressure information in short time.

Sand Compaction Piles (SCPs) are constructed by feeding and compacting sand into soft clay ground. Sand piles have been installed with irregular cross-sectional shapes, and mixtures of both sand and clay, which violate the design requirement of circular shape according to the replacement area ratio due to various factors, including side flow pressure. Therefore, design assumptions cannot be satisfied according to the conditions of the ground and construction and the replacement area ratio. Two case histories were collected, examined, and interpreted in order to study the effect of the shape of SCPs. The effects of the distortion of SCP shape and the mixture of sand and clay were studied with the results of large direct shear tests. The design internal friction angle was secured with the irregular cross-sectional sand piles regardless of the replacement area ratio. The design internal friction angle was secured regardless of mixed condition when the mixture of sand and clay was higher than the replacement area ratio of 65%. Therefore, systematic construction management is recommended with a replacement area ratio below 65%.
